  • The nursing vocalization of domestic pigs(Landrace$\times$Yorkshire) was investigated with respect to common features. All vocalizations uttered during nursings in 5 sows at 5 days after farrowing were recorded and 305 grunts were processed in a spectrograph. The sow's repeated grunting during nursing can be regarded as a contact call and a signal of the mother to start and synchronize the suckling behavior of the piglets. Analysis in the time domain revealed the gross structure of the call, whereas in the frequency domain the fine structure of single grunts was investigated. Nursing interval, duration of nursing behavior, duration of grunt, grunt rate per 10 seconds, fundamental frequency, 1 formant, 2 formant, 3 formant, 4 formant and spectrum were investigated. The results showed that mean interval between the nursing following one another was 25, 4.6 min and duration of nursing behavior was 3.2 $\pm$ 0.7 min. Average duration of grunt was 203.9 $\pm$ 63.6 ms. The formant contours could be identified. The nursing behavior might be disturbed by the grunts of alien sow.

  • We experienced a surgical case of esophageal atresia and tracheoesophageal fistula combined with imperforated anus, tracheal bronchus in a one day baby. A vacuum delivered full term baby, weighing 4.1 Kg showed grunting respiration, repeated regurgitation and distended abdomen after birth. Esophagogram revealed markedly dilated proximal esophagus as blind pouch and also noted displaced type of tracheal bronchus of right upper lobe by incidental bronchogram. Surgical correction with Haight anastomosis was performed successfully on the second day.

  • Esophagel atresia and tracheoesophageal fistula may occur as separate entities but usually occur in combination. First described by Durston in 1670, esophageal atresia was not successfully treated until 1939 when Ladd in Boston and Leven in St. Paul obtained the first survivors utilizing the methods of gastrostomy, esophagostomy and extrapleural ligation of the tracheoesophageal fistula as multiple operations which required months of hospitalization. Two years later Cameron Haight performed the first successful primary repair and afterward about 2000 cases of esophageal atresia with distal tracheoesophageal fistula reported in the world. In Korea, there appeared about 27 cases in the literature and 8 successful repaired cases noted in these year. Anther report two cases of esophageal atresia, of which one case was successfully treated with Haight`s method. Case 1.: Normal full term delivered boy with chief complaints of respiratory difficulty and persistent drooling with chocking, 3. lkg, was admitted with emergency 5 hours after delivery. Physical findings revealed no specific abnormal signs except distended abdomen and grunting respiration. Esophagograrn and bronchogram revealed proximal esophageal atresia and distal tracheoesophageal fistula proximal to the carina. Parent refused operative therapy and patient died 24 hours after discharge. Case 2. :3. lkg. normal full term delivered girl was admitted 4 days after delivery with chief complaints of regurgitation after feeding, chocking, cyanotic spell and fever since the day after delivery. Physical examination revealed persistent drooling, grunting respiration, and fever with moderate dehydration. Tracheoesophageal suction and fluid therapy with antibiotics improved her condition and subsided ]pneumonic condition. Esophagogram revealed markedly dilated proximal esophagus as blind loop and stomach distended with gas, and repairing operation as Haight`s method was performed on the 7th day after delivery. Patient tolerated all the operative procedure well and recovered uneventfully. Esophagogram on the 7th postoperative day showed passage of the lipiodol through the anastomotic side with moderate stricture, and feeding permitted. Patient tolerated all the feeding amount well and discharged on the 11th postoperative day. Followup revealed intermittent regurgitation after feeding and corrected with bougination.

  • Purpose : Transient tachypnea of the newborn (TTN) is usually benign and improves within 72 hours. However, it can also progress to prolonged tachypnea over 72 hours, profound hypoxemia, respiratory failure, and even death. The aim of this study is to find predictable risk factors and describe the clinical courses and outcomes of prolonged TTN (PTTN). Methods : The medical records of 107 newborns, >$35^{+0}$ weeks of gestational age with TTN, who were admitted to the NICU at Seoul Asan Medical Center from January 2001 to September 2007 were reviewed. They were divided into 2 groups based on duration of tachypnea. PTTN was defined as tachypnea ${\geq}72$ hours of age, and simple TTN (STTN) as tachypnea <72 hours of age. We randomly selected 126 healthy-term newborns as controls. We evaluated neonatal and maternal demographic findings, and various clinical factors. Results : Fifty-five infants (51%) with total TTN were PTTN. PTTN infants had grunting, tachypnea >90/min, $FiO_2$ >0.4, and required ventilator care more frequently than STTN infants. PTTN had lower level of serum total protein and albumin than STTN. The independent predictable risk factors for PTTN were grunting, maximal respiration rate >90/min, and $FiO_2$ >0.4 within 6 hours of life. Conclusion : When a newborn has grunting, respiration rate >90/min, and oxygen requirement >0.4 of $FiO_2$ within 6 hours of life, the infant is at high risk of having persistent tachypnea ${\geq}72$ hours. We need further study to find the way to reduce PTTN.

