Purpose: Pink gingival esthetic especially on the anterior teeth has been an important success criterion in implant-supported restoration. Inter-implant papillae are a critical factor for implant esthetics, and various techniques for inter-implant papilla reconstruction have been introduced. The aim of this study is to suggest and evaluate a surgical technique for reconstructing inter-implant papillae. Methods: A 28-year-old man had an implant placed on the #13 and #14 area. Four months after implant placement, a second stage surgery was planned for inter-implant papilla reconstruction. At the time of the abutment connection, I-type incisions were performed on the #13i & #14i area followed by full-thickness flap elevation and connection of a healing abutment on underlying fixtures without suture. Results: Two weeks after the second stage implant surgery, soft tissue augmentation between the two implants was achieved. Conclusions: I-shaped incisions for papilla reconstruction performed during the second stage implant surgery were useful for inter-implant papilla reconstruction and showed a good esthetic result.
The dental implants for edentulous Patients have been used for more than 20 years. After the introduction of osseointegration by $Br{\aa}nemark$, the commercially pure titanium implants were accepted by most practitioners. Recently dental implants are used for orthodontic anchorages as well as prosthetic abutment. Many researchers have reported implants as a good orthodontic anchorage through basic research and clinical evaluation. But previous researches were done after the healing time for osseointegration of inserted implants. If dental implants are to be used for prosthetic abutment the healing time for osseointegration is necessary, but orthodontic forces to implants are different from bite force regarding its amount of force, duration and direction. The authors evaluated the effect of orthopedic force to implants on bone tissue before osseointegration. 48 implants were placed at 12 rabbits. 2 implants into left side and 2 implants into right side were inserted along the long axis of femur respectively 2 weeks (2 weeks group), 4 weeks (4 weeks group) and 6 weeks (6 weeks group) after implants placement, 300g force had been applied to the implants at left side femur by Ni-Ti close coil spring for 4 weeks (experimental group) and no force applied to implants at right side femur (control group). After the force application for 4weeks, rabbits were sacrificed and microscopic evaluation was done by hematoxylin-eosin stain and Masson trichrome stain. The result3 were followed. 1. All implants in experimental group remained rigid after the force application for 4 weeks. 2. More fibrous tissue between bone and implants were noticed at 2 weeks experimental group than 2 weeks control group 3. More bone remodeling was noticed at 4weeks group than 2 weeks group and it was difficult to find out fibrous tissue between bone and implants at both experimental and control group of 4 weeks group. 4. It was hard to distinguish experimental group from control group at 6 weeks group. Therefore if initial stability can be obtained on implant insertion, it can be possible to use implants as a orthodontic anchorage before the healing time for osseointegration.
This study aims to investigate the clinical outcome following treatment of peri-implantitis lesions. Five subjects with 7 implants were treated with surgical approach. Four subjects with 6 implants were initially treated with non-surgical approach or hygiene control. However, inflammation was not resolved and more bone loss was found. Therefore, surgical treatment was performed. After surgical exposure of the defect, granulation tissue was removed and implant surface was treated using tetracycline and chlorhexidine. Then, the flaps were sutured. The wound healing was performed in a non-submerged mode. The present finding demonstrates stable results without progression of bone loss. In one subject, deep V shaped bone defect was filled with bone substitute (ICB, CanCellous Bone, Rockey Mountain Tissue Bank, USA), and resorbable membrane (Lyoplant$^{(R)}$, B.Braun Aesculap AG, Germany) was placed over the grafted defect and healing abutment was connected. However, the inflammation was not resolved and more bone loss was found. At one month after regenerative surgery, the implant was removed.

