Background: Health promotion services should be provided based on clients' health promotion needs. To provide health promotion services for elementary students in Korea, health promotion needs by geographical regions should be analyzed. Purpose: To analyze elementary students' health behaviors by regions. Method: Elementary students of 4018 from large cities, 1636 from medium sized cities, 328 from rural areas were included. Data collection was done using items from National Health and Nutrition Survey and was collected by mail from schools. For data analysis, Chi-square test and multiple logistic regression were utilized. Result: From comparison of health behaviors, there were no significant differences between large cities and medium sized cities. However, there were significant differences between urban students and rural students in terms of fast food intake, breakfast, soft drink intake, sleeping hours, tooth brush, and obesity. Conclusion: Health promotion services, especially correcting unhealthy eating behaviors for rural elementary students should be focused.
Purpose: The purpose of this study was to identify health promotion services in rural areas and factors influencing this service. Method: From March to April, 2007, a structured questionnaire on services in 2006 was used to collect data from community health practitioners in all of the Primary Health Care Posts (PHCP) in North Chungchong Province. Collected data were analyzed using SPSS 12.0 Win program. Results: The most frequently offered programs were health gymnastics, walking exercise, bathing and vaccination service, and hypertension management. The main obstacles to these health services were lack of adequate space, insufficient budget, and overwork. The most frequently offered health education programs were education on hypertension, exercise, diabetes, volunteer work, and smoking cessation. The main obstacles to health education were lack of adequate space, insufficient education materials and equipment, and lack of cooperation from the citizens. Improvement and reinforcement of health promotion programs should include support of specialist, development of appropriate methods of service delivery, and education materials, and increase ease in using community resources. Conclusions: The research results show that a new model of health promotion must be developed for efficient health promotion programs in rural PHCP.
Health policy is a historical product in the process of development, including the political and economic factors of the state as well as the social and cultural elements of the country. Bioethics began to debate the ethical questions that arise in the overall process of life's birth and death, and gradually evolved by presenting ethical directions for various social phenomena. Especially, according to the moral awakening of 'scientific medicine' which caused in some human problems in the rapidized scientific society from the late 19th century to the early 20th century, as a result of distress including the concept of various social relations, it is possible to say that it has reached the bioethics. Although health policy and bioethics are different in terms of starting and concept, they can be found in common with social, cultural, and political diversity in the times. In 2004, 'Bioethics Law' was enacted through the issue of research ethics in the life sciences. Therefore, in order to examine ethical aspects of current health policy direction and major issues, it can be divided into before and after enactment of 'Bioethics Law' in 2004. The authors would like to examine how the evolution of the ethical viewpoint on the health policy has changed in line with the enactment of the 'Bioethics Law' and how it is trying to solve it from an ethical point of view. Through the various events that took place in the 1990s and the 2000s, various discussions on bioethics were conducted in Korea. Prior to the enactment of the 'Bioethics Law,' ethical judgments of professions, distribution of healthcare resources, if the discussion focused on the ethical judgment of abortion, and the various events that appeared in the early 2000s became the beginning to inform that the ethical debate about the life, death, and dignity of human beings began in earnest in Korea with the enactment of the 'Bioethics Law.' Since then, 'Hospice and Palliative care Law' which was enacted in 2017, is based on the fact that the health policy of our country focuses on the treatment of the past diseases, health promotion, and delivery of health care services. It was an opportunity to let them know that even the quality problems were included. Therefore, considering the various circumstances, the ethical issue facing Korea's health care system in the future is the change of the demographic structure due to aging and what is to be considered as the beginning and the process of life in the overall process of life. It is the worry about how to die and when it sees as death. This has far exceeded the paradigm of traditional health care policies such as disease prevention and management and health promotion, and calls for innovative policy response at the national level that reflects the new paradigm, which in many cases creates a predictable ethical environment. And health policy should be shifted in the direction of future ethical review considering sustainability in the development process of future health care rather than coercive management.
