간흡충증 진단을 위한 피내반응검사의 의의 (Diagnostic Value of the Intradermal Test for the Infection with Clonorchis sinensis)

  • 1988년 8월초부터 9월말동안 영남의료원 내과를 내원한 사람중 1304명에서 대변도말검사를 실시하였으며, 이중 443명에서는 간흡충증 피내반응검사를 하였고, 간흡충증이 의심되는 79명에 대해서는 집란법을 동시에 시행하여 간흡충증을 조사한 결과 다음과 같은 결과를 얻었다. 1) 간흡충 감염율은 3.8%였다. 2) 간흡충증 피내반응검사의 감수성은 82.1%(32/39명)이었고, 특이성은 64.6%(261/404명)이었다. 3) 도말검사상 음성이었으나 피내반응검사가 양성이면서 간흡충증이 의심되거나, 양 검사 모두 음성이나 말초 호산구성 세포의 증가로 간흡충증이 의심되는 사람 79명을 집란법으로 조사한 결과 79명중 16명에서 간흡충란이 발견되었다. 4) 도말검사상 음성이었으나 집란법으로 충란이 발견된 16명중 13명은 경감염이었고 3명은 중등감염된 것으로 나타났다. 이상의 결과에서 간흡충증이 의심되는 환자에게는 반드시 대변도말검사와 함께 피내반응검사를 실시하고 필요시 집란법으로 EPG와 EPD을 조사한 후 확진하여 치료하는 것이 바람직하리라 생각된다.

Morphologic and Genetic Identification of Taenia Tapeworms in Tanzania and DNA Genotyping of Taenia solium

  • Species identification of Taenia tapeworms was performed using morphologic observations and multiplex PCR and DNA sequencing of the mitochondrial cox1 gene. In 2008 and 2009, a total of 1,057 fecal samples were collected from residents of Kongwa district of Dodoma region, Tanzania, and examined microscopically for helminth eggs and proglottids. Of these, 4 Taenia egg positive cases were identified, and the eggs were subjected to DNA analysis. Several proglottids of Taenia solium were recovered from 1 of the 4 cases. This established that the species were T. solium (n=1) and T. saginata (n=3). One further T. solium specimen was found among 128 fecal samples collected from Mbulu district in Arusha, and this had an intact strobila with the scolex. Phylegenetic analysis of the mtDNA cox1 gene sequences of these 5 isolates showed that T. saginata was basal to the T. solium clade. The mitochondrial cox1 gene sequences of 3 of these Tanzanian isolates showed 99% similarity to T. saginata, and the other 2 isolates showed 100% similarity to T. solium. The present study has shown that Taenia tapeworms are endemic in Kongwa district of Tanzania, as well as in a Previously identified Mbulu district. Both T. solium isolates were found to have an "African/Latin American" genotype (cox1).

Status of Helminthic Infections in Residents around River Basins in the Republic of Korea for 10 Years (2011-2020)

  • The positive rate of Clonorchis sinensis is the highest among intestinal parasites in the Republic of Korea (Korea). More than 1.2 million people were at risk of C. sinensis infection in Korea in 2012. An intensive control program is being implemented for residents of the 5 major river basins to reduce helminthic infections, including C. sinensis infection. This study evaluated the continuous intensive control program for parasitic diseases including clonorchiasis in areas near the 5 major river basins in Korea over the past 10 years (2011-2020). A total of 335,020 fecal samples (one sample per resident) prepared by the modified sedimentation technic were microscopically examined. Those who expelled helminth eggs were treated with anthelmintics through local health centers and re-examined 3 months later. The overall positive rate of helminths egg was 7.1%. The annual positive rates were dramatically decreased from 14.4% (2011) to 5.9% (2020). The egg positive rate was highest in C. sinensis (5.3%), followed by heterophyid flukes (1.5%) and Trichuris trichiura (0.2%). The prevalence of C. sinensis was significantly higher in males (7.6%) than in females (3.7%), and the highest in the 50-59 years (7.0%) age group. Our results are beneficial to establish prevention and control policies against helminthiases including clonorchiasis in endemic areas in this country.

