영양전공자의 영양지식, 식이 효능감, 식행동 변화간의 관련성 (Relationship of Nutritional Knowledge, Dietary Self Efficacy and Change of Dietary Behavior of Nutrition Professional)

  • 본 연구는 영양전공자들의 영양지식 수준, 식이 효능감과 식행동 간의 관련성을 파악하고 이들의 식행동 변화에 영양지식이나 식이효능감이 영향을 주는지 분석하고자 초 중 고등학교의 급식 영양사[(총 190명) 대전 84명(44.2%), 충남 106명(55.8%)]를 대상으로 영양지식 15문항, 식이 효능감의 4개 하부 영역으로 구성한 총 18 문항, 식행동조사로 권장 식품과 비권장 식품 총 12종의 6개월 전과 비교한 섭취량의 변화를 조사하였다. 학교급식 영양사들의 영양지식은 80.3%의 정답률을 보여 높은 편이었지만 특수 분야에 대한 정답률은 낮았다. 조사대상 영양사들은 74.7%의 자아효능감을 보여주었으며 각 영역별 효능감은 식사습관 영역 72.2%, 음식 선택 영역 85.4%, 주위 환경의 자극 영역 69.7%, 정서 상태 영역 74.6%로 음식 선택 영역의 효능감이 가장 높았고 주위 환경의 자극 영역의 효능감이 가장 낮았다. 권장 식품의 섭취량 변화에서 생선류와 채소류는 섭취량에 변화가 적은 식품류였고, 과일류와 콩류 및 된장 두부는 섭취량이 가장 많이 증가한 식품류였으며, 우유 및 유제품류는 섭취량에 증가와 감소가 비슷한 식품류였고, 미역 김 다시마 버섯류는 섭취량에 변화가 없거나 약간 감소한 식품류였다. 비권장 식품의 섭취량 변화 조사에서 대부분의 비권장 식품류들에 대해 응답자의 약 50%는 섭취량에 변화가 없었다고 응답하였지만 30% 이상은 섭취량에 감소가 나타났다고 응답하여 비권장 식품의 섭취량을 감소시키고자 노력하고 있었다. 조사 대상 급식 영양사들은 영양지식에 따라 식이효능감에 유의적인 차이를 보이지 않았으며 또한 식이효능감에 따라 영양지식에도 유의적인 차이를 보이지 않아 두 요인간에 서로 영향을 주지 않았다. 식이효능감에 따른 권장 비권장 식품의 섭취량 변화에서 식이효능감에 따라 권장식품의 섭취량에서는 유의적인 차이를 나타내지 않았지만 비권장 식품의 섭취량에서는 유의적인 차이(p<0.001)를 보여 식이효능감이 높을수록 권장하지 않는 식품에 대한 섭취량이 감소하였다. 식이 효능감의 4개 하위 영역과의 상관관계 분석 결과식사습관 영역(p<0.001)과 정서상태 영역(p<0.001)이 권장 식품 섭취량과 상관관계를 나타냈으며 비권장 식품 섭취량은 4개 영역 모두(p<0.001)와 상관관계가 있었다. 즉 식사습관과 정서상태 영역의 효능감은 특히 권장 식품의 섭취량을 증가시킬 수 있는 요인이나 모든 4개 영역의 효능감은 비권장 식품의 섭취량을 감소시킬 수 있는 요인들이었다. 이로써 식이효능감이 전문영양사들의 식행동 변화에 영향을 미칠수 있는 요인으로 나타나 학교급식 영양사를 대상으로 하는 교육에 있어서도 식이효능감을 강화시킬 수 있는 방안의 강구에 관한 연구가 필요할 것으로 사료된다.

대사증후군 환자의 영양소 섭취상태 및 식사의 질과 염증지표 농도의 상관성 (Relationship between Nutrients Intakes, Dietary Quality, and Serum Concentrations of Inflammatory Markers in Metabolic Syndrome Patients)