Objective : To determine the relationships between customer's attitude, the subjective norm and the intention to use hospital-based health promotion services. Methods : This study was based on the theory of reasoned action, suggested by Fishbein and Ajzen. The subjects of this study were 501 residents of Seoul, Bun-dang, Il-san and Pyung-chon city, under 65 years, who were stratified by sex and age. A covariance structural analysis was used to identify the structural relationships between attitude towards health promotion programs or services, their subjective norm and their intention to use the aforementioned services. Results : The subjective norm for using the health promotion programs or services provided by hospitals was a significant predictor of the intention to use, but the attitude towards the services was not significant Conclusions : Our results suggest that a customer's reference group affects their use of the hospital-based health promotion services. Because the subjects of this study were restricted to specific urban areas, there are limitations to generalizing the study results. Despite the limitations of these results, they can serve as baseline information for the understanding of consumers behavior toward hospital-based health promotion services.
With a recent increase in dietary supplements (DS) consumption among children in Korea, this study was performed to examine the influencing factors on children's DS consumption. A nationwide survey was conducted employing 3 representative samples of children for summer & fall of 2008 and spring of 2009 by stratified multistage sampling of 120 survey sites per season based on the 2005 census population. Approximately 30 households from each survey site were screened for residing children of 0-19 years and about 1,700 households remained as eligible samples per season. Trained dietitians visited households to perform face-to-face interview to children and/or parents regarding DS consumption including health functional foods (HFF), vitamins/minerals (V/M) supplements and other food supplements during 1 month prior to interview. Out of 5,328 children responded, 18.7% reported DS consumption. Consumption rate was higher in boys (19.9% vs. 17.3% in girls, P < 0.05) and youngsters (22.8% compared to 15.0% in adolescents, P < 0.001). Children from higher income family (P < 0.001), those living in apartments (P < 0.001), those residing in metropolitan area (P < 0.001), and those of mothers with higher education (P < 0.001) were more likely to take DS. Also, mother's employment status and occupation were significantly associated with children's DS consumption. The most popular DS was HFF (72.1%), which was consumed more in children of higher income family. It is revealed that socioeconomic factors affect children's DS consumption significantly. Also it is necessary to estimate children's V/M intake from DS and foods together especially because there are tolerable upper limits set for V/M for safety purposes.
Objectives: This job analysis of a staff in charge of quit-smoking policy at public health centers aims at providing fundamental information to establish strategies supporting various quit-smoking. Methods: The job analysis of a staff in charge of quit-smoking policy at public health centers was carried out through DACUM(Development of Curriculum) method from April through May 2006. Three experts had developed job description with staffs in charge of quit-smoking policy at public health centers through two workshops. The survey was practiced for staff in charge of quit-smoking policy at the other public health centers. The characteristics of the staffs such as age, years for working at public health center, years for charging with quit-smoking work, the proportion of responsibility for quit-smoking work, were surveyed. Results: The research has reached the conclusions below. 1. The job description have been developed considering input-process-outcome axis and plan-do-evaluation axis for quit-smoking policy at public health centers. The final job description is composed of 3 missions, 7 accountabilities, 20 sub-work items. 2. The quit-smoking activity mostly focused at direct education and counselling. But planing and evaluation activity for quit-smoking have been under-achieved. 3. The staffs for quit-smoking policy were feel it is easy to educate and counsel to comer to public health centers for quit-smoking. But having the high proportion of responsibility for quit-smoking policy have usually difficult to do that. So they want to education about counselling for smoker. 4. The staffs who worked over the 2 years for quit-smoking policy the public health center have responded that investigate the smoking rate of the jurisdiction community and the problem of the culture about smoking and smoking policy is important. Conclusions: The study helps reinforcing the initiatives of central government for quit-smoking policy at public health centers. Especially staffs want education in technology area for counselling smoker. And they want nationwide supporting for investigating smoking rate and related factors at the local level.
