Diagnostic Meaning of High Resolution Computed Tomography Compared with Chest Radiography for Screening of Welder's lung (용접공진폐증 집단검진을 위한 단순 흉부방사선 촬영과 고해상 흉부전산화 단층촬영의 진단적 의의)

  • Pneumoconiosis is one of the major problem in the field of occupational health at Korea. Therefore, the efficient diagnosis of pneumoconiosis is a hot issue on the occupational health program. The author executed this study to estimate the diagnostic value of high resolution computed tomography(HRCT) compared with chest radiography for screening of welder's lung. HRCT was introduced very recently for the diagnosis of pneumoconiosis, however, the diagnostic value for screening of welder's lung - principally nonfibrogenic and reversible - has not been evaluated. The subjects were fifty cases of welder's lung or suspected cases who had been collected between 1989 and 1994 from one shipyard and continuously followed-up on the basis of in-plant periodic health check program. We applied both chest radiography and HRCT on the same subjects from May 1 to 30, 1996. The images were evaluated by two careered radiologists independently. The findings of chest radiography were classified into four category by ILO classification, and the findings of HRCT according to the criteria of Bergin et al. The concordance between two radiologists expressed with Kendall's tau-b was 0.72 by chest radiography and 0.44 by HRCT- that is, interobserver variation of HRCT was bigger than that of chest radiography. The concordance between the two different methods was highly variable as 0.44 by radiologist A and 0.06 by radiologist B - that is, interobserver variation was very big. However, HRCT looked more detectable for the minor parenchymal change. These findings suggested that it is not appropriate to use HRCT routinely for screening of welder's lung due to lack of diagnostic criteria, and feasibility, acceptability and economic aspects. Nevertheless, HRCT might be recommendable in the case of equivocal parenchymal features on the chest radiography, unexplained respiratory symptoms, and/or lung function abnormalities suggestive of interstitial fibrosis.

  • There is no doubt that the national competitiveness, in 21st century, definitely depends on how effectively it has been producing high-qualify human resources. It is inevitable that we are required to produce outstanding people who are going to make the use of highly developed scientific technology. Every country has already set to concentrate their all efforts in cultivating competitive human resources, enabling it to strengthen its national competitiveness. We Korea, in order to keep up with it, have arranged legal and systematic basis and are putting spurs to producing competent human resources under the 영재교육진흥법 및 시행령, which took effect from March, 2002. With the lack of experience and short history of Gifted Education, however, it is true that we still have many problems in promoting it in reality, We are asked to improve it by finding out what problems we have in whole area of Gifted Education, such as defining conception, choosing target students, structuring system and managing students afterwards. Therefore, this study, especially focusing on Math of Gifted Education is to investigate the present situation of Gifted Education and to examine what we should do for administering Gifted Education in effective ways.

  • Recent years have witnessed a strong need for eco-friendly and energy-efficient systems owing to global environmental problems. A heat exchanger is a well-known mechanical rig that has long been used in many energy systems. The use of a heat exchanger in an airplane engine has been attempted. In this case, the heat exchanger should be redesigned to be compact, lightweight, and highly reliable, and the issue of mechanical integrity gains importance. Therefore, in this study, we proposed a method for evaluating the mechanical integrity of a tube-type heat exchanger. A U-shaped single tube was used as an example, and its behavior and stress distribution were studied using fluid-structure interaction (FSI) analysis.

A Study on Velocity Distribution Characteristics for Each Location and Effectiveness of Straight Duct Length in a Square-sectional 180° Bended Duct (정사각형 단면을 갖는 180° 곡관에서 위치별 속도분포특성 및 직관거리의 유효성에 관한 연구)

  • This study numerically analyzes the characteristics of the velocity distribution for each location of a square-sectional $180^{\circ}$ bent duct using a Reynolds Stress Turbulent model. The flow parameters were varied, including the working fluids, inlet velocity, surface roughness, radius of curvature, and hydraulic diameter. The boundary conditions for computational fluid dynamics analysis were inlet temperatures of air and water of 288 K and 293 K, inlet air velocity of 3-15 m/s, inner surface roughness of 0-0.001 mm, radius of curvature of 2.5-4.5 D, and hydraulic diameter of 70-100 mm. The working fluid characteristics were highly affected by changes in the viscous force. The maximum velocity profiles in the bent duct were indicated when the $90^{\circ}$ section was in the region of X/D=0.8 and the $180^{\circ}$ section was in the region of Y/D=0.8. Lower surface roughness and higher radius of curvature resulted in a higher rate of velocity change. Also, an efficient measuring location downstream of the bent duct is suggested since the flow deviations were the most stable when the straight duct length was in the region of L/D=30. The minimum deviations at the same velocity conditions according to the hydraulic diameter were mostly indicated in the range of L/D=15-30 based on the standard deviation characteristics.

