Promoter Methylation of CDKN2A, $RAR{\beta}$, and RASSF1A in Non-Small Cell Lung Carcinoma: Quantitative Evaluation Using Pyrosequencing

  • Background: While qualitative analysis of methylation has been reviewed, the quantitative analysis of methylation has rarely been studied. We evaluated the methylation status of CDKN2A, $RAR{\beta}$, and RASSF1A promoter regions in non-small cell lung carcinomas (NSCLCs) by using pyrosequencing. Then, we evaluated the association between methylation at the promoter regions of these tumor suppressor genes and the clinicopathological parameters of the NSCLCs. Methods: We collected tumor tissues from a total of 53 patients with NSCLCs and analyzed the methylation level of the CDKN2A, $RAR{\beta}$, and RASSF1A promoter regions by using pyrosequencing. In addition, we investigated the correlation between the hypermethylation of CDKN2A and the loss of $p16^{INK4A}$ immunoexpression. Results: Hypermethylation of CDKN2A, $RAR{\beta}$, and RASSF1A promoter regions were 16 (30.2%), 22 (41.5%), and 21 tumors (39.6%), respectively. The incidence of hypermethylation at the CDKN2A promoter in the tumors was higher in undifferentiated large cell carcinomas than in other subtypes (p=0.002). Hyperrmethylation of CDKN2A was significantly associated with $p16^{INK4A}$ immunoexpression loss (p=0.045). With regard to the clinicopathological characteristics of NSCLC, certain histopathological subtypes were found to be strongly associated with the loss of $p16^{INK4A}$ immunoexpression (p=0.016). Squamous cell carcinoma and undifferentiated large cell carcinoma showed $p16^{INK4A}$ immunoexpression loss more frequently. The Kaplan-Meier survival curves analysis showed that methylation level and patient survival were barely related to one another. Conclusion: We quantitatively analyzed the promoter methylation status by using pyrosequencing. We showed a significant correlation between CDKN2A hypermethylation and $p16^{INK4A}$ immunoexpression loss.

Magnetic Resonance Imaging Diagnosis: Nasal Adenosquamous Cell Carcinoma in a Dog (개 비강종양의 자기공명영상학적 평가 1예)

  • Nasal tumors have been reported frequently in dogs and nasal discharge has been the primary presenting complaint in nasal tumor patients. A 10-year-old 7 kg male mixed breed dog was presented to the animal hospital for evaluation of severe bloody nasal discharge. After physical examination, blood examination and X-ray examination, magnetic resonance imaging (MRI) examination was also conducted. Based on MRI nasal tumor which does not extend cranial vault was diagnosed. The nasal tumor was confirmed as adenosquamous cell carcinoma by histopathological examination after necropsy. The invasive characteristics of the neoplasm were documented by MRI. MRI was very valuable diagnostic tool for identifying extracranial and intracranial tumor invasion in a small animal.

Pathological Interrelations of Soil-Borne Diseases in Cucurbits Caused by Fusarium Species and Meloidogyne incognita

  • Pathological interrelations of two soil-borne diseases in cucurbits (watermelon, oriental melon, shintosa and cucumber) caused by Fusarium isolates (FI) and the root-knot nematode (RKN), Meloidogyne incognita were characterized by the fusarium disease severity index (DI), RKN gall index (GI) and eggmass index (EI) in inoculation tests using FI and RKN. Virulence of FI as determined by DI at 4 weeks after inoculation was mostly in the higher order of Fusarium proliferatum F6, F5 and Fusarium oxysporum f. sp. melonis or Fusarium oxysporum f. sp. niveum with no significant differential interactions among the cucurbits and RKN co-infection. Significant increases of DI due to RKN coinfection were noticed in watermelon and oriental melon infected with F. proliferatum isolates, suggesting the DI increase due to RKN coinfection may depend upon the virulence of FI relative to aggressiveness of RKN on the cucurbits. For the coinfection of FI and RKN, GI and EI were mostly reduced logarithmically with the increase of DI, largely more in EI than GI, in all cucurbits except for shintosa. Microscopic examination of the root tissues showed histopathological features characteristic to infection types; formation of fungal hyphae and/or spores and plant defense structures (tyloses and mucilage) in variable degrees and formation of giant cells at variable developmental stages and with variable cytoplasmic depletion or degeneration which were visualized in relations with the values of DI, GI and EI. These findings will be helpful to develop control strategies of the soil-borne disease complex based on their pathological characteristics.

