식품산업체에서 확률분포 모델을 이용한 불규칙적인 미생물 수 분포 활용 (Application of the Fluctuating Microbial Counts Using Probability Approaches in Food Industries)

  • 식품산업체에서 검사한 자료의 일반세균수는 대부분 불규칙 분포를 나타낸다. 최근 이러한 불규칙적인 경향에 대하여 미생물수의 lognormal 분포 특성을 이용하는 확률분포 모델을 적용함으로써 좀더 정확한 변화의 특성을 밝히고 있다. 확률분포 모델을 이요하면 일반세균수와 대장균군 등의 분석결과에 대해 과거의 단순한 경향분석을 벗어나, 유용한 분석이 가능한 것으로 나타나고 있다. 본 여구는 냉동만두류를 생성하는 2개 업체의 각각 1년 동안 실험된 일반세균 자료를 갖고, 확률분포 모델 활용의 타당성과 업체간의 위생수준 상호비교 그리고 예상되는 위해발생 예측에 적용 검토하였다. 그 결과 정량적인 미생물 자료에 대한 확률분포 모델의 적용은 식품산업체에서 일반세균수에 의한 위해발생 가능성을 예측하고 안전관리 수준을 결정하는데 유용한 것으로 판단된다.

예방처치경험이 진료만족도 및 치과공포도에 미치는 영향 (The Effect of Preventive Treatment Experience on Treatment Satisfaction and Dental Fear)

  • 본 연구는 성인의 예방처치경험 여부가 진료만족도 및 치과공포도에 미치는 융복합적인 영향을 확인하고자 수도권에 거주하는 20세 이상 성인 292명을 대상으로 2017년 1월부터 4월까지 설문조사를 실시하였다. 수집된 자료는 SPSS 22.0 프로그램을 이용하여 $x^2-test$, t-test(검증), 다중회귀분석(multiple regression analysis)을 실시하였다. 성인의 예방처치경험은 불소도포가 가장 많았으며, 불소도포경험이 있는 경우 진료만족도가 높게 나타났다. 한편 예방적 스케일링경험이 있는 경우 치과 공포도는 낮게 나타났다. 다중회귀분석을 한 결과 진료만족도에 영향을 미치는 변수는 불소도포경험이었으며, 치과공포도에 영향을 미치는 변수는 구강건강상태, 예방적 스케일링경험으로 나타났다. 본 연구의 결과는 예방처치경험이 있는 경우 진료만족도는 높아지고 치과공포도는 낮아지는 것을 확인하였으므로 구강건강교육을 통한 예방처치의 중요성을 국민에게 인식시키고 치과예방처치 서비스를 제공함으로써 구강건강증진 향상을 기대할 수 있을 것이다.

ICR mouse에 있어 UVB조사로 유도된 접촉 과민반응에 대한 EGb 761의 억제 효과 (Prevention of Ultraviolet B Radiation (280-320 nm) Induced Contact Hypersensitivity by EGb 761)

  • Exposure of skin to UVB radiation can cause the induction of inflammation and impairment of contact hypersensitivity(CHS) response. Several studies have shown that polyphenolic compounds isolated from EGb 761 afford protection against UVB. In this study, we demonstrated that topical application of EGb 761, before 1MED(1.4 KJ/$m^2$), 1.5MED (2.1 KJ/$m^2$), 2MED (2.8 KJ/$m^2$) of UVB exposure to ICR mice prevented UVB-induced inflammation and inhibition of the contact hypersensitivity response. The skin-fold swelling from 1MED, 1.5MED, 2MED of UVB exposure highly significantly increased after twice irradiation. Topical application of EGb 761(0.1%, 1%, 4%), 5 days prior to UVB exposure reduced skin thickness compared to non-treated mice. Exposure of shaved abdominal skin of mice to 1MED, 1.5MED and 2MED of UVB radiation resulted in suppression of contact sensitization through the skin to 56.23%, 65.12%, 74.02%, compared to normal unirradiated skin. Topical application of EGb 761(0.1%, 1%, 4%), 5 days prior to or 5 days after exposure to 1MED and 2MED of UVB resulted in protection against suppression of contact hypersensitivity in mouse dorsal skin. These protective effects were dependent on the dose of EGb 761 employed. The present study show that EGb 761 protect UVB-induced inflammation and immune suppression. Also, we suggest that EGb 761 can provide protection from photoimmunosuppression.

의료서비스의 대기시간 요인이 고객만족과 서비스 가치에 미치는 영향 (A Study on the Effect of Waiting Time factor of Hospitals on Customer Satisfaction and Service Values)

  • One of the important complaint factors by the patients using medical institutions is various kinds of waiting time. This research is about the causal relationships among waiting time(application of waiting time, perceived of waiting time, fairness of waiting, explanation of the delay, acceptance possibility), service values, and customer satisfaction. The subjects for this study were 256 out-patients currently using seven general hospitals in the Busan area. The findings of the empirical analysis are as follows: First, the good application of waiting time or fair waiting time procedure didn't actually affect perceived waiting time. Second, though the application of waiting time didn't affect acceptance possibility, the fair procedure of waiting time had an affirmative effect on acceptance possibility. Third, even when there was enough explanation about the need to wait, it could shorten perceived waiting time but failed to get sufficient sympathy or agreement from the patients. Fourth, the possibility of acceptance following the sufficient sympathy of patients had an effect on shortening perceived waiting time. Fifth, perceived waiting time lowered customer satisfaction, but had no negative effect on service values. Finally, the ranges of acceptance possibility by the patients were found to increase customer satisfaction and maximize service values. In conclusion, every medical institution needs to focus on, first of all, shortening waiting time for more customer satisfaction and improved service values.

