Gingival Squamous Carcinoma with Metastatic Lymph Node Involvement of Papillary Thyroid Carcinoma

  • The development of multiple primary tumors is a problem leading to the treatment of patients diagnosed with gingival squamous cell carcinoma (SCC). The occurrence of multiple primary cancers in patients with SCC of the head and neck is uncommon. Thyroid carcinomas have been found incidentally in the cervical lymph nodes after histopathologic examination. A 72-year-old male with SCC of the lower gingiva at the clinical stage T2N0M0 was treated with partial mandibulectomy and selective neck dissection. Histopathologic examination showed the foci of papillary thyroid carcinoma metastasis. The patient subsequently underwent total thyroidectomy. We report a case of papillary thyroid carcinoma associated with SCC of the oral gingiva along with a review of literatures.

Percutaneous Ultrasound-Guided Fine-Needle Aspiration Cytology and Core-Needle Biopsy for Laryngeal and Hypopharyngeal Masses

  • Objective: To evaluate the feasibility and diagnostic performance of ultrasound (US)-guided fine-needle aspiration cytology and core-needle biopsy (US-FNAC/CNB) for the diagnosis of laryngo-hypopharyngeal masses. Materials and Methods: This was a single-center prospective case series. From January 2018 to June 2019, we initially enrolled 40 patients with highly suspicious laryngo-hypopharyngeal masses on laryngoscopic examinations. Of these, 28 patients with the mass involving or abutting the pre-epiglottic, paraglottic, pyriform sinus, and/or subglottic regions were finally included. These patients underwent US examinations with/without subsequent US-FNAC/CNB under local anesthesia for evaluation of the laryngo-hypopharyngeal mass. Results: Of the 28 patients who underwent US examinations, a laryngo-hypopharyngeal mass was identified in 26 patients (92.9%). US-FNAC/CNB was performed successfully in 25 of these patients (96.2%), while the procedure failed to target the mass in 1 patient (3.8%). The performance of US caused minor subclinical hematoma in 2 patients (7.7%), but no major complications occurred. US-FNAC/CNB yielded conclusive results in 24 (96.0%) out of the 25 patients with a successful procedure, including 23 patients with squamous cell carcinoma (SCC) and 1 patient with a benign mass. In one patient with atypical cells in US-FNAC, additional direct laryngoscopic biopsy (DLB) was required to confirm SCC. Among the 26 patients who received US-FNAC/CNB, the time from first visit to pathological diagnosis was 7.8 days. For 24 patients finally diagnosed with SCC, the time from first visit to the initiation of treatment was 25.2 days. The mean costs associated with US-FNAC/CNB was $272 under the Korean National Health Insurance Service System. Conclusion: US-FNAC/CNB for a laryngo-hypopharyngeal mass is technically feasible in selected patients, providing good diagnostic performance. This technique could be used as a first-line diagnostic modality by adopting appropriate indications to avoid general anesthesia and DLB-related complications.

성문상부 종물 양상의 일차성 후두 Large B세포 림프종 1예 (A Case of Primary Large B-cell Lymphoma of Larynx Presenting as Supraglottic Mass)

  • The extranodal non-Hodgkin lymphoma is uncommonly occurred in larynx, accounting for less than 1% of all laryngeal neoplasm. In general, the laryngeal lymphoma is appeared as submucosal mass without mucosal ulceration and is most commonly found in supraglottis. The primary laryngeal lymphoma constitute a diagnostic challenge because they are characterized by absence of clinical and gross differential criteria, compared with squamous cell carcinoma (SCC). We encountered a 74-year-old man with hoarseness and lump sensation in the throat. On direct laryngoscope, multiple ulcerative and exudative mass in glottis and supraglottic areas were observed. The patient was finally diagnosed as large B-cell lymphoma through the laryngeal microsurgery. He received radiation therapy and there is no evidence of recurrence. Although the laryngeal mass has superficial mucosal change, primary laryngeal lymphoma must be included in the differential diagnosis.

