The Intention of Having a Second Child by the Employment Status of Married Women (취업여부에 따른 기혼여성의 둘째자녀 출산의도)

  • The goal of this study was to examine the economic and cultural factors that influenced the intention of having a second child. In particular, this study intended to examine whether the factors related to the intention of having a child differed by the employment status of married women. The data for this study were drawn from the National Survey on the Trend of Marriage and Birth. This survey was conducted in 2005 by Korean Institute for Health and Social Affairs. The sample consisted of 690 married women who had one child(282 employed and 408 unemployed). The data were analyzed by logistic regression with SAS 9.1 program. The results showed that the employed women's intention of having a second child was affected by the husband's hours of housework, having a own mother(child's grandmother) and the perception of value of child. The employed women's intention of having a second child was affected by the perception of emotional value of child, and the sex of the first child. In sum, there were differences between the employed women and the unemployed women in the variables affecting the intention of having a second child. Perception of value of child was found important for both the employed and the unemployed women. Besides husband's hours of housework, having a mother and the needs for public support about child birth and rearing were found significant for the employed women. For the unemployed women, the costs of child care/education and the sex of the first child were found significant. The findings of this study suggested that different policy interventions be developed to satisfy the needs of the employed women and the unemployed women.

Incidences of Lower Extremity Injuries in Korea (국내 하지손상의 발생현황에 대한 분석)

  • Purpose: We conducted this retrospective epidemiological study to assess the incidence and severity of lower extremity injuries in Korea Methods: For this study, we retrospectively reviewed nationwide lower-extremity injury data compiled from 2001 to 2003 based on the National Injury Database, what included National Health Insurance Corporation (NHIC), Car Insurance, and Industry Insurance data. Data were standardized in terms of demographic characteristics, region, and socioeconomic status by using NHIC data. To assess the degree of the injuries, we used the Modified Abbreviated Injury Scale (MoAIS), what has been changed from the International Classification of Disease-10 (ICD-10) code. By using the Excess Mortality Ratio-adjusted Injury Severity Score (EMR-ISS), we classified the degree of severity into four categories: mild, moderate, severe and critical. Results: From 2001 to 2003, lower extremity injuries increased slightly, with a yearly average of 2,437,335. Insurance data should that lower-extremity injuries were the most common, followed by upper-extremity injuries. Significant difference were seen in the numbers of lower extremity injuries based on gender and age. As for provinces, Seoul and Gyeongi provinces had the highest numbers of cases. Junlabukdo had the highest rate of 55,282 cases per 1 million people for standardized gender and population. The annual incidence of the insured patients with lower extrimity injuries was higher than the employer's medical insurance contributions to the medical insurance program. Daily cases occur most often in May and June, with the lowest occurrences being in January and February. Conclusion: The result of this study shows that lower extremity injuries comprised common cause of all injuries. In addition, differences associated with gender, location and socioeconomic status were observed. Further studies are needed to find reasons and then this knowledge will allow strategies to prevent the lower extremity injuries.

An Experience of Korean Consumer's Monitoring on Nanoproducts (국내 나노제품에 대한 시민 모니터링 결과 고찰)

  • Our team carried out a new program for public engagement on nanotechnology in Korea. We chose 22 monitors, who majored in science or technology and graduated from universities long time ago. Most of them were married and housewives. This 'focus group' had not only general knowledge about science or technology but also much interest in social activities. The 167 nanoproducts to be monitored were for daily life, e.g. home appliances (washing machine, refrigerator, water purifier, etc), clothing, cosmetics, food, toy, and others. And the period of it was one month. The monitors had a sheet with 10 questions, and filled them out in essay form. All of them submitted 2~3 sheets every weekend to our team. Before monitoring, our team had a meeting for introduction and explanation about the potential risk of nanotechnology as well as benefits from it. Another meeting was held after finishing monitoring to share their experience one another. The main results of the monitoring were as follows: the number of nanoproducts describing both the definition of 'nano' and the size of nanomaterials was just 2 (1.2%) the number of them explaining the technical methods enough was 15 (9/0%) the number of them accounting for the reason of functional improvement enough was 14(8.4%); the number of them doubtful as if there would be exaggeration or false knowledge was 27 (16.2%); the number of them commenting potential hazards to human health or environment was almost zero; the number of them describing about safety certification acceptable was 9 (5.4%). The monitors made a proposal containing recommendation to Government and industry. The contents were as follows: industry should make the manual in detail and correctly, Describe Certificate detailed and correctly, Do research on risk and toxicity continually, Educate employee about nanoproducts at consumer's center; Government should make indication of nanoproducts compulsory, Appoint Certificate Authority and make Certificate Mark guaranteeing the safety on nanoproducts, Make detailed explanation about nanoproducts compulsory.

