Past, Present, and Future of Home Visiting Healthcare Services based on Public Health Centers in Korea (우리나라 방문건강관리사업의 과거, 현재와 미래)

  • Purpose: We present improvements to the Korean home visiting healthcare service based on analysis of Korean home visiting healthcare services considering recent sociodemographic changes and demands for healthcare services. Methods: This is a review study in which the results are derived through a literature review and data analysis. We collected data through a search of electronic databases, Google Scholar, and governmental websites. Results: Changes in Korean home visiting healthcare services are classified into four stages: 'introduction (1990-2000)', 'pilot project (2003-2006)', 'nationwide expansion (2007-2012)', 'various types (2013-2018)'. Korean home visiting healthcare service based on public health centers has achieved outcomes such as improved health behavior and health management, increased health management ability, and establishment of comprehensive healthcare infrastructure. Conclusion: In the future, the demand for home visiting healthcare service will increase steadily because of deepening social polarization, rapid aging of the population, and increases in chronic diseases. To improve health management and health equity, we suggest that Korean home visiting healthcare service will expand to all the people as a core public health service. It is necessary to establish a management team for various types of home visiting healthcare service in the public health center.

  • This paper implemented the u-Healthcare Context Information System (HCIS) supporting ubiquitous healthcare by using location, health and titrating environment information collected from sensors/devices equipped in home for healthcare home service. The HCIS is based on the Distributed Object Group Framework (DOGF), a management model which can customize distributed resources, and manages various context information, applications and devices as a group in healthcare home environment, as one more logical units. Also, this system provides continuous healthcare multimedia service considering a resident's location using Mobile Proxy, and the healthcare context information through Context Provider to a resident in home. For verifying execution of our system, we implemented the seamless multimedia service based on resident's location and the prescription/advice and schedule notification/alarm service as healthcare applications in home. And we showed the executing results of healthcare home service by using service device existed in the residential space on which the resident is located according to the healthcare scenario.

Survey of Home Healthcare Nursing Services to Establish Quality Assessment Standards (가정간호 서비스 질 평가기준 설정을 위한 조사연구)

  • Purpose: The study aimedto provide basic data to improve the quality of home healthcare nursing services by evaluating quality of care in representative nationwide sites. Method: The current quality of home care service in 104 nationwide sites was evaluated in terms of structures, processes, and outcomes based on published standards of the Joint Commission on Accreditation of healthcare Organizations. Results: The mean score for three dimensions of quality of home care service was as follows in descending order: structures (77.6), outcomes (60.4), and processes (38.7). Additionally, by specific item compared level of quality of home care servicein each site, the highest score was 97.3 and the lowest score was 42.3 out of 100, with a mean score of 74.7. Conclusions: These findings provide a base for establishing the quality management system and to develop a tool for evaluating the quality of home healthcare nursing. The result should be continuous management and improvement of home healthcare nursing quality.

  • In this paper, we propose the active model based on the distributed object group framework which provides adaptive information sharing service to support it to various Healthcare home service. And we applied it to Healthcare home service. This proposed model provides information that collected from physical environment of the home network and sensors for healthcare service according to situation of the user. We uses information of the healthcare information database which was constructed previously, and we uses TMO scheme for to solve each other different periodic qualify. Specialty, We wrote about interaction of various Healthcare Home services for adaptive information services, and wrote about u-healthcare framework based on DOGF.   • In this paper, we propose a service component based on active model for supporting a variety of u-healthcare application services. It implemented that component as a classification of function for developing healthcare application services. Especially we focus on the adaptive information service in integrated environment using a distributed object technologies of the various healthcare home service based on distributed object group framework. And we shows the service component applying to Healthcare application services such as healthcare home monitoring, mobile monitoring and web based monitoring. Also, we show the performance evaluation results such as response time, system load and network load.

