A Thirteen-week Oral Dose Subchronic Toxicity Study of Isaria sinclairii in Rats

  • Isaria sinelairii (IS) was orally administered at doses of 0, 0.04, 0.2, and 1 g/kg/day over a 13-week period. There were no observed clinical signs or deaths related to treatment in all the groups tested. Therefore, the approximate lethal oral dose of I. sinclairii was considered to be higher than 1 g/kg in rats. Throughout the administration periods, no significant changes in diet consumption, ophthalmologic findings, organ weight, clinical pathology (hematology, clinical chemistry, coagulation, and urinalysis) or gross pathology were detected. Minor changes were found in hematological parameters for the 0.04 g/kg/day and 0.2 g/kg/day IS treated groups (triglyceride reductions of $20.1{\sim}46.6%$ and platelet increases), but all changes were within physiological range. Microscopic examination failed to identify any treatment-related histopathologic changes in the organs of the IS-treated rats other than nuclear enlargement (cellular atypia) of the tubular regions in the medulla of the kidney in the high dose group. From these results, one can conclude that the no-observed effect level (NOAEL) of I. sinclairii is less than 0.04 g/kg/day in rats.

뽕잎 첨가량에 따른 콩 다식의 품질 특성 (A Study on the Quality Characteristics of Soybean Dasik by Addition of Mulberry Leaf)

  • As the modern medicine develops, the physique and average life span are increasing. In proportion to that, modern diseases of adult people such as cancer, obesity, arteriosclerosis, cardiac disorder become great social issues. In the meantime, as the development of a new diet brings a new attention, the study is designed to examine the availability of functional food by adding Korean traditional food, soybean Dasik, mulberry leaf. For this study, mulberry leaf was added 0, 5, 10, and $15\%$ respectively to soybean Dasik in proportion to the total weight of soybean power. These are compared with the soybean Dasik itself in relation to the nutritional composition, sensory evaluation, and mechanical characteristics. The results of the research were as follows. Nutritional composition showed that mulberry leaf soybean Dasik increased Na, Ca, K, crude fiber, and ash. As for the result of sensory evaluation of mulberry leaf soybean Dasik, the followings were turned out to be good : color - $10\%$, mulberry leaf smell - $5\%,\;10\%,\;15\%$, bitterness - $5\%,\;10\%$, softness and moistness - $0\%,\;5\%,\;10\%$, and sweetness and overall quality - all the added food group. As the test results of mechanical characteristics, mulberry leaf soybean Dasik showed statistically significant difference only in the hardness, especially high on $5\%,\;10\%,\;and\;15\%$. There were no significant differences in other characteristics. In color, mulberry leaf soybean Dasik showed that L, a and b values decreased as the percentage of addition of mulberry leaf went up. Therefore, the appropriate amount of addition for the production of mulberry leaf soybean Dasik was $5\~10\%$ mulberry leaf in proportion to the weight of soybean power.

사상체질병증 임상진료지침: 예방 및 위험인자 (Clinical Practice Guideline for Sasang Constitutional Medicine: Prevention and Risk Fators of Sasangin Disease)

  • Objectives This research was proposed to present Clinical Practice Guideline (CPG) for Prevention of Sasangin disease pattern of Sasang Constitutional Medicine (SCM) and diseases closely related with Sasang constitution. Each CPG was developed by the national-wide experts committee consisting of SCM professors. Methods At first, we searched the literatures related to SCM such as "Dongeuisusebowon", Textbook of SCM and Clinical Guidebook of SCM. Also we searched the articles related to the studies about risk factors for Sasangin disease pattern of both at home and abroad. Finally, we selected leading risk factors of Sasangin disease pattern and developed CPG for prevention of Sasangin disease pattern of SCM. And then, we searched the literatures related SCM such as "Dongeuisusebowon" and the articles on the correlation between disease and Sasang constitution using case-control studies, observational studies or cross sectional studies of both at home and abroad. Next, we selected diseases closely related with Sasang constitution on the basis of articles including prevalence rate and odds ratio between disease and Sasang constitution and finally developed CPG for these diseases. Results and Conclusions We categorized risk factors of Sasang disease pattern into 2 types: non-modifiable and potentially modifiable. 3 items (age, sex and genetic factors) were classified as non-modifiable risk factors of Sasang disease pattern. 6 items (original symptom, stress, diet and nutrition, physical activity, alcohol and drug misuse) were classified as less well-documented or potentially modifiable risk factors of Sasangin disease pattern. We found out Sasang constitution is more likely to develop some diseases. It was proven that Sasang constitution increase the risk of hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. And there is high probability of Sasang constitution being potential risk factor for obesity, hyperlipidemia, allergy and cancer. Also, we found out Taeeumin is independent risk factor for hypertension, diabetes mellitus, metabolic syndrome, stroke, nonalcoholic fatty liver and obstructive sleep apnea. Therefore we recommend that Taeeumin need to prevent these disease by regular checkups and aggressive management.

