건강마을의 건축적 치유요소 연구 (A Study on the Healing Architecture of Health Promotion Villages)

  • This study aimed to analyze health promotion villages from the perspective of therapeutic architecture, and to provide basic information for planning relevant health facilities. On the basis of a review of previous literature, the characteristics of therapeutic architecture were classified into eight categories: safety, comfort, sociality, autonomy, openness, diversity, privacy, and natural environment. Field study and observation were performed in four health promotion villages that conduct programs for lifestyle improvement, physical fitness, and treatment; these villages were also considered to have a healthy residential environment. The researcher and three assistants visited the facilities and checked utilization of space through interviews with the staff and guided tours. Basic information about the subject facilities, such as the purpose of establishment, was gathered from websites before visits. Two of the villages examined in this study had a traditional Korean wooden structure, while the other two were reinforced concrete and masonry block structures. All the facilities emphasized the use of environmentally friendly material and harmony with nature. Each site was divided into the following areas: office, residence, health/treatment, public/rest, and outdoor. The safety standards of the healing architecture were not completely satisfactory at all facilities. In particular, it was found that they lacked adequate universal design equipment. However, the healing characteristics of autonomy and diversity were strong. With regard to autonomy, the facilities allowed visitors to select indoor-outdoor moving lines and the use of front- and backyards constructed on slopes. In addition, they were equipped with many entries, staircases, void, terraces, and crossroads. The architecture was aesthetically designed using feng shui symbolism, and visitors found the scenes depicted in various spaces in the villages to be stimulating. Besides, the facilities were constructed using natural materials, had a natural indoor environment, and provided a sense of spaciousness and flexibility.

High Prevalence of Opisthorchis viverrini Infection in a Riparian Population in Takeo Province, Cambodia

  • Opisthorchis viverrini infection was found to be highly prevalent in 3 riverside villages (Ang Svay Chek A, B, and C) of the Prey Kabas District, Takeo Province. This area is located in the southern part of Cambodia, where the recovery of adult O. viverrini worms was recently reported. From May 2006 until May 2010, fecal examinations were performed on a total of 1,799 villagers using the Kato-Katz thick smear technique. In the 3 villages, the overall positive rate for helminth eggs ranged from 51.7 to 59.0% (av. 57.4%), and the percentage positive for O. viverrini was 46.4-50.6% (47.5%). Other helminths detected included hookworms (13.2%), echinostomes (2.9%), Trichuris trichiura (1.3%), Ascaris lumbricoides (0.6%), and Taenia spp. (0.06%). The prevalence of O. viverrini eggs appeared to reflect a lower infection in younger individuals (<20 years) than in the adult population (>20 years). Men (50.4%) revealed a significantly higher (P=0.02) prevalence than women (44.3%). The Ang Svay Chek villages of the Prey Kabas District, Takeo Province, Cambodia have been confirmed to be a highly endemic area for human O. viverrini infection.

A survey of Brugia malayi infection on the Heugsan Islands, Korea

  • Lymphatic filariasis due to Brugia malayi infection was endemic in several areas of South Korea. The infection was controlled, or disappeared, in most areas, with the exception of the remote southwestern islands of Jeonranam-do, including the Heugsan Islands. To discover its current situation, a small-scale survey was performed on the Heugsan islands in September 2000. A total of 378 people, 151 male and 227 female, living in 8 villages (6 on Daeheugsan-do, 1 on Daejang-do, and 1 on Yeongsan-do) were subjected to a night blood survey for microfilaremia, and physical examination for elephantiasis on the extremities. There were 6 (1.6%) microfilaria positive cases, all in females aged 57-72 years, and from only two villages of the Daeheugsan-do area. There were 4 patients with lower leg elephantiasis, but they showed no microfilaremia. The results show that a low-grade endemicity of filariasis remains on the Daeheugsan-do.

농촌노인의 건강증진 생활양식과 건강증진프로그램 요구도에 관한 연구 (Health Promoting Lifestyle and Need assessment of Health Promotion Program for the Rural Elderly)

  • Purpose: The purpose of this study is to identify the health promoting lifestyle and need assessment of a health promotion program. and to develop a health promotion program for rural elderly. Method: The subjects of the study were 366 adults chosen from 24 villages located in Geochang Gun, Korea. Data sampling used a quota sampling method. Analysis of the data was done by using descriptive statistics, t test, ANOVA and the Scheffe test with SPSS. Results: 1) The average score of performance in the health promoting lifestyle was 1.85. In the subscales, the highest degree of performance was 'nutrition', followed by 'interpersonal relationships', 'health responsibility', 'stress management', 'spiritual growth', and the lowest degree of performance was 'physical activity'. 2) Health promoting lifestyles were significantly correlated with such demographic variables as age (F=2.684, p=.047), education (F=10.989, p=.000), monthly pocket money (F=3.516, p=.008), religion (F=7.160, p=.000), current health status F=3.375, p=.035), health education (t=2.476, p=.014). 3) Health promoting lifestyles were significantly correlated with such life style pattern variables as milk drinking (F=3.767, p=0.035), hobbies (t=3.072, p=0.002), exercise (t=7.186, p=0.000). 4) There is a high level of need for the need assesment of the health promotion program for the elderly in the rural area. Conclusion: With the above findings. I propose that it is necessary to understand a health promoting lifestyle and need assessment for a health promotion program, and to develop a health promotion program considering regional and environmental elements.

