Infantile Marfan syndrome in a Korean tertiary referral center

  • Purpose: Infantile Marfan syndrome (MFS) is a rare congenital inheritable connective tissue disorder with poor prognosis. This study aimed to evaluate the cardiovascular manifestations and overall prognosis of infantile MFS diagnosed in a tertiary referral center in Korea. Methods: Eight patients diagnosed with infantile MFS between 2004 and 2014 were retrospectively evaluated. Results: Their median age at the time of diagnosis was 2.5 months (range, 0-20 months). The median follow-up period was 25.5 months (range, 0-94 months). The median length at birth was 50.0 cm (range, 48-53 cm); however, height became more prominent over time, and the patients were taller than the 97th percentile at the time of the study. None of the patients had any relevant family history. Four of the 5 patients who underwent DNA sequencing had a fibrillin 1 gene mutation. All the patients with echocardiographic data of the aortic root had a z score of >2. All had mitral and tricuspid valve prolapse, and various degrees of mitral and tricuspid regurgitation. Five patients underwent open-heart surgery, including mitral valve replacement, of whom two required multiple operations. The median age at mitral valve replacement was 28.5 months (range, 5-69 months). Seven patients showed congestive heart failure before surgery or during follow-up, and required multiple anti-heart failure medications. Four patients died of heart failure at a median age of 12 months. Conclusion: The prognosis of infantile MFS is poor; thus, early diagnosis and timely cautious treatment are essential to prevent further morbidity and mortality.

A Study on Selection Method of Management Watershed for Total Pollution Load Control at Tributary (지류총량관리를 위한 관리유역 선정 방법에 관한 연구)

  • The purpose of Total Pollution Load Control at Tributary is to obtain maximum improvement effect of water quality through finding the most impaired section of water-body and establishing the proper control measure of pollutant load. This study was implemented to determine the optimal management of reach, period, condition, watershed, and pollution source and propose appropriate reduction practices using the Load duration curve (LDC) and field monitoring data. With the data of measurement, LDC analysis shows that the most impaired condition is reach V (G4~G5), E group (flow exceedance percentile 90~100%) and winter season. For this reason, winter season and low flow condition should be preferentially considered to restore water quality. The result of pollution analysis for the priority reach and period shows that agricultural nonpoint source loads from onion and garlic culture are most polluting. Therefore, it is concluded that agricultural reuse of surface effluent (storm-water runoff with non-point sources) and low impact farming that includes reducing fertilization and controlling the height of drainage outlet are efficient water quality management for this study watershed.

The Study of the Blood Pressure, Blood Sugar, and Blood Cholesterol in Obese Children (비만아의 혈압, 혈당 및 콜레스테롤에 관한 연구)

  • This study surveyed 146 obese elementary school children(94 male and 52 female) in Seoul, and measured height, body weight, degree of obesity, blood pressure, fasting blood sugar, serum lipid profiles and liver function. The hypertension was above 95 percentile depending on age and sex. and the hyperglycemia was defined as above 110mg/dl. The hypercholesterolemia and hypertriglyceridemia were defined as above 200mg/dl, 160mg/dl, repectively. The abnormal liver function was considered as elevated alanine aminotransferase(ALT>35IU/L)or aspartate aminotransferase(AST>35IU/L). The results were as follows: 1. The mean degree of obesity was $29.56\pm12.56\%$, and the percentage of overweight was $18.5\%$, mild obesity $41.1\%$, moderate obesity $35.6\%$, severe obesity $4.8\%$ respectively in surveyed children. 2. The systolic blood pressure was $108.34\pm13.73mmHg$, diastolic blood pressure was $67.46\pm8.27mmHg$. 3. FBS was $93.79\pm6.51mg/dl$. 4. Total cholesterol, triglyceride, ALT and AST were $183.34\pm31.38mg/dl$. $115.55\pm56.43mg/dl,\;24.08\pm18.42IU/L,\; 28.73\pm10.45IU/L,$ respectively. 5. The prevalence of complications was $47.2\%$ : hypertension$(13\%)$, hyperglycemia$(0.7\%)$, hypercholesterolemia$(23.3\%)$, hypertriglyceridemia$(17.1\%)$, and liver dysfunction$(21.9\%)$. In conclusion. childhood obesity is associated with various risk factors. Therefore, the aggressive approaches to successful prevention, early detection, and effective treatment of obesity in children are urgently required.

