Workplace Health Promotion in Thai Occupational Health Nursing

  • The increase of health care expenditure for Thai worker calls for the need of workplace health promotion. The purpose of this article is to describe the status of workplace health promotion in Thailand, emphasizing the roles of occupational health nurse. Secondary data analysis and extensive literature reviews were conducted. Results showed that Thailand is committed with implementing health promotion concepts in various settings including workplace. Several public organizations have developed national workplace health projects with different strategies and approaches. Role of occupational health nurses in workplace health promotion has gradually expanded. The new law specifying the functions of occupational health nurse in providing comprehensive health services is in the process. Occupational health nursing standard as related to workplace health promotion has been developed. A research based case study on workplace health promotion program is also presented to elaborate the proactive roles of occupational health nurse. Findings of this study suggest the transitional roles of Thai occupational health nurses in which training and technical supports from related organizations are in need.

선원유관단체의 선원 보건의료실태 인식도 조사 (A Research on the Perception Level of Seafarer Related Organizations in Seafarer's Actual Health Care Conditions)

  • 이 연구는 선내의료 및 선원 건강관리 여건 개선 방안을 마련하기 위하여 선원 건강관리 정책에 영향을 미치는 정부를 비롯한 선원 유관단체 종사자 139명을 대상으로 선원들의 건강과 질병관리에 대한 보건의료실태에 대한 인식도 조사를 실시하였다. 조사결과 선원들의 건강관리여건(p<0.01)과 건강관리 능력(p<0.01), 현행의 무선응급의료 정책(p<0.01), 선내 의료함 및 의료장비의 도움(p<0.05) 및 선박 의료관리자 제도에 대해 유관기관 간에 인식도 차이는 있었으나 대체적으로 부정적인 응답률이 높았으며, 선원 건강관리를 위한 선내의료 지원제도 개선을 위해서는 간이 건강측정장비 비치, 정기적 건강검진 강화, 원격의료 및 건강관리를 위한 선원건강증진센터의 설치(p<0.01)가 시급히 필요하다고 조사되었다.

보육교사와 학령전기 아동 어머니의 아동 건강증진 교육요구 (Needs for Children's Health Promotion Education as Perceived by Child Care Center Teachers and Mothers)

  • Purpose: The purpose of this study was to determine the needs for children's health promotion education programs as perceived by child care center teachers and mothers. Methods: This study conducted a survey of 88 child care center teachers and 70 mothers of preschool children in Seoul from January 4 to February 5, 2016. This study aimed to characterize the current conditions of health promotion education for preschool children and the needs for health promotion education as perceived by child care center teachers and mothers of preschool children. Results: Areas of high need for health promotion education included lifestyle improvements for preventing diseases in children and awareness of the importance of health in educational objectives, standardized educational manuals, health educators as educators, child care centers as educational places, local health centers as educational support organizations, regular class hours as educational time, role play for training, and actual models in the educational medium. The educational subjects for which a high need was reported included safety and accident prevention, the role of smartphones and TV watching in mental health, and personal hygiene and disease prevention. Conclusion: These findings suggest that it is necessary to develop a health promotion education program for preschool children.

Capability for Change at Community Health Centers Serving Asian Pacific Islanders: An Exploratory Study of a Cancer Screening Evidence-based Intervention

  • Background: Understanding and enhancing change capabilities, including Practice Adaptive Reserve (PAR), of Community Health Centers (CHCs) may mitigate cancer-related health disparities. Materials and Methods: Using stratified random sampling, we recruited 232 staff from seven CHCs serving Asian Pacific Islander communities to complete a self-administered survey. We performed multilevel regression analyses to examine PAR composite scores by CHC, position type, and number of years worked at their clinic. Results: The mean PAR score was 0.7 (s.d. 0.14). Higher scores were associated with a greater perceived likelihood that clinic staff would participate in an evidence-based intervention (EBI). Constructs such as communication, clinic flow, sensemaking, change valence, and resource availability were positively associated with EBI implementation or trended toward significance. Conclusions: PAR scores are positively associated with perceived likelihood of clinic staff participation in cancer screening EBI. Future research is needed to determine PAR levels most conducive to implementing change and to developing interventions that enhance Adaptive Reserve.