  • Elevated serum concentration of inflammation markers is known as an independent risk factor of metabolic syndrome (MS) and dietary intake is an important factor to control MS. The purpose of this study was to investigated the hypothesis that inflammatory indices are associated with dietary intake and diet quality index-international (DQI-I) in subjects with MS. A cross-sectional study was conducted on 156 men and 73 postmenopausal women with MS, defined by three or more risk factors of the modified Adult Treatment Panel III criteria. Serum levels of high sensitive C-reactive protein (hs-CRP), adiponectin were examined and nutrients intake and DQI-I were assessed using a semiquantitative food frequency questionnaire. The total DQI-I score was significantly higher in female subjects ($65.87{\pm}9.86$) than in male subjects ($62.60{\pm}8.95$). There was a positive association between hs-CRP and polyunsaturated fatty acid intake (p < 0.05) and a negative association between adiponectin and lipid (p < 0.05), total sugar (p < 0.01), and total fatty acids (p < 0.05). When the subjects were divided into 5 groups by quintile according to serum adiponectin and hs-CRP level, there was no association between DQI-I score and hs-CRP levels. Moderation score of DQI-I was significantly higher in highest quintile group than the lower quintile groups. Therefore, our results provide some evidence that dietary intake and diet quality are associated with inflammation markers and dietary modification might be a predictor to decrease risk for metabolic syndrome complications. However further research is needed to develop the dietary quality index reflecting the inflammatory change by considering the dietary habit and pattern of Koreans.

어린이집 아동의 영양교육을 위한 사전조사 연구 - 식습관과 영양지식을 중심으로 - (A Preliminary Study on Nutrition Education for Preschool Children in Day-Care Center - Dietary Habit and Nutrition Knowledge -)

  • 본 연구에서는 어린이집에 다니는 미취학 아동의 식습관 및 영양지식상태를 파악하여 보건소의 영양 및 건강증진사업을 위한 영양교육 자료개발에 유용한 기초자료로 제공하고자 울산시 북구에 소재한 어린이집에 다니는 아동 1,200명을 대상으로 조사한 결과는 다음과 같다. 조사대상자의 성별은 남아 429명(52.6%), 여아 386명(47.4%)이었으며, 연령은 $3{\sim}7$세로 4세 240명(29.4%), 5세 294명(36.1%), 6세 163명(20.0%)으로 대부분을 차지하였고 3세와 7세는 각각 14.0%와 0.5%로 적었다. 부모의 직업으로는 지역의 특성상 생산직 근로자(38.7%)와 사무직(33.9%)이 높게 나타났고, 교육 정도에서 아버지는 전문대 졸업(51.6%)이 가장 많았고, 어머니는 고등학교 졸업이 58.0%로 가장 많았다. 월평균 소득은 $200{\sim}300$만원 미만이 42.0%로 가장 많이 나타났으며, 다음이 $100{\sim}200$만원 미만으로 20.6%로 나타났다. 조사대상 아동의 평균 신장과 체중은 108.0cm와 18.7kg으로 한국소아협회의 연령별 체위기준인 111.0cm, 19.0kg보다 약간 낮게 나타났다. PIBW에 근거한 저체중은 14.6%, 정상체중은 54.4%, 과체중 및 비만은 31.0%로 나타났다. 아동들의 일상적인 식습관을 나타내는 식습관점수는 전체 평균 22.0점(30점 만점)으로 남아 22.0점, 여아 21.9점으로 거의 비슷하게 나타났으며, 항목별 점수에서 가장 높은 점수는 '아침을 매일 규칙적으로 먹는다'였고, 가장 낮은 점수는 '당근, 시금치등 녹황색 채소의 섭취'로 나타났다. 식행동에서는 전체 아동의 69.1%가 편식을 하였으며, 편식식품으로는 채소류가 51.2%, 다음이 육류 및 생선류로 나타났다. 간식섭취 실태에서는 피자나 햄버거 등 패스트푸드의 섭취빈도가 가장 많았고 다음이 탄산음료, 튀김류의 순으로 나타났다. 영양지식점수는 교육실시 이전의 전체 평균은 7.7점(10점 만점)으로 남아가 여아보다 약간 높은 점수를 보였고 영양교육을 4주간 실시 후에는 전체 평균점수가 8.9점으로 향상되었으며 역시 남아가 여아보다 높게 나타났다(p<0.001). 이상의 연구결과를 종합해볼 때 미취학 아동들은 저체중과 과체중 및 비만의 문제를 동시에 가지고 있었고, 식습관에서 편식이 많았으며, 간식의 섭취비율이 클 뿐만 아니라 종류에서도 인스턴트 및 패스트푸드 식품을 선호하는 것으로 나타났다. 그러므로 건전한 식습관으로 아동의 영양상태 개선과 소아비만의 예방 및 올바른 식사예절의 정착을 위한 정기적인 영양교육이 필요하다고 하겠다.

