Modified Chevron Osteotomy for the Treatment of Hallux Valgus (Modified chevron 절골술을 이용한 무지외반증의 치험)

  • Hallux valgus has been characterized by a valgus deformity of the great toe at the metatarsophalangeal joint, along with medial deviation of the first metatarsal, and by three components. First, there is a valgus angle more than $20^{\circ}$ at the first, metatarsophalangeal joint. Second, there is a greater angle than $9^{\circ}$ between the first. and second metatarsals. Third, there is bursal hypertrophy at the medial eminence of the first metatarsals head. The etiology is multifactorial and many procedures have been reported in the treatment of hallux valgus. Most of the procedures are directed towards pain relief, correction of deformity, and preservation of dorsiflexion in the first metatarsophalangeal joint. One such treatment is the Modified chevron osteotomy. It is technically simple, and provides greater stability than a standard osteotomy, and allows early ambulation after surgery. We a reviewed 19 cases with 13 patients of hallux valgus deformity. They were all treated with the Modified chevron osteotomy at the Department of Orthopedic Surgery, Choong ang Gil Hospital, between June 1988 and May 1994. The results of the study were as follows; 1. The mean age was 36 years. Three patients(5 case) were male and ten patients(14 cases) were female. 2. The mean value of the hallux valgus angle was $34.1^{\circ}$, and the first to second intermetatarsal angle was $12.1^{\circ}$, preoperatively. These angles were corrected to $15.8^{\circ}$ and $8.5^{\circ}$, respectively. 3. The metatarsalgia subsided in 17 cases (89.5%). avascular necrosis, non union, and dorsal angulation complicatious were nonexistant. Early bone healing occurred in all cases. 4. The Modified chevron osteotomy is technically simple. It provides excellent pain relief, early ambulation, increased mechanical stability, and many avoids many complications such as AVN, non-union, and dorsal angulation.

Match Injuries Related to Adolescent Taekwondo Athletes (성장기 태권도 선수의 경기 손상)

  • Purpose: To research match injuries related to Taekwondo in adolescent athletes and find out preventing injuries. Materials and Methods: We analyzed 162 match injuries of middle and high school athletes, who were surveyed in 2005 and 2006 national tournament. Results: Among 162 match injuries, injuries in foot and hand were the most common and there were 35 cases in serious injuries such as fracture, open dislocation, open fracture of epiphyseal plate and so on. The very fast speed and destructive power of Takwondo athlete can cause open dislocation of great toe and can cause fracture of foot and upper extremity when kicking. Conclusion: We consider that injured athletes need proper treatments and sufficient rest not to accumulate training injuries and match injuries and need to improve mats of stadium, braces for feet and hands and put on braces for elbows to prevent match injuries.

The Wearing Sense of Male Adult Shoes - Comparison of Common Shoes with Elevated Shoes -

  • This research was administered in order to know the effects of heels on the foot by comparing the foot environmental characteristics when common shoes and elevated shoes are worn. First, 157 male adults in their 20s through 40s living in Busan were the inquiry subjects to reveal the shoes-wearing reality of adult males. Second, 7 male adults in their early 20s became the subjects for the experiments of wearing common shoes and elevated shoes. 1. Inquiry Results of Shoes-Wearing Reality Common-shoes wearers were in the order: 20s (43.9%) > 30s (24.8%) > 40s (8.3%). Elevated-shoes wearers were mostly 20s (12.1%), followed by 30s (8.3%) and 40s (2.5%). Among the wearing effects of elevated shoes were 'looking taller' (66.7%), 'no height complex & more confidence' (30.6%), and 'higher work efficiency' (2.8%). In sum, 97.3% of the male subjects believed in great positive effects by wearing elevated shoes. 2. Shoes-Wearing Experiment Results In foot skin temperature, significant differences between the two groups were admitted in outer foot a (p<0.05) and other areas (p<0.001), except in the instep. Elevated-shoes group had bigger skin temperature, while the order of temperature was the instep, the big toe, inner foot a/b/c and outer foot a/b/c. Significant difference was accepted in total sweat rate (p<0.05) and local sweat rate (p<0.01). Elevated-shoes group appeared higher in both rates. Significant difference (p<0.001) between the two groups was recognized in fatigue degrees after wearing, whereas significance (p<0.05) in elevated-shoes group was approved in fatigue before and after exercise. So elevated-shoes group experienced more fatigue, especially after exercise.