Short-Term Outcomes of Intracorporeal Delta-Shaped Gastroduodenostomy Versus Extracorporeal Gastroduodenostomy after Laparoscopic Distal Gastrectomy for Gastric Cancer

  • Background: Billroth I anastomosis is one of the most common reconstruction methods after distal gastrectomy for gastric cancer. Intracorporeal Billroth I (ICBI) anastomosis and extracorporeal Billroth I (ECBI) anastomosis are widely used in laparoscopic surgery. Here we compared ICBI and ECBI outcomes at a major gastric cancer center. Methods: We retrospectively analyzed data from 2,284 gastric cancer patients who underwent laparoscopic distal gastrectomy between 2009 and 2017. We divided the subjects into ECBI (n=1,681) and ICBI (n=603) groups, compared the patients' clinical characteristics and surgical and short-term outcomes, and performed risk factor analyses of postoperative complication development. Results: The ICBI group experienced shorter operation times, less blood loss, and shorter hospital stays than the ECBI group. There were no clinically significant intergroup differences in diet initiation. Changes in white blood cell counts and C-reactive protein levels were similar between groups. Grade II-IV surgical complication rates were 2.7% and 4.0% in the ECBI and ICBI groups, respectively, with no significant intergroup differences. Male sex and a body mass index (BMI) ${\geq}30$ were independent risk factors for surgical complication development. In the ECBI group, patients with a BMI ${\geq}30$ experienced a significantly higher surgical complication rate than those with a lower BMI, while no such difference was observed in the ICBI group. Conclusion: The surgical safety of ICBI was similar to that of ECBI. Although the chosen anastomotic technique was not a risk factor for surgical complications, ECBI was more vulnerable to surgical complications than ICBI in patients with a high BMI (${\geq}30$).

Relationship between metabolic syndrome and intake of ultra-processed foods in Korean adults: based on 6th and 7th Korea National Health and Nutrition Examination Survey (2013-2018)

  • BACKGROUND/OBJECTIVES: Metabolic syndrome is closely associated with lifestyle factors, including diet and nutritional intake. Modern trends show a shift in food consumption from healthy home-cooked meals to processed and instant foods. Therefore, this study analyzed the association between ultra-processed food intake and the development of metabolic syndrome in Korean adults based on the data from the Korea National Health and Nutrition Examination Survey (KNHANES) 2013-2018. SUBJECTS/METHODS: The subjects of this study were 17,414 adults aged 19-80 years who participated in the 6th-7th KNHANES. Processed food was classified into four categories, NOVA1 to NOVA4, using 24-h recall data. The higher the NOVA category, the more processed the food. Statistical analysis was conducted using logistic regression to investigate the prevalence of metabolic syndrome according to the consumption of ultra-processed foods. RESULTS: Among the diagnostic criteria for metabolic syndrome, hypertension (odds ratio [OR], 0.72; 95% confidence interval [CI], 0.62-0.85; Q4 vs. Q1, P-trend < 0.001) and high triglycerides (OR, 0.83; 95% CI, 0.72-0.94; Q4 vs. Q1, P-trend < 0.001) showed a correlation with the percentages of energy consumed from ultra-processed foods. The OR for metabolic syndrome, according to the percentages of energy consumed from ultra-processed foods, is shown only for men. The OR showed that the percentages of energy consumed from ultra-processed foods were associated with increased metabolic syndrome. CONCLUSIONS: This study suggests that consumption of ultra-processed foods raises the risk of metabolic syndrome especially among men. To prevent metabolic syndrome, it is necessary to prepare appropriate dietary guidelines for Korean adults.

한열변증에 따른 중풍 환자의 제반특성 비교 (The Comparison on the General Characteristics of Acute Stroke Patient between Heat pattern group and Cold pattern group)

  • Object : This study was conducted as part of the national project to standardize stroke diagnosis in Korean medicine. In this study, we aimed to evaluate the characteristics of acute stroke between heat pattern group and cold pattern group. Methods : We recruited stroke patients from 5 universities(Kyung-Hee University oriental medical center, Kyung-Hee University East-West Neo medical center, Kyungwon university lncheon oriental medical center, Kyungwon university Songpa oriental medical center and DongGuk university llsan oriental medical center) from April, 2007 until February, 2010. We diagnosed them and selected 463 heat pattern patients and 182 cold pattern patients. Results : We find that the risk factor of smoking, alcohol, diet(prefer to meat) are more associated with the heat pattern group. On the other hand, inflammation history in recent 3 months and diet(prefer to sea food) are more associated with the cold pattern group. RBC, Hg, Hct, TG, CK and Cl are more related to the heat pattern group. CPT, total cholesterol and HDL are more related to the cold pattern group. Tae-eum type takes high distribution of the heat pattern group, and So-eum type takes high distribution of the cold pattern group. Conclusions : This study provides evidence that the heat pattern group have more risk factor than the cold pattern group.