Persistent endemicity of Gymnophalloides seoi infection in a southwestern coastal village of Korea with special reference to its egg laying capacity in the human host

  • Follow-up studies have been conducted every three years on the endemicity of Gymnophalloides seoi infection in a small coastal village of Chollanam-do (Province), Korea, since it was first known as an endemic area in 1994. Special attention was given to its egg laying capacity in the human host. In fecal examinations, the overall helminth egg and/or cyst positive rate was 78.7% (74/94) in 1997 and 76.6% (82/107) in 2000. Among them G. seoi eggs showed the highest rate; 71.3% (67/94) in 1997 and 72.0% (77/107) in 2000. The average number of eggs per gram of feces (EPG) was 1,015 in 1997, while a reduced rate of 353 was observed in 2000. In 1997, total of 320,677 adult flukes of G. seoi (av. 10,344/person, 94-69,125 in range) were collected from the diarrheic stools of 31 treated patients. The EPG/worm obtained from 21 cases ranged from 0.04 to 0.77 (av. 0.23), suggesting density-dependent constraints on the worm fecundity. The relationship between the worm burden (X) and EPG/worm (Y) can be expressed as $Y=0.42{\cdot}e{\cdot}^{-l.2x}(r=0.49)$. The results showed that G. seoi infection is persistently endemic in this village.

A new culture system for in situ observation of the growth and development of Eucyclops serrulatus (Copepoda: Cyclopoida)

  • A practical and convenient method of rearing Eucyclops serrulatus in a microculture environment is described. A complete life cycle of E. serrulatus was maintained in a narrow space on a microscope slide glass on which a cover glass of $22{\times}40mm$ in size was mounted at a height of 0.8mm. The culture medium was constituted by bottled mineral water boiled with grains of Glycine max (soybean). Chilomonas paramecium, a free-living protozoan organism, was provided as live food. Growth of nauplii hatched from eggs to the first stage of copepodite took an average of 7.7 days, and the growth of copepodite 1 to the egg-bearing adult female took an average of 20.1 days in the microculture cell with an average life time of 44.7 days. Continuous passage of cope pods was successfully maintained as long as sufficient medium and food were provided. The microculture method enables an in situ microscopic observation on the growth and developmental process of helminth larvae experimentally infected to copepods as well as of copepod itself. Furthermore, it does not require anesthetization and, therefore, minimize the amount of stress exposed to cope pods during the handling process.

V-shaped Pits in Regions of Ancient Baekje Kingdom Paleoparasitologically Confirmed as Likely Human-Waste Reservoirs

  • In a paleo-parasitological analysis of soil samples obtained from V-shaped pits dating to the ancient Baekje period in Korean history, we discovered Ascaris lumbricoides, Trichuris trichiura, and Clonorchis sinensis eggs. In light of the samples' seriously contaminated state, the V-shaped pits might have served as toilets, cesspits, or dung heaps. For a long period of time, researchers scouring archaeological sites in Korea have had difficulties locating such structures. In this context then, the present report is unique because similar kind of the ancient ruins must become an ideal resource for successful sampling in our forthcoming paleoparasitological studies.

Zoonotic Echinostome Infections in Free-Grazing Ducks in Thailand

  • Free-grazing ducks play a major role in the rural economy of Eastern Asia in the form of egg and meat production. In Thailand, the geographical location, tropical climate conditions and wetland areas of the country are suitable for their husbandry. These environmental factors also favor growth, multiplication, development, survival, and spread of duck parasites. In this study, a total of 90 free-grazing ducks from northern, central, and northeastern regions of Thailand were examined for intestinal helminth parasites, with special emphasis on zoonotic echinostomes. Of these, 51 (56.7%) were infected by one or more species of zoonotic echinostomes, Echinostoma revolutum, Echinoparyphium recurvatum, and Hypoderaeum conoideum. Echinostomes found were identified using morphological criteria when possible. ITS2 sequences were used to identify juvenile and incomplete worms. The prevalence of infection was relatively high in each region, namely, north, central, and northeast region was 63.2%, 54.5%, and 55.3%, respectively. The intensity of infection ranged up to 49 worms/infected duck. Free-grazing ducks clearly play an important role in the life cycle maintenance, spread, and transmission of these medically important echinostomes in Thailand.