대전지역 대학생들의 주거지에 따른 식행동과 야식 섭취실태 (The Dietary and Late-night eating Behavior according to Residence Type of University Students in Daejeon)

  • The purpose of this study was to investigate the dietary behavior and late-night eating habit of university students in Daejeon. Questionnaires were collected from 432 students and statistically analyzed according to residence type. One thirds of students took meals irregularly and skipped breakfast because of no time. Both Relative self-boarding and dormitory boarding group skipped breakfast more frequently than home group(p<0.01). The carbonated drink was most frequently chosen during late-night eating. The most favorite alcoholic beverage was soju and 54.7% of students drank soju 1~2 times per week near campus. Almost half of them took late-night foods 1~2 times per week at home(51.8%), near campus(25.8%), near home(15.6%), down town(6.9%). The percentage of late-night eating near campus was higher in dormitory boarding group than both home group and relative self-boarding group(p<0.001). Late-night foods were selected based on taste in 1~2 man won and enjoyed with 2~3 friends until 12 pm. The more in relative self-boarding group chose late-night foods based on taste than both home and dormitory boarding group(p<0.05). The main intake method for late-night foods was delivery. The results of this study suggest that nutritional education is required for relative self-boarding group to change dietary behavior and to select proper late-night foods for their health.

폐경 후 비만 여성의 식이 및 운동 교육이 체지방향 및 혈중지질농도에 미치는 효과 (Effects of Nutrition and Exercise Education on Fat Mass and Blood Lipid Profile in Postmenopausal Obese Women)

  • Obesity is an independent risk factor fur coronary artery disease in the postmenopausal women, which may be mediated by alteration of blood lipid metabolism. We are aimed to evaluate the effects of low energy diet, restriction of high fat foods and exercise education on weight, fat mass and blood lipid profile. Fifteen postmenopausal obese women were studied. Subjects received detailed advice about how to achieve a reduction of weight by a low-energy diet (1,200kcal), restriction of high fat food and increased exercise in every 4 wks during 8 wk. To evaluate the effectiveness for education programs, dietary fat habit and daily nutrient intakes, exercise were tested before and after intervention. Anthropometry, computerized tomography, and blood lipid profile were assessed before and after intervention. According to the nutrition education, energy intake (from 1776.1 ${\pm}$ 28.2 kcal to 1268.7 ${\pm}$ 115.2 kcal, p < 0.001) and percent of energy from fat (21.9%, p < 0.01) were significantly decreased. However, the index of nutritional quality was over 1.0. On the contrary, exercise (from 341.3 ${\pm}$ 222.1 kcal to 569.4 ${\pm}$ 309.8 kcal, p < 0.05) was increased and dietary fat habit scores (from 30.8 ${\pm}$ 5.2 to 36.1 ${\pm}$ 3.0, p < 0.01) were improved. Also, body weight (4.2%, p < 0.001), BMI (4.5%, p < 0.001), body fat mass (3.4%, p < 0.05) and waist to hip ratio (1.1%, p < 0.05) were decreased. Among abdominal fat, visceral fat (26.1%, p < 0.05) and subcutaneous fat (14.8%, p < 0.01) were decreased. But there was no difference in visceral fat to subcutaneous fat ratio. We observed HDL-cholesterol increase (11.7%, p < 0.05), triglyceride reduction(14.8%, p < 0.05) and atherogenic index improvement (from 2.7 ${\pm}$ 0.7 to 2.3 ${\pm}$ 0.7, p < 0.05) However, there was no change in LDL-cholesterol and total cholesterol. Theses results showed that low energy diet, restriction of high fat foods and exercise education could result in reduction of body weight, fat mass, visceral and subcutaneous fat, and in improvement of blood lipid profile in the postmenopausal obese women.

초등학생의 올바른 식습관 형성을 위한 연관교과 내 식생활교육의 효과판정 -식습관 변화를 중심으로- (Effect-Evaluation on Nutrition Education in Related Curriculums for Elementary School Children -Focused on Change of Dietary Habits-)

  • The purpose of this study was to develop of nutritional education activities and effect evaluation in related curriculums for all Dongbu Elementary School children in Jinhae by the teacher in charge. This educational purpose was to build a desirable dietary behavior for optimal growth and health in elementary school children. The educational program contents were developed according to 1) selection of content related curriculums 2) analysis of contents related on dietary habits by grade 3) preparation teaching plans to build desirable dietary habits 4) development of teaching manual by turns. Twelve hours of nutrition education at all school classes were done from March 2 to December 10 2004 in Dongbu Elementary School in Jinhae, Gyeongnam. The education effects were evaluated through the questionnaire surveys before and after the education and their comparative analysis. The results of this study were summarized as follows : Forty percent of the underweight group, $22.8\%$ of the normal group, $42.2\%$ of the obese group had the wrong perceptions on their body figures, but after nutrition education $80.5\%$ of the underweight group, $94.3\%$ of the normal group, $97.9\%$ of the obese group gleaned the correct perceptions on their body figures (p < 0.001). As the effects of the education, the subjects who exercise more than 3 times in a week increased in all groups (p < 0.01-0.001), notably in the obese group. Meal regularity (p < 0.001) , skipping meals (p < 0.001), eating rate and snack selection patterns (p < 0.01-p < 0.001) also improved by nutrition education. We could also observe the effects of the nutrition education through the affirmative statements which appeared in their dietary records. These results showed a possibility of nutrition education activities in related curriculums to improve in dietary habits of school children. Next year, we will start with the system of nutrition teachers in elementary school, but nutrition teacher cannot undertake the task of teaching and food service management alone. Because of that, we intend to develop this program as an alternative proposal for the nutrition education in elementary schools. (Korean J Community Nutrition 10(6) $795\∼804$, 2005)