Tissue Expression, Serum and Salivary Levels of IL-10 in Patients with Head and Neck Squamous Cell Carcinoma

  • Background: Head and neck SCC is a common cancer related to various factors. IL-10, a pleiotropic cytokine produced by macrophages, T-helper-2 cells, and B lymphocytes, is thought to play a potential pathogenetic or therapeutic role in a number of human conditions, such as inflammation, autoimmunity and cancer. The present study was designed to evaluate the relation between tissue expression, serum and salivary levels of IL-10 in head and neck squamous cell carcinomas (HNSCCs) and their correlation with clinicopathologic features. Materials and Methods: Samples were collected from 30 patients with HNSCCs and 24 healthy volunteers. IHC analysis was used to examine the tissue expression and ELISA was employed to measure serum and salivary levels. Results: Our study showed tissue expression of IL-10 to be significantily higher in patients (P: 0.001), but there was no relation between tissue expression, serum and salivary levels of the marker (P>0.05). Also except for a positive correlation between tissue expression of IL-10 and stage (P: 0.044), there was no relation between this marker and clinicopathologic features. There was no correlation between serum and salivary levels in either patients or controls. Conclusions: It seems there is no correlation between level of IL-10 in serum and saliva and this marker in saliva and serum does not reflect tissue expression.

Head and Neck Squamous Cell Carcinoma in Iranian Patients and Risk Factors in Young Adults: a Fifteen-Year Study

  • Background: Head and neck squamous cell carcinoma (HNSCC) is the 8th most common cancer worldwide. Although older age, male gender, smoking and alcohol consumption are known risk factors, an increasing number of HNSCC patients are without typical risk factors. Our aim was to define demographics of HNSCC in Iran and the potential risk factors related to Iranian ethnicity and lifestyle. Methods: We conducted a cross-sectional analytical study on 262 patients with primary SCC of the larynx, hypopharynx or tongue referred to our pathology department during 1995-2010. Patients' demographics, tumor characteristics and risk factors such as smoking, alcohol consumption and anemia were analyzed and compared in two groups of patients: over 40 years (older group) and 40 years or less (young group); Chi-square and Mann-Whitney analytical tests were employed. Results: 5.7% of patients were young adults. The male to female ratio was 1.5 in the younger group and 5.6 in the older group. In young adults, 40% of tumors were located in larynx and 40% in the tongue. Age >40 was significantly associated with laryngeal location (P<0.001). History of smoking and drinking was significantly associated with age >40 and SCC of larynx in both age groups. Cervical lymph node involvement was significantly correlated with SCC of tongue (P<0.001), however, considering young adults only, SCC of hypopharynx was most frequently accompanied by lymph node involvement (60%). The most prevalent tumor among men was SCC of larynx whereas SCC of hypopharynx was the most prevalent tumor among women (61%), of whom 18.2% were ${\leq}40$. Conclusions: The incidence of HNSCC among young adults seems to be higher in Iran compared to other countries. Reduction in exposure to known risk factors, especially tobacco smoking in forms of cigarettes and bubble pipes, and search for other causative agents of HNSCC in young population is recommended.

  • Cutaneous squamous cell carcinoma (cSCC) is the second most common nonmelanoma skin cancer, and its incidence is increasing globally. In Korea, there were 12,516 diagnosed cases of cSCC between 1999 and 2014. Surgical treatment, for which several options are available, is the standard of care for cSCC and securing a sufficient surgical resection margin is always important. cSCC of the scalp sometimes exhibits unusually aggressive behavior. In this article, we report a case of cSCC of the scalp with invasion into the skull and dura mater.

A Retrospective Clinical View of Basal Cell Carcinoma and Squamous Cell Carcinoma in the Head and Neck Region: A Single Institution's Experience of 247 Cases over 19 Years