Chest Radiological Changes after Cessation and Decrease of Exposure to Welding Fume in Shipyard Welders (조선업 용접공진폐증에서 용접 흄 폭로력에 따른 방사선 소견의 경시적 변화양상)

  • 27 shipyard welders were diagnosed as pneumoconiosis and suspected pneumoconiosis(1976-1988) by chest radiographs and were observed over three years. 27 welders were divided into three groups by the state of exposure to welding fume i.e. cessation, decresase or continuity of exposure. And we observed the changing pattern of the chest radiographs of 27 welders with the passage of time. The results were as follows; 1. Grour I (ceased exposure to welding fume) were 10 cases(3 cases: suspected pneumoconiosis,7 cases: pneumoconiosis). Chest radiographs of all cases were improved. The shape and size of small opacities was improved in 6 cases(85.7%) and did not changed in 1 case(14.3%) out of 7 pneumoconiosis welders. 2. Group II (decreased exposure to welding fume) were S cases(2 cases: suspected pneumoconiosis, 3 cases: pneumoconiosis). Chest radiographs were progressed in 2 cases(40%), did not changed in 1 case(20%), were improved in 2 cases(40%) out of 5 cases. The shape and size of small opacities was progressed in 1 case(33.3%) and was improved in 2 cases(66.7%) out of 3 pneumoconiosis welders. 3. Group III(continued expoxsure to welding fume) were 12 cases(1 case: suspected pneumoconiosis, 11 cases: pneumoconiosis). Chest radiographs were progressed in 9 cases(75%), did not changed in 3 cases(25%) out of 12 cases. The shape and size of small opacities was progressed in 1 case(9.1%) and did not changed in 10 cases(90.9%) out of 11 pneumoconiosis welders. 4. The average duration for development into suspected pneumoconiosis was 6.6 years and for progression of each one category after that was 2.2 years(p<0.01). The radiological appearance of pneumoconiosis had disappeared or decreased after cessation of exposure to the welding fume. So that, early detection and control e.g., change of department of pneumoconiosis of welders by screening program will be important for medical surveillance of welders.

Analysis on the Relationships among the Total Cholesterol, Fasting Blood Sugar, Hypertension and Ischemic Heart Disease on EKG Findings (혈청 총콜레스테롤, 공복시 혈당, 혈압의 분포와 심전도상 허혈성 심질환과의 관련성)

  • Since the 1960s, Korea has achieved rapid economic growth, longer life expectancy, accelerated urbanization and a westenization of diet. Cardiovascular disease has become the leading cause of death; however the prevalence of ischemic heart disease(IHD) remains low. A cross-sectional multiphasic screening service for 18,426 persons aged $30\sim64$ years in a Medical Aid Program in Kyonggi-do Province was conducted from 1991 to 1993. Total cholesterol(TC), fasting blood glucose(FBG), blood pressure(BP), and electrocardiographic(EKG) data were collected. The result as follows ; 1. On the EKG findings, the age-adjusted prevalence of myocardial ischemia and myocardial infarction was 1.45% in men and 2.06% in women. 2. The mean blood pressure was 122.9/78.8mmHg. The age-adjusted prevalence of hypertension was 11.05% in men and 9.02% in women. The prevalence of hypertension showed increasing tendency according to age increase. In all age group, the prevalence of hypertension was higher in men than women. 3. The mean total cholesterol level was 184.4mg/dl in men and 189.2mg/dl in women. The age-adjusted prevalence of hypercholesterolemia was 4.88% in men and 5.67% in women. The total cholesterol level showed increasing tendency according to age increase, except $55\sim64$ age group in men. 4. The prevalence of hyperglycemia is 5.8%. The age-adjusted prevalence of hyperglycemia is 6.72% in men and 4.50% in women. The prevalence of hyperglycemia showed increasing tendency according to age increase. 5. On the EKG findings, the prevalence of myocardial ischemia and myocardial infarction was higher in hypertension than normal, in all age group of men and women less than 40 years-old. Only in women more t]fan 40 years-old, the prevalence of myocardial ischemia and myocardial infarction was higher in hypercholesterolemia and hyperglycemia. Nevertheless there is not statistical association between ischemia heart disease and previous risk factors in other age group, the prevalence of myocardial ischemia and myocardial infarction was higher in hypertension, hypercholesterolemia and hyperglycemia than normal. The result of this study suggest that relationships between major risk factor of ischemic heart disease and ischemic heart disease is similar to existing theory.

