Use of an Ultrasonic Osteotome for Direct Removal of Beak-Type Ossification of Posterior Longitudinal Ligament in the Thoracic Spine

  • Direct removal of beak-type ossification of posterior longitudinal ligament at thoracic spine (T-OPLL) is a challenging surgical technique due to the potential risk of neural injury. Slipping off the cutting surface of a high-speed drill may result in entrapment in neural structures, leading to serious complications. Removal of T-OPLL with an ultrasonic osteotome, utilizing back and forth micro-motion of a blade rather than rotatory-motion of drill, may reduce such complications. We have applied the ultrasonic osteotome for posterior circumferential decompression of T-OPLL for three consecutive patients with beak-type OPLL and have described the surgical techniques and patient outcomes. The preoperative chief complaint was gait disturbance in all patients. Japanese orthopedic association scores (JOA) was used for functional assessment. Scores measured 2/11, 5/11, 2/11, and 4/11 for each patient. The ventral T-OPLL mass was exposed after posterior midline approach, laminotomy and transeversectomy. The T-OPLL mass was directly removed with an ultrasonic osteotome and instrumented segmental fixation was performed. The surgeries were uneventful. Detailed surgical techniques were presented. Gait disturbance was improved in all patients. Dural tear occurred in one patient without squeal. Postoperative JOA was 6/11, 10/11, 8/11, and 8/11 (recovery rate; 44%, 83%, 67%, and 43%) respectively at 18, 18, 10, and 1 months postoperative. T-OPLL was completely removed in all patients as confirmed with computed tomography scan. We hope that surgical difficulties in direct removal of T-OPLL might be reduced by utilizing ultrasonic osteotome.

  • A hologram scanner for POS bar code symb9ol readers has been developed. This system is composed of scanning optics, optical detector, video signal circuitary and preprocessor. In contrast to conventional scanners using polygonal mirrors, which complicate the scanning optics, the hologram scanner developed in this research was made up with simple optics and higher reading performance was achieved. And in order to read abar code symbol omnidirectionally with highdensity scan patterns, the new real time decoding technique was proposed. The advantage of this technique is less hardware and lower clock rate. High speed processing and improved readability for tilted symbol was confirmed experimentally.

  • 목적: To evaluate the feasibility of the event-related functional MR study using power grip studying the hand motor system 대상 및 방법: Event-related functional MRI was performed on a 1.5T MR unit in seven norm volunteers (man=7, right-handedness=2, left-handedness=5, mean age: 25 years). A single-shot GRE-EPI sequence (TR/TE/flip angle: 1000ms/40ms/90, FOV = 240 mm matrix= 64$\times$64, slice thickness/gap = 5mm/0mm, 7 true axial slices) was used for functiona MR images. A flow-sensitive conventional gradient echo sequence (TR/TE/flip angl 50ms/4ms/60) was used for high-resolution anatomical images. To minimize the gross hea motion, neck-holders (MJ-200, USA) were used. A series of MR images were obtained in axial planes covering motor areas. To exclude motion-corrupted images, all MR images wer surveyed in a movie procedure and evaluated using the estimation of center of mass of ima signal intensities. Power grip task consisted of the powerful grip of all right fingers and hand movement ta used very fast right finger tapping at a speed of 3 per 1 second. All tasks were visual-guid by LCD projector (SHARP, Japan). Two tasks consisted of 134 phases including 7 activatio and 8 rest periods. Active stimulations were performed during 2 seconds and rest period were 15 seconds and total scan time per one task was 2 min 14 sec. Statistical maps we obtained using cross-correlation method. Reference vector was time-shifted by 4 seconds an Gaussian convolution with a FWHM of 4 seconds was applied to it. The threshold in p val for the activation sites was set to be 0.001. All mapping procedures were peformed usin homemade program an IDL (Research Systems Inc., USA) platform. We evaluated the activation patterns of the motor system of power grip compared to hand movement in t event-related functional MRI.

  • Defects of corrugated board were limiting factors to the corrugated industry's growth. On-line detecting systems of defects are beneficial to increase profits of the corrugated board companies by reducing base paper consumption, increasing process reliability, and increasing productivity. By replacing naked eye inspection of defects to a defects detecting system, continuous inspection without fatigue may guarantee final products quality. The system was developed, which was consisted of line scan CCD camera, lens, illuminating parts, high speed image processor, software, various input parts, and output parts. First installation location of the system was at before fluting process after base paper unwinding, and surface of liner board was inspected by the system. Second installation location was after fluting process and combining process for liner board. Production loss includes ragged appearance of edge, irregular flute shape, wrong combination of flute, score cracking, defects in base paper, and flute cracking. The production loss was analyzed before and after the system installation at a commercial domestic mill. The production loss ratio was defined as a production loss weight per total production weight. The production loss ratio before the installation was decreased a lot from 1.28%(during 3 months before installation) to 0.76%(during 12 months after the first installation) and to 0.25%(during 6 months after the second installation).