치과 의료서비스 질이 재이용의사에 미치는 영향 (Factors of the Quality of Dental Care Service Upon Intention to Revisit)

  • Purpose: Identify the factors that influence following up control and reuse intention of patient who used dental health care institution after dental heath care service and in order to prepare the method which improve the quality of dental health service which dental heath care institution service afford. Methods: Data were collected through random sampling from June 20th to August 20th 2010 (for 60 days). Once we explained the purpose of our survey to people who experienced the dental service within one year, we distributed the questionnaires to someone who volunteered to respond and they answered all questions by themselves based on the actual experience of dental health care organizations. Even if the survey was conducted for 610 people, only 585 properly answered questionnaires were analyzed because responses which had many unanswered questions and had errors in responsive way were excluded. Results: Result of multiple regression analysis, the value of dental clinic service, the following up control after dental treatment, the technique and kindness of dentist, the environment of treatment, type of dental service and the kindness of dental staff is significant main cause to intention of reuse dental clinic. Conclusion: In order to increase the rate of patient reuse, enhance the value of service with following up control after health treatment and the high quality of dental health service.

의료기관의 조직 책무성 : 분석을 위한 모형 개발 (Organizational Accountability in Health Care : Developing a Model for Analysis)

  • Past studies on organizational accountability have had similar limitations. First, empirical evidence of organizational accountability is rare as the majority of research takes a conceptual approach of the topic. Only a few of these studies are applicable to health care organizations (HCOs). To fill these gaps, we attempted to develop a model for analysis of organizational accountability for HCOs. Accountability for HCOs was conceptualized by two axes: answerability(X, horizontal) and value-creation(Y, vertical). Our concept building could relieve competing accountability mechanism which past studies stressed. Four elements of accountability(legal, economical, social, and clinical) were applied to specify each of the two features of organizational accountability. And then four types of accountability behavior were coordinated by this x-y axis : high A/high VC, high A/low VC, low A/high VC, low A/low VC. Finally, a multidimensional model of HCOs' accountability, enabling an empirically testable multi-level analysis, was proposed.

간호 연구와 조직 이론의 통합적 접근: 동향과 함의 (The Application of Organization Theories to Nursing Research: A Critical Review and Implications)

  • This article explores the potentials of the application of organization theories to nursing research on health care organizations in Korea. The critical review of the literature reveals that the important topics on organizations in health services research have been also dealt with by nursing researchers. Notwithstanding this theoretical closeness between the two fields, some issues remain: the paucity of attention to 'level issue', which requires to increase the attempts at 'meso-' or 'macro-' analysis on nurses' behavior in organizations; the need for theoretical frameworks specifying organizational factors relating to nursing processes; the necessity of interdisciplinary approaches to bridge the recent developments of organizational study and nursing research. This article finishes with a few of research agendas for future research to fill these gaps.

지역공공보건조직의 역량과 조직성과 (Organizational Capacity and Performance of Local Public Health in Korea)

  • Objectives: The purpose of this study was to investigate the differences of capacity of local health organization to regional characteristics and the influence of organizational capacity on organizational performance. Methods: The study used the secondary data for 160 local public health organizations from $5^{th}$ Community Health Plans and 2009 Community Health Survey. The collected data were analyzed using one-way ANOVA and multiple regression analysis. Results: Work force and budget showed differences in regional size and elderly population rate. And consumer satisfaction and health care utilization showed differenced in work force and budget. The regression model with total number of employee, number of registered nurses, number of doctors and budget against consumer satisfaction was statistically significant (F=14.70, p=<.001), and number of registered nurses was identified as a factor influencing consumer satisfaction. This model also explained 20.5% of service satisfaction. The regression model for consumer satisfaction was statistically significant (F=45.98, p=<.001), and total number of employee nurses was identified as a factor influencing health care utilization. This model also explained 53.1% of utilization. Conclusions: The findings of this study imply that organizational capacity as work force and budget should be increased to improve the organizational performance as consumer satisfaction and health care utilization.

