One Case Report of Acute Cholestatic-Hepatitis(Drug Induced Hepatitis) After Taking Herbal-Medicine (한약 투여후 발생한 급성담즙정체성(약제유인성)간염 치험 1례)

  • In western medicine, there are some reports about herbal medicine induced hepatitis, but in oriental medicine, there are few reports about that. We experienced one case of drug acute cholestatic-hepatitis in the treatment of oriental medicine for HNP. We treated the patient with acupuncture, physical therapy and herb medicine. The patient's symptoms improved after two weeks of treatment. In the course of treatment, the patient intermittently complained of general weakness, nausea, yellowish urin, dyspepsia, and abdominal discomfort. We recognized that total bilirubin(7.2mg/dl), direct bilirubin(5.5mg/dl), serum transaminase(AST 360U/L, ALT 354U/L), alkaline phosphatase(16.6 K/A), urobilinogen(++) and bilirubin(++) were elevated. We diagnosed drug induced hepatitis. We stopped giving herb medicine and began giving Saeng gan gunbi-tang and Injin-oryung-san. Saeng gan gunbi-tang and Injin-oryung-san have been used to treat hepatic disease and have been known to have beneficial effects. After 3weeks on medication, the clinical symptoms and liver function improved. So, we report this case to bring more attention to the safety and toxicity of herbal medicine.

Two Cases of Severe Eczema Herpeticum after Herbal Medication in Children with Atopic Dermatitis (아토피 피부염 환아에서 한방 치료 후 발생한 중증 포진상 습진(Eczema Herpeticum) 2례)

  • It has been known that the eczema herpeticum is the one of most severe skin manifestation of external herpetic infection. It is developed by skin infection of Herpes simplex Virus(HSV) that superimposed on children with atopic dermatitis, and this could be fatal. The secondary bacterial infection happens frequently in eczema herpeticum. Staphylococcus aureus is the most frequently isolated aerobic strain. The important point of treatment of eczema herpeticum is the immediate start of systemic antiviral treatment and use of antibiotics to decrease the secondary infection. We report two cases of severe eczema herpeticum after herb medical treatment in atopic dermatitis. They have been improved after systemic acyclovir and antibiotic therapy with burn dressing.

Acupuncture and Herbal Medicine Improved the Quality of life of Obese Women (한방치료를 받은 여성 비만환자의 삶의 질 연구)

  • To examine the difference of health-related quality of life (QoL) of obese patients between before and after treatment of Korean traditional medicine. This study was assessed in 18 obese-women (age 29.72${\pm}$7.38) treated by acupuncture and herbal medicine using SF-36 quality of life questionnaire between January and July 2006, a 36 item instrument yielding 8 dimensions (Social functioning, Role limitation-Physical, Role limitation-Emotion, Mental health, Vitality, Bodily pain, and General health) and a total score. Their weight was measured by Inbody 3.0 (Biospace co. Korea). QoL and body composition was assessed twice at baseline (B) and 4 weeks after treatment. Auricular acupuncture and electrical needle stimulation to abdomen, thigh, and arm fat was used twice a week and complex herb formula was prescribed according to their constitution and symptoms for 4 weeks. Herb medication was taken 2 times a day. Respective variables were analyzed with Wilcoxon signed ranks test and the level set for statistical significance was p <0.05. The mean of the body weight (P=0.000;B64,68${\pm}$6.86, 4 WKs 60.47${\pm}$5.69), fat percentage (P=0.000;B33.14${\pm}$4.86, 4 WKs 30.16${\pm}$5.34), body mass index (P=0.000;B25.18${\pm}$2.44, 4 WKs 23.46${\pm}$2.09) and fat weight (P=0.000;B21.66${\pm}$5.06, 4 WKs 18.41${\pm}$4.57) of some obese patients decreased significantly between before and after treatment for 4 weeks. Physical functioning (P=0.48;B27.06${\pm}$3.17, 4 WKs 28.00${\pm}$1.71), Mental health (P=0.01 ;B18.83${\pm}$5.25, 4 WKs 22.00${\pm}$3.73), Vitality (P=0.028;B13.89${\pm}$3.03, 4 WKs 15.44${\pm}$2.53), Bodily pain (P=.0014;B8.84${\pm}$1.75, 4 WKs 10.15${\pm}$1.78), and the total scores (P=0.001;B104.99${\pm}$12.60, 4 WKs 114.58${\pm}$11.11) of SF-36 were increased significantly after treatments. These data show some differences in QoL and BMI between before and after treatment on obesity and suggest that the treatment with acupuncture and herbal medicine have a positive impact on several domains of QoL of some obese patients. Further randomized clinical trials (RCTs) including follow-up are needed to examine whether acupuncture and herb medicine have a positive effect on QoL of treatment group compared with control group.