Safety of Thyroidectomy in Thyroid Cancer Patients Older than 75 Years (고령 갑상선암 환자의 갑상선 절제술의 안정성)

  • Background and Objectives: The aim of this study was to evaluate the safety and the feasibility of thyroidectomy for aged (${\geq}75$ years old) thyroid cancer by reviewing postoperative morbidity and pathologic characteristics. Materials & Methods: The clinical records of patients submitted to thyroid operation between 2014 and 2015 with histopathological diagnosis of thyroid cancer were analyzed. Clinical variables included age, gender, preoperative symptom, final pathology, length of stay, comorbidities, American Society of Anesthesiologists score, postoperative complications, and mortality. Results: There were 30 patients aged more than 75 years old, and only one patient passed over with postoperative pneumonia. There were 8 male and 22 female patients. Half of the patients presented with symptoms such as palpable mass (33.3%), voice change (6.7%) and dyspnea (6.7%). The pathologic diagnosis was thyroid cancer in all cases. The median postoperative hospitalization time was 4 days. There was one perioperative mortality case observed in this study. Conclusion: Thyroid surgery in patients 75 years or older can be performed except extensive surgical resection. Aggressive histology and extent of surgery may be an important factor of perioperative mortality and morbidity.

Characterization and pathogenicity of extracellular products (ECPs) of Vibrio harveyi (Vibrio harveyi에서 분리한 extracellular products (ECPs)의 특성과 독성)

  • Vibrio harveyi, one of the major causal agent of vibriosis, affects a diverse range of marine vertebrates and invertebrates over a wide geographical area. The aims of this study were to investigate the characteristics of extracellular products (ECPs) from an isolate of the pathogenic non‐luminous V. harveyi and the luminous V. harveyi. And ECPs of V. harveyi were examined the pathogenicity to the black rockfish, Sebastes schlegeli, and histopathological traits of internal organs injected by ECPs. Four strains of V. harveyi cultures produced ECPs showing various enzymatic activites (caseinase, gelatinase, phospholipase, lipase, haemolysin). The ECPs showed strong cytotoxicity on macrophages of black rockfish, Sebastes schlegeli and olive flounder, Paralichthys olivaceus. Especially, the virulence of ECPs from the isolate of V. harveyi FR 2 was higher in the intraperitoneally injected black rockfish. Also, the ECPs of V. harveyi FR 2 caused the expansion of sinusoids in the liver, the activation of ellipsoid in the spleen and the sloughing of the epidermal cell in the intestine. It was suggested that the ECPs from V. harveyi play an important role in the pathogenicity process of the V. harveyi.

The Infection Characteristics of Vibrio scophthalmi Isolated from Olive Flounder, Paralichthys olivaceus (양식 넙치, Paralichthys olivaceus에서 분리한 Vibrio scophthalmi의 감염 특성)

  • Recently high mortality of cultured olive flounder, Paralichthys olivaceus occurred frequently at the fish farms in Ulsan, Korea. The diseased fish showed skinny body and swimming behavior around the water surface with liver atrophy and white enteritis as internal signs. The isolated bacteria were identified to V. scophthalmi by biochemical test, nucleotide analysis of 16S rRNA and dnaJ gene sequencing. The pathogen of this study showed strong pathogenicity as 75% mortality to olive flounder by intraperitoneal injection of $1{\times}10^6$ CFU/fish. The pathological sign was not different between the naturally diseased fish and the artificially infected fish. Histopathological changes were shown to liver atrophy, desquamation of the intestinal mucosa and hyaline droplet like as other previous studies.

Celiac disease in children: increasing prevalence and changing clinical presentations

  • Background: Celiac disease (CD) is a chronic autoimmune enteropathy. It results from genetic predisposition and exposure to gluten-containing food. The prevalence and presentation of CD vary among populations. Purpose: This study aimed to describe the prevalence and clinical characteristics of CD in children in Bahrain. Methods: We retrospectively reviewed the medical records of children diagnosed with CD in the pediatric department, Salmaniya Medical Complex, Bahrain, in 1988-2018. Their clinical, biochemical, serological, and histopathological findings were documented. Adherence to the recommended gluten-free diet (GFD) was assessed. Results: Of 86 patients with CD, 67 were included. The CD prevalence was 0.02%. A significant increase in prevalence in the last decade was observed (P<0.0001). Thirty-eight patients (56.7%) were males. The median (interquartile range) age at presentation was 4.45 (1.5-7.3) years. A family history of CD was positive in 13 out of 43 patients (30.2%). Pallor and failure to thrive were the most common presentations. The most frequent associated disease was iron-deficiency anemia in 23 patients (69.7%). Positive serology was found in 32 of 45 patients (71.1%). Marsh-Oberhuber type III was found in 16 of 35 patients (45.7%). Seropositive patients were significantly older (P=0.025) and had more severe duodenal histology (P=0.002). Adherence to GFD was poor in 27 patients (64.3%). Conclusion: This study revealed a significant increase in CD prevalence over the last decade. Atypical presentations were frequent. Most patients had poor adherence to GFD.