  • Esophageal duplication cysts are rare congenital lesions that occur as a result of a failure in the tubulation of the esophagus. They are most frequently single, tubular, or cystic. They may cause compressive symptoms or may be discovered incidentally on chest radiographs. They become symptomatic when complications develop. Symptoms often are related to the location of the duplication; esophageal lesions can create respiratory difficulties. The definitive diagnosis of esophageal duplication cysts requires the pathological evaluation of the cyst after surgical removal. We experienced a rare tubular esophageal duplication, in a 2-month old girl who presented with fever and grunting. This is the first reported case in which the sequence of events of ruptured tubular esophageal duplication with empyema, mediastinitis and aneurysm occured.

  • The aim of the present study was to analyse grunts of the sows and to extract parameters from the time and frequency signals in nursing behavior. Five crossbred $Landrace{\times}Yorkshire$ sows were used on day 5 or 6 postpartum. The grunts and the behaviors of the five sows were recorded with five digital camcorders. Three parameter groups [Group I: Formant vector alone, Group II: Formant vector+parameters from time signal, Group III: Formant vector+parameters from time signal-parameters eliminated by stepwise discriminant analysis backward (SDAB)] with parameter vectors extracted from single grunts in the maximum grunting rate period were used for individuality of the sows. The parameter groups were compared by a discriminant function analysis. The classification system adopted in the Group II represented the higher discriniation rate than those in other groups (Group I: 63.3%, Group II: 83.0%, Group III: 80.0%). This study demonstrated that formant, intensity, and pitch were available sound parameters for individuality of the sows during nursing behavior.

  • We describe here our experience with a neonate presenting with cyanosis, grunting, and cardiomegaly, who was diagnosed with isolated left ventricular noncompaction (IVNC) by echocardiography. The patient had high levels of N-terminal pro-B-type natriuretic peptide (NT pro-BNP) and symptoms of heart failure including poor feeding and tachypnea. During the period in which NT pro-BNP levels steadily increased, the patient suffered sudden cardiac arrest despite heart failure management. Following cardiopulmonary resuscitation, cardiac arrest was resolved, NT pro-BNP levels decreased, and all symptoms showed improvement. We consider that assessment of NT pro-BNP with cardiac functional analysis using echocardiography could help in the prediction of disease progress in IVNC.

  • With the increasing number of underground projects, the problem of rock-water coupling catastrophe has increasingly become the focus of safety. Grouting reinforcement is gradually applied in subway, tunnel, bridge reinforcement, coal mine floor and other construction projects. At present, cement-based grouting materials are easy to shrink and have low strength after solidification. In order to overcome the special problems of high water pressure and high in-situ stress in deep part and improve the reinforcement effect. In view of the mining conditions of deep surrounding rock, a new type of cement-based reinforcement material was developed. We analyses the principle and main indexes of floor strengthening, and tests and optimizes the indexes and proportions of the two materials through laboratory tests. Then, observes and compares the microstructures of the optimized floor strengthening materials with those of the traditional strengthening materials through scanning electron microscopy. The test results show that 42.5 Portland cement-based grouting reinforcement material has the advantages of slight expansion, anti-dry-shrinkage, high compressive strength and high density when the water-cement ratio is 0.4, the content of bentonite is 4%, and the content of Nano Silica is 2.5%. The reinforcement effect is better than other traditional grouting reinforcement materials.

  • Piglets communicate with their sow by using special sounds and synchronize their suckling behavior with their mother’s grunting. This study sought to find an effective sound for controlling the nursing/suckling behavior of pigs. Eighteen crossbred Landrace${\times}$Yorkshire sows and their piglets were studied on days 1, 7, and 14 post-partum. The parity of the sows was three or four. The litter size ranged from 9 to 11, with a mean of 10.4 piglets. Thirty minutes after the end of the last suckling, either the nursing-suckling sound (NSS) or the nursing-suckling sound + click sound (NSSCS) was played for 90 s randomly, and this experiment was repeated three times. The behavior of the sows and their piglets was recorded using camcorders and observed each second. When comparing NSS and NSSCS, there was less lying and sleeping and more sitting, standing, walking, and massaging when NSSCS was played than when NSS was played. These results suggest that NSSCS may induce more nursing and suckling in pigs than NSS.

  • Purpose: This study was designed to investigate rotavirus infection by comparing the clinical characteristics in neonates and infants. Methods: We enrolled 104 neonates and 250 infants wiht gastroenteritis and a rotazyme test positive reaction at the Soonchunhyang University Bucheon Hospital from February 2001 to January 2003. Results: The seasonal peaks of infection in infants occurred from February to June. However, in neonates, it occurred from October to December due to nursery outbreaks. Diarrhea, vomiting, fever and convulsions were significant symptoms in infants; however, metabolic acidosis with dehydration, jaundice, irritability, apnea, bloody stool, gastric residual, grunting, poor oral intake, lethargy as well as fever and diarrhea were more common in the neonates. Upper respiratory infection, pneumonia and bronchitis were present in the infants; however, necrotizing enterocolitis was more commonly observed in the in neonates. Among the patients with rotaviral infection, formula feeding was more popular than breast milk feeding in both the neonates and infants; however, this finding was not statistically significant. Conclusion: Rotavirus can be a significant pathogen in neonates as well as infants. Neonates suffering from fever, poor oral intake, lethargy and apnea should be investigated for rotaviral infection. A new vaccine, rotaviral specific immunoglobulin and treatment guidelines are needed for eradicating rotavirus infection. Further studies on isolation, infection pathway, immune response and treatment of rotavirus are needed.