Clinic Study on Herb Medication for Patients Who have a Chronic Respiratory Disease from the Cold Sequelae (감모후유증(感冒後遺症)으로 기허형(氣虛型)호흡기 질환을 호소하는 환자의 한약치료에 관한 임상연구)

  • Objectives : In order to confirm a remedial effect and related influence of the clinic treatment by prescribing Samchuljojung-tang(蔘朮調中湯), Bojungikqi-tang(補中益氣湯) and Samso-um(蔘蘇飮) to patients who suffering from a chronic cough and rhinorrhea. Methods : 1. In a clinical observation and analysis method, we have selected the 23 patients who have visited National Oriental Medical Center, date from 2007.8.21 to 2008.7.31 and suffering from a chronic cough and rhinorrhea caused by the common cold sequelae. 2. Separated them into three types(I: cough type, II: rhinorrhea type, III: cough & rhinorrhea type). 3. Observed thoroughly the level of symptom and recorded it for each type with questionnaire survey at the beginning set 4. Prescribed Samchuljojung-tang, Bojungikqi-tang and Samso-um for type I, II, III respectively. 5. Observed the level of symptom and treatment afterwards affect, and recorded movements of intensity level for 3 types with same questionnaires. Results : 1. Type I : The survey turned out to be from 9 patients, initial level recorded 31.89 at the entry diagnosis, and next level that means changing of symptoms, recorded to 12.67 after prescribing herb medication. 2. Type II : From 11 patients, initial level was 32.82 and next level was 13.45. 3. Type III : From 3 patients, initial level was 37.67 and next one was 16.00. Conclusions : It is concluded that there is a significant remedial effect and related influence of the clinic treatment for each type of patients who suffering from a chronic cough and rhinorrhea caused by the common cold sequelae.

The Statistical Study of Dermatoses Outpatients in Oriental Hospitals Located in Gwangwon Province (강원지역 소재 한방병원 피부과 외래환자의 임상적 분석)

  • Objective : The purpose of this study is to expand our treatment field in oriental dermatology by understanding the patterns of outpatients who had visited the oriental hospitals located in Gangwon province. Methods : We studied 510 outpatients out of 2240 people from Sangji University Oriental Hospital(SUOH) and 333 outpatients out of 2030 people from Chuncheon Oriental Hospital(COH) from January 1, 2007 to December 31, 2009. We categorized the results by gender, age, disease types, residences, months and number of visiting times. Results : The results were as follows 1. Comparing the number of outpatients that had visited the hospital by gender in SUOH shows that female's visiting rate is 57% and male is 43%. In COH shows that female's visiting rate is 54% and male is 46%. 2. Most frequent visiting age group were 21-30 year-old group(30%) in SUOH and 11-20 year-old group(23%) in COH. 3. In regard to the monthly visiting period, January(13%) is the highest in SUOH and July(10%) is the highest in COH. 4. Regional distributions of the outpatients that had visited SUOH and COH show Wonju(73%) and Chuncheon(74%). 5. In terms of treatment methods, SUOH showed the external application(65%), herb medication(54%), and acupuncture(52%) while COH showed acupuncture(74%), the external application(69%), and herb medication(66%). 6. Visiting frequency of outpatients in SUOH showed that 1 time visiting cases were 51%. In COH case, 1 time visiting cases were 19%. 7. The most prevalent diseases were urticaria(15%), eczema(14%) acne(12%), and seborrheic dermatitis(11%) in SUOH and atopic dermatitis(27%), urticaria(20%), acne(16%), and psoriasis(10%) in COH. 8. Most outpatients had visited SUOH for 1 time(51%) and COH for over 11 times(32%) with the major 16 diseases. Conclusions : This study suggests that outpatients who had visited the department of oriental dermatology preferred to get a treatment for urticaria, eczema, atopic dermatitis and acne. Considering this aspect, it needs to improve current research to expand the oriental medical treatments further in dermatological field.