  • Background: The two most common skin cancers are basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). The purpose of this study was to describe the detailed clinical behavior of BCC and SCC in the head and neck region over 19 years at a single institution. Methods: A retrospective analysis was performed for all patients with non-melanoma skin cancer who had undergone surgical resection over an 18-year period. Patient charts were reviewed for demographic information, tumor size, onset-to-diagnosis, anatomic location, clinical subtype, histologic differentiation, method of surgical treatment, and recurrence. Results: The review identified 265 cases of either BCC or SCC in 226 patients. Of the 226 patients, 80 (35.4%) were men and 146 (64.6%) were women. BCC (n=138, 55.9%) was more frequent than SCC (109, 44.1%). The most frequent age group was 70-to-79 year olds (45 patients, 35.2%) for BCC and 80-to-89 year olds (41 patients, 41.8%) for SCC. By aesthetic units of the face, the most common location was the nasal unit (44 cases, 31.9%) for BCC and the buccal unit (23 cases, 21.1%) for SCC. The most common clinical subtype of BCC was the nodular type (80 cases, 58.0%). Local flaps were most commonly used to cover surgical defects (136 cases, 55.1%). Recurrent rates were 2.2% for BCC and 5.5% for SCC. Conclusion: In our study, many characteristics of BCC and SCC were compared to previously published reports were generally similar, except the ratio of BCC to SCC. Further study can help to establish the characteristics of BCC and SCC.

Diagnostic Utility of p63 (Ab-1) and (Ab-4) Tumor Markers in the Squamous Cell Carcinomas of Head and Neck

  • P63 is a gene product required in cell cycle regulation which plays vital roles in tumor differentiation. Aims of the present study were to assess the frequency, pattern, sensitivity and specificity of two p63 protein clones P63 4A4 and P63 4A4+Y4A3 in squamous cell carcinomas (SCCs). Thirty cases of head and neck region SCC diagnosed on the basis of H&E staining were examined along with 60 cases of head and neck region biopsies other than squamous cell carcinoma, negative on H&E staining, were taken as control. Immunostaining was performed on slides according to the Thermo Scientific UltraVision LP detection System. P63 4A4+Y4A3 clone is more sensitive 96.6% in comparison to 86% in P63 4A4 with having greater NPV of 98.3%. The results signify the importance of P63 4A4+Y4A3 marker over the old markers and may be used as a confirmatory marker of squamous cell carcinoma.

두경부 편평상피암 환자에서 수술 전 경부림프절 전이 평가에 대한 F-18 FDG PET/CT의 정확도: CT/MRI와의 비교 (Accuracy of F-18 FDG PET/CT in Preoperative Assessment of Cervical Lymph Nodes in Head and Neck Squamous Cell Cancer: Comparison with CT/MRI)

  • 목적: 두경부 편평상피암 환자에서 경부림프절전이에 대한 F-18 FDG PET/CT의 진단성적을 평가하고 CT/MRI와 비교하였다. 대상 및 방법: 수술전 조직검사로 편평상피암으로 진단받은 17명(남: 15명, 여: 2명, 평균나이: $59{\pm}11$세)의 환자를 대상으로 F-18 FDG PET/CT를 시행하였다. 경부림프절의 F-18 FDG섭취 정도는 세 등급(1=배후방사능, 2=간방사능보다 낮거나 같은 경우, 3=간방사능보다 높은 경우)으로 나누었고, 3등급일 때 전이로 진단하였다. 경부림프절전이에 대한 F-18 FDG PET/CT의 예민도와 특이도를 CT/MRI와 비교하였다. 결과: 17명의 환자에서 123개의 경부림프절이 절제되었고 이 중 29개가 전이로 확진되었다. F-18 FDG PET/CT의 예민도는 69%(20/29), 특이도는 99%(93/94), CT/MRI의 예민도는 62%(18/29), 특이도는 96%(90/94)로 경부림프절전이에 대한 두 검사의 진단성적은 유의한 차이가 없었다. 그러나 F-18 FDG PET/CT는 각각의 환자에서 간에 생긴 이차성 원발암과 늑골전이를 발견할 수 있었다. 결론: 두경부 편평상피암에서 경부림프절 전이에 대한 F-18 FDG PET/CT의 특이도는 매우 높았지만 예민도는 비교적 낮았고 CT/MRI와 유의하게 다르지 않았다. F-18 FDG PET/CT의 비교적 낮은 예민도의 원인은 최근 많이 절제되고 있는 잠재 전이의 가능성이 있는 작은 경부림프절을 발견하지 못하는 F-18 FDG PET/CT 의 한계 때문이었다. 그러나 F-18 FDG PET/CT는 이차성 원발암과 원격전이를 부가적으로 평가할 수 있어 환자의 치료 방침 결정에 유용하였다.
