A Clinical Case Study of Low Back Pain Caused by the Herniated Lumbar Disc in Soeumin patient (요추(腰椎) 추간판(椎間板) 탈출증(脫出症)으로 인하여 발생한 소음인(少陰人) 요각통(腰脚痛) 치험 1례)

  • 1. Objectives The purpose of this study is to give an account of the effects of Korean herbal medicine therapy which is based on the Sasang Constitutional Medicine for the patient who has suffered from low back pain caused by the Herniated Lumbar Disc. 2. Methods Consulting her sungjeong(性情), Chehyungkisang(體形氣像) and QSCCII, we have diagnosed her as Soeumin(少陰人), and have prescribed Hyangbujapalmul-tang(香附子八物湯). 3. Results After the treatment, she felt her symptoms, low back pain, headache, and general condition, improved. 4. Conclusions This paper describes the process of management and changes of symptoms. We hope this paper useful to rehabilitatable treatment for low back pain caused by the Herniated Lumbar Disc through Sasang Constitutional Medicine.

Case Report on 4 Patients with Lumbar Disc Herniation Treated with Concurrent Embedding Therapy under Conventional Korean Medical Treatments (매선요법을 병행한 요추 추간판 탈출증 환자 치험 4례)

  • Embedding Therapy keep the stimulation to acupoint. Recently, Embedding Therapy is used for various disease and also studied in many different fields especially in China. In Korea, Embedding Therapy is studied and used for dermatology, face-lift, obesity, facial palsy most frequently. Chinese studies shows us Embedding Therapy could help musculoskeletal system disorder, lumbar herniated intervertebral disc, sprain, spinal canal stenosis. In Korea study concerning lumbar herniated intervertebral disc treatment used Embedding Therapy is few. This study's purpose is to ensure effect of Embedding Therapy at lumbar herniated intervertebral disc treatment. Four lumbar disc herniation patients had been treated with Embedding Therapy were described in this article. The patients(n=4) had been suffering from chronic lower back and leg pain that continued for more than 6 months. All patients had been diagnosed as lumbar disc herniation by MRI or CT, and treated with Embedding Therapy at least 2 times(7 days interval). 2 patients had been treated with concurrent treatment of acupuncture therapy, and the other 2 patients with concurrent treatment of acupuncture and herbal therapy. We observed 4 patients for more than 2 weeks, and their symptoms and quality of daily life had been improved. This suggests that lumbar disc herniation can be successfully recovered with Concurrent Embedding Therapy under conventional Korean Medical Treatment without side effects.

Percutaneous Discectomy of Herniated Intervertebral Disc with a Dekompressor(R) (요추 추간판 탈출증에서 Dekompressor(R)를 이용한 경피적 수핵 감압술)

  • Background: Radicular pain, associated with herniated intervertebral disc, has been treated with either conservative treatments or a traditional surgical discectomy. Several modalities for minimally invasive percutaneous procedures have been developed as alternatives to a surgical discectomy. Percutaneous decompression using a $Dekompressor^{(R)}$ probe has been recently introduced. Herein, we report the outcome of percutaneous decompression with a $Dekompressor^{(R)}$ for the treatment of a herniated intervertebral disc. Methods: Between August 2004 and April 2005, percutaneous decompression was performed using a $Dekompressor^{(R)}$, 1.5-mm percutaneous lumbar discectomy probe in 17 patients with a herniated lumbar intervertebral disc resistant to conservative treatments, with the results reviewed retrospectively. The procedure was performed under fluoroscopic guidance after local anesthesia. Disc access was gained with a posterolateral approach on the symptomatic side and intradiscal placement of the discectomy probe in the herniated disc confirmed from the anteroposterior and lateral views on the fluoroscopy. Results: We obtained satisfactory clinical results in 14 patients with a decrease in the initial Visual analogue scale (VAS) of more than 55% and the elimination or reduction of analgesic medication, with a follow-up of 3 to 11 months. Conclusion: We concluded that a percutaneous discectomy with a $Dekompressor^{(R)}$ probe might be an effective alternative for the treatments of painful disc herniations resistant to conservative managements when performed under proper selection criteria.

