Influence of the coexistence of visual impairment, hearing impairment, and masticatory discomfort on the quality of life of middle-aged adults: an analysis based on the 2019 and 2020 Korea National Health and Nutrition Examination Survey

  • Purpose: Visual impairment, hearing impairment, and masticatory discomfort each influence quality of life (QoL). However, little is known regarding the impact of their coexistence on QoL. Therefore, we aimed to investigate the influence of the coexistence of visual impairment, hearing impairment, and masticatory discomfort on QoL among middle-aged adults aged 40-64. Methods: This study involved a secondary data analysis utilizing the data from years 1 and 2 of the eighth Korea National Health and Nutrition Examination Survey. To evaluate the influence of the coexistence of visual impairment, hearing impairment, and masticatory discomfort on QoL, we conducted a complex sample hierarchical multiple regression analysis. Results: When visual impairment, hearing impairment, and masticatory discomfort coexisted, the QoL was significantly lower than in individuals without any of these conditions. Conclusion: The coexistence of visual impairment, hearing impairment, and masticatory discomfort was negatively correlated with QoL. Therefore, it is important to prepare for old age through appropriate health management during middle age.

노인의 시력 및 청력 저하가 우울 및 인지기능에 미치는 영향: 2008년 고령화연구패널조사 (The Effect of Visual and Hearing Impairment on Depression and Cognitive Function in Community-dwelling Elderly : The Korean Longitudinal Study of Aging 2008)

  • Purpose: The purpose of this study was to identify the prevalence of visual and hearing impairment and to investigate the effect of visual and hearing impairment on depression and cognitive function in community-dwelling Korean elderly. Methods: The study population consisted of a representative community sample of 4,028 persons aged 65 and older from the Korean Longitudinal Study of Aging 2008. Depression and cognitive function were measured by CES-D 10-item scale and K-MMSE. Visual and hearing impairment were measured by 5-Likert scale. Data were analyzed using Rao-Scott ${\chi}^2$-test, simple and multiple logistic regression. Results: The prevalence of visual and hearing impairment were 37.3% and 14.0%, respectively. Of the participants, the prevalence of depression was 58.4% and cognitive impairment was 50.6%. There were significant differences in depression and cognitive function according to visual and hearing impairment. Far vision and hearing impairment were predictors for both depression and cognitive impairment even after covariates had been adjusted. Conclusion: These data suggest that visual and hearing impairment in older adults may increase their probability of experiencing depression and cognitive impairment. Thus sensory impairment should be considered a risk factor for mental health and the cognitive function of elderly.

재가노인의 시청각기능장애가 일상생활수행능력에 미치는 영향 (The Influences of Visual and Hearing Impairment on Activities of Daily Living for the Community Dwelling Elderly)

  • Purpose: The aim of this study is to identify the influence of visual and hearing impairment on the activities of daily living of community dwelling elderly. Methods: Data were collected by home visiting interviewers from 452 older people aged 65 years or older living in community. Resident Assessment Instrument MDS-HC(2.0version) was used for data collection. Data analysis for descriptive statistics, Chi-square test and multiple regression was made by SAS 6.2 Results: 34.7% of the subject had hearing impairment and 64.3% had visual impairment Among IADL. one half of them were dependent in ordinary house work and meal preparation. In the case of ADL. 13.9% of subjects were dependent in bathing and 8.9% in personal hygiene. There was significant difference in IADL performance by visual and hearing impairment On the other hand, ADL performance showed the significant difference. only in the case of hearing impairment. As the result of input of visual and hearing impairment in the process of regression. variances were increased from 3% to 11%. Conclusions: Large proportions of older people living in the community have visual and hearing impairment. It could be confirmed that hearing and vision were significant factors influencing on IADL performance of older people. Intervention and support policy for elderly needs to focus on improvement of visual and hearing impairment.