Subacute Oral Toxicity and Bacterial Mutagenicity Study of a Mixture of Korean Red Ginseng (Panax ginseng C.A. Meyer) and Salvia plebeia R. Br. Extracts

  • As various populations are rapidly becoming an aging society worldwide and interest in health issues has increased, demand for functional foods including herbal products has increased markedly to maintain a healthy state which has led to safety issues about their intake as an inevitable result. The objective of this study was to identify the safety profile of a Korean red ginseng and Salvia plebeia R. Br. extract mixture (KGC-03-PS) which is a valuable ingredient that can be used as a functional food. In the present study, the subacute oral toxicity and bacterial reverse mutagenicity of KGC-03-PS were evaluated. Sprague Dawley rats were administered KGC-03-PS orally for 28 days by gavage. Daily KGC-03-PS dose concentrations were 0, 500, 1,000, or 2,000 mg/kg body weight (bw) per day. Bacterial reverse mutation test with KGC-03-PS dose levels ranging from 312.5 to $5,000{\mu}g/plate$ was carried out by OECD test guideline No. 471. Five bacterial strains (Salmonella typhimurium TA98, TA100, TA1535, TA1537, and Escherichia coli WP2) were tested in the presence or absence of metabolic activation by plate incorporation method. There were no toxicological effects related with test substance in the clinical evaluation of subacute oral toxicity test including clinical signs, body weight, and food consumption. Moreover, no toxicological changes related to KGC-03-PS were observed in the hematological and serum biochemical characteristics as well as in the pathological examinations, which included organ weight measurements and in the gross- or histopathological findings. KGC-03-PS did not induce an increase in the number of revertant colonies in all bacterial strains of the bacterial reverse mutation test. The no-observed-adverse-effect level of KGC-03-PS is greater than 2,000 mg/kg bw/day, and KGC-03-PS did not induce genotoxicity related to bacterial reverse mutations under the conditions used in this study.

  • The infection status of domestic farmed eels Anguilla japonica, Anguilla bicolor and Anguilla marmorata with Japanese eel endothelial cell-infecting virus (JEECV) and anguillid herpesvirus 1 (AnHV) was examined at the major eel farming areas in Korea. These viruses were detected in all areas examined, regardless of the eel species or age. Any farm with a history of viral infection in adult fish confirmed the infection to be transmitted to stocked fry within 3 to 5 months. It is proposed that both viruses are horizontally transmitted within a given farm. The primary symptoms and histopathological lesions produced by the two viral infections are similar, making it difficult to distinguish the two diseases through clinical symptoms. Both viruses displayed 100% detection in the gills, suggesting that the gills are an optimal tissue for JEECV and AnHV monitoring. This study concluded that JEECV and AnHV were prevalent on eel farms across the country and caused very high mortality when the two viruses co-infected fry. Additional studies, including experimental infections, are needed to clearly understand the pathogenicity of each virus and the risk of co-infection.

Evaluation of P57, P53 and Ki67 Expression in Meningiomas

  • Objective : We conducted this study with the aim of predicting the biological behavior of meningiomas, and determining the benefits of associating histological subtype and grade with the expression of proliferative markers and tumor suppressor proteins. Methods : The study included 29 patients with primary intracranial and intraspinal meningioma diagnosed in the pathology laboratory of Konya City Hospital between January 2014 and December 2020. Clinicopathological characteristics of the patients including parameters such as age and gender were obtained from the hospital records. Histopathological findings were obtained by re-evaluating the preparations stained with Hematoxylin-Eosin, which were extracted from the archive, and by evaluating new sections obtained from paraffin blocks of patients stained with Ki67, p53, and p57 immunohistochemical stains. Results : A moderate correlation was found between tumor size and Ki67 proliferation index (PI) (p=0.003, r=0.530). There was no significant difference between grade I and grade II tumors in terms of p53 (p=0.184) and p57 (p=0.487) expressions. There were higher levels of Ki67 PI in grade II tumors. The histological subtypes of the tumor had no significant difference with Ki67 PI (p=0.018), p53 (p=0.662), and p57 (p=0.368) expressions. Conclusion : In order to obtain more definitive results, there is a need for studies, which are conducted with a greater number of patients and in multiple centers, and in which a long prospective follow-up is planned. The combination of histological, surgical, and imaging markers could make a more sensitive tool for predicting recurrence, and this could also be tested in future studies.