고등학생의 영양 관련 문제점 분석 및 영양 교육 프로그램 개발 ( II ) - 고등학생 대상 영양 교육 프로그램 개발 - (Problems Analysis Related to Nutrition and the Development of Nutrition Education Programs for High School Students(II) - A Study Centered on the Development of Nutrition Education Programs for High School Students -)

  • Previously, we analyzed for nutrition knowledge and the use of nutrition knowledge in the everyday lives of students in order to develop nutrition education programs that focus on desirable behavior change. From this, we found that female students desired to participate in nutrition education more than male students, and regarding their concerns for nutrition education, 73.2% of the females and 50.0% of the males displayed interest in 'obesity and the regulation of body weight'. Therefore, this study showed female students give more attention to the obesity and the regulation of body weight than male students(p=.000<.001). In addition, female students had higher interests($8.63{\pm}1.67$) than male students ($7.45{\pm}2.03$) in nutrition knowledge(p=.000<.001). By investigating the use of nutrition knowledge in everyday life, our research indicated that the actual use of nutrition knowledge was less. To encourage students to persue dietary lives addressing the concerns confirmed above, the following needed to occur. 1) Provide them nutrition information for the main processed foodstuffs encountered when dining out(breads, cakes, cookies, and carbonated beverage). 2) Teach them to read food nutrition labels. 3) Help them find a lifestyle connection through lasting self-management methods and the generation of social support. Accordingly, this required developing effective and practical nutrition education programs that considered regional characteristics and gender differences. The most important factors considered during nutrition education program development were the need for motivation and ongoing education by stage of change, rather than temporal education through specific problem analysis, in order that those being educated may bring about a change of behavior by themselves. Therefore, from this study, we have suggested the use of multilateral operating strategies for successful nutrition education. In addition the phase model of behavior change should be applied. Our programs were aimed at self owned nutritional management so that students could master their own methods for acquiring skills and enjoying dietary life. The research may be summed up as follows. First, the purpose of education at the recognition stage of change was to attempt motivation for nutrition improvement, by analyzing the problems such as food buying habit and the main purchasing viewpoints when dining out. Second, the purpose of education at the action stage of change was to help students acquire of concrete methods for behavior modification by linking the program to their home as well as to teachers with various activities that suited the situation at school. This was done by analyzing the processes and decisions pertaining to dining out the main processed foodstuffs and principal components, etc. through data and experimental practice. Third, the purpose of education for changing of habits and values, or the maintenance stage, was to investigate the various reasons that undesirable behaviors were induced, and then determine a lasting self-management method as well as how to generate social support. If the nutrition education program developed in this study is utilized on site, someone in the primary role as the nutrition educator and trained specifically in nutrition, can help induce the health promotion in the community as well as lasting dietary management, by executing a link with families in parallel with educating teaching staff and students' parents. In addition, this program can playa role in the government policies related to the health promotion for our youth who are the foundation of our nation and who can enhance our national competitive power.

COVID-19 팬데믹 전후 한국 성인 남녀의 건강 및 식생활행태 비교연구: 국민건강영양조사 제8기(2019-2021년도) 자료 활용 (Comparative study on the health and dietary habits of Korean male and female adults before and after the coronavirus disease 2019 pandemic: utilizing data from the 8th Korea National Health and Nutrition Examination Survey (2019-2021))