청력저하 여부에 따른 노인의 삶의 질 관련 요인: 국민건강영양조사 자료(2016-2018년) 분석 (Comparison of quality of life and related factors according to hearing impairment in elders using the Korea National Health and Nutrition Examination Survey (2016-2018))

  • 본 연구는 청각저하 여부에 따른 노인의 삶의 질 관련 요인을 파악하고자 시도되었다. 제 7기 국민건강영양조사 자료를(KNHANES 2016-2018) 활용하여 65세 이상 노인을 청각저하 여부에 따라 분류하여 최종적으로 4,754명을 연구대상으로 하였다. 인구사회학적 특성, 건강 행태, 신체적 건강과 심리사회적 건강 특성(주관적 건강상태, 스트레스, 우울, 걷기운동, 근력운동, 활동제한, 의료 미충족 요구)을 조사하였다. 수집된 자료는 SPSS ver. 22.0를 이용하여 복합 표본 분석하였다. 청각저하 여부에 따라 노인의 삶의 질은 인구사회학적 특성, 건강 행태, 신체적 건강과 심리사회적 건강에 따라 차이가 있었다. 다중로지스틱회귀분석 결과, 청력저하가 없는 노인의 경우 결혼상태, 활동제한과 걷기운동이 삶의 질에 영향을 미치는 요인이었다. 청력저하가 있는 노인의 경우에는 걷기운동이 삶의 질에 영향을 미치는 요인이었다. 청력저하 노인의 삶의 질 향상을 위한 환경, 신체, 심리적 건강관리 및 운동이 포함된 다학제적 협력과 프로그램 개발 및 적용이 필요하다.

  • 1976년 1월부터 동년 12월까지 본병원 당뇨병진료실을 방문하여 당뇨병으로 진단된 환자 중 청력검사를 시행한 43례(남자 25례, 여자 18례)와 그중 청각장애자 8례를 대상으로 임상적 소견 및 그 상호관계를 관찰하여 다음과 같은 결론을 얻었다. 1) 당뇨병환자 43례 중 청각장애자는 8례이며, 청력형은 고음급추형 및 고음점경향이 대부분이다. 2) 중연속 및 고연령층에 청각장애자의 출현빈도가 높다. 3) 이식기간과는 무관하나, 혈당치 및 혈청총 cholesterol치와는 어느 정도 상관관계가 있다. 4) 강막증, 고혈압증 및 신경증 등 합병증을 동반한 례에 청각장애자가 많다. 5) 당뇨병환자의 청각장애의 성립요인은 단일한 것이 아니고 여러가지 요소가 관여하는 것으로 생각된다.

난청인의 주파수 선택도와 비대칭적 청각 필터를 고려한 난청 시뮬레이터 개발에 관한 연구 (A Study on Development of a Hearing Impairment Simulator considering Frequency Selectivity and Asymmetrical Auditory Filter of the Hearing Impaired)

  • In this paper, we propose a hearing impairment simulator considering reduced frequency selectivity and asymmetrical auditory filter of the hearing impaired, and we verified the reduced frequency selectivity and asymmetrical auditory filter affected in speech perception through experiments. The reduced frequency selectivity has made embodied by spectral smearing using LPC(linear prediction coding). The shapes of auditory filter are asymmetrical different with each center frequency. Hearing impaired person which has hearing loss was differently changed with that of normal hearing people and it has different value for speech of quality through auditory filter. The experiments confirmed subjective test and objective test. The subjective experiments are composed of 4 kinds of tests: pure tone test, SRT(speech reception threshold) test, and WRS(word recognition score) test without spectral smearing, and WRS test with spectral smearing. The experiment of the hearing impairment simulator was performed from 9 subjects who have normal ears. The amount of spectral smearing was controlled by LPC order. The asymmetrical auditory filter of proposed hearing impairment simulator was simulated and then some tests to estimate the filter's performance objectively were performed. The objective experiment as simulated auditory filter's performance evaluation method used PESQ(perceptual evaluation of speech quality) and LLR(log likelihood ratio) for speech through auditory filter. The processed speech was evaluated objective speech quality and distortion using PESQ and LLR value. When hearing loss processed, PESQ and LLR value have big difference according to asymmetrical auditory filter in hearing impairment simulator.

Improved Self-Esteem in Women with Hearing Impairment after Horticultural Therapy and Subjects' Program Preference

  • This study was conducted to determine the effects of horticultural therapy on self-esteem of women with hearing impairment. Ten women with hearing impairment registered in the Mokpo Branch of the Korean Association of the Deaf participated in the horticultural therapy programs (HTP) for self-esteem. The self-esteem scale was used to evaluate the effects of the programs and scores were compared to determine the difference before and after the programs. Self-esteem was improved in all 10 subjects, and its mean value increased with statistical significance from 22.5 points before the programs to 29.8 points after the programs. The programs preferred by women with hearing impairment included 'making a centerpiece using scented candles and flowers', 'packing and planting pot spray chrysanthemum', 'making a topiary', 'planting Hedera helix', and 'planting cyclamen'. Therefore, it was suggested that self-esteem of women with hearing impairment were improved by increasing social and family support, emotional stability, positive feedback, and self-expression through horticultural therapy programs.

