A Case of Hepatitis B Virus Associated Nephropathy (B형 간염 바이러스(Hepatitis B Virus)에 의한 신장병증 1예)

  • Since Combes' first description of hepatitis B virus associated membranous glomerulonephritis, many reports have shown a strong association of hepatitis B virus infection with various types of nephropathies, especially membranous glomerulonephritis and membranoproliferative glomerulonephritis. Recently, the authors experienced a case of membraoproliferative glomerulonephritis, type I in a 16-year-old male patient with persistent HBs and HBe antigenemia. One year prior to admission the patient was told of hepatitis at routine check, but he remained asymptomatic throughout. He was admitted to our hospital with chief complaints of proteinuria and microscopic hematuria found on routine urinalysis. Liver and kidney biopsy revealed chronic persistent hepatitis and membranoproliferative glomerulonephritis type I, respectively. We report a case of hepatitis B virus associated nephropathy with review of the literatures. Considering the endemic nature of hepatitis B virus infection in Korea, the incidence of hepatitis B viurs associated nephropathy would be proportionally high in comparison with those of other countries. So, the importance of this entity merits special consideration in our country. Further study concerning pathogenesis, epidemiology, and treatment may be needed.

Knowledge about hepatitis B among dental hygiene students in one college (일부 M지역 치위생과 학생들의 B형간염에 대한 지식도 조사)

  • The purpose of this study was to evaluate knowledge and differences about hepatitis B among dental hygiene students in Masan university. The study surveyed by means of self-administered questionnaire consisted 193 of them. The converted score of knowledge of hepatitis B among 1st and 2nd grade students was 61.3(${\pm}22.4$) and 73.1(${\pm}18.3$) point respectively if 100 point for all statements were correctly answered. They have a good understanding of knowledge of hepatitis B except air bone infection and transmission by fomites. There is no differences of knowledge about hepatitis B according to socio-economic factor like father's education, mother's education and income. In conclusion, dental hygiene students should be educated more about hepatitis B.

  • Interferon (IFN) alpha has been the first line therapy of chronic hepatitis B in children, but HBeAg seroconversion occurred in 26% of treated children compared to 11% of controls in multinational randomized controlled study. Recently, lamivudine was shown to be a potent inhibitor of Hepatitis B virus (HBV) reproduction both in HBeAg positive and in HBeAg negative (the pre-core mutant form) chronic hepatitis in randomized studies worldwide. Lamivudine therapy led to considerable improvement in the seroconversion rate of HBeAg in children with chronic hepatitis B, though long-term therapy resulted in the expansion of lamivudine-resistant mutant viruses. Combination therapy with lamivudine plus alpha-IFN does not seem to improve HBe Ag seroconversion. Above all, the most effective way to prevent hepatitis B is universal HBV vaccination.

  • This case was designed to evaluate the improving effect of hepatitis on drug metabolism. Gleditsiae Spina(?甬刺) was included Taeumin' herb. but it maybe induced hepatitis on drug metabolism, if Taeumin take this herb long time. We use Taeyeumin Chungsimyeunjatang to the hepatitis on drug metabolism, and we improved it. so we report it.

  • Hepatitis is a common and serious disease for the Korean population. It is caused by a virus, the A and B types of which are plentiful in Koreans. In this study, we tried to find genetic factors for hepatitis through genome-wide association studies. We took 368 cases and 1,500 controls from Anseong and Ansan cohort data. About 300,000 single-nucleotide polymorphisms and 20 epidemiological variables were analyzed. We did not find any meaningful significant single nucleotide polymorphisms, but we confirmed the influence of major epidemiological variables on hepatitis.

Effect of the Structured Education for Knowledge of Hepatitis B Type and Self Care Behavior in Chronic Hepatitis B Patients (구조화된 환자교육이 만성 B형 간염환자의 B형 간염에 관한 지식과 자가간호 수행에 미치는 효과)

  • The purpose of this study was to identify the effect of structured patient education on knowledge of Hepatitis B type and behavior about self care in chronic hepatitis B patients, and to fine the strategy to promote their self care behavior. The research design was quasi-experiment research. The study method had been done by investigating the experimental group and control group through the questionnaire on 50 patients who had been out patient medicine department in U university hospital in Ulsan from september 1st 1997 to the end of October, 1997. The analysis of the collected material had been done for the homogeneity test in which general characterics of experimental group and control group had been tested by $x^2-test$ and the homogeneity test of the knowledge of hepatitis B type and self care behavior before by t-test. To test the hypothesis the t-test had been given for the difference of the knowledge of hepatitis B type and self care behavior between the two groups and the correlation between knowledge of hepatitis B type and self care behavior performance had been tested by Pearson's correlation coefficient. The results as follows : 1. The 1st hypothesis 'The experimental group which received the structured education should be higher in the knowledge of hepatitis B type than the control group' was supported(t=-6.25, P=.000). 2. The 2nd hypothesis 'The experimental group which received the structured education whould be higher in the self care behavior performance than the control group' was supported(t=-5.15, P=.000). 3. The 3rd hypothesis 'The higher the knowledge of hepatitis B type in the patient the higher the self care behavior performance degree' was supported(r=.492, P=.001). In conclusion, the patients who received the structured education showed the increase in the degree of knowledge of hepatitis B type and self care behavior performance. so the structured education had been judged the nursing intervention had been prerequisite in increasing knowledge of hepatitis B type and self care behavior performance of the chronic hepatitis B patients.