  • Phenol sympathetic block is valuable for the treatment of ischemic pain, gangrene, intermittent claudication, Paget's disease of the bone and pain associated with pancreatitis, pancreatic carcinoma, etc. The author has experienced a case of successful lumber sympathetic block using 7% phenol under fluoroscopy and given to a patient with Buerger's disease who had severe pain and ulceration of the right great toe for several years. After the sympathetic block, 2 epidural blocks with 2.5% bupivacaine were done in order to augment the effects of this sympathetic block. Subsidence of rest pain, increase in walking distance from under 100 M to over 500 M and circulatory improvement of the affected limb were observed, Improvement of circulation was confirmed by strain gauge plethysmography.

An optical fibre monitoring system for evaluating the performance of a soil nailed slope

  • Conventional geotechnical instrumentation techniques available for monitoring of slopes, especially soil-nailed slopes have limitations such as electromagnetic interference, low accuracy, poor longterm reliability and difficulty in mounting a series of strain sensors on a soil nail bar with a small-diameter. This paper presents a slope monitoring system based on fibre Bragg grating (FBG) sensing technology. This monitoring system is designed to perform long-term monitoring of slope movements, strains along soil nails, and other slope reinforcement elements. All these FBG sensors are fabricated and calibrated in laboratory and a trial of this monitoring system has been successfully conducted on a roadside slope in Hong Kong. As part of the slope stability improvement works, soil nails and a toe support soldier-pile wall were constructed. During the slope works, more than 100 FBG sensors were installed on a soil nail, a soldier pile, and an in- place inclinometer. The paper presents the layout and arrangement of the instruments as well as the installation procedures adopted. Monitoring data have been collected since March 2008. This trial has demonstrated the great potential of the optical fibre monitoring system for long-term monitoring of slope performance. The advantages of the slope monitoring system and experience gained in the field implementation are also discussed in the paper.

A study on the characteristics of female shoes in 2000's (여성구두의 디자인 특성에 관한 연구 -2000년대를 중심으로-)

  • The purpose of this thesis is to conduct a thorough research on shoes of 2000s, understand the changes and analyze the differences and the characteristics of the shoe designs of the past and present in order to predict and indicate the future direction of the women shoe designs. The shoes of the millennium not only kept the old fashioned shapes but also showed futuristic shapes. With the increasing importance of shoes in fashion, shoes with various images were created. As far as the materials are concerned, natural materials and artificial materials were both used. The colors were vivid and the futuristic colors. On the other hand, pastel toned colors and romantic colors were used in many different designs and shapes regardlees of the seasons. Straps and hills differed from the oldies. Heavy decorations were added to strap shoes to form a whole new design and hills show geometrical platforms or the avantgarde like heights. Feminine images overwhelmed the shoe designs of the millennium. To elaborate, Pumps displayed feminine images the most for it showed a high frequency number regardless of the seasons. Shoes are presented in aesthetical perspective rather then functional or practical perspectives that are based on human nature. Cultural, social, environmental and some part religious factors had a great influence on the evolution on the shoes. Especially in the 2000s, future oriented concepts were implemented on shoe designs, resulting a wider range of expressiveness and in the end leading to a more creative shoe designing. Changes in shoe designs could be pointed out in many ways according to the shapes materials, colors, modeling of decorations or also with the diverting trends.

Proximal Dome Osteotomy for Hallux Valgus (반월형 절골술을 이용한 무지 외반증의 치료)