서울지역 가정간호사의 업무수행능력 실태조사 (A Study on the Nursing Performance of the Home Care Nurses in Seoul)

  • The purpose of this research was to find out the state of the nursing performance of the home care nurses in Seoul in order to provide the data for the practical work guide. The data were collected from fifty home care nurses working in 22 institutions located in Seoul. The research tool used for this research was modified and tested by Song Jong-Rye(1999) which was originally modified the standard tool of American Nurses Association (1998) and was partly supplement and adjusted for this research. The reliability of this tool was Cronbach's $\alpha$=.0982. The collected data were statistically processed using SAS for t-test, ANOVA. $Scheff\'{e}$ test. Among the surveyees, 50% have been working as a home care nurse more than three years and 74.0% of them were married. And 52% have B.S degree with certification of home care nurse from the certification program for Home Care Nurses affiliated at nursing schools. Half of the home care nursing organizations were based on the general hospitals, and most(78%) of the responsible senior personnels of those organizations were nurses. The following results are drawn from this research. 1) The level of nursing performance by the task sectors General performance level of home care nurses was relatively high in grade of 3.06 from total 4.00. Among the task sectors. the ethical field scored the highest points, and the next were nursing intervention, professional training, and data gathering, and the research sector scored the lowest point. 2) The level of nursing performance by general features of home care nurses Statistically relevant correlation between performance according to the duration of working experience as a nurse(p=0.8951) and performance according to the duration of working as home care nurse(p=0.2263) did not emerge. Also, performance by marriage status(p=0.2218), education(p=0.5733), and taking the certification program for home card nurse(p =0.1560) has no statistically meaningful correlation. 3) The level of nursing performance by the type of home care nursing organizations There exists a significant difference(p=0.002) between performances by the types of organizations. Most of the responsible senior personnels of the home care nursing organization were nurses. The level of nursing performance of the home care nursing organization under nurse management was relatively higher than that of home care nursing organizations led by medical doctors or non-medical professional, but this was not proved as statistically meaningful(p =0.3617). 4) The level of nursing performance by task sectors according to the characteristic of home care nursing service organization There exists a significant difference between nursing performances by task sectors according to the characteristics of home care nursing organization(p=0.002). In case of model research center of one College of Nursing, the nursing performance in the sectors of organization, theory, and data gathering were lower than that of in hospital based home care nursing service. And in case of local home care centers, performances in sectors of organization, theory, data gathering, nursing intervention. professional training, and research sectors were significantly low. Based on the obtained results, overall performance of home care nurses can be appraised as relatively good. Especially, performances in sectors of the nursing intervention, nursing plan. and data gathering including the in direct nursing were recorded high scores. From this, it can be concluded that high quality of nursing is relatively practicing for home patients at these days. Since the high quality of nursing for patients was directly related to the level of nursing performances of home care nurses, it is required to improve practical performance level of them by making constant evaluation and running continual education program and supplementing curriculum for the sectors with low scores.

영남 지역 중년여성의 화장품 사용 및 피부관리 실태 파악 연구 (The Study of the use of Cosmetics and Skin Care of Middle-aged Women in Young-nam Province)

  • A study on skin care of middle-aged women was conducted to develop proper skin care program. The study was performed from January 25, 2003 through March 24, 2003 by using questionnaires. The subjects were 380 from some area in Youngnam province. All subjects were females from 40 to 59 years. The results of this study are summarized as follows. 1. According to general characteristics of the subjects, 36.8% of them was from 40 to 44years old; 61.3% 'high school graduate' ; 61.1% 'housewife';89.2% 'married' ;76.1% 'middle class' ;55.8% 'living in small or medium sized city'. In the case of physical health conditions, 55.8% of the respondents was in good health and 63.9% answered that their face skins were healthy. Those who don't drink were 54.5%, and those who don't smoking were 92.9%. 2. In case of the knowledge level of skin care, it showed significant difference in the variables of education, occupation, and economic status. According to the knowledge level of skin care by physical health conditions, it was highest in the case of physically healthy respondents(7.77 point). Those who do not smoke marked higher knowledge level of skin care than those who smoke. Those who exercise also marked higher knowledge level of skin care, than those who don't exercise. 3. 86.8% of the subjects experienced the massage at home. It showed significant difference in the variable of education and place of residence. The knowledge and habit of skin care of middle-aged women showed significant difference according to the variable of education, economic status, and place of residence. Therefore, related professionals and organizations relating to the skin care must make efforts to develop education program for effective skin care for middle-aged women in order to enhance the knowledge level of skin care and information about skin health.