The concept of implant stability was basically originated from the relative condition of bone-implant interface and has some meanings for evaluation of that interface. In addtion, it has been used for the investigation of initial bone healing process after fixture installation because a degree of micromotion around interface can affect unfavorable clinical results. The purpose of this study is to investigate the mode of initial bone healing from fixture installation through prospective trial. Thirty fixtures were consecutively installed in mandibles of 26 patients with single tooth loss area and then healing abutment were secured for one-stage surgery meothod. Resonance frequency analysis was performed with one week interval during 12 weeks and periapical radiographs were taken at each month. Although marginal bone level change was not shown through observation period (P>0.05), statistical difference of implant stability was shown through 4 and 6 week (P<0.05) and was not shown after 6 week (P>0.05) according to the bone quality. Initial bone healing process is a successive process of bone resorption and favorable bone healing result might be postulated at 4 week interval after installation through RFA.

Purpose: The various suture techniques can be utilized in order to maximize the keratinized tissue healing around dental implants. The aim of this study is to compare the soft tissue healing pattern between two different suture techniques after implant placement. Materials and Methods: 15 patients with 18 implants were enrolled in this study. Simple implant placement without any additional bone graft was performed. Two different suture techniques were used to tug in the mobilized flap near the healing abutment after paramarginal flap design. Digital intraoral scan was performed at baseline, post-operation, stitch out, and 3 months after operation. The scan data were aligned using multiple points such as cusp, fossa of adjacent teeth, and/or healing abutment. After subtracting scan data at baseline with other time-point results, closed space indicating volume increment of peri-implant mucosa was selected. The volume of the close space was measured in mm3. The volume between two suture techniques at three time-points was compared using nonparametric rank-based analysis. Results: Healing was uneventful in both groups. Both suture technique groups showed increased soft tissue volume immediately after surgery. The amount of volume increment significantly decreased after 3 months (P < 0.001). Flap folding suture group showed higher median of volume increment than interrupted suture group after 3 months without any statistical significance (P > 0.05). Conclusion: After paramarginal flap reflection, the raised flaps stabilized by flap folding suture showed relatively higher volume maintenance after 3-month healing period. However, further studies are warranted.

A variety of digital technologies are being used throughout the entire implant treatment process of diagnosis, surgery, impression, design, and fabrication of prostheses. In this case, using a digital surgical guide, sinus floor elevation was performed without complications, and the implants were placed in the planned position. After the healing period for osseointegration, CAD-CAM (Computer-aided design-Computer-aided manufacturing) customized abutments and provisional prostheses were delivered. While using the provisional prosthesis, occlusal change was observed. To transfer the intermaxillary relationship and abutment position that reflect occlusal change and axial displacement, double scanning and abutment-level digital impressions were taken. Abutment superimposition was used to capture the subgingival margin without gingival retraction. Then, the definitive prosthesis was designed and fabricated with digital system. We report a case applying digital system, to achieve the predictable result as well as the efficient treatment process from implant surgery to fabricating prosthesis in the posterior area.
The aim of this study was to achieve the healing of peri-implantitis defects and the hard tissue regeneration using the augmentation of a xenograft on defect site. Two patients were treated with the surgical approach. With a full muco-periosteal flap elevation, the implant surfaces were exposed and taken the debridement of granulation tissue around the abutment. Each surface of the abutments was prepared with the air-abrasive device (PerioFlow$^{(R)}$) for decontamination. Bovine-derived bone mineral (Bio-Oss collagen$^{(R)}$) was then used to fill the defects, and no membrane was placed on the grafting site. Radiographs and clinical photo was taken to compare from baseline status. Within the limits of the present case, this case shows the significance of the surgical treatment of peri-implantitis. And this also verifies the stability of bovine-derived bone mineral and effectiveness of Air-abrasive device (PerioFlow$^{(R)}$).