고추씨 에탄올 추출물이 콜레스테롤 흡착능 및 UDP-glucuronyl transferase 활성에 미치는 영향 (Effects of Ethanol Extracts from Red Pepper (Capsicum annuum L.) Seeds on Cholesterol Adsorption Capacity and UDP-Glucuronyl Transferase Activity)

  • 본 연구에서는 고추씨 에탄올추출물이 콜레스테롤 흡착능 및 UDP-glucuronyl transferase 활성에 미치는 영향을 관찰하였다. In vitro 콜레스테롤 흡착능에서는 대조군에 비해 고추씨 에탄올추출물 모든 농도에서 유의적으로 증가하였다. In vivo에서 실험동물은 체중 $100{\pm}10$ g 내외의 Sprague-Dawley계 수컷 흰쥐를 사용하였으며, 실험군은 정상식이를 공급한 정상군과 고지방 고콜레스테롤 식이 실험군으로 나누고 고지방 고콜레스테롤 실험군을 다시 고추씨 에탄올추출물을 공급하지 않은 군(HF groups), 0.1% 공급한 군(HEA group), 0.2%을 공급한 군(HEB group) 및 0.5%을 공급한 군(HEC group), 5군으로 나누고 각 군당 8마리씩 3주간 사육하였다. 체중증가량은 정상군에 비해 HF 군에서 유의적으로 증가하였고 고추씨 에탄올추출물을 공급한 모든 군에서 HF 군에 감소하였으나 유의적인 차이는 없었다. 식이효율은 고추씨 에탄올추출물을 각각 0.1%, 0.2% 및 0.5% 공급한 군에서는 HF군에 비해 유의적이지는 않았지만 감소하는 경향을 보였다. 간조직의 총콜레스테롤 및 중성지질 함량은 정상군에 비해 HF 군에서 유의적으로 증가하였고 HF 군에 비해 HEA, HEB 및 HEC군에서는 유의적이지는 않았지만 다소 감소하는 경향을 나타내었다. UDP-glucuronyl transferase 활성에서는 정상군에 비해 HF 군에서 증가되었고, 고추씨 에탄올추출물 공급군인 HEC 군에서는 HF 군에 비해 유의적으로 증가되어 가장 높은 활성을 나타내었다. 혈청중의 총콜레스테롤 및 중성지방 농도는 정상군에 비해 HF 군에서 유의적으로 증가하였고, HEA, HEB 및 HEC 군에서 HF 군에 비해 감소하였다. HDL-콜레스테롤은 정상군에 비해 HF 군에서 유의적으로 감소하였으나 HEA, HEB 및 HEC 군에서는 HF 군에 비해 정상군 수준으로 증가하는 경향을 보였다. 반면, LDL-콜레스테롤은 HF 군에서 정상군에 비해 유의적으로 증가하였으나 HEA, HEB 및 HEC 군에서는 모두 유의적 감소를 나타내었다. 대변중의 총콜레스테롤 함량은 정상군에 비해 HF 군에서 유의적으로 증가하였고 HF 군에 비해 특히 고추씨 에탄올추출물 공급군 HEB 군 및 HEC 군에서 증가하는 경향을 나타내었으나 유의적인 차이는 없었다. 중성지질 함량은 정상군에 비해 HF 군에서는 유의적이지는 않았지만 증가되었고, 고추씨 에탄올추출물을 공급한 HEB군 및 HEC 군에서 HF 군에 비해 다소 증가하는 경향을 나타내었다. 따라서 고추씨 에탄올 추출물은 콜레스테롤 흡착능에서 효과적으로 작용하였으며 고지방 고콜레스테롤 식이 흰쥐의 UDP-glucuronyl transferase 활성 및 지질대사에 효과적으로 작용하였음을 규명하였다.

뽕나무발효톱밥을 이용한 흰점박이꽃무지(딱정벌레목: 꽃무지과) 유충 사육에서 적정 사육밀도 및 저온처리 조건 (Optimal Larval Density and Low Temperature Storage Conditions for Rearing of Protaetia brevitarsis (Coleoptera: Cetoniidae) using a Fermented Mulberry Sawdust-base Diet)