  • We derive priority decision making on healthcare service technology standardization in the home network through the decision support process with industry professionals. We configured a research group with 4 industrial areas including Industry, Academic, Research Institution and Medical Institution. And we also applied AHP methodology for the priority decision making. The research group decides an evaluation criteria which are consisted of marketability, technology, ripple effect, strategy for national policies in order to make a priority for healthcare service on a home network. And it is also decided 7 fields and 24 sub-fields, technically. In order to make a priority for the standardization, we use an AHP methodology, that is more objective and feasible, as a decision tool. After two-phase survey that consists of paper survey and face to face meeting, we get a conclusion that home healthcare content is at the top and then wireless home network follows it.

  • This paper suggests a healthcare home service system based on the distributed object group framework that can not only provide healthcare application services using the information obtained from the physical healthcare sensors and devices, and but also monitor and control these services remotely. The distributed object group framework supports the object group service, the interaction service between object groups and the real-time service in order to execute the healthcare application. Here object group means the unit of logical grouped objects or healthcare sensors and devices for a service. Our suggested system consists of 3 layers. The first layer presents the physical sensors and devices for healthcare, as a physical layer. The second layer lays the distributed object group framework, and the third layer, the upper's one, implements healthcare applications based on lower layers. With healthcare applications providing for this system, we implemented the location tracking service, the health information service and the titrating environment service. Also the integrated executing results of these services can be monitored and controlled via remote desktop systems or PDAs.

  • This paper is to construct a healthcare database using information obtained from healthcare home environments, and use this one for healthcare home services, Especially, our researching focus in this paper is how to design healthcare database scheme and how to use this constructed database on the Framework for Supporting Healthcare Integrated Service(FSHIS) we developed previously. Healthcare information is designed to database schemes in accordance to the specific save types of the data collected from various typed-sensors. The healthcare database constructed by using this information for the purpose of healthcare home services is divided into the base information with real schemes and the context based information with view schemes. Firstly, the base information includes low data obtained from physical sensors relevant to locations, healths, environments, and the personnel healthy profiles. The other is the context based information that is produced and fused by using the based information. This context based information might be got via various view schemes according to healthcare application services. Finally, for verifying the practical use of healthcare database constructed in this paper, Via interconnecting this database to our FSHIS, we show an example of healthcare home monitoring service using information (basic and context based information), emergency call, home appliance control, and so on needed from living activity area for elderly living alone.

  • To improve the quality of healthcare and build up the health delivery system, electronic health record system is important for healthcare institutions providing health services. However, the systems currently operated in domestic healthcare institutions use independent software interface with distributed environment for data search and process. Therefore, it raises an additional expense to buy or develop each interface module when there is a link to new system and the problem of complexity. To solve these problems, this study implemented the service oriented architecture basis for home nursing service and carried out an assessment. The study defined the process modeling and business requirements based on the scenario of service and drew 17 suitable services based on five verification items for service design. To draw the final service, seven services were selected by service litmus test (SLT). The study found that, by the improvement of business process for healthcare service support department, SOA based information system has an effect of time reduction of patient processing time. In conclusion, to enable the hospital information system to cope with various demands of healthcare consumers, it is necessary to consider the application of SOA technology which has excellent interoperability, reusability and maintenance.

Factors Affecting the Entrapment of Primary Caregivers of Cancer Patients Receiving Home Healthcare Nursing Service (가정간호서비스를 받는 암환자 주부양자의 속박감 영향요인)

  • Purpose: The purpose of this study was to examine the factors influencing the entrapment of primary caregivers of cancer patients. Methods: From 6 general hospitals, 146 primary caregivers of cancer patients who were receiving home healthcare nursing service were selected for the study. Regarding data collection, structured questionnaires were distributed to the caregivers for data collection. T-test, ANOVA and hierarchical regression were used for data analysis. Results: The significantly influential factors on their entrapment were caregiving time, taking turms to look after the patient, disease duration, home healthcare nursing period, quality of relationship, perceived health status, and social support. And the explanatory power was 55.1% Conclusion: To reduce primary caregivers' entrapment, it is necessary to perform comprehensive and continuous nursing intervention, and to develop a standardized home healthcare nursing intervention program, and to come up with a system for using resources available in local communities.
