건강신념모형을 적용한 일개 병원 임상간호사의 손씻기 수행도에 대한 연구 (A study on the hand washing practice of a clinical nurse in a hospital based on health belief model)

  • 본 연구는 건강신념모형을 적용하여 임상에서 근무하는 간호사의 손씻기 수행도를 분석하기 위한 서술적 조사연구이다. 본 연구의 대상은 G 광역시에 위치한 일개 병원에 근무하는 임상 간호사 162명이다. 자료수집은 2017년 9월 1일부터 2017년 10월 31일까지였다. 수집된 자료는 SPSS (SPSS Version 20.0, IBM, US)를 이용하여 빈도분석, 백분율, 일원분산분석, t-검정, pearson 상관계수분석 및 다중회귀분석으로 통계처리 하였다. 일반적 특성에 따른 손씻기에 대한 수행도는 모든 영역에서 평균은 3.1점으로 높았으나 통계적으로 유의한 차이는 보이지 않았다. 일반적 특성에 따른 손씻기에 대한 건강신념은 연령에서는 지각된 유익성 영역에서 유의한 차이를 보였으며(p<0.05), 근무연수에서는 지각된 유익성, 지각된 장애성 영역에서 유의한 차이를 보였다(p<0.05). 다섯 개의 하위영역간의 상관관계를 분석한 결과는 지각된 유익성과 지각된 장애성은 부의 상관관계를 나타냈으며, 수행을 위한 동기는 정의 상관관계를 보였다. 본 연구의 결과를 통해 손씻기에 대한 건강신념을 고려한 지속적인 병원에서의 감염관리교육과 올바른 손씻기 교육을 진행한다면 손씻기 수행도가 높아질 것으로 판단된다.

중환자실 간호사의 호흡기병원감염 예방에 관한 연구 (A Study on the Prevention of Nosocomial Respiratory Infection in Critical Care Nurses)

  • The purpose of study is to prevent Nosocomial Respiratory Infection after understand the Correlation about Recognition and Performance of Critical Nurses. Data were collected from Critical Nurses 300 persons who has worked in 14 Subsidary Hospitals of University in Seoul and the collection periode is from Nov. 1st, 1997 to Jul. 14th. 1998. There are 40 questions and constructed by 6 Domains which Hand Washing, Suction Management, Airway Management, Respiratory Instrument. Ventilation and Respiratory Intervention. Also, the Data were analized with SPSS program. The obtained results are as follows : (1) The Mean of Recognition for Prevention of Nosocomial Respiratory Infection is 4.649 and by Domains, Respiratory Intervention(4.758), Suction Management (4.669), Airway Management(4.660), Hand Washing(4.651), Ventilation(4.605) and Respiratory Instrument(4.561) according to the Mean. (2) The Mean of Performance for Prevention of Nosocomial Respiratory Infection is 3.991 and by Domains, Respiratory Intervention(4.498), Airway Management (4.107), Hand Washing(4.084), Suction Management(3.898), Respiratory Instrument(3.860) and Ventilation(3.690) according to the Mean. (3) In the Correlation of Recognition and Performance for the Prevention of Nosocomial Respiratory Infection, Hand Washing(r=0.755, p=0.000), Airway Management(r=0.724, p=0.000), Respiratory Intervention(r=0.693, p=0.000) are mentioned significant correlation level. The Performance is good whenever the Recognition is high for Respiratory Instrument(r=0.143, p=0.054) but it's not significant level and Suction Management and Ventilation has no Correlation. (4) In Normal Properties, The Recognition is good(p<.05) when Nurses has plenty Clinical Career, Attendance of the Education and Exclusive Nurse for the Respiratory Infection is in Hospital. The Age is not significant correlation level statistically but represent a little correlation. The Performance is good(p<.05) when Elder Age, Attendance of the Education and the plenty Clinical Career is not significant correlation level statistically but represent a little correlation. Another properties has no menas. According to the results, Suggestion is as follows ; (1) Required Education to advance Recognition and Performance about Prevention of Nosocomial Respiratory Infection for a little clinical career of Nursing and younger Nurses. (2) It needs to analyze Performance about Prevention of Nosocomial Respiratory Infection by observation of research worker.