  • 흰점박이꽃무지(Protaetia brevitarsis seulensis) 유충을 사육상자(가로 543 × 세로 363 × 높이 188 mm) 안에서 뽕나무 발효톱밥 기반 사료를 이용하여 집단으로 사육할 때(25℃, 16:8 L:D), 투입될 수 있는 갓 부화한 유충의 밀도를 검토하였다. 100-175마리 투입구는 90일후 생존율이 80% 이상으로 사육밀도 사이에 차이가 없었다. 그러나 200마리 밀도에서는 생존율이 약 10% 이상 감소하였다. 100-150마리 밀도에서 유충 평균 무게는 처리 밀도 사이에 유의한 차이가 없었으나, 175마리와 200마리 밀도에서는 100마리 밀도에 비해 유충 무게가 유의하게 낮았다. 사육밀도가 낮을수록 유충 무게가 더 빨리 증가하였다. 이상의 결과를 종합하여 100-150마리가 처음 투입밀도로 적절하다고 판단하였다. 두 번째 실험으로, 종령 유충을 저온에 보관하는 조건을 검토하였다. 종령 유충을 4개의 무게 집단(1.8-2.0, 2.0-2.3, 2.3-2.5, 2.5 g 이상)으로 구분하여 4, 8, 10℃에 보관하였을 때, 4℃에서는 70일 보관된 유충들은 무게에 관계없이 모두 사망하였다. 그러나 8℃에서는 70일 보관집단에서 약 80%의 유충이 생존하였다. 보관하는 유충의 무게가 무거울수록 생존율이 높았다. 4℃ 보관에서는 무게가 2.3 g 이상인 유충을 50일까지 보관한 후 우화한 성충들만 적은 수의 산란을 하였다. 8℃와 10℃ 처리에서는 유충 무게에 관계 없이 모두 산란을 하였다. 이상의 결과로부터 무게가 2.3 g 이상인 흰점박이꽃무지 종령 유충을 8℃에서 30-50일 범위에서 저장할 수 있다고 판단하였다.

어류병원성 연쇄구균의 생장을 억제하는 Bacillus pumilus CPB-St의 생물학적 특성 (Biological Characters of Bacillus pumilus CPB-St Inhibiting the Growth of Fish Pathogenic Streptococci)

  • 어류병원성 연쇄상구균에 대하여 항 미생물 효과를 갖는 것으로 보고된 Bacillus sp. CPB-St를 수산용 probiotics로 개발하기 위한 기초 조사로서 미생물의 생물학적인 특성을 알아보았다. Bacillus sp. CPB-St는 16S rRNA 유전자 염기서열에 근거하여 Bacillus pumilus로 동정되었으며 B. pumilis CPB-St (이하 CPB-St)로 명명되었다. CPB-St를 배양 온도 및 배지의 종류에 따라 Streptococcus sp.에 대한 생장억제능력을 평가한 결과, $20^{\circ}C$(억제대: 11~29 mm)에서 배양하였을 때가 $30^{\circ}C$ (12~21 mm)에서 배양했을 때보다 생장억제효과가 높았고, NA (29 mm)와 TSA (29 mm)에서는 유사하였으나 BHIA (22 mm)에서는 낮게 나타났다. 또한 생장억제대의 크기는 pH 7.0에서 18 mm로 가장 높게 나타났고, 염분농도 0.5~3%에서는 동일하였다. 한편 CPB-St 의 생물학적 특성을 알아본 결과 $30^{\circ}C$에서 완만한 증식을 보였으며 배양 후 48시간에 최고 생장을 나타내었다. pH 4.0로 조정한 인공위액에서는 5시간까지 약간의 감소만 나타내었다. 넙치에서 채취 한 담즙에서 CPB-St는 농도에 관계없이 동일한 성장을 보였으며 deoxycholic acid가 첨가된 TSA배지에서 $300{\mu}g/ml$의 농도까지 증식 가능하였다. 또한 1%의 염분농도에서 최적의 성장을 보였으며 7%의 농도까지 증식에 큰 차이를 보이지는 않았고 저온 내성의 경우 $-20^{\circ}C$$-70^{\circ}C$에서는 45분경과 시점부터 대조군에 비해 성장 감소가 나타났으나 $4^{\circ}C$에서는 실온과 차이를 보이지 않았다. 따라서 CPB-St는 어류의 소화관 내에서의 생존가능성이 높으며 사료에 첨가하여 저온에 보관하여도 균수가 감소되지 않을 것으로 판단된다.

서방성 제제의 생물학적동등성시험을 위한 가이드라인 (Guideline for Bioequivalence Studies of Controlled Release Products)

  • The "Guidance Document for Bioequivalence Study" was revised for adding to bioequivalence studies of controlled-release products after meal(Korea Food & Drug Administration Notification #2008-22, 2008.5.7). The bioequivalence study design for controlled-release products is $2{\times}2$ crossover under fast and fed condition in respect. For studies of controlled-release products under fed study, the same high-fat diet should be taken within 20 minutes in at least a 10-hour fasting state. The drug products should be administered 30 minutes after the meal started. A high-fat(more than 35 percent of total caloric content of the meal) and high-calorie(over 900 calories) meal is recommended as a test meal for fed BE studies.