  • Objective : p16/INK4a, a kind of tumor suppressor genes, encodes a specific inhibitor of the cyclin D-dependent kinases CDK4 and CDK6. This prevents the association of CDK4 with cyclin D1, and subsequently inhibits phosphorylation of retinoblastoma tumor suppressor protein(pRb), thus preventing exit from the G1 phase. According to previous reports, over 50% of glioma tissue and 80% of glioma cell lines have been demonstrated inactivation of p16/INK4a gene. The purpose of this study was to determine whether recombinant adenovirus-p16 virus is a suitable candidate for gene replacement therapy in cases of glioma. Methods : Three human glioma cell lines(U251MG, U87MG and U373MG) that express mutant p16 protein were used. Replication-deficient adenovirus was utilized as an expression vector to transfer exogenous p16 cDNA into the cells ; control cells were infected with the Ad-${\beta}$-gal expressing ${\beta}$-galactosidase. To monitor gene transfer and the expression of exogenous genes, we used Western Blotting analysis. Flow cytometry studies of cellular DNA content were performed to determine the cell cycle phenotype of the glioma cells before and after treatment. Results : We showed here that restoration of p16/INK4a expression in p16 negative U87MG, U251MG and partially deleted U373MG by Ad-CMV-p16 induced growth suppression in vitro. Flow cytometric study revealed that Ad-CMV-p16 infected U87MG cells were arrested during the G0-G1 phase of the cell cycle. Expression of p16 transferred by Ad-CMV-p16 in glioma cells was highly efficient and maintained for more than seven days. Conclusions : Our results suggest that Ad-CMV-p16 gene therapy strategy is potentially useful and warrants further clinical investigation for the treatment of gliomas.

Study on the Detoxification of Asbestos-Containing Wastes (ACW) Using SiC Plate (SiC 플레이트를 이용한 석면 함유 폐기물의 무해화 연구)

  • Even asbestos-containing waste (ACW) are highly harmful to humans, it continues being produced due to the massive disposal of asbestos-containing products. A development of asbestos detoxification and recycling technologies is required. Heat treatment using microwave is the most efficient method for ACW detoxification. However, microwave heat treatment method has the limitation that asbestos does not absorb microwave at room temperature. That is why, in this study, ACW was detoxified by microwave heat treatment adding the ACW between SiC plates, which are inorganic heating elements that absorb microwaves at room temperature. In order to improove the heat transfer, ACW was crushed and pulverized and then heated using microwave. Microwave heat treatment temperature and time variables were adjusted to investigate the detoxification properties according to heat treatment conditions. After heat treatment, treated ACW was analyzed for detoxification properties through crystal structure and microstructure analysis using X-ray diffraction (XRD) and scanning electron microscopy (SEM). Microwave heat treatment method using SiC plate can be heated up to the target temperature within a short time. Finally, complete asbestos detoxification was confirmed from the crystal structure and the microstructure when the microwave heat treatment was performed at 1,200℃ for at over 60 minutes and at 1,300℃ for at over 10 minutes.

Structured Assessment to Evaluate a Family Medicine Clerkship Program (가정의학 임상실습과정에 대한 구조화된 평가)

  • This study was conducted to analyze the strengths and weaknesses of a 3-week family medicine clerkship program based on the results of an online survey taken by the students (N=127) and a structured interview with a focus group (n=10), aimed to improve the quality of the clerkship program. The online survey contained questions pertaining to goals, schedule, contents, arrangement, atmosphere, environment, evaluation, and satisfaction regarding the clerkship. The focus group interview addressed the schedule and achievements of the program. Scores were reported on a 5-point Likert scale. Most students were highly satisfied with the overall quality of the clerkship. The structured interview results showed that 97.6% of the clerkship program was executed according to the schedule. The focus group reported a perfect score of 5 points on several measures including: accomplishment of the educational goals of the family medicine clerkship, providing many chances to obtain medical histories and perform physical examinations on real patients, experience with various symptoms and diseases, positive attitudes of faculty members when teaching, notification of the guidelines for evaluation beforehand, well-constructed and effective clerkship schedule, and reflection of student feedback. However, the focus group gave low scores on: support for health accidents of students, access to patient information, enough opportunities to practice clinical skills, appropriate rest facilities for students, and fairness of clerkship evaluation process. In conclusion, the structured evaluation performed after the 3-week clerkship program motivated students and helped them ensure an efficient clerkship. This structured evaluation also suggested basic data to make the professor who is subject of the assessment. This study shows that structured assessment is an effective method which can be used to improve the quality of clerkships.

