  • This study aims to provide an opportunity to enhance public health through suggesting development of Health Management program for industrialization of Health Management Field in Oriental Medicine. Industial Model for Health Management Field in Oriental Medicine has been Development with following characteristics. Allowed easy application of Oriental Medicine through everyday life methods. Efforts were made to industrialize and generalize Health Management Field in Oriental Medicine by shifting emphasis to mass training and edacation from individual training. This will enable Oriental Medicine to participate in the public health program of 21st century and hence contribute to positive overall public health model.

The Effects of Smoking Cessation Clinics in the Korean Armed Forces: Application of the Transtheoretical Model and Life Skill Approaches (범이론적모형과 생활기술접근법을 통합적용한 군인.전의경 대상 금연클리닉프로그램의 효과)

  • Objectives: The aim of this study was to evaluate the effectiveness of smoking cessation clinics for soldiers in the Korean armed forces. In so doing it attempted to identify the predictors of smoking cessation and positive transition of stages of change. Methods: In a single-group pre-post test study design, 5,362 smokers from Korean armed forces participated in the smoking cessation clinic programs based on both the Transtheoretical model and life skills approaches. Changes in stages of change and related variables were measured before the intervention and at 3-month follow-up after the intervention. Results: The cessation rates was 66.0% at 3-month follow-up. With the positive transition to action/maintain stage, proportion of participants in action/maintain stage increased more than 5 times after the intervention. Cons of smoking, self-efficacy, processes of change, and life skills increased, and pros of smoking decreased after the intervention. Predictors of smoking cessation included nicotine dependency, pros of smoking, high levels of self-efficacy, processes of change and life skills. Positive stage transition was predicted by pros of smoking and high self-efficacy. Conclusions: Application of the Transtheoretical model and life skills approaches provided useful strategies for a smoking cessation program in armed forces.

  • This paper reviews the available published literature on the antioxidant, anti-inflammatory, and COX-2 inhibition properties of phycocyanin from Spirulina. The potential application of for the prevention and mitigation of such radical-induced chronic diseases like cancer and heart disease, and age-related degenerative diseases like Alzheimer's disease, Parkinson's disease, diabetes and other conditions are discussed based on the available evidence.

Effects of Cerebrovascular Disease Prevention Program Designed for Workers in Small Scale Workplaces (소규모 사업장 근로자에 대한 뇌.심혈관질환 예방 프로그램 적용 효과)

  • Objectives: This study was conducted in order to prepare fundamental data and assess the short-term effects of applying cerebrovascular disease prevention programs by the Ministry of Employment and Labor and the Korea Occupational Safety and Health Agency (KOSHA) in Korea. Methods: The number of study subjects was 2,676 workers (58.5%) who were able to evaluate the level of incidence risk at pre- and post-applications of the program, among the 4,576 total workers who were enrolled in the cerebrovascular disease prevention program during 2011. The guidelines for this prevention program were adopted from KOSHA GUIDE H-1-2010. To determine the program' effectiveness, the workers'risks for cerebrovascular disease were assessed pre- and post-application of the program. Results: The blood pressure level was significantly reduced by 4.09 mmHg for the mean systolic blood pressures and 5.47 mmHg for diastolic blood pressures, respectively. The mean level of total cholesterol and BMI were also reduced significantly by 2.07 g/dl and 0.1 $0.1kg/m^2$. The rate of smoking was decreased by 4.0% and the percentage of workers engaging in regular exercise was increased by 29.8%. The level of overall cerebrovascular disease risk was reduced among 1,451 (70.7%) of 2,052 workers at low risk and above. The level of cerebrovascular disease risk was lower in the improved group for health behavior than the unimproved group (odds ratio =1.7, p < 0.001). Conclusions: The author considers that the application of the cerebrovascular disease prevention program by the Korea Occupational Safety and Health Agency reduced cerebrovascular diseases risks among workers who were enrolled in the cerebrovascular disease prevention program and it must be accompanied by an improvement in health behavior for prevention of cerebrovascular disease.

An Implementation Analysis of the National Health Insurance Coverage Expansion Policy in Korea: Application of the Winter Implementation Model (건강보험 보장성 확대정책의 집행분석: Winter의 정책집행모형의 적용)

  • Background: Most studies on the national health insurance benefit expansion policy have focused on policy tools or decision-making process. Hence there was not enough understanding on how policies are actually implemented within the specific policy context in Korea which has a national mandatory health insurance system with a dominant proportion of private providers. The main objectives of this study is to understand the implementation process of the benefit coverage expansion policy. Unlike other implementation studies, we tried to examine both the process of implementation and decision making and how they interact with each other. Methods: Interviews were conducted with the ex-members of the Health Insurance Policy Review Committee. Medical doctors who implement the policy at the 'street-level' were also interviewed. To figure out major variables and the degree of their influences, the data were analyzed with Winter's Policy Implementation Model which integrates the decision making and implementation phases. Results: As predicted by the Winter model, problems in the decision making phase, such as conflicts among the members of committee, lack of applicable causal theories application of highly symbolic activities, and limited attention of citizen to the issue are key variables that cause the 'implementation failure.' In the implementation phase, hospitals' own financial interests and practitioners' dependence on the hospitals' guidance were barriers to meeting the policy goals of providing a better coverage for patients. Patients, the target group, tend to prefer physicians who prescribe more treatment and medicine. To note, 'fixers' who can link and fill the gap between the decision-makers and implementers were not present. Conclusion: For achieving the policy goal of providing a better and more coverage to patients, the critical roles of medical providers as street-level implementers should be noted. Also decision making process of benefit package expansion policy should incorporate its influence on the implementation phase.

