Background: Passive straight leg raising (PSLR) is the common clinical test to measure of hamstring muscle length. Hip flexion angle contributes to change the lumbopelvic rotation during PSLR. Pressure biofeedback unit (PBU) is commonly used to detect lumbopelvic movement during lower limb movements. Thus, there may be the relationship between pressure of PBU and lumbopelvic motion during PSLR. Objects: The objective of this study was to determine the relationship between pressure of PBU and lumbopelvic motion during PSLR. Methods: Thirty two subjects participated in this study. A three-dimensional motion analysis system were used to measure the lumbopelvic angle during PSLR, while recording the pressure of PBU according to angle of PSLR by 10 degree increments. Pearson product moment correlations and linear regression analysis were used to describe the relationship between variables. Results: The results showed that there was a significant relationship between the lumbopelvic and angle of PSLR (Pearson's r=.83, p<.05), between the pressure of PBU and angle of PSLR (Pearson's r=.75, p<.05), and between lumbopelvic motion and pressure of PUB (Pearson's r=.83, p<.05). Linear regression equation using lumbopevic angle as an independent factor was as follows: Pressure of PBU = 47.35 + (2.55 ${\times}$ angle of lumbopelvic motion) ($R^2=.69$, p<.05). Conclusion: Results of the present study indicate that pressure of PBU can be used to indirectly detect the amounts of lumbobevic motion during muscle length test or stretching of hamstring.
Purpose: The purpose of this study was to investigate the effect of strengthening exercises of hip abductors on muscle strength and ambulation for patients with ACL reconstruction. Methods: The subjects were randomly assigned to the intervention group (general exercise plus strengthening of hip abductors) or the control group (general exercise without the strengthening of hip abductors). Both groups participated in a six-week exercise protocol after the surgery for ACL reconstruction. The knee strength test (quadriceps, hamstring, hip abductor) and gait analysis were performed in pretest and post-test. Results: Muscle strength was measured using the Biodex system III model (Biodex Medical System, Inc, NY, US). The results showed no significant difference in knee strength (quadriceps, hamstring) between the two groups (p>0.05), however, there was a significant difference in hip abductors (p<0.05). The gait analysis was measured with Gaitrite system (CIR System Inc, US). Results of comparison between groups showed a significant increase in the step length of both groups. (p<0.05), and there was a significant difference in the affected leg of the intervention group(p<0.05). Conclusion: Exercise of hip abductors for patients with ACL reconstruction provides significant benefits with respect to improvement of muscle strength in hip abductors, increasing the stride. This result indicates that a training program designed for ACL reconstruction patients should include strengthening of the hip abductor. We think that further study is needed to determine the relationship of hip muscles and knee joint for patients with ACL reconstruction.
Background: This study was conducted to investigate the effects of pressure belt during proprioceptive neuromuscular facilitation sprinter pattern training using a Thera-band on leg muscle strength and gait in stroke patients with stroke. Methods: Nine patients with stroke underwent training five times a week for four weeks, and changes in the muscle strength and walking ability of the paralyzed leg before and after training were measured. Muscle strength was measured using a Digital muscle tester, and walking ability was measured using a G-WAKER and the timed up and go (TUG) test. Results: Results showed that the quadriceps, hamstring, tibialis anterior, gastrocnemius, cadence, stride length, and stance phase significantly increased (p<.05). The swing phase, gait cycle duration, and TUG test results significantly decreased (p<.05). Conclusion: This study demonstrated that a pressure belt is a very useful tool for improving muscle strength and walking ability in patients with stroke.
The purpose of this study was to investigate the dynamic balance and activity of internal oblique muscle, multifidus muscle, gluteus maximus muscle, biceps femoris muscle during the Y balance test following the wearing of pelvic compression belt. Forty healthy adults were recruited for this test. The dynamic balance score was estimated as the following: (anterior+posteromdial+posterolateral)/($3{\times}leg$ length)${\times}100$. The electromyography signals were measured through %reference voluntary contraction, which was normalized by reference voluntary contraction of Y balance test without wearing the pelvic compression belt. The paired t-test was carried out to compare the dynamic balance score and the activity of the trunk and hip extensor with and without the wearing of pelvic compression belt. The dynamic balance score of the Y balance test when wearing pelvic compression belt was significantly than when measured without wearing the pelvic compression belt (p<.05). The muscle activity of the internal oblique and the multifidus was significantly decreased when wearing pelvic compression belt (p<.05). The muscle activity of the gluteus maximus was significantly increased when wearing pelvic compression belt (p<.05). However, there was no significant difference in hamstring muscle activity, with or without wearing the belt (p>.05). In conclusion, this study shows that the wearing of pelvic compression belt affects trunk muscle and hip extensor muscle activity related to the pelvic mobility and stability and increases dynamic balance and also contributes to the stabilization of the external pelvic stabilization.