  • Hearing loss is one of the most common birth defects among infants. Most hearing-impaired children are not diagnosed until one to three years of age, which is too late to treat for normal speech and language development. If a hearing impairment is identified and treated in its early stage, child's speech and language skills could be comparable to his or her normal-hearing peers. In this study, we applied the 'Fsp' method to distinguish between normal and impaired hearing. We have developed a battery-operated portable A - ABR(automated auditory brain stem response) system that automatically detects hearing impairment for neonates or infants in a nursery room, as well as in a sound-proof room. We partially validated the accuracy of the system in five normal-hearing adults.

난청인의 주파수 선택도를 고려한 난청 시뮬레이터 개발 (Development of a Hearing Impairment Simulator considering Frequency Selectivity of the Hearing Impaired)

  • In this paper, we propose a hearing impairment simulator considering reduced frequency selectivity of the hearing impaired, and verify it's performance through experiments. The reduced frequency selectivity was embodied by spectral smearing using linear prediction coding(LPC). The experiments are composed of 4 kinds of tests; pure tone test, speech reception threshold(SRT) test, and word recognition score(WRS) test without spectral smearing and with spectral smearing. The experiments of the hearing impairment simulator were performed with 9 subjects who have normal hearing. The amount of spectral smearing was controlled by LPC order. The percentile score of WRS test without smearing is $89.78{\pm}2.420%$. The scores of WRS with 24th LPC order and with 8th LPC order are $88.00{\pm}3.556%$ and $83.78{\pm}2.123%$ respectively. It is verified that WRS score is lowered by decreasing LPC order. This is a reasonable result considering that spectral smearing is getting heavier according to decreasing LPC order. It is confirmed that spectral smearing using LPC simulates the reduced frequency selectivity of the hearing impaired and affects the clearness of speech reception.

청각증강 기술 개발을 위한 청각장애인의 욕구조사: 위험상황 인식 및 의사소통 분야를 중심으로 (A Needs Assessment of People with Hearing Impairment for Hearing Augmentation Technology Development: Focusing on Risk Context Awareness Communication)

  • 본 연구의 목적은 청각장애인이 경험하는 위험상황과 청각 대체기술로 사용하는 보조기기 사용실태를 조사하여 청각증강 기술 개발의 적용점을 모색하는 데 있다. 이를 위해 청각장애 등급을 판정받은 청각장애인을 대상으로 설문조사하여 총 355명의 자료를 분석에 사용하였다. 연구결과 첫째, 소리 및 진동을 인지한 경험이 없는 상황으로 교통수단, 사물, 자연 영역 순으로 높게 나타났다. 이 중 교통수단의 소리와 진동을 인지하지 못한 비율이 높아 일상생활에서 청각장애인이 경험하는 위험 가능성이 높은 것으로 조사되었다. 둘째, 청각장애인이 경험한 위험상황은 교통사고, 보행사고, 가정에서의 일상생활 순으로 높게 조사되었다. 셋째, 위험상황 인식과 알림을 위한 보조기기로 2G폰/스마트폰, 진동디지털알람시계, 경광등, 진동손목시계의 인지도가 높았으며, 2G폰/스마트폰의 만족도가 가장 높았다. 넷째, 의사소통을 위한 보조기기로 보청기, 스마트폰, 영상전화기, 인공와우, 2G폰 순으로 인지도가 높았으며, 스마트폰을 사용하였을 경우 만족도와 의사소통 개선정도가 가장 높게 조사되었다. 마지막으로 청각증강 기술 개발을 위해 고려할 점으로 휴대/착용 편의성, 가격, 동작 정확도 순으로 선호하였으며, 알림 전달 방식은 시각(문자와 빛)을 활용한 알림방식을 선호하였다. 연구결과에 근거하여 위험상황에서 청각장애인의 청각증강 기술 개발을 위한 정책적, 실천적 방안을 제시하였다.