Objectives: This study performed to verify Korea National Health and Examination Survey(KNHAES) to monitor recommended physical activity(PA) and to propose an monitoring tools for evidence-based PA projects. Methods: The characteristics of the Global Physical Activity Questionnaire(GPAQ) and its reliability and validity were overviewed based on the current scientific evidence in the field of PA. Relevant articles were identified through multiple sources including PubMed, Science Direct, Scopus, CINAHL, ASP, RISS4U, and KISS. Results: The GPAQ has strong points over the Short Form of International Physical Activity Questionnaire(IPAQ-SF) as a monitoring tool to meet recommended PA levels for three domains separately and reflecting a better understanding about walking. Its reliability and validity also were reported as acceptable in existing studies overall. Some limitations of this questionnaire, however, need to be addressed to improve its quality. Conclusions: For monitoring of evidenced-based physical activity projects, we considered that it is able to be provided an opportunity for in-depth discussion among academic experts and policy-makers about the necessity of complements for KNHAES and applying new tools, and it could be the basis for development of physical activity monitoring tool in the future.
Objectives: A great number of the elderly are not aware of having metabolic syndrome, and this pattern differs among regions of the nation. This issue is particularly important for the elderly people with chronic diseases. This study was conducted in order to identify the prevalence, predicting factors of perception and exercise behaviors of among the elderly population. Methods: Using data from the 2009 Korean Community Health Survey, we employed two-level logistic regression models to determine whether individual and community factors are associated with perception and exercise behaviors among the elderly population. Results: Results from weighted two-level logistic regression analyses indicated the individual-level factors of smoking, education level, health center education, community exercise program participation and unmet needs in men, and marital status (being separated), health center education women predicted the elders' perception. As for the community-level predictors, higher smoking and living in regions with high suicide rates significantly predicted the elders' perception and exercise behaviors of metabolic syndrome. Conclusions: These findings demonstrate that education according to stage would increase the perception and motivation toward healthier behavior particularly for the elderly with lower perception levels of metabolic syndrome.
Objectives: The purpose of this study was to analysis the health behaviors differences of adolescents depending on whether parenting behavior perceived by adolescents. Methods: The data which is used in this study was extracted from the 2011 second korean adolescents' penal data collected by the National Youth Policy Institute. Multivariate logistic regression analysis was performed in order to find how degree parents relationship factors are associated with adolescents' health behavior after adjusting sex, smoking(alcohol drinking) friends, and other significant variables. The data was processed by SAS 9.1. Results: Respondent's general characteristics such as smoking(alcohol drinking)friends, parents' job were positively relative to smoking and alcohol drinking experience. Also Parents's affection and experience of being abused from parents were significantly associated with smoking(alcohol drinking) experience. Conclusions: Negative vertical attitude for children is important for the health risk behaviors of adolescents. To increase effectiveness of the health policy for the adolescents in korea, it is important to adapt new strategy to include the positive empowerment of adolescents' social networks except for smoking(alcohol drinking)friends and policy support to turn negative vertical attitude for children into positive attitude for children.
Oriental public health programs have been introduced for the purpose of providing comprehensive oriental health care services to community people including vulnerable classes, increasing the accessibility of oriental medicine and the public benefit, and further more elevating the health promotion and the quality of life of community people. Promoting these programs since 2002 in earnest, it is evaluated that it has made a lot of performances. In the other side, it showed many problems and policy issues also. This study is accomplished to analyze the problems made since the system was introduced and until now, 2007, and with this analysis, to examine policy issues and the reasonable recommendations for its development Major problems are as follows. First, fundamental notions and identity of oriental public health programs are not positioned accurately. Second, the infra-structure construction for effective propulsion of business is insufficient. Third, it is short of the capacity for program implementation of oriental public health doctors, related manpower, and health centers. Fourth, oriental health promotion programs that can fulfill the various health needs of community people are deficient. Fifth, active aid of the government and the oriental medical world as well as legal and systematical support for oriental public health programs is insufficient. As a result, to solve the problems and induce the successful settlement of the program, the policy recommendations such as (i) the fundamental notions of the program and establishment of approach strategies, (ii) reinforcement of the foundation of the program implementation, (iii) capacity enhancement of the manpower of the program implementation, and (iv) effective building of supportive system of the program are presented.