서울지역(地城)의 양내기생충(賜內寄生蟲) 감염상태(感染狀態) 조사(調査) -한양대학병원(漢陽大學病院) 내원환자(來院患者)를 중심으로- (Intestinal Parasite Survey in Seoul by Stool Examination at Hanyang University Hospital)

  • The present study was undertaken to evaluate the present status of intestinal parasitic infection in Seoul area, Korea. During the period from June 1985 to July 1986, a total of 5,251 stool samples were collected in Department of Clinical Pathology, Hanyang University Hospital and examined by formalin-ether sedimentation technique once for helminth ova and protozoan cysts. The results were summarized as follows: 1. The overall egg positive rate of intestinal helminthes was 2.53%; and 1.43% for Clonorchis sinensis, 0.7% for Trichuris trichiura, 0.13% for Metagonimus yokogawai, 0.06% for hookworm, and 0.02% for Ascaris lumbricoides, Diphyllobothrium latum and Hymenolepis nana, respectively. 2. The overall cyst positive rate of intestinal protozoa was 1.07%. Cyst positive rate was 0.06% for Entamoeba histolytica and 0.13% for Giardia lamblia, respectively. 3. Higher prevalence of intestinal parasitic infection was observed in male and in $21{\sim}50$ year-old groups. 4. The highest prevalence of Clonorchis sinensis infection was observed in 40th age group (3.4%), and male (2.3%) was more infected than female (0.7%) in general.

  • National surveys on the prevalence of intestinal parasitic infections have been carried out every 5-7 years since 1971 in the Republic of Korea in order to establish control measures. The present nationwide survey was conducted from June to December 2004. The 10% population sampling data of Population and Housing Census by the Korean government in 2000 was used as the survey population. One sample was selected randomly from each of the 22,858 registered subjects, and a total of 20,541 people were ultimately included in this survey. Fecal examinations were performed by the cellophane thick smear and saturated brine flotation techniques. Pinworm infection was examined by cello-tape anal swab method. This survey also included a questionnaire study for a socioeconomic analysis. The total helminth egg positive rate was 3.7%, and the estimated total positive number among nationwide people was 1,780,000. The rates in urban and rural areas were 3.1% and 6.8%, respectively. As the total egg positive rate in the 6th survey in 1997 was 2.4%, the present survey showed that there was a considerable degree of increase in the prevalence rate of intestinal parasitic infections over the 7-year period following the 6th survey. The largest increases occurred in the egg positive rates of Clonorchis sinensis and heterophyids including Metagonimus yokogawai.

전북 부안군과 경남 사천군 해안지역에서 발견한 이형흡충류 및 기타 장흡충류의 유행지 (Two endemic foci of heterophyids and other intestinal fluke infections in southern and western coastal areas in Korea)

  • 우리 나라 일부 해안 지역의 장내 기생충 감염현황을 파악하기 위해 전북 부안군 주민 153명 및 경남 사천군 주민 138명의 대변을 수집하여 셀로판 후층 도말법 및 포르말린-에테르 침전법을 실 시하였다. 전북 부안군의 기생충 감염률은 21.5% (33/153명)이었으며, 감염 기생충별 양성률은 이형흡충류 17.6%, 간흡충 2.6%, 회충 0.7%, 람블편모충 2.6%, 대장아메바 0.7% 및 작은와 포자충 0.75이었다. 경남 사천군의 기생충 감염률은 39.1% (54/138명)로서 이형흡충류 18.8%, 간흡충 12.3%, 편충 2.2%, 람블편모충 2.2%, 대장아메바 5.0%, 이질아메바 0.7%, 왜소아메바 3.6% 및 작은와포자충 4.3%의 양성률을 보였다. 부안군에서 이형흡충란 양성자 22 명으로부터 회수한 충체는 유해이형흡충이 총 3,284마리 (평균 149.3마리)였으며 표주박이형흡 충, 수세미이형홉충, 요코가와흡충, 미야타흡충, 자루이형흡충, 참굴큰입흡충, 긴이형흡충 및 Aconthopowphium kurogamo등이 중복 감염되어 있었다. 사천군에서 이형흡충란 양성자 중 6명으로부터 회수한 충체는 요코가와흡충이 총 3,007마리 (평균 501.2마리)였으며 유해이형흡충, 수 세미이형흡충, 긴이형흡충, 갈매기이형흡충 등의 이형흡충류가 중복감염되어 있었다. 이상의 결과로 우리 나라 남해 및 서해안 지역 주민들에서 담수어 또는 반염수어의 생식 습관으로 장내 흡충, 특히 이형흡충류의 유행이 지속되고 있음을 확인하였다.