Correlation between the morning hypertension on ambulatory blood pressure monitoring and the left ventricular mass in children

  • Purpose: Although high morning blood pressure (BP) is known to be associated with the onset of cardiovascular events in adults, data on its effects in children with hypertension are limited. Our retrospective study aimed to define the clinical characteristics of children with morning hypertension (MH) and to determine its associated factors. Methods: We reviewed 31 consecutive patients with hypertension, confirmed by the ambulatory blood pressure monitoring (ABPM). We divided these patients into 2 groups: the MH group (n=21, 67.7%), morning BP above the 95th percentile for age and height (2 hours on average after waking up) and the normal morning BP group (n=10, 32.3%). We compared the clinical manifestations, laboratory results, and echocardiographic findings including left ventricular hypertrophy (LVH) between the groups. Results: The early/atrial (E/A) mitral flow velocity ratio in the MH group was significantly lower than that in the normal morning BP group. In addition, LV mass was higher in the MH group than in the normal morning BP group, although the difference was not statistically significant. The age at the time of hypertension diagnosis was significantly higher in the MH group than in the normal morning BP group (P =0.003). The incidence of hyperuricemia was significantly higher in the MH group than in the normal morning BP group. Conclusion: Older patients and those with hyperuricemia are at higher risk for MH. The rise in BP in the morning is an important factor influencing the development of abnormal relaxation, as assessed by echocardiography. Clinical trials with longer follow-up periods and larger sample sizes are needed to clarify the clinical significance of MH.

  • Immunodeficiency affected by antibody formation is most common among primary immuno-deficiencies. Selective IgA deficiency is more common but, one or more IgG subclass level is low or deficient in some patients. Patients with antibody production deficiency are vulnerable to pneumococci, staphylococci and H.influenzae leading to sinusitis, otitis media and pneumonia. A 10-year-old girl had suffered from frequent upper respiratory infections, a history of tuberculous lymphadenitis tuberculosis medication, and frequent pneumonia that requires hospital adimission. Her height and weight were below 3 percentile normal growth as a manifestation of failure to thrive. When she had another severe pneumonia, all the immunologic test was normal at first, and then we checked the IgG subclass levels. Her IgG1 was within normal, IgG2 was very low, IgG3 and IgG4 was not detected. We report a case of IgG subclass deficiency in frequent upper respiratory infection and failure to thrive.

The Comparison of Waist Circumference, Waist to Hip Ratio and Body Mass Index in Female College Students (여대생의 허리둘레, 허리둔부 둘레비와 체질량지수 비고)

  • Purpose: The purpose of this study was to validate waist circumference (WC) and waist to hip ratio (WHR) corresponding to body mass index (BMI) of $25kg/m^2$ and to survey the prevalence of hypertension according to obesity in female college students. Method: Weight, height, waist and hip circumferences and blood pressure were measured for each of 286 female college students in J City, Chungbuk. Results: The subjects' mean BMI was $21.4kg/m^2$, and the estimated prevalence of obesity was 11.2%. The subjects, the 80th-90th percentile of whom had BMI exceeding $25kg/m^2$, were found to have WC of 75-78.1 cm and WHR of 0.79-0.80, respectively. Of the subjects with obesity($BMI{\geq}25kg/m^2$), 53.1% exhibited abdominal obesity ($WC{\geq}80cm$) and 40.6% had WHR over 0.80. Systolic and diastolic blood pressure were correlated with BMI, WC, and WHR. The prevalence rate of hypertension in subjects with obesity($BMI{\geq}25kg/m^2$) was 25%. Additionally, the prevalence rate of hypertension in subjects with abdominal obesity ($WC{\geq}80cm$) was 39.1%. Conclusions: Our study indicates that WC corresponding to BMI of $25kg/m^2$ in female subjects in their twenties is different from that of adult women. We also found that the prevalence rate of hypertension in subjects with abdominal obesity was high. Therefore. we suggest further studies to determine the cutoff value of WC for evaluating abdominal obesity and to investigate the long-term effects of obesity on women in their twenties.

Analysis on the Results of Developmental Screening Test in One Public Health Center (일개 시 보건소의 영유아 성장발달 스크리닝 결과 분석)

  • This descriptive exploratory study was conducted to analyze the results of developmental screening test using DDST and the follow-up diagnostic evaluation in one public health center, and to evaluate the feasibility of developmental screening in nursing. Samples were 373 children under 6 years who visited the public health center. Mothers' satisfaction on the developmental clinic was also determined. Summaries of the results were as follows:v 1. 0.5% of children in height and 1.9% of children in weight were included in below 3 percentile of Korean children's growth chart, but none of them showed developmental delay. 2. The results of DDST showed 92.1% of subjects were classified as normal, 3.7% as abnormal, and 4.2% as questionable among 354 children. 3. Most of children who showed the abnormal development at the first DDST were confirmed having the developmental delay at the follow-up diagnostic evaluation. On the other hand, most of children who showed the questionable development at the first DDST revealed having the normal development in follow-up screening test. 4. The result of the DDST was influenced by the birth order of the subject and delivery type. 5.The mean satisfaction score by mothers on developmental clinic was 4.35 in 5 Likert scale. In conclusion, we could certain the feasibility and usefulness of developmental screening in community and child care nursing. To fulfil the increasing needs of mothers with infants on the child development, nurses have to provide anticipatory guidance and parent education in addition to the developmental screening test. We hope to expand the developmental screening in nursing field not only of clinical setting but also of community.

