불안장애에 동반된 우울증상에 대한 Psychotherapies의 효과비교 : 체계적 문헌고찰 (Comparison of Effectiveness of Psychotherapies for Relieving Depressive Symptoms in Anxiety Disorders : A Systematic Review)

  • Objective : The purpose of this study was to compare and analyze effectiveness of various psychotherapies of depressive symptoms in patients with anxiety disorders by means of a systemic review. Methods : A systematic search using key words was conducted using EMBASE, MEDLINE and CENTRAL. Results : Five studies were included in this systematic review. The Scottish Intercollegiate Guidelines Network (SIGN) was used for quality assessment. Diagnostic-specific cognitive behavioral therapies for various anxiety disorders (ds CBT) were also effective in reducing comorbid depression. Transdiagnostic cognitive behavioral therapies targeting anxiety and depression (tCBT-AD) were not superior than ds CBT for anxiety disorders. Mindfulness-based stress reduction (MBSR) was more effective than traditional cognitive behavioral therapy. Conclusion : ds CBT relieves anxiety symptoms as well as depressive symptoms comorbid with anxiety disorders effectively. In addition, MBSR is more successful for anxiety disorders with co-occurring depression by focusing broadly on the shifting relationship to internal experience rather than focusing narrowly on specific symptoms.

  • Objective : Brief screening for anxiety symptoms in clinical practice can further facilitate the diagnosis and evaluation of anxiety disorders. This study examined the factorial validity of the Korean version of the State-Trait Inventory (STAI)-Form X, one of the most frequently used self-report questionnaires for anxiety. Methods : Data from the STAI and Beck Depression Inventory were obtained from a consecutive sample of 200 outpatients diagnosed with DSM-IV anxiety disorders at a psychiatric unit of a university hospital. The factor structures of the State and Trait Scales were assessed using exploratory factor analysis. Results : Three-factor components, including 'State anxiety present', 'State anxiety absent' and 'Selfconfidence', were extracted from the State Scale, explaining 59% of the total variance. A four-factor solution involving 'Trait anxiety and depression present', 'Trait anxiety and depression absent', 'Anxiety proneness' and 'Stability' (59% of total variance) was extracted from the Trait Scale. The internal consistency of the STAI and factors were satisfactory. There were significant correlations between depressive symptoms and factors of the STAI. Conclusion : The STAI-form X showed factorial validity for Korean patients with anxiety disorders. However, our finding that this anxiety scale also measures depressive symptoms should be interpreted with caution.

Effects of shirodhara in generalized anxiety disorder

  • Anxiety is a common clinical presentation. Primary anxiety poses a significant problem in its management. Many among the current treatment options of anxiety are habit forming causing significant withdrawal symptoms. There are dose dependent responses often associated with drug adversities. Day time sedation is an undesired effect of many drugs used for anxiety management limiting its usage. Ayurveda recommends a bio- physical procedure to manage anxiety. Shirodhara, a dripping procedure is utilized as a front line therapy for anxiety in Ayurveda. Seeing the limitations of conventional biomedical management of anxiety requiring an improvement upon the existing protocols of managements, and also seeing the use of shirodhara for anxiety management in ayurvedic clinics, a pragmatic study to evaluate the effect of shirodhara in generalized anxiety disorder was done. This was a pragmatic study consisting of shirodhara with ksheer bala oil as an intervention upon generalized anxiety disorder patients. The study was conducted upon 13 patients and observations were done for six weeks. The responses observed through the changes in Hamilton Anxiety Scale were evaluated using paired t test to observe the pre-post significance.