가계의 세탁기사용방식과 사용정도에 관한 연구 (A Study on the Pattern and Extent of Washer Use in Household)

  • In this study one aspect of consumer behavior in household equipment utilization was investigated the pattern, frequency, rate of washer use and their relation to the following factors a) Washer related factors : extent of the presence of desired characteristics, the evaluation of washer's intrinsic features and related household facilities. b) Psycho-social factors : attitude of energy conservation, preference & ability to wash by hand, standard of washing of the respondent homemaker. c) Socio-demographic factors : age, education level and employment status of homemaker, house-hold income, the presence of children under seven years, size of family, the presence of a paid help. The subjects of this study were 286 homemakers with washer in Seoul. Analysis methods were used to fuequency, one-way ANOVA, Gamma test, Pearson's Correlation Coefficient, t-test and multiple regression of SPSS program. The major findings are the following; 1) The pattern, frequency, rate of washe use appeared various in every household. 2) Extent of the presence of desired characteristics was very low and respondents evaluated their washer's intrinsic features moderate. 3) The pattern of washer use was affected by the evaluation of washer's intrinsic features, preference & ability to wash by hand, wife's employment and household income. The frequency of washer use was affected by family size and preference & ability to wash by hand. The rate of washer use was affected by extent of the presence of desired characteristics, the evaluation of washer's intrinsic features and preference & ability to wash by hand. Therefore, washing by hand is major substitute for washer. If more desired characteristics are added to washer, intrinsic features are improved, and maintenance costs are reduced or household income is raised, every houshold with washer will use washer more than washing by hand in washing ask so that it may gain more utility from washer.

치과위생사의 임파워먼트에 따른 감염관리 인식 및 실천도 (Perception and practice of the infection control by empowerment in the dental hygienists)

  • Objectives: The purpose of the study was to investigate the perception and practice of the infection control by empowerment in the dental hygienists. Methods: A self-reported questionnaire was filled out by 200 dental hygienists in Gyeongbuk from January 3 to February 20, 2013. Data were analyzed by SPSS 12.0 program. The instrument of impowerment was adapted from Spreitzer and consisted of 12 questions including meaning(4 questions), competency(4 questions), self-decision(4 questions), and impact(4 questions). Impowerment was score by Likert 5 scale and higher score means higher impowerment. The instrument for hand washing recognition and practice was adapted from Kim and consisted of hand washing(5 questions), personal protective clothing management(5 questions), contaminated appliance management(3 questions), sterilization(3 questions), and infection control environment(8 questions). The empowerment instrument was score by Likert 5 scale and the mean was 3.83 points. Based on 3.83, infection control recognition and practice were divided into upper group and lower group. Cronbach alpha was 0.951 in empowerment, 0.931 in recognition, and 0.924 in practice in the study. Results: Based on the average points of 3.83, the groups were divided into two groups including upper group and lower group. The upper group showed higher score in hand washing than the lower group. In the protective clothing management, the upper group changed the mask at one-hour interval(p<0.001). Conclusions: In the viewpoint of empowerment, it had a significant influence on the perception and practice of the dental infection control in the dental hygienists.

국내병원의 유형별 손위생 수행실태 조사분석 (A Feasibility Study of Hand Hygiene Status in Korea Hospitals)

  • Purpose : The easiest and most cost-effective way to prevent medical-related infections is known as proper hand washing of health care workers. The experience of MERS in domestic medical institutions has increased the importance of hand hygiene for medical workers to prevent infections in hospitals. It is necessary to investigate the level of hand hygiene practice by type of medical institutions and the factors influencing the infection prevention. Methods : Domestic and overseas hand hygiene related business cases and literature data were collected and analyzed in order to investigate the hand hygiene status of medical institutions in Korea. Result : As a result of hand hygiene monitoring of all hospital-level medical institutions in 2016, the total number of observations was 24,328 and the hand hygiene performance rate was 75.9%. The hand hygiene performance rate of hospitals was 71.5% for general hospitals, 75% for general hospitals, and 81.3% for hospitals. Implications : In general hospitals and hospitals, the HR(Hand Rubbing) method is preferred as a way of performing hand hygiene, whereas the HW(Hand Washing) method is relatively high in the small hospitals. It is estimated that the HW system is preferred because of the cost burden at the hospital medical institution. Therefore, it is necessary to compensate the related expenses to improve the hand hygiene performance of the physicians who are engaged in the hospitals.