The resistance rate of anti-tuberculosis drug isolated from initial tuberculosis patients at a general hospital in Daejeon area (대전지역 일개 종합병원 초진결핵 환자들로부터 분리(分離)된 결핵균(結核菌)의 약제(藥劑) 내성률(耐性率))

  • According to the nationwide survey of tuberculosis from 1965 to 1995, the incidence and drug resistance rate of tuberculosis have been decreased in Korea, but the prevalence of multidrug resistance of Mycobacterium tuberculosis is still a serious problem. The purpose of this study is to investigate the drug resistance rate and pattern of tuberculosis in Daejeon from 2001 to 2008. Of the total 581cases where the drug susceptibility test was performed, resistance to at least one anti-TB drug was found in 104 cases(17.9%) of these, 68(11.7%) were resistant to at least INH and 41(7.1%) were resistant to at least RFP. Single-drug resistance was found for isolates from 37(6.4%) ; 18(3.1%) of these were resistant to INH and 5(0.9%) to RFP. Multidrug resistance, where TB was resistant to at least isoniazid and refampin, was found in 35 cases(6.0%). and Factors associated with MDR-TB included age under 40-60.The drug-resistance rate of pulmonary TB, especially MDR-TB, is higher in the initial treated patients at a private referral hospital than in those in the pubulic sector. Initial drug resistance is common and the drug susceptibility test is informative for pulmonary TB patients who have not received previous TB treatment. The need for an improved control program, coupled with early diagnosis of MDR-TB, to reduce the spread and development of resistance. Multidrug resistance rate is still problem in korea. Efforts to decrease multidrug resistance rate either independently or in cooperation with the pubulic sector will be needed.

Development of the Multidimensional Scale of Addictive Behavior for Adolescents (청소년 중독행동의 다차원적 척도 개발)

  • Purpose: This study was done to develop the multidimensional scale of addictive behavior for adolescents. Methods: The process involved construction of a conceptual framework, initial items, verification of content validity, selection of secondary items, and extraction of final items. The participants were 636 adolescents in six middle schools and four high schools. Results: Seventy items were selected for the final scale, and categorized 8 factors explaining 56.5% of total variance. The factors were labeled as game addictive behavior, shopping addictive behavior, mobile phone addictive behavior, nicotine addictive behavior, television addictive behavior, gambling addictive behavior, alcohol addictive behavior, and internet addictive behavior. The scores for the scale were significantly correlated with addictive personality and self-control. Cronbach's alpha coefficient for the 70 items was .94. Scale scores identified adolescents as addictive behavior group, risk group, and average group. Conclusion: The above findings indicate that the multidimensional scale of addictive behavior for adolescents has good validity and reliability when used with adolescents. More importantly, it provides the first step toward developing a addiction prevention program. Additionally the scales provide an education or guideline, and proper physical and mental health management of youth in research and practice for the promotion of education.

A Study on the Factors Giving Effects to Patient's Satisfaction in the Selection of Implant Operation (임플란트 시술선택에 있어 환자 만족도에 영향을 미치는 요인)

