Spontaneous Femoral Capital Physeal Fracture with Proximal Tibial Epiphyseal Dysplasia in a Cat

  • An 11-month-old, 5.6 kg, grade 4 on a 5-point scale body condition score, castrated male Korean short hair cat was presented with right hindlimb lameness for 3 days without trauma. On physical examination, severe pain with crepitation was elicited at the hip region but not at the stifle. Radiographic examination and computed tomography revealed a Salter-Harris type 1 fracture of the femoral capital physis and dysplasia of the proximal tibial epiphysis. Spontaneous femoral capital physeal fracture was suspected and femoral head and neck osteotomy was performed. Treatment for proximal tibial dysplasia was not performed. On histopathologic examination of femoral head, a cluster of chondrocytes in a proliferative zone at the epiphyseal plate was observed. Therefore, spontaneous femoral capital physeal fracture was diagnosed. The patient recovered his gait, and no related clinical signs were observed during 6 months of follow-up.

Korean Medical Treatment after Surgical Treatment of Necrotizing Fasciitis on Thigh : A Case Report (대퇴부 괴사성 근막염의 외과적 수술 후 한방치료에 대한 증례 보고)

  • Objectives : The aim of the present study was to report the clinical progress of a patient who underwent surgical treatment of necrotizing fasciitis. Methods : During the admission period, The patient was treated with acupuncture, cupping, Korean Medicine, Chuna therapy every day and observed by Numeric Rating Scale(NRS), Pain Disability Index(PDI), Beck Depression Inventory(BDI), and assessment about range of motion(ROM) and manual muscle test(MMT) for hip & knee joint. Results : After treated by using above treatment, NRS, PDI, BDI and ROM, MMT were improved. Conclusions : Korean Medical treatment are estimated to be good for patients after surgical treatment of necrotizing fasciitis. But more cases are required to prove the effectiveness of the treatment.

Influence of successive active knee extension tests on hamstring flexibility (연속적 능동무릎폄 검사가 뒤넙다리근의 유연성에 미치는 영향)

  • Background: In clinical practice, active knee extension (AKE) test has been widely used to measure hamstring flexibility. During the AKE test, the knee extension is repeated six times. The first five repetitions are considered as warm-up, and the sixth is used as baseline. In order to accurately measure the subject's inherent flexibility, warm-up trials have been proposed as mentioned above; but currently, the evidence is insufficient. Design: Cross-sectional study. Methods: Forty-three healthy adults participated in this study. The AKE was performed slowly after flexing the hip and knee joints by 90° in a supine position. The knee was extended as far as could be done without causing discomfort or pain. When reaching the end range, knee flexion was performed without stopping. As per standard protocol, the subjects performed the knee extension six times. Results: There was no significant difference between the AKE value in the first trial with those in the other five trials. During the repetitions, the average value in the group with higher flexibility tended to decrease and the Intraclass correlation coefficient gradually decreased. Conclusion: Successive repetitions of the AKE test can misrepresent an individual's inherent hamstring flexibility. In order to avoid a decrease in hamstring flexibility, it is strongly recommended that individuals with high initial hamstring flexibility avoid unnecessary repetitions of the AKE test.

Effect of Taping and Pilates Core Stabilization Exercise on Physical Alignment and Improvement of Genu Recurvatum in Women

  • Background: More women than men experience genu recurvatum, which can lead to knee pain and arthritis if left unattended. Pilates combined with taping is a suitable rehabilitation method for women with genu recurvatum. Objectives: To aimed the effect of taping and Pilates stabilization exercise on physical alignment and improvement of genu recurvatum in women with genu recurvatum. Design: A Randomized controlled trial. Methods: Twenty-one women were divided into three groups: taping group (back of knee taping and quadriceps femoris taping, n=7), Pilates exercise group (hip, knee and abdominal muscles exercise, n=7), and combined group (taping with pilates exercise, n=7). The measured variables were sagittal plane alignment and back and abdominal muscle, knee flexor and extensor strength. Results: After the intervention, all sagittal plane alignments were significantly improved in both the Pilates and combined groups. Sagittal plane alignment was significantly improved in the combined group compared with the taping group. Back and abdominal muscle strength were significantly improved in the Pilates and combined groups compared with the taping group. Knee flexor and extensor were significantly improved in the combined group compared with the Pilates group and in the Pilates group compared with the taping group. Conclusion: Pilates exercise with taping or Pilates exercise alone was effective intervention methods to improve physical alignment and strength in women with genu recurvatum.