치위생과 학생의 감염관리에 대한 인지도와 실천도 조사 (A study on recognition and practice of dental hygiene students for Infection control dental hygiene major courses)

  • The purpose of the study was to provide the basic for the prevention of infection control by analyzing the level of recognition and practice of dental hygiene students. It is important for dental clinic staffs to know and practice infection control measures in favor of effective infection control. Thus, this study conducted an one-month questionnaire survey (during October 2008) for dental hygiene students who ever experienced in 16-week clinical practice, and analyzed total 206 questionnaire forms as collected from them, so that it could determine potential associations between their recognition and practice of infection control and thereby suggest an efficient solution for infection control in dental hygiene. 1. It was found that our students' recognition about infection control averaged $4.49{\pm}.58$ points on the whole, and they were best aware of instrumental disinfection and sterilization among other measures, which was followed by hand washing, extirpation control, post-sterilization management, personal protective outfit and instrumental surface maintenance respectively. 2. It was found that our students' practice of infection control averaged $3.85{\pm}.57$ points on the whole, and they practiced hand washing most actively among other measures, which was followed by extirpation control, post-sterilization management, instrumental disinfection and sterilization, personal protective outfit and instrumental surface maintenance respectively. 3. It was found that our students scored higher mean points in recognition about every measure of infection control than those in practice (t=15.676, p=0.000). Particularly, it was notable that there were significant differences between their mean points in recognition and practice of some infection control measures, such as instrumental surface maintenance (t=15.361, p=0.000), personal protective outfit (t=15.245, p=0.000) and instrumental disinfection and sterilization (t=11.169, p=0.000). But there was least significant difference between mean points in recognition and practice of hand washing (t=5.460, p=0.000). 4. For potential associations between recognition and practice of infection control, it was found that higher recognition was in significantly positive associations with higher practice in every measure of infection control (r=.478, p=.000), such as extirpation control (r=.630, p=.000), instrumental disinfection and sterilization (r=.477, p=.000) and post-sterilization management (r=.433, p=.000). 5. It was found that there were differences in our students' recognition depending upon availability of infection control guideline (t=4.587, p=.011), and there were significant differences in practice depending on necessity of infection control education on a statistical basis (t=2.229, p=.027). Overall, it is found that our dental hygiene students are very likely to practice hand washing and extirpation control, because both of these measures are relatively easy to practice or are considered legally binding.

의류제품에 대한 소비자의 인식과 취급실태 (Survey on Consumer's Cognition for Management of Clothing Products)

  • To investigate the cognition of labeling system and its problems, problems in laundry, and consumer's dissatisfaction and to decrease problems in management of clothing products, total 476 subjects were surveyed in Taejon. The major results were as follows ; 1. The cognition of labeling system was understood by 93.1% of respondents, and 72.9% of them prefer to recognized labeling by figures and letters than by either of them. Most of respondents got the knowledge of labeling system from school. The most frequently experienced mislabeling was the label for management. 2. In cleaning, 60.4% of respondents made their decisions of the laundry method after seeing labeling system. When the label recommanded either of hand washing or dry cleaning, they usually laundered with hand washing after a few times of dry cleaning. The first consideration factor for laundry was fiber composition of textiles. 3. About 70% of respondents understood ironing, laundry, and drying mark on labeling system, and 53.8% understood fiber composition and bleaching. 4. More than 90% of respondents experienced dissatisfaction in handling clothing products. The reason of dissatisfaction was deformation and decoloring after laundry. Most of respondents experienced change of tactile sensation, too.