PURPOSE. To describe and characterize the surface topography and cleanliness of CAD/CAM manufactured zirconia abutments after steaming and ultrasonic cleaning. MATERIALS AND METHODS. A total of 12 ceramic CAD/CAM implant abutments of various manufacturers were produced and randomly divided into two groups of six samples each (control and test group). Four two-piece hybrid abutments and two one-piece abutments made of zirconium-dioxide were assessed per each group. In the control group, cleaning by steam was performed. The test group underwent an ultrasonic cleaning procedure with acetone, ethyl alcohol and antibacterial solution. Groups were subjected to scanning electron microscope (SEM) analysis and Energy-dispersive X-ray spectroscopy (EDX) to verify and characterize contaminant chemical characterization non- quantitatively. RESULTS. All zirconia CAD/CAM abutments in the present study displayed production-induced wear particles, debris as well as organic and inorganic contaminants. The abutments of the test group showed reduction of surface contamination after undergoing an ultrasonic cleaning procedure. However, an absolute removal of pollutants could not be achieved. CONCLUSION. The presence of debris on the transmucosal surface of CAD/CAM zirconia abutments of various manufacturers was confirmed. Within the limits of the study design, the results suggest that a defined ultrasonic cleaning process can be advantageously employed to reduce such debris, thus, supposedly enhancing soft tissue healing. Although the adverse long-term influence of abutment contamination on the biological stability of peri-implant tissues has been evidenced, a standardized and validated polishing and cleaning protocol still has to be implemented.
A titanium based screw shaped dental implant was first introduced by Branemark and a treatment protocol where the restoration of edentulous area by connecting abutment after the osseointegration of the titanium surface of the implant and surrounding bone structure has been proposed. Although this protocol is widely accepted as a standard up to date, the healing duration of 3-6 months as well as the need for provisional prostheses during this period present as a major drawback. Immediate loading has been accomplished through the advent of various implant designs, enforced surface treatments, diverse forms of abutment, and delicate surgical techniques together with the increase in demand from the patients. The success rate of the immediate loading technique has been first reported as 85.7% by Dr. Schnitman in 1990 which recently has been reported up to 100% in the case of immediate loading in single tooth by Dr. Kan. To ameliorate the success rate of immediate loading technique, selection of patients presenting a sound bone quality and quantity, acquiring primary stability through delicate surgical techniques and fabrication of prostheses which accounts for biological stabilities should all be taken into consideration. This presentation introduces the understanding of biological stability of immediate loading, various methods for measurement of stability and clinical cases regarding immediate loading technique.

Introduction: Possible etiologic factors associated with bone loss around implants after implantation are surgical trauma, occlusal overload, periimplantitis, presence of micro gap and the formation of biologic distances. Tarnow et al. observed that the crestal bone loss was greater when the distance between the implants was <3mm than when the implants were ${\geq}\;3mm$ apart. The aim of this study was to evaluate the influence of different interimplant distance on marginal bone and crestal bone resorption in the beagle dogs. Materials and methods: The mandibular premolars of 5 dogs were extracted bilaterally. After 12 weeks of healing, each dog received 7 implants. On each side, implants were separated by 2mm (Group 1) and by 5mm (Group 2). After 16 weeks of healing, the dogs were sacrificed. Marginal bone loss was determined through linear measurements made between the implant-abutment junctions and the most coronal portions of the bone in contact with the implant surface. A line was drawn uniting the implant-abutment junctions of the adjacent implants, and a linear measurement was made at the midpoint in the direction of the most coronal peak of the interimplant bone crest to determine the crestal bone loss. Both of them was measured radiologically and histologically. Result and conclusion: In radiological analysis, the mean of marginal bone loss was $1.26{\pm}0.14mm$ for group 1 and $1.23{\pm}0.34mm$ for group 2, the mean of crestal bone loss was $1.10{\pm}0.14mm$ for group 1 and $1.02{\pm}0.30mm$ for group 2. The results were not statistically significant between 2 groups. In histological analysis, the mean of marginal bone loss was $1.63{\pm}0.48mm$ for group 1 and $1.62{\pm}0.50mm$ for group 2, the mean of crestal bone loss was $1.23{\pm}0.35mm$ for group 1 and $1.15{\pm}0.39mm$ for group 2. The differences were also not statistically significant. The clinical significance of this result is that the increase in the crestal bone loss results in the increase in the distance between the base of the interproximal contact of the crowns and the bone crest, and this determines if papilla will be present or absent between implants. Considering this fact, keeping up sufficient interimplant distance is important to minimize crestal bone loss.