청소년기 심담허겁(心膽虛怯) 환아의 온담탕(溫膽湯) 치험 1례 (A clinical study on the effect of Ondam-tang on anxiety disorder in adolescence)

  • Objectives : The purpose of this study is to find out whether Ondam-tang has any effects on adolescence with anxiety disorder Methods : The subject is in regard to 1 case with anxiety disorder who adnutted at ${\bigcirc}{\bigcirc}$ Oriental Medical hospital from 29th of June to 7th of July. The herbal therapy (Acupuncture, Moxa, Aroma therapy) is applied. Results : After treatment, in a week, most of symptoms (insomnia, anxiety, the problem of verbal expression, chest discomfort, palpitation, anorexia, digestion, headache, dizziness, thirst, eye discomfort, etc) were subsided. Conclusion : Anxiety disorder is similar to Simdamhugup in Oriental medicine. Ondam-tang has been used for long time for treatment on Simdamhugup. Ondam-tang is effective medication for adolescence with anxiety disorder, but more studies about adolescence and Oriental approach of psychiatries are needed.

당뇨병 환자에서 우울 및 불안증상과 자아탄력성, 사회적 갈등 및 지지 사이의 연관성 (Association of Depressive/Anxiety Symptoms with Ego Resilience and Social Conflict/Support in Patients with Diabetes Mellitus)

  • Objective : The purpose of this study was to investigate the association of depressive/anxiety symptoms with psychosocial factors including ego resilience, social conflict, and social support as well as hemoglobin A1c in patients with diabetes mellitus (DM). Methods : The subjects were 144 patients with DM. Depressive/anxiety symptoms were evaluated by the Hospital Anxiety and Depression Scale. Sociodemographic factors, hemoglobin A1c, ego resilience, social conflict, and social support were measured. Multiple regression analyses were performed to examine the impact of hemoglobin A1c, ego resilience, social conflict, and social support on depressive/anxiety symptoms. Results : A total of 32.6% and 24.3% of participants were identified with depressive and anxiety symptoms, respectively. Depressive/anxiety symptoms positively correlated with the social conflict score. Conversely, ego resilience and social support negatively correlated with depressive/anxiety symptoms. In the final model of the multiple regression analyses, ego resilience was associated with a lower level of depressive (β=-0.083, p=0.019)/anxiety (β=-0.125, p=0.001) symptoms whereas social conflict was related to a higher level of depressive (β=0.353, p=0.011)/anxiety (β=0.460, p=0.003) symptoms. Also, hemoglobin A1c positively associated with anxiety symptoms (β=0.495, p=0.012) whereas social support negatively related to depressive symptoms (β=-0.464, p=0.004). Conclusion : We found possible risk and protective psychosocial factors of underlying depressive/anxiety symptoms among patients with DM. Our findings suggest that enhancing ego resilience and social support as well as decreasing social conflict would be crucial in the prevention and management of depressive/anxiety symptoms in patients with DM.

  • Although treatments of anxiety symptom have been available for decades, the biological basis for anxiety disorders in humans is just beginning to emerge. Recently, there is a growing body of literature suggesting that group II metabotrpic glutamate (mGlu) receptors and group I mGlu receptors are important in the physiological and behavioral sequelae associated with stressful stimuli. Moreover, compounds selective for mGlu receptors, particularly mGlu2/3 and/or mGlu5, have proven as effective as classical anxiolytics in various animal models of anxiety without producing many of the unwanted side effects that are typical of current therapies. This article will focus on the emerging preclinical and clinical data that implicate modulation of the mGlu receptors as a potential anxiolytic strategy.

  • Interest in anxiety has a long history, and this topic has played a significant role in the field of mental health. The concept of anxiety has evolved diversely over time and across cultures, influencing our understanding, diagnosis, and treatment approaches for anxiety disorders. In this paper, we have reviewed and summarized various concepts and theories related to anxiety that have evolved over several centuries. Furthermore, we have examined how the diagnostic criteria in the DSM (Diagnostic and Statistical Manual of Mental Disorders) have changed within this historical context. A precise understanding of the history of the concept of anxiety and anxiety disorders is believed to have a positive impact on shaping the future path of anxiety disorders in psychiatry.