The change of ankle of plantar pressure and range of motion joint according to treadmill gradients (트레드밀보행 시 경사도에 따른 족저압과 발목관절의 관절가동범위의 변화)

  • Purpose : The purpose of this study was to investigate the change of the peak plantar pressure distribution under the foot areas and the range of motion (ROM) of ankle joint according to gradients in treadmill gait. Method : Thirty normal subjects (15 male and 15 female) walked on treadmill at three gradient conditions ($0^{\circ}$, $10^{\circ}$, and $15^{\circ}$) in normal speed. The ankle ROM was measured using the CMS70P that is three dimensional analyzer for excursion of ankle ROM, plantar flexion, and dorsi flexion. The peak plantar pressure distribution under the hallux, 1st metatarsal head (MTH) and heel was measured using the F -Scan system with an in-shoe sensor. Data was collected from 9 steps of left sife foot in at each gradient condition while all subjects walked. Result : As the treadmill gradient increased, the excursion of ankle joint was significantly increased (p<.05). Also, plantar flexion and dorsi flexion was significantly increased according to treadmill gradients (p<.05). The peak plantar pressure under the 1st MTH was significantly increased (p<.05) and the peak plantar pressure under the heel was significantly decreased (p<.05) as the treadmill gradient increased. No significant different in the peak plantar pressure under the hallux was observed. Conclusion : This study suggests that physical therapy for patients who have limited ankle ROM should be considered sufficient range of motion for functional ambulation. And individuals that have painful forefoot syndromes, including metatarsalgia, hallux valgus, and plantar ulceration should be careful in walking to uphill, as there is high plantar pressure under the forefoot.

Dual Sensing with Voltage Shifting Scheme for High Sensitivity Touch Screen Detection (고감도 터치스크린 감지를 위한 양방향 센싱과 전압쉬프팅을 이용한 센싱 기법)

  • This paper proposes a new touch screen sensing method that improves the drawback of conventional single-line sensing methods for mutual capacitance touch screen panels (TSPs). It introduces a dual sensing and voltage shifting method, which reduces the ambient noise effectively and enhances the touch signal strength. The dual sensing scheme reduces the detection time by doubling the integration speed using both edges of excitation pulse signals. The voltage shifting method enhances the signal-to-noise ratio (SNR) by increasing the voltage range of integrations, and maximizing the ADC's input dynamic range. Simulation and experimental results using a commercial 23" large touch screen show an SNR performance of 43dB and a scan rate 2 times faster than conventional schemes - key properties suited for a large touch screen panels. We implemented the proposed method into a TSP controller chip using Magnachip's CMOS 0.18um process.

Development of distance sensor module with object tracking function using radial arrangement of phototransistor for educational robot (교육용 로봇을 위한 포토트랜지스터의 방사형 배열을 이용한 물체추적기능을 갖는 거리 센서 모듈 개발)

  • Radial distance sensors are widely used for surveying and autonomous navigation. It is necessary to train the operation principle of these sensors and how to apply them. Although commercialization of radial distance sensor continues to be cost-effective through lower performance, but it is still expensive for educational purposes. In this paper, we propose a distance sensor module with object tracking using radial array of low cost phototransistor which can be used for educational robot. The proposed method is able to detect the position of a fast moving object immediately by arranging the phototransistor in the range of 180 degrees and improve the sensing angle range and track the object by the sensor rotation using the servo motor. The scan speed of the proposed sensor is 50~200 times faster than the commercial distance sensor, thus it can be applied to a high performance educational mobile robot with 1ms control loop.

  • In the underwater image, it is not clear to distinguish the shape of the target due to underwater noise and low resolution. In addition, as an input of deep learning, underwater images require pre-processing and segmentation must be preceded. Even after pre-processing, the target is not clear, and the performance of detection and identification by deep learning may not be high. Therefore, it is necessary to distinguish and clarify the target. In this study, the importance of target shadows is confirmed in underwater images, object detection and target area acquisition by shadows, and data containing only the shape of targets and shadows without underwater background are generated. We present the process of converting the shadow image into a 3-mode image in which the target is white, the shadow is black, and the background is gray. Through this, it is possible to provide an image that is clearly pre-processed and easily discriminated as an input of deep learning. In addition, if the image processing code using Open Source Computer Vision (OpenCV)Library was used for processing, the processing speed was also suitable for real-time processing.

  • Laser-generated ultrasonic inspection system is a non-contact scanning inspection device with high spatial resolution and wide bandwidth. The amplitude of laser-generated ultrasound is varied according to the energy of pulse laser and the surface conditions of an object where the CW measuring laser beam is pointing. In this paper, we correct the generating errors by measuring the energy of pulse laser beam and correct the measuring errors by extracting the gain information of laser interferometer at each time. h dynamic stabilizer is developed to stably scan on the surface of an object for an laser-generated ultrasonic inspection system. The developed system generates ultrasound after adaptively finding the maximum gain time of an laser interferometer and processes the signal in real time after digitization with high speed. In this paper, we describe hardware configuration and control algorithm to build a stable laser-generated ultrasonic inspection system. Also, we confirmed through experiments that the proposed correction method for the generating errors and measuring errors is effective to improve the performance of a system.

  • The major advantage of slip-ring technology in Spiral CT is that it facilitates continuous rotation of the x-ray tube, so that volume data can be acquired from a patient quickly. Not only for such a fast scan, but also for the dose reduction purpose, high signal-to-noise ratio and fast data acquisition system is required. In this study, we have built a multi-channel photodetector and multi-channel data acquisition system for CT application. The detector module consisted of CdWO4 crystal and Si photodiode in 16 channels. For the performance test of the preamplifier stage, both the transimpedance and switched integrator types are optimized for the photodetector modules. Switched integrator showed better noise performance in the limited bandwidth which is suitable for the current CT application. The control sequence for data acquisition and 20 bit ADC is designed with VHDL(Very High Speed Integrated Circuit Hardware Description Language) and implemented on FPGA(Field Programmable Gate Array) chip. Our Si photodiode detector module coupled to CdWO4 crystal showed comparable signal with other commercially available photodiode for CT. Switched integrator type showed higher SNR but narrower bandwidth compared to transimpedance preamplifier. Digital hardware is designed by FPGA, so that the control signal could be redesigned without hardware alteration.