The Effect of Thread Embedding Acupuncture on Lumbar Herniated Intervertebral Disc Patients : A Retrospective Study (요추 추간판 탈출증 환자의 매선요법 병용치료 효과에 대한 후향적 연구)

  • Objectives : This study was conducted to examine the clinical effects of thread embedding acupuncture on lumbar herniated intervertebral disc patients. Methods : This is a retrospective study based on clinical charts. Despite the treatments of acupuncture, pharmacopuncture, herbal medicine, and physical therapy during admission period, 10 lumbar herniated intervertebral disc patients who had shown few improvements in their symptoms were selected as subjects. Thread embedding acupuncture was conducted to reduce patient pain. Patient progress was categorized into three periods based on when the embedding procedure performed. Verbal numeric rating scale (VNRS), Oswestry disability index (ODI), and a 5 step evaluation scale for satisfaction were used to analyze the outcome. Results : Regarding comparison of VNRS improvement rate, both 3 and 6 days after embedding progress showed significant difference compared to before embedding progress. ODI improvement rate showed significant difference only 6 days after embedding progress. Patients' satisfaction with treatment had increased by several degrees. Conclusion : Thread embedding acupuncture for lumbar herniated intervertebral disc patients may be effective in reducing pain and improving their quality of life.

The comparative study with administrative and clinical standard of conservative treatment on herniated intervertebral lumbar disc patients (요추 추간판 탈출증 환자에 대한 보존적 치료의 행정기준과 임상기준에 관한 비교 연구)

  • The purpose of this study is comparative with administrative and clinical standard of conservative treatment on herniated intervertebral lumbar disc patients. The persons who diagnosed to herniated intervertebral lumbar disc were attended in this study. The number of cases were 120 cases. We evaluated their chart, X-ray and MRI. The result of the group 1 study(60 cases) were as follows; 1. The patients who treated for 41days were improved 100%, which was 3cases(5%). 2. The patients who treated for 45days were improved 90%, which was 13cases(22%). 3. The patients who treated for 43days were improved 80%, which was 28cases(47%). 4. The patients who treated for 39days were improved 70%, which was 6cases(10%). 5. The patients who treated for 28days were improved 60%, which was 5cases(28%). 6. The patients who treated for 22days were improved 50%, which was 4cases(7%). 7. The patient who treated for 28days were improved 40%, which was 1case(2%). The result of the group 2 study(60 cases) were as follows; 1. The patients who treated by administrative standard were improved 40%, which was 8cases(13%). 2. The patients who treated by administrative standard wereimproved 30%, which was 32cases(53%). 3. The patients who treated by administrative standard wereimproved 20%, which was 7cases(12%). 4. The patients who treated by administrative standard were improved 10%, which was 9cases(15%). 5. The patients who treated by administrative standard wereimproved 0%, which was 4cases(7%). Conclusion ; In herniated intervertebral lumbar disc patients who had conservative treatment. The highest improve patients were 28 cases(47%), who treated for 43 days in group 1. The lowest improve patient were 4 case(7%), who treated by administrative standard in group 2. The Effective duration of conservative treatment was more than 43 days in group 1. Group 1 which applied to clincal standard was much batter than group 2 which applied to administrative standard.

The Effects of Korean Medical Treatment Combined with Acupotomy on Patients with a Herniated Intervertebral Disc of the Lumbar Spine: a Retrospective Study (요추 추간판 탈출증에 도침치료를 병행한 한의학적 복합치료 효과에 대한 후향적 연구)

  • Objectives : The purpose of this study was to investigate the clinical effects of Korean medical treatments combined with acupotomy on patients diagnosed with a herniated intervertebral disc of the lumbar spine. Methods : We reviewed the medical records of twenty in patients diagnosed with a herniated intervertebral disc of the lumbar spine. They had received Korean medical treatment combined with acupotomy on tender points in the gluteus minimus muscle and piriformis muscle at Gwangju Korean Medicine Hospital of Dong-Shin University from April, 2014 to December, 2014. To evaluate the efficacy of the treatments, the twenty in patients were asked to complete the Numerical Rating Scale(NRS) and the Oswestry Disability Index(ODI) before acupotomy, the first day after acupotomy, the third day after acupotomy, and the fifth day after acupotomy. Results : 1. NRS significantly decreased from $6.15{\pm}1.27$ to $3.90{\pm}1.45$. 2. ODI significantly decreased from $23.64{\pm}8.76$ to $15.95{\pm}7.35$. Conclusions : Korean medical treatment combined with acupotomy on tender points in the gluteus minimus muscle and piriformis muscle might be effective in reducing pain and improving the life quality of patients with a herniated intervertebral disc of the lumbar spine. We hope that further studies will be done to produce more clinical data and ensure effective application of these results.