  • The study performed questionnaire to the patients receiving implant operation, who visited dental clinics and hospitals(2 hospitals and 5 clinics) from Jul. 1, 2009 to Sep. 12, 2009 in order to study the factors giving effects to patient's satisfaction in the selection of implant. 145 questionnaire papers collected from patients were analyzed with SPSS12.0 program. The result revealed that the satisfaction with implant operation was usually normal when implant was operated because of tooth extraction due to dental caries and periodontal disease and it was usually high when implant was operated because of wound(P<0.05). When considering each cause of tooth extraction, the satisfaction with pronunciation function was usually normal(P<0.001), the satisfaction with functional part including foreign body sense was very low when the cause of operation was periodontal disease(P<0.01). The satisfaction with hospital environment depending on the cause of hospital selection was usually high when hospital was selected because of doctor's fame. It was low when hospital was selected because it was close to home(P<0.001). The satisfaction with the number of hospital visits depending on the number of implanted teeth was usually low when many teeth were implanted(P<0.01). The analysis on the satisfaction with tooth shape and color showed that the satisfaction was high when '1~2' teeth and 'more than 6' teeth were implanted(P<0.05). In order to improve patient's satisfaction with implant, it is necessary to recognize individual's oral status and request correctly. It is determined that proposal of operation period, number of hospital visits, function and expected aesthetic satisfaction degree will contribute to the improvement of mutual reliance between doctor and patient and post-operation satisfaction degree.

A study on necessity of education and safety awareness to high school students when accessing laboratories (고등학생들의 연구실 이용에 따른 안전 인식 및 교육 필요성에 관한 연구)

  • The purpose of this study is to investigate the necessity of safety awareness and education regarding the use of laboratories, and to foster safety awareness during intramural and extracurricular activities in high schools. The study conducted a questionnaire to 290 eleventh grade students of three high schools located in Seoul. The collected data were statistically analyzed. The result illustrates that 64.8% of students have experienced safety education during a regular class, 6.6% have experienced accidents, and 68.0% have experienced safety education during field activities. The result concludes that there is a lack of prior safety education as well as overall low awareness on the necessity of safety education. In particular, students who have experienced safety education recognizes the necessity of emergency education regarding laboratory accident more than those who did not receive such education. Thus, a systematic management of laboratory safety education and multilateral management measures for students' safety are required. There is a relatively high correlation between safety intention of R&D and Education factors and Community and Laboratory Safety Education. As much as there is a high safety demand regarding the laboratory instruments and experiment, it shows high intention of safety lab operation as well as count measurement of lab accident. This study recommends developing a laboratory safety education program and management methods appropriate for high school students.

Blood Pressure, Salt Threshold, Salt Preference, Urinary Excretions and Nutrition Knowledge About Blood Pressure of Elementary School Children in Rural Area (농촌 국민학교 아동의 혈압, 짠맛에 대한 역치, 최적 염미도, 뇨 중 배설성분 및 혈압에 관한 영양지식)

  • The purpose of this study was to observe the relationship of dietary factors to blood pressure in 5th and 6th grade school children. Salt threshold, salt preference and nutrition knowlege about blood pressure were tested. Twenty-four hour urines were collected for the measurements of the volume and concentrations of sodium, potassium, chloride, calcium, phosphorus, creatinine and urea nitrogen. 1) Mean systolic and diastolic blood pressure of elementary school children were 106.8/67.6mmHg in males and 108.7/69.5mmHg in females. Seven children(4%) of total subjects were found to be hypertensive. Their mean blood pressure was 130.0/86.4mmHg. 2) Urinary excretions of creatinine and urea nitrogen during twenty-four hours were 621.1mg and 1524mg, respectively. The mean daily urinary sodium excretion was 2940mg, which is equivalent to 7.37g NaCl. 3) The daily urinary excretions of other minerals were as follows; potassium was 1301mg, chloride, 4991mg, calcium, 55.7mg and phosphorus, 700.03mg. 4) Salt preference of hypertensive children90.473$\pm$0.275) was significantly higher than those of normotensive children(0.473$\pm$0.213%), but salt threshold was lower in hypertensives(0.105$\pm$0.04%) than normotensives(0.081$\pm$0.022%) (p<0.05). INdices for assessing obesity, such as body weight, triceps skinfold, weight for heigth and body surface area, were high in hypertensive children than normotensive children(p<0.05). But there was no significant difference in energy and nutrient intakes between two groups. 5) Various anthropometric measurements had positive correlations with blood pressure, but body muscle mass(%) had a negative correlation with blood pressure. And daily energy and nutrient intakes were not related to blood pressure. 6) Blood pressures-both systolic and diastolic-were significantly correlated with urinary excretions of potassium and chloride. Daily sodium excretion was related to systolic blood pressure, but no related to diastolci blood pressure. There was no relationship of salt threshold to salt preference, and neither threshold nor preference was related to blood pressure. Results of this study suggest that nutrition education program for children including the information about desirable food habits for the prevention of hypertension should be developed.