일부 농촌지역 여성노인들의 건강특성과 물리치료와의 상관관계 (A Study on Relations between Health Status and Physical Therapy in Rural Area Elderly Women)

  • Purpose: This study examined the health status and physical therapy in elderly women in rural areas to provide basic data on the promotion of health and physical therapy management of elderly women in rural areas. Methods: The subjects were 110 elderly women. The data was collected through individual interviews, and the tool developed for this study was a structured questionnaire based on the literature. The reliability coefficient was 0.57 ~ 0.79(Chronbach' $\alpha$). Results: Village 1 made the most of an oriental medicine clinic and a neighborhood clinic, whereas village 2 mainly used a public health center. Arthritis/ neuralgia and high blood pressure/low blood pressure were the most common complaints in the two villages; a medical institution was used once a month by more than 50% of subjects. Most rural elderly women used physical therapy, and the prevalence of arthritis/neuralgia was high. The most common problem in the two villages was poor health. The improvement in mobility was higher in the more healthy women, who also had less need for treatment at a medical institution. Conclusion: Most rural elderly women were concerned about poor health and used physical therapy. Therefore, rural medical institutions need to pay attention to the medical service and preventive activity to reduce the incidence and severity of neuromuscular syndrome in rural elderly women.

국내 생활터 중심의 건강증진사업: 건강도시를 중심으로 (The Setting Approaches in Health Promotion Projects)

  • 본 연구는 건강도시 지자체를 대상으로 국내 생활터 중심의 건강증진사업 사례들을 살펴봄으로써 국내 건강증진사업의 성공적인 확산을 위한 기초자료를 제공하고자 실시되었다. 2008년부터 2010년까지 국내 건강도시에서 실시한 생활터 중심 건강증진사업 자료, 건강도시 현황자료, 보고서, 논문, 워크숍 및 심포지엄 자료집 등 관련 이차자료를 수집하고 전문가 회의를 통해 시사점을 도출하였다. 2008년 수행한 건강도시 총 사업 중 건강한 생활터 사업은 27개 사업(14.7%), 2009년의 경우에는 42개(21.8%)였다. 생활터 종류를 살펴보면 2008년에는 마을과 학교가 각각 6개(22.2%)로 가장 많았다. 2009년에는 학교 12개(28.6%), 아파트 8개(19.0%), 마을 6개(14.3%), 어린이집 5개(11.9%), 직장 및 경로당이 각각 4개(9.5%), 시장 3개(7.1%)의 순이었다. 대도시 지역의 경우 학교, 아파트를 중심으로 생활터 접근을 하고 있고, 농촌 지역의 경우 마을이나 경로당을 중심으로 생활터 접근을 하고 있다. 국내 생활터 중심의 건강증진사업 사례를 건강증진학교, 건강한 작업장, 건강마을과 건강한 아파트, 건강증진병원, 건강한 시장, 건강경로당을 중심으로 살펴보았다. 생활터 중심의 건강증진사업을 지속적으로 하기 위해서는 '생활터'와 '건강증진'에 대한 인식을 바꾸고 이를 위해 생활터가 건강한 도시(지역사회)를 만들기 위한 중요한 역할을 할 수 있도록 노력하여야 할 것이다.

Assessment of Village Health Worker Training Program in Tuguegarao, Philippine

  • Objectives : This study was performed to evaluate the effectiveness of 'village health worker training program' which aimed to build community participatory health promotion capacity of community leaders in villages of low developed country and to develop methods for further development of the program. Methods : The intervention group were 134 community leaders from 25 barangays (village). Control group were 149 form 4 barangays. Intervention group participated 3-day training program. Questionnaire was developed based on 'Health Promotion Capacity Checklist' which assessed capacity in 4 feathers; 'knowledge', 'skill', 'commitment', and 'resource'. Each feather was assessed in 4 point rating scale. Capacity scores between intervention group and control group were examined to identify changes between the pre- and post-intervention periods. A qualitative evaluation of the program was conducted to assess the appropriateness of the program. The program was conducted in Tuguegarao city, Philippine in January, 2009. Results : The result showed significant increases in the total health promotion capacity and each feather of health promotion capacities between pre and post assessment of intervention group. But there was no significant change in that of control group. Participants marked high level of satisfaction for preparedness, selection of main subjects and education method. Qualitative evaluation revealed that training program facilitated community participatory health promotion capacity of participants. Conclusions : This study suggested that the Village health worker training program is effective for building health promotion capacity of community leaders and it can be a main method for helping low developed countries with further development.