Effects of Decompression Therapy for 6 Cases with Lumbar Herniated Disc (감압치료가 요추간판탈출 6(증)례에 미치는 영향)

  • The Purpose of this study was to report magnetic resonance imaging(MRI) changes of six cases of Lumbar herniated disc which was treated with spinal decompression therapy, oriental medical therapy. 'Disc heights' were measured on sagittal view of $T_2$-weighted MRI. Size of the herniated disc was measured by MRI and 'disc herniations index'. The grading system and algorithm for 'disc degeneration' were based on MRI signal intensity, disc structure, distinction between nucleus and anulus, and disc height. Data for disc height, disc herniations index and disc degeneration grade were collected before and after the treatment together with calculation from the MRI. Case studies were showed that decompression therapy for the herniated disc has an positive effect on disc herniations index, disc regeneration, not disc heights. The clinical evidence for the use of decompression in herniated disc remains inconclusive because of limited researches. Further trials, which give attention to these areas, are needed before any firm conclusions may be made.

  • A herniated intervertebral disc is the most common type of soft tissue mass lesion within the lumbar spinal canal. Magnetic resonance imaging (MRI) is a useful tool for the assessment of patients with lower back pain and radiating pain, especially intervertebral disc herniation. MRI findings of intervertebral disc herniation are typical. However, from time to time, despite an apparently classic history and typical MRI findings suggestive of disc herniation, surgical exploration fails to reveal any lesion of an intervertebral disc. Our patient underwent lumbar disc surgery with the preoperative diagnosis of lumbar disc herniation; however, nothing could be found during the surgical procedure, except a swollen nerve root.

  • Lumbar discs are one of the first parts of the musculoskeletal system to age in the human body. Lumbar disc for females start to age at twenty years of age and completely age between fifty and sixty. The number of lumbar herniated intervertebral disc patients are increasing rapidly; however, studies on protective inner wear are insufficient. This study was conducted to develop protective inner wear equipment that retains an aesthetic appreciation of the body. The methods and procedures of this study are as follows. First, foreign and domestic lumbar pads were researched and analyzed by visiting medical shops and internet shopping malls that sell lumbar pads. Second, the survey was conducted on forty patients with disc disease in hospitals located in Seoul and surrounding suburbs. The following are the study results and observations. First, the lumbar pads selling on the market had a variety of designs, textiles, and fabrics. The result of the research was to develop additional inner protective equipment for lumbar herniated intervertebral disc patients. Second, the survey results of the forty patients with disc disease were that housewives were the most vulnerable. All patients had experience purchasing lumbar pads. The prevention of pain and stress were the most selected responses for the motivation to buy lumbar pads. Most individuals said that they used lumbar pads for less than 1 year. Dissatisfaction came from fabric, activity and design. 'Being uncomfortable to wear' was the most common complaint. 'Protection and how easy the clothes were to wear' were the most selected for the necessity of developments. The Inner wear shape was the most selected in shape. Except for six respondents, all favored the lumbar protective inner wear design.

Effectiveness of ShinBaro Pharmacopuncture on Lumbar Spinal Herniated Intervertebral Disc : A Randomized Controlled Trial (요추추간판탈출증에 대한 신바로약침의 효과 : 무작위 대조군 시험)

  • Objective : The aim of this study is to investigate the effectiveness of ShinBaro Pharmacopuncture in the treatment of patients with Lumbar spine Herniated Nucleus Pulposus. Methods : We investigated 20 cases of patients with lumbar disc herniation and devided into two groups. Experimental group were treated with ShinBaro Pharmacopuncture with integrated package treatment and control group were treated the same therapies without ShinBaro Pharmacopuncture. To evaluate the treatment effects of two groups, we used numeric rating scale(NRS), oswestry disability index(ODI). Results : In NRS of lumbar and sciatica pain and ODI, that of experimental group was much more improved than control group, and decrement of NRS of lumbar pain showed statistical significance along with the duration of treatment. Conclusions : ShinBaro Pharmacopuncture was thought to be effective for relieving symptoms of lumbar spine herniated nucleus pulposus, although further study is needed.