A Study on the Musculoskeletal Pain Experience of Dental Hygienist's Treatment Postur (치과위생사의 진료자세에 따른 근골격계 통증 경험에 대한 연구)

  • This study was to investigate the muscoulosckeletla pain from 214 dental hygienist during treatment process in Ulsan and Kyungnam. The data collection was done from March to May in 2009 with questionnaire. The study results were as follows. In the participant's age characteristic, under 23 years old(34.1%) was majority, other participant age group were 24~26 years old(33.6%), 25~29 years old(20.6%), and over 30 years old(11.7%). Other participant characteristic were unmarried participant(86.4%), none religion(43.5%), and graduation from college(88.3%). In the participant's business characteristic, working place were dental clinic(57.5%), and dental hospital(42.5%), career was 1~2 years(42.5%) and a number of change job was once(45.1%). Salary were 1,300,000~1,490,000 won (33.6%), 1,500,000~1,990,000 won(29.9%), under 1,300,000 won(26.2%), 2,000,000~2,490,000 won(7.5%). A number of dental hygienist was over 20 person(85.9%) and the average dental hygienist was 15.6 person. A number of dentist were one person(28.5%), 2 person(22.4%), 4 person(19.2%) and the average dentist was 3.2 person. Dental hygienist's working hour was 8~9(49.5%), a large part of dental hygienist was assistance of general treatment(70.1%). Other results were 5 days/week working(60.3%), and night shift(49.1%). In an effect of musculoskeletal pain during working, neck pain was significantly influenced by body posture such as "head forward 15 degree or head round", "non balanced shoulders", "bend waist", "bend wrist", and "sit astraddle on a chair". Shoulders pain was significantly influenced by body posture such as "head forward over 15 degree", "bend waist", and "stand up straightly". Knee pain was significantly influenced by body posture such as "stand up straightly", and hip pain was influenced by "sit astraddle on chair".

Narrative Review of Clinical Trial on Scolopendrid Pharmacopuncture in Korean Literature (국내 데이터 베이스 검색을 통한 오공 약침 시술에 대한 임상 논문고찰)

  • Objectives The purpose of this review is to investigate studies of Scolopendrid pharmacopuncture and the evidence of it's effects in order to suggest a better research method in the future. Methods We retrieved numbers of clinical studies about Scolopendrid pharmacopuncture from 7 Korean web databases, using key words such as 'Scolopendrid', 'Scolopendrid AND Pharmacopuncture'. This study had been conducted from 1st May 2013 to 31th July 2013. Controlled studies and case studies were only used for this study. Clinical studies that we picked from the databases were classified according to the diseases that those studies are about, and from these clinical studies, we are to research what has to be improved generally in clinical researches. Results 18 case studies, 4 controlled studies had been under research. Scolopendrid pharmacopuncture has a therapeutic effect mainly in musculoskeletal and neurological diseases such as herniated intervertebral disc, carpal tunnel syndrome, swollen leg, feeling of cold on legs, wrist ganglion, lateral epicondylitis, radial nerve palsy, cervical myelopathy, cauda equina syndrome, postauricular pain; as an early symptom of Bell's palsy, pain of popliteal part, gout, plantar fasciitis, cellulitis, frozen shoulder, pain of hip adductors. However objectivity and reliability of the Scolopendrid pharmacopuncture studies still remains controversial. Conclusions It has been suggested that there are positive effects of Scolopendrid pharmacopuncture therapy in treating specific diseases (especially neuromusculoskeletal diseases). However, this narrative review can't conclude and prove that the Scolopendrid pharmacopuncture has positive effectiveness on these diseases unlike systematic review. So, in order to put Scolopendrid pharmacopuncture therapy to use for many kinds of diseases in more reasonable ways, it is essential to build well-designed clinical research tools. In the future, abundant case studies, more follow-up trials and randomized controlled trials based on the korean medicine should be done to use Scolopendrid pharmacopuncture for a clinical purpose.

Effects of Applying the Pelvic Compression Belt on the Trunk and Hip Extensor Electromyography Pattern in Female Patients With Sacroiliac Joint Pain During the One-Leg Standing (골반압박벨트 착용이 한발서기 시 여성 천장관절통증 환자의 체간과 고관절 신전근 활성 양상에 미치는 영향)