The Development and Application of Core Fundamental Nursing Skills Improvement Education Program for New Nurses in Comprehensive Nursing Care Ward (간호·간병통합서비스 병동 신규간호사 대상 핵심기본간호술기향상 교육프로그램 개발 및 적용)

  • Purpose: The purpose of this study was to investigate the effect of the development and application of the new nurses' core skill program on the clinical skill, confidence, knowledge and performance of new nurses. Methods: This study was a nonequivalent control group post-test design on 46 new nurses with 3 to 6 months work experience in a general hospital. Data were collected from August 2015 to August 2016. A training program of core fundamental nursing skills was applied on 23 new nurses from 2016 in the experiment group, and an existing training method was applied on 23 new nurses from 2015 in the comparison group. The collected data were analyzed using SPSS/WIN 23.0. Results: The experimental group ($89{\pm}4.29$) showed higher score in practical skill assessment compared to the control group ($85.09{\pm}3.99$) and showed a statistically significant difference (p<001). Conclusion: A training program of core fundamental nursing skills for new nurses proved effective in improving practical skills. Further research on the development of a systemic training program that can improve clinical nursing knowledge and work ability for new nurses according to the extended enforcement of a Nursing Care Integrated Service ward is needed.

Health related behavior patterns and associated factors among marriage immigrant women using latent class analysis (잠재계층분석을 활용한 결혼이주여성의 건강관련행동 군집유형과 영향요인)

  • Objectives: This paper aims to identify the health related behaviors patterns and its associated factors among marriage immigrant women in Korea, and discusses their application to health promotion strategies. Methods: The study participants were 7,591 immigrant wives in Gyeonggi province who participated in health examinations conducted by the Korea Association of Health Promotion in 2011-2013. The participants completed self-administered questionnaires on sociodemographics, psychological characteristics, health status and health care factors, and health related behaviors. Results: A 3-latent-class model of health behaviors was identified related to 'lack of physical activity', 'abnormal diet', and 'not experienced medical check-up': 'high risk class', 'middle risk class', and 'low risk class'. Most of the participants belong to 'middle risk class'. Country of origin, age, length of stay, number of children, work status, health insurance status, and unmet health care needs were associated with problematic health behaviors in middle risk health behavior class. Conclusions: Health promotion and intervention programs for marriage immigrant women and their family members need to consider the health behavior patterns of physical inactivity, abnormal diet and no medical check-up and develop multiple behavior intervention with pre-existing program modification.

International Trends of Digital Health and It's Political Implication for Health Technology Assessment (디지털헬스 정책환경의 국제 동향과 의료기술평가에 대한 시사점)

  • As technologies develop, the digital health sector is gradually expanding. Internationally, the global summit for Digital Health named Global Digital Health Partnership (GDHP) was launched in 2018. Many countries are participating in GDHP and share their policy experiences on digital health and find the ways to cooperate with participating countries (13 countries, including South Korea, and Hong Kong). This article reviewed the international trends in digital health policy environment and evidence assessment focusing on GDHP activities, and derived implications for health technology assessment of digital health. Consequently, to assess the intervention effects of digital health is very complex and the assessment should be considered multidimensional aspects (social, clinical, and technical). In addition the patient experience should be assessed qualitatively. Health technology assessment (HTA) should assess the effect of digital health policies to changes in health care systems resulting from the application of advanced technologies related to the 4th Industrial Revolution. Digital health is also related to new HTA, HTA of existing technologies, and R&D on the promising health technology. Therefore, it is necessary to review the trends of the technology's management policy consistently through the HTA of digital health.

Production of Conjugated Linoleic Acid by Lactobacillus acidophilus and Their Industrial Application (Lactobacillus acidophilus의 산업적 이용과 CLA 생성)

  • Application of lactic acid bacteria in the markets are divided into four categories: dairy industry, health food industry, animal feed industry and pharmaceutical industries. Recently, Lactobacillus acidophilus have been used in the food industry and have obtained great attention as key cultures for health benefit. Since commercial application of L. acidophilus has become a common practice, characterization of these cultures were made. Futhermore, the strains selected should produce a final dairy product possessing good taste and acceptable body and texture, a selection step that cannot be achieved unless the product is actually manufactured. Conjugated linoleic acid (CLA) have been recognized as antioxidants, cancer inhibitors, cholesterol depressing agents, and growth promoting factors. Food products from ruminants, particularly dairy products, are the major dietary source of CLA f3r humans. The CLA content in yogurt or cheese can be increased by action of the starter cultures. The finding of the production of CLA by food starter culture opens interesting perspectives far the future in producing fermented products enriched in CLA.