목적: 자가 슬괵건을 이용한 전방 십자 인대 재건술에서 Cross pin 및 Graft Tensioner를 이용한 수술의 단기 추시 결과를 보고하고 조기 경험에서 발견된 수술 술기상의 문제점과 그 해결방안을 모색하고자 한다. 대상 및 방법: 2008년 1월부터 2009년 3월까지 자가 슬괵건을 이용한 전방 십자인대 재건술 후 최소 18개월이상 추시가 가능하였던 35예를 대상으로 하였다. 이식건의 대퇴골측 고정을 위해 Cross pin을, 경골측의 고정을 위해 Intrafix와 Post-tie를 사용하였다. 이식건은 Graft Tensioner을 이용하여 80N의 힘으로 고정하였다. 결과는 임상적, 방사선학적 결과와 합병증으로 평가하였다. 결과: IKDC 주관적 점수 및 Lysholm 점수는 술 후 89.1점 및 91.4점로 호전되었으며 축이동 검사 및 One-leg hop 검사도 술후 양호한 소견을 보였다. KT-1000 관절계 검사 및 Telos 부하 방사선 검사상 최종 추시시 건측에 비해 평균 $2.8{\pm}1.6$ mm, $2.6{\pm}1.3$ mm로 호전을 보였으며 대퇴골 터널은 평균 $2.3{\pm}1.1$ mm로 확장되었다. 수술 술기상 문제점으로 대퇴골 터널과 횡고정 터널의 불일치, 대퇴골 터널과 Graft Harness의 길이 불일치 그리고 연부 조직 자극이 관찰되었다. 결론: Cross pin 및 Graft Tensioner를 이용한 전방 십자인대 재건술은 단기 추시 상 좋은 안정성을 보여주는 수술 방법 중의 하나로 사료된다. 그러나 술기상 문제로 대퇴 터널-cross pin 불일치를 줄이기 위해 대퇴 터널을 짧게 만들거나 cross pin 터널을 피질골에 가능한 수직으로 만들어야 하며 대퇴 터널 형성시 위치 막대를 3 mm가량 더 진행시켜야 대퇴 터널-graft harness 불일치를 줄일 수 있을 것으로 생각된다.
본 연구에서는 만성 파킨슨병 환자의 동적탄력튜빙 보행 훈련 (I, II)이 자세정렬, 보행, 삶의 질에 미치는 영향을 조사하였다. 총 3명의 만성 파킨슨병 환자를 모집하여 사례 연구를 시행하였다(Hoehn 및 Yahr 1-3단계 각 1명). 동적탄력튜빙 보행 훈련 (I 및 II)은 파킨슨병 환자에게 하루 30분, 주 5일, 5주 동안 25세션 적용되었다. 본 연구의 효과를 알아보기 위해, 자세정렬검사, 근 활성도검사, 보행분석 및 파킨슨병 환자의 삶의 질을 평가하였다. Dynamic tubing 보행 훈련(I, II) 중재 후 몸통 굽힘은 감소되었다. 또한 초기 접촉기(IC)에서 중간 디딤기(Mst)까지 보행 시 넙다리네갈래근, 뒤넙다리근, 앞정강근(TA)의 근 활성도가 증가하고 장딴지근의 근활성도가 감소하였다. 척추세움근(ES T12, L3)의 근활성도는 H&Y I, III 단계에서 증가하고 H&Y II 단계에서 감소하였다. 압력 중심(COP) 매개변수인 보행선의 길이, 한 발 지지선, 앞/뒤 위치, 좌우대칭 값이 개선되었다. 시공간 보행 매개변수인 보폭, 보폭, 의 속도는 증가하고 분당걸음수는 감소되었다. 나아가 파킨슨 병 환자의 삶의 질도 개선되었다. 이러한 연구 결과를 바탕으로 동적탄력튜빙 보행 훈련 (I 및 II)은 약물 내성이 절반으로 감소된 5년 이상된 만성 파킨슨병 환자의 자세, 보행, 삶의 질을 개선하기 위한 새로운 접근 방식으로 적용될 수 있을 것이다.
본 연구는 라이프케어 증진을 위한 협응적 이동훈련 프로그램이 비만 여성노인의 균형에 미치는 영향을 알아보았다. 본 연구의 대상자는 비만 여성노인 10명으로, 최근 1년 이내 허리나 엉덩관절 및 무릎관절 수술경험이 없고 독립보행이 가능한 자로 선정하였다. 협응적 이동훈련을 일일30분씩 2회, 주5회, 4주간 총 20회 실시하였다. 협응적 이동훈련 프로그램의 효과를 알아보기 위해 인바디 BMI검사, myoVIDEO를 이용한 관절가동범위검사, 표면근전도 검사(척추세움근, 배바깥빗근, 넙다리네갈래근, 넙다리뒤근육), 힘판 트레드밀을 통한 동적 균형검사, 버그 균형 척도(BBS)와 일어서서 걷기 검사(TUG)를 중재전후 시행하였다. 본 연구의 연구결과, 보행 시 디딤기 및 흔듦기 시기에 몸통과 엉덩관절의 관절가동범위 전-후 비교에서 통계학적으로 유의한 차이가 있었으며(p<0.05), 표면근전도를 통한 근 활성도검사에서는 척추세움근과 배바깥빗근의 중재 전-후 비교에서 통계학적으로 유의한 차이가 있었다(p<0.05). 보행 시 동적균형검사에서는 보행선길이와 한발지지선의 중재 전-후 비교에서 유의한 차이가 있었으며(p<0.05), 버그 균형 척도(BBS)와 일어서서 걷기검사(TUG)에서도 중재 전-후 비교에서 유의한 차이가 있었다(p<0.05). 본 연구결과를 통해 라이프케어 증진을 위한 협응적 이동훈련 프로그램이 비만 여성노인의 자세불안정과 근 약화, 균형능력감소 및 낙상위험도를 개선에 유의한 영향을 주는 것을 알 수 있었다. 따라서 비만 여성노인의 균형능력 개선 및 낙상방지를 통한 라이프케어증진을 위해 이동적 협응훈련 프로그램 적용을 권장한다.