Modelling Stem Diameter Variability in Pinus caribaea (Morelet) Plantations in South West Nigeria

  • Stem diameter variability is an essential inventory result that provides useful information in forest management decisions. Little has been done to explore the modelling potentials of standard deviation (SDD) and coefficient of variation (CVD) of diameter at breast height (dbh). This study, therefore, was aimed at developing and testing models for predicting SDD and CVD in stands of Pinus caribaea Morelet (pine) in south west Nigeria. Sixty temporary sample plots of size $20m{\times}20m$, ranging between 15 and 37 years were sampled, covering the entire range of pine in south west Nigeria. The dbh (cm), total and merchantable heights (m), number of stems and age of trees were measured within each plot. Basal area ($m^2$), site index (m), relative spacing and percentile positions of dbh at $24^{th}$, $63^{rd}$, $76^{th}$ and $93^{rd}$ (i.e. $P_{24}$, $P_{63}$, $P_{76}$ and $P_{93}$) were computed from measured variables for each plot. Linear mixed model (LMM) was used to test the effects of locations (fixed) and plots (random). Six candidate models (3 for SDD and 3 for CVD), using three categories of explanatory variables (i.e. (i) only stand size measures, (ii) distribution measures, and (iii) combination of i and ii). The best model was chosen based on smaller relative standard error (RSE), prediction residual sum of squares (PRESS), corrected Akaike Information Criterion ($AIC_c$) and larger coefficient of determination ($R^2$). The results of the LMM indicated that location and plot effects were not significant. The CVD and SDD models having only measures of percentiles (i.e. $P_{24}$ and $P_{93}$) as predictors produced better predictions than others. However, CVD model produced the overall best predictions, because of the lower RSE and stability in measuring variability across different stand developments. The results demonstrate the potentials of CVD in modelling stem diameter variability in relationship with percentiles variables.

Evaluation of Dietary Zinc, Copper, Manganese and Selenium Intake in Female University Students (여대생의 아연, 구리, 망간, 셀레늄 섭취 상태 평가)

  • This study aimed to measure and evaluate the intakes of four antioxidant trace elements, namely, Zn, Cu, Mn, and Se in 19-29y-old female university students in Korea. Diet data were collected by 3-day dietary records in 644 subjects. The mean age, height, weight and body mass index of the subjects were 20.08 years, 161.77 cm, 54.26 kg and $20.82kg/m^2$, respectively. The mean, median and 25th-75th percentile intakes of Zn, Cu, Mn, and Se in the subjects were 12.83 mg (12.40 mg, 9.59 to 15.34), 1.30 mg (1.27 mg, 1.00 to 1.57), 3.19 mg (3.12 mg, 2.45 to 3.86), and $50.90{\mu}g$ ($50.17{\mu}g$, 37.59 to 64.35), respectively. The proportion of subjects whose Mn intake was adequate or less was 62.89%, and the proportions of subjects whose Zn, Cu and Se intakes were at the estimated average requirements or less were 10.09, 4.97, and 39.60%, respectively. The major food group for dietary intakes of Zn, Cu, Mn, and Se was cereal, providing 8.55 mg (66.60%), 0.78 mg (59.93%), 2.09 mg (65.50%), and $16.83{\mu}g$ (32.43%), respectively. Many female university students were deficient in Mn and Se compared with the dietary reference intakes. Therefore, except for cereal, it is required to consume a diet consisted of various food sources for increasing the intakes of antioxidant trace minerals, especially animal food groups.

Relationships between Obesity and Dietary Habits of Preschool Children and Their Parents in Dongducheon Based on the Nutrition Quotient (NQ) (어린이 영양지수를 활용한 동두천시 지역의 유아 및 부모의 식습관과 유아의 비만과의 관련성)

  • Objectives: This study was conducted to investigate the associations between obesity and the children's Nutrition Quotient (NQ) and to further examine the relationships between NQ and mini dietary assessment (MDA) of their parents. Methods: The subjects were 355 children aged 3 to 5 years and their parents in Dongducheon. We collected parental-reported NQ questionnaires for children and MDA of parents. Anthropometric measurements, height, weight and BMI by BIA were measured in children. Overweight and obesity were determined according to age- and sex-specific BMI percentile from the 2007 Korean national growth chart. Statistical analyses consisted of the chi-squared test, ANOVA, partial correlations and logistic regression analysis adjustments for parents BMI. Results: Approximately 20.8% of preschool children were classified as overweight or obese. Underweight children showed a significantly higher score for balance than overweight children. The NQ of the children was $61.9{\pm}11.6$, and NQ scores and their parents' MDA did not exhibit any significant differences according to degree of obesity. After adjusting for parent's BMI, children's BMI was significantly correlated with balance and moderation among NQ factors. Parent's MDA showed significant correlation with their children's NQ, balance, diversity, moderation, regularity, practice and NQ grade, except for diversity of father. Additionally, NQ grade had a significantly increased (150.1%) odds ratio (OR) of being overweight (95% CI 1.008-2.234). Conclusions: These results show that NQ for children is influenced by their parents' MDA and BMI. Furthermore, our findings support the association between overweight prevention and improvement of NQ grade among preschool children.