  • Optical characteristics in polymer films of MEH-PPV/DFPP blends were for the first time investigated. DFPP (N, N'-diperfluorophenyl-3,4,9,10-perylenetetracarboxylic diimide) used here was a novel n-type polymer, which had good stability in air and solubility in common solvents. For a 1:9 DFPP:MEH-PPV blend, highly efficient quenching of photoluminescence (PL) was observed. In addition, the photocurrent responses of these MEH-PPV/DFPP photovoltaic cells were measured. When the light intensity was $50mW/cm^2$, short-circuit photocurrent densities were two times higher than those of single layer MEH-PPV devices.

A safe, stable, and convenient three-dimensional device for high Le Fort I osteotomy

  • Background: Le Fort I osteotomy is a highly effective treatment for skeletal jaw deformities and is commonly performed. High Le Fort I osteotomy is a modified surgical procedure performed for improving the depression of the cheeks by setting the osteotomy higher than the conventional Le Fort I osteotomy. Developments in three-dimensional (3D) technology have popularized the use of 3D printers in various institutions, especially in orthognathic surgeries. In this study, we report a safe and inexpensive method of performing a high Le Fort I osteotomy using a novel 3D device and piezosurgery, which prevent tooth root injury without disturbing the operation field for patients with a short midface and long tooth roots. Results: A 17-year-old woman presented with facial asymmetry, mandibular protrusion, a short midface, and long tooth roots. We planned high Le Fort I osteotomy and bilateral sagittal split ramus osteotomy. Prevention of damage to the roots of the teeth and the infraorbital nerve and accurate determination of the posterior osteotomy line were crucial for clinical success. Le Fort I osteotomy using 3D devices has been reported previously but were particularly large in size for this case. Additionally, setting the fixing screw of the device was difficult, because of the risk of damage to the roots of the teeth. Therefore, a different surgical technique, other than the conventional Le Fort I osteotomy and 3D device, was required. The left and right parts of the 3D device were fabricated separately, to prevent any interference in the surgical field. Further, the 3D device was designed to accurately cover the bone surface from the piriform aperture to the infra-zygomatic crest with two fixation points (the anterior nasal spine and the piriform aperture), which ensured stabilization of the 3D device. The device is thin and does not interfere with the surgical field. Safe and accurate surgical performance is possible using this device and piezosurgery. The roots of the teeth and the infraorbital nerve were unharmed during the surgery. Conclusions: This device is considerably smaller than conventional devices and is a simple, low-cost, and efficient method for performing accurate high Le Fort I osteotomy.

Crashworthiness Analysis and Shape Design Optimization of Thin-walled Corrugated Tubes under Axial Impact (축 방향 충격을 받는 박판 파형관의 충돌안전도 해석 및 형상 최적설계)

  • Thin-walled tubes have been widely used as energy absorbing devices because they are light and have high energy-absorption efficiency. However, the downside is that conventional thin-walled tubes usually exhibit an excessive initial peak crushing force (IPCF) and a large fluctuation in the load-displacement curve, and thus lack stability as energy absorbing devices. Corrugated tubes were introduced to reduce IPCF and to increase the stability of collision energy-absorbing devices. Since the performance of corrugated tubes is highly influence by geometry, design optimization methods can be utilized to optimize the performance of corrugated tubes. In this paper, we utilize shape design optimization based on an adaptive surrogate model for crashworthiness analysis. The amplitude and wavelength of the corrugation, as well as curvature changes in the features, are the design variables. A morphing methodology is adopted to perform shape design parameterization. Through numerical examples, we compare optimal design results based on the adaptive surrogate model, with optimal results based on conventional surrogate models, and we show that direct optimal design methods produce more efficient results.