Use of an animated emoji scale as a novel tool for anxiety assessment in children

  • Background: Dental anxiety in children is a major barrier in patient management. If dental anxiety in pediatric patients is assessed during the first visit, it will not only aid in management but also help to identify patients who are in need of special care to deal with their fear. Nowadays, children and adults are highly interested in multimedia and are closely associated with them. Children usually prefer motion pictures on electronic devices than still cartoons on paper. Therefore, this study was conducted to evaluate a newly designed scale, the animated emoji scale (AES), which uses motion emoticons/animojis to assess dental anxiety in children during their first dental visit, and compare it with the Venham picture test (VPT) and facial image scale (FIS). Methods: The study included 102 healthy children aged 4-14 years, whose dental anxiety was measured using AES, VPT, and FIS during their first dental visit, and their scale preference was recorded. Results: The mean anxiety scores measured using AES, FIS, and VPT, represented as $mean{\pm}SD$, were $1.78{\pm}1.19$, $1.93{\pm}1.23$, and $1.51{\pm}1.84$, respectively. There was significant difference in the mean anxiety scores between the three scales (Friedman test, P < 0.001). The Pearson's correlation test showed a very strong correlation (0.73) between AES and VPT, and a strong correlation between AES and FIS (0.88), and FIS and VPT (0.69), indicating good validity of AES. Maximum number of children (74.5%) preferred AES. Conclusion: The findings of this study suggest that the AES is a novel and child-friendly tool for assessing dental anxiety in children.

항암치료 및 교통사고로 유발된 불안에 대한 한의학적 치료 1례: 증례 보고 (Korean Medicine Treatment for Anxiety Induced by Anticancer Therapy and Traffic Accident: A Case Report)

  • Objectives: The purpose of this study was to report the efficacy of using Korean medicine treatment, for anxiety induced by chemotherapy and traffic accident. Methods: We treated a patient's anxiety with Korean medicine treatments (Yangshimtang-Gamibang, ear acupuncture, acupuncture, cupping, interferential current therapy, and herbal acupuncture therapy). To evaluate the results of this treatment, we measured the Beck Anxiety Inventory, EuroQol-Five Dimensions, Numeric rating scale, Neck Disability Index, Oswestry Disability Index, total sleep duration, time duration to fall asleep, and the number of wake-up times during sleep. Results: Improvements of the Beck Anxiety Inventory, EuroQol-Five Dimensions, Numeric rating scale, Neck Disability Index, Oswestry Disability Index, and total sleep duration were observed after the treatments. Conclusions: This study showed one case of a patient who received Korean medicine treatment for the anxiety induced by anticancer therapy and traffic accident, and improvement was observed.

주의력결핍-과잉행동장애 초등학생에서 불안증상이 아동의 행동 및 주의력과 가지는 관련성에 관한 연구 (Associations of Anxiety Symptom with Behavior and Attention in Elementary School Children with Attention-Deficit/Hyperactivity Disorder)

  • Objective : This study aimed to investigate the associations of anxiety symptoms with behavior and attention in elementary school children with attention-deficit/hyperactivity disorder (ADHD). Methods : A total of 195 elementary school children with ADHD participated in the study. The Korean Version of ADHD Rating Scale (K-ARS) and Behavior Assessment System for Children(BASC-2) were used to measure the children's behavior. Anxiety and attention was assessed with State-Trait Anxiety Inventory for Children (STAIC) and ADHD Diagnostic System (ADS), respectively. Children with ADHD were categorized to two groups of ADHD with low anxiety (ADHD-LA) and ADHD with high anxiety (ADHD-HA) according to the total STAIC scores. Scores on K-ARS, BASC-2 and ADS were compared between two groups of ADHD-LA and ADHD-HA. Results : The Scores on K-ARS total and both subscales in ADHD-HA group were significantly higher than ADHD-LA group. Scores on the BASC-2 subscales including hyperactivity, aggression, conduct problem, anxiety, depression, somatization, withdrawal, attention problems were also significantly higher in ADHD-HA group compared to ADHD-LA group. In contrary, scores on ADS subscales were not significantly different between the both groups. Conclusion : Our study identified that the anxiety accompanied with ADHD was associated with the negative behavioral aspect in children with ADHD. However, the performances on attention task were not affected by the anxiety comorbid with ADHD. Future studies to reveal underlying mechanism are needed for further understanding the association with anxiety and ADHD.