  • A female visited the Dept. of Oriental Ophthamology & Otolaryngology & Dermatology, Dong-eui University with Facial Nerve Palsy in Herpes Zoster Oticus. She had been taking ill with chronic hepatitis B and taking western medicine. We treated a patient with only Oriental Medicine.(the herbal medication and acupuncture etc). Because she was afraid of herbal medicine -induced hepatitis, went through an examination about LFT profile regularly. The symptom of Herpes Zoster Oticus was improved and there was no abnormality in LFT profile. Through this case, we thought that it is possible to treat the other disease of the patient with chronic hepatitis using herbal medication without hepatotoxic hepatitis. But for the safety of patient and doctor in several case, we need to accumulate objective data about the side effect of herbal medications inducing hepatotoxic hepatitis.

Two Cases of Hepatitis Treated with Gagamsaenggan-tang (가감생간탕 투여후 호전된 급성 간염 2예)

  • Acute hepatitis is caused by virus, alcohol. toxins and drugs. Clinical symptoms of acute hepatitis are severe fatigue, jaundice, nausea, hepatomely, lymphadenopathy. We encountered two cases of acute hepatitis, one C type viral hepatitis and the other hepatitis by overuse of medicine. The symptoms of the latter are severe fatigue, petechia and heartburning. The symptoms of the latter are severe fatigue, nausea, dyspepsia, yellow urine and skin. We recognized that total bilirubin. direct bilirubin, serum transaminase, alkaline phosphatase and gamma glutamyl transferase were elevated. We treated both patients with herb medicine(Gagamsaenggan-tang). Gagamsaenggan-tang has been used to hepatic diseases and have been known to have beneficial effects. The patients' symptoms began to improve after about two weeks of treatment. After 3 weeks on medication, the clinical symptoms and liver function of two patients were improved.

Current status of hepatitis A virus infections in Korea (한국 소아 A형 간염 현황)

  • The age-specific anti-hepatitis A virus (HAV) seroprevalence rates in South Korea have changed markedly since the last 2030 years with an improvement in the socio-economic, housing, and environmental-sanitation conditions. These changes are characterized by very low anti-HAV seropositive rates among individuals less than 30 years of age; however, nowadays, most adolescents and young adults at an increased risk of developing symptomatic HAV infections. The Korea Center for Disease Control Sentinel Surveillance System has recently revealed an increase in the incidence of hepatitis A infection since 2001 and has revealed a potential endemic nature of the hepatitis A infection. Hepatitis A vaccines that were introduced in 1997 in Korea have made the current anti-HAV IgG positive rates in children (less than 10 years of age) approximately 50% of the rates observed in Seoul in 2006. However, in the same year, a few children were diagnosed as having anti-HAV IgG antibodies in Busan. This suggests the presence of some difference in the vaccination policy among doctors practicing in Seoul and Busan. Thus, the current recommendation of vaccinating 12-year-old child with HAV vaccination should be emphasized and a new strategy should be developed for the vaccination program to cater to the adolescents and young adults who are not immune, as well as for persons who are at a high risk for hepatitis A viral infection such as military personnel and hospital and day care center employees. Further, urgent hepatitis A vaccinations are also needed in patients with chronic liver diseases.

Relationship between knowledge and infection control behavior about Hepatitis B patient in health care-related majors (일부 보건의료계열 학생들의 B형 간염환자 감염관리에 대한 이론적지식과 임상감염관리 실천도의 관련성)

  • Purpose: The purpose of this study was to identify relationship between knowledge and infection control behavior about Hepatitis B patient in health care-related majors. Methods: Data was collected from health care related majors-nursing, emergency rescue and dental hygiene with questionaire. Total respondents were 306 and we use all of them. We analysed students' knowledge and infection control behavior about Hepatitis B patient and their relationship. Uni-variate analysis, t-test, ANOVA and correlation analysis test were conducted using SAS version 9.2. Results: Students who go the Hepatitis B lecture were significantly high Knowledge level about Hepatitis B(p<.05). Students who go to clinical education about Hepatitis B were significantly better behavior about infection control in clinical practice(p<.05). The relationship between knowledge about Hepatitis B and infection control in clinical practice was significantly negative(p<.05). Conclusions: It needs to be educated both infection control lectures and clinical practice to health care-related majors to prevent cross-infection between patients and health care providers.