The Relationship between the Use of Korean and Western Medicine in treating Musculoskeletal Disease (근골격계 질환에 대한 한방의료기관 이용이 양방의료기관 이용에 미치는 영향 - 한국의료패널 자료를 이용하여 -)

  • Objectives: The aim of this study was to identify the complementary and substitute relationships between the use of Korean medicine (KM) and that of Western medicine (WM) in the treatment of musculoskeletal disease. Methods: We analyzed the 2009 Korea Health Panel dataset. General characteristics and the medical utilization of respondents were analyzed descriptively. Logistic regression, negative binominal regression, and Tobit regression analysis were used to identify the relationships between the use of KM and the use, visit frequency, and expenses of WM, respectively. Results: In the treatment of musculoskeletal disease, KM use and non-herbal treatments with Korean medicine significantly reduced WM use. Herb medication use significantly increased WM visit frequency. There were no significant relationships between KM use and WM expenses. Conclusions: There are substitute relationships between WM use and KM use, especially non-herbal treatments in KM. Therefore we need to develop the clinical protocols of KM and WM treatments in the treatment of musculoskeletal disease for proper distribution medical resources.

Clinical-Case Study on a Patient with Organic Mental Disorder after Head Trauma Caused by Traffic Accident (교통사고로 인한 외상 후 기질성 정신장애 환자 치험 1례)

  • Objectives: This study evaluated the effect of oriental treatments and rehabilitation on a patient with organic mental disorder after head trauma caused by traffic accident. Methods: We treated the patient with acupuncture, herb-medication, oriental psychotherapy, and rehabilitation. MMSE-K, BDI, K-MBI, FIM were evaluated before and after treatment. Results: The results indicated that MMSE-K was maintained at 27 points; BDI was reduced from 16 points to 5 points; K-MBI was increased from 75 points to 99 points; and FIM was increased from 86 points to 115 points. Furthermore, the treatment resulted in reduced mental symptoms and improved work capacity. Conclusions: Oriental medical treatment has potential validity in improving symptoms of organic mental disorder.

  • Multiple system atrophy (MSA) is an idiopathic and progressive neurodegenerative disorder. Up to now, treatment of MSA patients had not been reported in Oriental medicine and only rarely reported in Western medicine. We experienced a 75-year-old female diagnosed with MSA and had quadriparesis with dysphagia. She was admitted to the hospital and received herbal medication, acupuncture, moxibustion and rehabilitation therapy. After 2 months, her motor power and swallowing function on video fluoroscopic swallowing test significantly improved. This report shows that Oriental medicine may play a role in the treatment of MSA, and further study is merited.

A Clinical Study of Twenty-five Patients Admitted with Headache (두통(頭痛)을 주소(主訴) 입원한 환자 25명에 관한 임상적(臨床的) 고찰(考察))

  • Objective : The purpose of this study is to investigate clinical characteristics with 25 patients who have suffered from headache and were treated. Methods : We classified 25 patients into several groups by IHS classification and evaluated the effects of oriental medical therapy on headache. Results : 1. According to the statistics, instances of migraine were more prevalent than tension headache, especially among women. 2. 28% of patient had entered for treatment within one month of onset. 3. Common associated symptoms included dizziness, nausea, dyspepsia, palpitation and insomnia. 4. In classification by the oriental medical differentiation of symptoms and signs, the rate of stagnation of the humid dam and deficiency of qi, these two types were highest. 5. 56% of patients said they were satisfied with the treatment, reporting half the frequency of headache or better from before treatment. Conclusions : The present results suggest that oriental medical therapy has effects on headache. Further clinical comparative studies on herb-medication versus acupuncture therapy for headache are urged.

A Case Report : Remission of Spasmodic Torticollis Confirmed with Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS) And Tsui Score for Outcome Measure (사경증 평가도구를 통해 관해를 확인한 경련성 사경증 환자 1례)

  • Objective : A case of idiopathic spasmodic torticollis rapidly improved after Oriental Medical treatments and supplementary trigger point injection is reported. Methods & Results : The oriental medical treatment for spasmodic torticollis including acupuncture, bee-venum therapy, herb medication, taping therapy, chuna therapy and physical therapy was performed for pain alleviation, and trigger point steroid injections were also performed. The Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS) and the Tsui Score were used for outcome measure of the spasmodic torticollis. After treatment for 15 days, the TWSTRS showed a decrease from 56 to 22.3 and the Tsui score also decreased from 13 to 5. Conclusions : We concluded that the oriental medical treatment including acupuncture, bee-venum therapy and taping therapy rapidly induced spasmodic torticollis to remission.