  • Objectives: This study aims to compare changes in physical factors, health behaviors, eating habits, and nutritional intake among Korean male and female adults over a period of three years (2019-2021) before and after the outbreak of coronavirus disease 2019 (COVID-19). Methods: This study utilized raw data from the 8th Korea National Health and Nutrition Examination Survey (2019-2021). The participants in this study included 6,235 individuals in 2019, 5,865 individuals in 2020, and 5,635 individuals in 2021. Individuals whose daily energy intake was less than 500 kcal or exceeded 5,000 kcal were excluded from the study. Results: In comparison to 2019, overweight/obesity rates, weight, waist circumference, weekend sleep hours, and resistance exercise days/week increased in both male and female during the COVID-19 pandemic. Regarding eating habits, the proportions of people skipping breakfast, not eating out, consuming health supplements, and recognizing nutritional labels increased in 2020 and 2021, whereas the rate of skipping dinner decreased. Total energy intake has continued to decrease for the two years since 2019. A comparison of nutrient intake per 1,000 kcal before and after the outbreak of COVID-19 revealed that intake of nutrients, including protein, phosphorus, iron, vitamin A, riboflavin, and niacin increased, while folic acid intake decreased. In male, calcium, phosphorus, riboflavin, and niacin intakes increased, whereas iron, vitamin C, and folic acid intakes decreased. In female, phosphorus, iron, vitamin A, and riboflavin intake increased significantly, while protein and niacin intake decreased significantly. Conclusions: After COVID-19, the obesity rate, breakfast skipping rate, health supplement intake, and nutritional label use increased, while the frequency of eating out, dinner skipping rate, and total energy intake decreased. These environmental changes and social factors highlight the need for nutritional education and management to ensure proper nutritional intake and reduce obesity rates in the post-COVID-19 era.

어린이 영양지수 (NQ, Nutrition Quotient) 개발을 위한 평가항목 선정 (Selecting items of a food behavior checklist for the development of Nutrition Quotient (NQ) for children)

  • The objective of this study is to select a simple and easy measurable food behavior checklist for the development of Nutrition Quotient (NQ) for children, which reflects children's diet quality, as well as to evaluate the validity of the items in the food behavior checklist. The first 36 items in the checklist were established by an expert review, modifying the preliminary 50 items in the checklist, which had been selected by a literature review and the Korean National Health and Nutrition Examination Survey data. 341 children in 5th and 6th grades at an elementary school participated in a one-day dietary record survey, and later responded to 36 food behavior questions of the checklist. Pearson's correlation coefficients between the responses to the food behavior checklist items along with the mean nutrient intakes of the children were calculated. From the result, in which responses of food frequency and food behavior items showed certain association with the dietary record data, a second checklist with 22 items was selected. A survey was conducted by using the second checklist. 1,393 children in the 5th and 6th grades at 12 elementary schools in metropolitan cities, such as Seoul, Busan, Gwangju, Daegu, Daejeon, and Incheon, participated in the survey. Further, an exploratory factor analysis was performed. After the analysis, 19 items (10 items from food frequency and 9 items from food behavior) were finalized as the food behavior checklist items for the NQ. The final 19 food behavior checklist items were composed of 5 factors: 'Balance', 'Diversity', 'Moderation', 'Regularity', and 'Practice'. This study is a significant first trial to establish a comprehensive system for evaluating children's food habit and diet quality. This checklist might need continuous modification and revision reflecting the change of children's dietary life and the social environment.

중 고등학생 대상 영양교육 목표 설정을 위한 영양 관련 문제점 분석 (Setting Instructional Goals for Nutritional Education Program Through an Analysis of Problems Identified in Junior/senior High School Students)

  • For the purpose of developing 'Web-Based Nutritional Education Program', this study analyzed nutritional problems of junior/senior high school students and set appropriate instructional goals for nutritional education program. Survey questionnaires were distributed from October 21,2002 to October 26, 2002 to a total of 564 students at three junior high schools and five senior high schools located in Seoul. The total number of questionnaires collected and used in our study was 479, which is 84.9% of the total questionnaires distributed. According to the survey results, three student groups (male and female students at junior high schools and female students at senior high schools) had lower than an ideal body weight. In case of their Basal Metabolic Indices (BMI), the female students and junior high school students showed a tendency to be underweight, while the other three student groups were within the normal range. The result of the survey showed that all four groups of students showed a deficiency in nutrient intake of calories, calcium and thiamin when compared with Korean RDA. Particularly, in the case of the female students at senior high schools, the deficiency of iron intake was a problem. The survey about food attitudes and food habits of the students showed that the senior high school students had more problems when compared with their junior high school counterparts (p < .001). Therefore, the instructional goals of nutritional education program were set as follows: 1) To improve the level of understanding and the perception of junior and senior high school students about the importance of a dietary lifestyle. 2) To change food attitudes and nutritional knowledge related to individual dietary 1ifestyle, so as to maintain good health through the enhancement of the students' physical activities. We found out that it is critically important to factor in the existing problems of food attitudes and food habits, as well as nutrient intake among junior and senor high school students when setting objectives for nutritional education programs.