노인들의 건강증진생활양식에 관한 연구 - 전북 농어촌지역을 중심으로 - (Study on Life Style of Health Promotion for the Elderly - Centering on farming villages in Jeollabuk-do Province -)

  • This investigation grasps the level and relevant elements of performance of health promotional activities for the elderly in Korea. It provides fundamental data on health promoting projects targeting the elderly population from farming villages. Hence, this study gropes for an effective approach and measures of health promoting programs. The program needs to be developed with a focus on elderly people from farming villages. In addition, it was carried out in order to provide basic data for development of health projects for local communities. Data gathering was based on survey data targeting patients from the free clinic service. Service was rendered for the residents of farming villages, and conducted at the Offices of CheonBuk Province from October 2000 to December 2000. Analytical results were used to examine the health promotional method for the elderly in the aspect of Oriental Medicine. SPSS 9.0 version as well as T-test and ANOVA were used for survey data analysis. Piersons correlation coefficient was utilized for the relationship for each area, obtaining the following analytical results. 1. The average score for the activities of health promotion was 2.28. Looking at each subcategory, stress management was the highest at 3.65; interpersonal relationship, 3.00; nutrition, 2.55; health responsibility, 2.15; self-realization, 2.03; and exercise was the lowest at 1.89. 2. With respect to lifestyle of the health promotion secondary to general features of elderly people from farming villages, the level of activities of health promoting lifestyle was shown to be higher for males than that of females. Self-realization area was high among males in detailed particulars while the level of execution was high as age decreases in the stress area. 3. Regarding health promoting life style secondary to socioeconomic characteristics, the level of execution was higher for the individuals with a higher level of education and further utilization of spare time. With respect to occupation, the level was highest for people from the fishery. The level decreased in the order of other occupations such as trade, unemployed and agriculture, which was shown to be the lowest. In detailed particulars, it revealed that higher the individuals educational level, the higher the self-realization and stress management areas. The level of interpersonal relationship was the highest among people with little or no education. With respect to self-realization area, the level was highest among the cases where one paid living expenses along with their children. The lowest level of living expenses was seen in the cases where an individual pays for living expenses by himself/herself. There were significant results in all areas except for nutrition areas depending on occupation. The fishery was shown to be the highest. The level of activities was higher as one utilizes more spare time in all areas except for the area of interpersonal relationship.

키르키즈스탄 이주민 마을 지역사회기반 영유아 건강증진 예비사업 평가 (Evaluation of a Community-based Child (Infants and Toddlers) Health Promotion Pilot Project in a Migrant Village in Kyrgyzstan)

  • Purpose: Children in migrant villages in Kyrgyzstan have a high incidence of anemia and need effective health promotion programs. The purpose of this study was to evaluate a pilot study of a health promotion project for infants and toddlers based on community participation. Methods: This was a retrospective study conducted to evaluate the effectiveness of the project. The project was carried out by nurses at a public health center, community health workers, and local residents in a migrant village. After the operational system of the project was established, health assessments, child-care education, provision of vouchers for iron supplements, and activities to improve residents' awareness were conducted during 6 months among 100 children, of whom 85 were finally analyzed. Results: In international health projects, close cooperation of the project team with community residents and health workers is important. Access to the community-based program was feasible in the socially and economically poor migrant village, and improvements were shown in children's anemia and awareness of health care. Conclusion: By focusing on the effective aspects of this preliminary project, plans to utilize community health workers and promotion strategies can be added to the main project to improve health promotion among children in this area.

농촌주민의 적정음주를 위한 보건교육 전략 (Health Education Strategies for Adoption of Moderate Drinking Habits among Rural Residents)

  • This study, based on stages of behavioral change, was aimed at suggesting strategies for the adoption of moderate drinking habits for community-based health education designed to help rural people. An interview survey was conducted during the period from March 4 to April 5, 2002 by sampling 467 rural people living in 6 villages covered by a public healthcare clinic. The results of this study can be summarized as follows; 1. The perceived non-moderate drinkers were less prepared for behavioral change. 2. The heavier drinking habits were ‘drinking alone’, ‘meals accompanied by drinking’ and ‘drinking twice or more at a time’. The agricultural off-season and the custom of brewing liquor at home were negative environmental factors for moderate drinking. 3. The predisposing factors affecting moderate drinking were recognition of health, expectation of the drinking effect, etiquette encouraging overdrinking and control of drinking. The reinforcing factors were stress from ordinary life and perception of being loved. The enabling factor was accessibility to the public healthcare clinic. 4. Rural residents are less motivated to participate in health education for moderate drinking. Based on the above findings, health education strategies for each stage can be suggested as follows: 1) Pre-contemplation stage: improvement of perception, motivation, sharing of experiences, and reawakening. 2) Contemplation/preparation stag e: measurement of value, departure from the inertia against a change, formation of a habit, and reinforcement of the behavior. 3) Action/maintenance stage: creation of a social atmosphere, encouragement of participation, change of life style, and improvement of environment.