일 대학병원 간호사의 다약제 내성균 감염관리지침에 대한 지식과 수행정도 (Hospital Nurses' Knowledge and Compliance on Multidrug-resistant Organism Infection Control Guideline)

  • Purpose: This study was done to investigate nurses' knowledge of, and compliance with the multidrug-resistant organism (MDRO) infection control guidelines. Methods: A survey questionnaire was developed based on the institutional and national guidelines and was administered to a convenience sample of 306 nurses in a university hospital. Results: The mean score for knowledge was 33.87 (percentage of correct answers: 82.61%). The percentages of correct answers for basic concepts, route of transmission, hand washing/protective devices and environment management were 74.27%, 94.29%, 92.90% and 75.54% respectively. The mean compliance score was 4.15 (range: 1-5). The compliance scores for education, communication, contact precaution, disinfection, surveillance culture, and hand washing were 3.29, 4.05, 4.20, 4.50, 4.40 and 4.48 respectively. Nurses indicated "lack of time (30.06%)", "lack of means (10.78%)" and "lack of knowledge (9.48%)" as reasons for noncompliance. Conclusion: While most educational programs have focused on hand washing or use of protective devices to prevent transmission of MDRO in acute care settings, hospital nurses' knowledge of the basic concepts of MDRO and environmental management has remained insufficient. Nurses are relatively non-compliant to the guidelines in the areas of education (staff, patient, family) and communication. Comprehensive educational programs are needed to decrease hospital infection rates and to improve the health of patients.

다약제 내성균 감염관리지침에 대한 간호학생의 지식과 수행정도 (Nursing Students' Knowledge and Compliance with Multidrug-resistant Organism Infection Control Guidelines)

  • Purpose: This study was done to investigate knowledge of and compliance with the multidrug-resistant organism (MDRO) infection control guidelines among student nurses on clinical practicum, Methods: survey questionnaire on MORO infection control was administered to a convenience sample of 259 nursing students from 3 different nursing schools Results: The mean knowledge score was 28.01/39 (71.82%). The percentages of correct answers for basic concepts, route of transmission, hand washing/ protective devices and environment management, were 55.40, 81.14, 84.94 and 69.17 respectively. The mean compliance score was 3.83/5. The compliance scores for education, communication, contact precaution, environment management, and hand washing were 3.06, 3.33, 3.86, 4.50, 3.92 and 4.29 respectively. 96.9% of subjects knew that they should wash hands after touching MORO patient while only 22.8% of subjects knew how to collect samples for VRE surveillance culture, The highest compliant item was hand washing after touching MORO patient. The Lo-west compliant item was referring to infection control manual. Conclusion: Comprehensive MDRO infection control education programs for nursing students should be developed to decrease MORO infection.

병원감염관리에 관한 간호대학생의 지식 및 실천에 관한 연구 (The Study of the Knowledge and Performance of Nursing Students for the Nosocomial Infection Control)

  • Purpose: This study was designed to investigate nursing students' exposure experience to infectious diseases and their knowledge and performance for the nosocomial infection control. Method: The data was obtained from 292 nursing students having clinical experience at two university hospitals in Seoul from November 2005 to February 2006. Specific questionnaire was constructed and used for this study. The obtained data was analyzed by using the SPSS window program. Result: The results were as follow : 1. Among the evaluation scores of knowledge for the nosocomial infection control, the score of sterilization and the score of the intravenous therapy management were low(.14). 2. Among the evaluation scores of performance for the nosocomial infection control, the score of hand washing is low(1.63). 3. 86.3 percent of the nursing students had exposure experience to infectious disease. Many of them did not use the protective equipments in clinical experience nor took any treatment after the exposure to infectious diseases. Conclusion: Based on these results, we suggest that the programs for the nosocomial infection control should be developed for nursing students. It is also suggested that nursing students should be educated before their clinical experience to keep protected from the infection. In particular, the hand washing, sterilization and intravenous therapy management should be emphasized in the educational programs.