  • The pelvic compression belt (PCB) contributes to improving sacroiliac joint stability, and it has been used as an additional therapeutic option for patients with sacroiliac joint pain (SIJP). This study aimed to investigate whether the muscle activation patterns of the supporting leg was different between asymptomatic subjects and subjects with SIJP during one-leg standing, and how it changes with the PCB. 15 subjects with SIJP and 10 asymptomatic subjects volunteered to participate in this study. Surface electromyography (EMG) data (reaction time [RT] and muscle activation) were collected from the internal oblique, lumbar multifidius, gluteus maximus and biceps femoris muscles during one-leg standing with and without the PCB. Without the PCB condition, in the SIJP group, the biceps femoris muscle showed the fastest RT among all muscles (p<.05), whereas in the asymptomatic group, the RT of the internal oblique muscle was the most rapid (p<.05). In condition without the PCB, the biceps femoris EMG amplitudes in the SIJP group were significantly greater than that in the asymptomatic group (p<.05). After the application of the PCB, the RT of the biceps femoris muscle was significantly increased only in the SIJP group (p<.05). Moreover, the biceps femoris EMG amplitudes significantly decreased and the gluteus maximus EMG amplitudes significant increased only in the SIJP group by applying the PCB (p<.05). However, this had no such effect on the gluteus maximus and biceps femoris EMG patterns in the asymptomatic group (p>.05). Thus, this study supports the applying the PCB to patients with SIJP can be used as a helpful option to modify the activation patterns of the gluteus maximus and biceps femoris muscle.

The study of difference between HIVD group and simple low back pain group by Current Perception Threshold test (요추추간판탈출증군과 단순요통군간의 Current Perception Threshold상의 차이에 관한 연구)

  • Objective : The aim of this study is to assess the ability of the CPT test to dignosis radiculopathy in Korean with NeuvalTM CPT database. Method : Electrical stimulation(at 2000, 250, and 5Hz) using Neurometer was applied to the 19 patients who felt back and sciatic pain with herniated intervertevral disc(HIVD) of L-spine, dignosisd by lumbar CT or MRI, and the 33 patients who felt only back or hip or leg pain without HIVD, stenosis, spondylolisthesis which causing radiculopathy. The test sites were toe1, toe3 and toe5 related to L4, L5 and S1 nerve roots. Results : The mean values of the CPT of HIVD group was stiatically lower than LBP group at toe1-250Hz, toe3-2000Hz, 250Hz and toe5-250Hz, 5Hz. The grading CPT score of HIVD group was stastically higher than LBP group at the toe3(L5). The VAS of HIVD group was stastically higher than LBP group. Conclusion : These results suggest that the CPT test can be a valuable testing for diagnosing radiculopathy in Korean. In using CPT test further study is needed for the diagnosis and evaluation of sensory nerve dysfunction in the musculoskeletal disease.

Percutaneous Radiofrequency Therapy of Benign Bone Tumors in the Femoral Head (대퇴골두 부위에 발생한 양성 골 종양에 대한 경피적 고주파치료 (증례보고))

  • Purpose: To report two cases of bone tumors other than osteoid osteoma in the proximal femur and treated with percutaneous high frequency radioablation method. Cases: We reviewed two cases with intracortical chondroma and enchondroma in the femoral head retrospectively. The patient with intracortical chondroma was a thirty one year old woman and had suffered right hip pain of 1 year duration. The lesion was located in the head of right femur and treated with CT guided percutaneous high frequency radioablation after needle biopsy under general anesthesia. The symptom was gone immediately after the procedure and was discharged postop. 1 day. 15 months has passed without symptom recurrence. Second case having enchondroma, was 56 year old woman complaining of gluteal area pain for 3 months. Radiologic evaluation showed osteolytic lesion with sclerotic rim on the inferior portion of the left femoral head. She received a same therapy with CT guided radiofrequency ablation following needle biopsy. She reported dramatic pain relief after the procedure and was discharged postop. 1 day. No symptom has occurred for 3 months until now. Conclusion: We present 2 cases of bone tumor occurred in the hip joint area other than osteoid osteoma which were treated with CT guided radiofrequency ablation.

Effects of Height of the Shoe Heel in a Static Posture on Muscle Activity of the Rectus Abdominis, Erector Spinae, Rectus Femoris and Hamstring (구두 굽의 높이가 한다리 서기 자세에서 배곧은근, 척추세움근, 넙다리곧은근, 뒤넙다리근의 근활성도에 미치는 영향)

  • The purpose of this study was to examine the effects of height of the shoe heel in a one leg standing posture on the muscle activity of the rectus abdominis, erector spinae, hamstring, and rectus femoris. Twenty healthy female subjects were asked to perform a one leg standing posture with three types of shoes, which had different heights of shoe heel, 0cm, 3cm, and 7cm. Surface electromyography was used to evaluate the activities of the right rectus abdominis, erector spinae, hamstring, and rectus femoris muscles. The data was analyzed by repeated one-way ANOVA and the muscle activities among three heights of shoe heel were compared. The results showed that the muscle activities of the erector spinae and rectus femoris increased significantly according to increase height of shoe heel (p<0.05). The activity of the rectus abdominis and hamstring were not significantly different among the three height of shoe heel (p>0.05). These results suggest that the height of shoe heel affects the lumbo-pelvic alignment and center of mass; therefore, high-heeled shoes lead to musculoskeletal pain in the lumbopelvic, hip, and knee areas.