환자안전문화 측정을 위한 설문조사 수행 및 결과 활용 기법 (A Strategy for Administration and Application of a Patient Safety Culture Survey)

  • Objectives : A safety culture is the bedrock for all patient safety improvement initiatives; thus, many resources have been invested in measuring hospital culture. However, many of these endeavors have failed to yield meaningful results. This article proposes a practical checklist to ensure successful administration of a safety culture survey and describes current methodologies for analyzing survey results to develop safety improvement programs. Methods : We reviewed currently used safety culture surveys and summarized their strengths and weaknesses. We also reviewed studies using safety culture surveys and found several pitfalls leading to failure in survey administration. With this information, we developed a checklist that covers critical items in the survey process. We also reviewed newly developed methodologies for survey results analysis and application and described them using the Korean version of the Safety Attitudes Questionnaire as an example. Results : The checklist consists of three steps: survey preparation, administration, and analysis and application. Each step contains clear action items. The content even describes how to get buy-in from hospital executives and manage communication channels with them. Also, common misunderstandings regarding survey scores are described and possible solutions are suggested. In the analysis section, we demonstrate new methods for obtaining more accurate survey results and how to utilize these methods to develop and implement hospital-wide safety improvement programs. Conclusion : A successful safety culture survey is the foundation of all future safety improvement projects. This review is intended to guide hospitals in enhancing safety.

시설재배 딸기의 생산단계에서 살충제 잔류량에 따른 반감기 및 감소상수 산출에 의한 안전성 평가 (Residue Dissipation Kinetics and Safety Evaluation of Insecticides on Strawberry for the Harvest Periods in Plastic-covered Greenhouse Conditions)

  • BACKGROUND: Dissipation patterns of insecticides (acequinocyl, clothianidin, diflubenzuron, thiacloprid, and thiamethoxam) on strawberry grown in plastic-covered greenhouse were evaluated at 7 intervals including the preharvest interval after application. This study was performed to determine the residue concentrations, half-lives and dissipation rates in strawberry for the harvest periods. METHODS AND RESULTS: Acequinocyl, clothianidin, diflubenzuron, thiacloprid, and thiamethoxam were applied in accordance with critical good agricultural practices for strawberry. Strawberry samples were collected at 0, 1, 2, 3, 5, 7 and 10 days after application. Quantitaion was performed by HPLC/DAD and HPLC-MSMS system with C18 column. The limit of quantitation (LOQ) values varied between were 0.00025~0.05 mg/kg on strawberry. The recoveries of acequinocyl, clothianidin, diflubenzuron, thiacloprid and thiamethoxam at two concentrations ranged from 70.9~104.9% on strawberry. The half-lives of pesticide dissipation on strawberry for two fields ranged from 1.3~8.2 days. The dissipation rates were evaluated according to the statistics method with a 95% confidence. CONCLUSION: The residue levels of insecticides (acequinocyl, clothianidin, diflubenzuron, thiacloprid, and thiamethoxam) on strawberry at 0-day after application were below the specified values by Korean MRL. To estimate dissipation rate constant of pesticides on crop for harvest periods, it is important to set the pre-harvest residue limit for human health and consumer protection in Korea.

전과정평가에 있어 확률론적 건강영향분석기법 적용 -Part II : 화학제품의 환경부하 전과정평가에 있어 건강영향분석 모의사례연구 (Application of Probabilistic Health Risk Analysis in Life Cycle Assessment -Part I : Life Cycle Assessment for Environmental Load of Chemical Products using Probabilistic Health Risk Analysis : A Case Study)

  • Health risk assessment is applied to streamlining LCA(Life Cycle Assessment) using Monte carlo simulation for probabilistic/stochastic exposure and risk distribution analysis caused by data variability and uncertainty. A case study was carried out to find benefits of this application. BTC(Benzene, Trichloroethylene, Carbon tetrachloride mixture alias) personal exposure cases were assumed as production worker(in workplace), manager(in office) and business man(outdoor). These cases were different from occupational retention time and exposure concentration for BTC consumption pattern. The result of cancer risk in these 3 scenario cases were estimated as $1.72E-4{\pm}1.2E+0$(production worker; case A), $9.62E-5{\pm}1.44E-5$(manger; case B), $6.90E-5{\pm}1.16E+0$(business man; case C), respectively. Portions of over acceptable risk 1.00E-4(assumed standard) were 99.85%, 38.89% and 0.61%, respectively. Estimated BTC risk was log-normal pattern, but some of distributions did not have any formal patterns. Except first impact factor(BTC emission quantity), sensitivity analysis showed that main effective factor was retention time in their occupational exposure sites. This case study is a good example to cover that LCA with probabilistic risk analysis tool can supply various significant information such as statistical distribution including personal/environmental exposure level, daily time activity pattern and individual susceptibility. Further research is needed for investigating real data of these input variables and personal exposure concentration and application of this study methodology.