  • Purpose: The purpose of this study was to evaluate the radiographic results and complications after the proximal dome osteotomy for hallux valgus. Material and Methods: 127 cases of clinically moderate to severe hallux valgus from October 1994 to September 1997 were included in this study. All had been surgically corrected with proximal dome osteotomy, bunionectomy, and distal soft tissue release. We compared the hallux valgus angle(HVA) and intermetatarsal angle(IMA) at preoperative, postoperative 3 weeks, postoperative 6 weeks, and postoperative 3 months. Also we reviewed the postoperative com plications. Result: The HVA averaged $34.1^{\circ}$ at preoperative, $4.3^{\circ}$ at 3 weeks after operation, $8.1^{\circ}$ at 6 weeks after operation, and $10.2^{\circ}$ at 3 months after operation. The lMA averaged $14.6^{\circ}$ at preoperative, $5.1^{\circ}$ at 3 weeks after operation, $5.6^{\circ}$ at 6 weeks after operation, and $7.3^{\circ}$ at 3 months after operation. We experienced 7 cases of malunion, 5 cases of limitation of motin at the first metatarso-phalangeal joint, 3 cases of hallux varus deformity, 2 cases of delayed union. Conclusion: Proximal dome osteotomy for moderate to severe hallux valgus deformity was considered as one of the effective treatment methods. And we try to avoid limitation of motion at the first metatarso-phalangeal joint after operation.

A Case of Subungal Abscess and Onycholysis Induced by Docetaxel (Docetaxel 투여 후 발생한 조갑하 농양 및 조갑 박리증 1예)

  • Docetaxel is a taxoid antineoplastic drug, which is widely used to treat locally advanced or metastatic non-small cell lung cancer (NSCLC). Among the adverse dermatological reactions, nail disorders such as bending, onycholysis, hypoor hyperpigmentation are rare. We report a case of a 62-year-old male with advanced NSCLC (cT4N3M1, stage IV), who developed purulent discharge and onycholysis in the nail of all his fingers and the left great toe after five courses of anti-neoplastic chemotherapy, which included docetaxel (cumulative dose: $370mg/m^2$, 590 mg). Seven days after the final session of chemotherapy, the patient had become aware of discoloration and swelling of the nail beds with out pain. Three days later, greenish-yellow purulent discharge oozed out from the involved nails. Microbiologic studies revealed Pseudomonas aeruginosa. Intravenous and topical antibiotics (mupirocin) were applied. After 2 weeks, regrown nails were observed and the onycholysis had improved.

The Anthropometric Factors in Isokinetic Peak Torque of Young Female Dancers (무용전공 여대생들의 등속성 최대우력에 영향을 미치는 인체계측적 요인)

  • Dance movements consist of combinations of movements such as jumping, rotation, maintaining balance, leg lifts, and plantar flexion with toe shoes. Dance movements require great muscle strength of lower extremities as well as muscular endurance. The purposes of this study were to investigate correlation between the anthropometric variables and the peak torque in young female dancers and to identify variables that affect isokinetic peak torque. Twenty-six female dancers ($19.7{\pm}1.2$ years of age) performed concentric maximum force efforts on the knee extensors and flexors at $60^{\circ}/sec$ and $120^{\circ}/sec$, the ankle plantar flexors (PF) and dorsiflexors (DF) at $30^{\circ}/sec$ and $120^{\circ}/sec$. Antropometric variables such as age, height, weight, body mass index (BMI), thigh girth, calf girth and duration of dance training were measured. To identify antropometric variables related to muscle strength, Pearson correlations were computed and a stepwise multiple regression analysis was performed. Pearson correlation coefficients of Knee extensor at $60^{\circ}/sec$ and $120^{\circ}/sec$ revealed moderate positive associations with BMI and thigh girth. Pearson correlation coefficients of ankle PF revealed low-to-moderate positive associations with height. Ankle DF also revealed moderate positive associations with BMI and calf girth. The main predictor variables of knee extensor at 60/sec, were stepwise multiple regression, age, height, thigh and girth.

Comparison of Sesamoid Bone Position and Hallux Valgus Angle in Weight Bearing Conditions between Subjects with and without Hallux Valgus

  • Purpose: Previous studies reported changes in the first metatarsophalangeal (MTP) joint angle in relation with and without weight bearing, but it is unclear whether sesamoid bone of the great toe changes in weight bearing conditions particularly in subjects with hallux valgus (HV). To investigate how weight bearing conditions can affect the position of the medial sesamoid bone (MSB), first MTP joint angle, and second intermetatarsal angle (IMA) in the recruited subjects. Methods: Subjects were recruited 24 with HV and 21 without HV in study. X-rays were taken in the weight bearing and non-weight bearing conditions. The distance of the MSB, first MTP joint angle, and second IMA were measured from the radiographs. Data were analyzed by paired and Independent t-test. The statistical significance level was p<0.05. Results: In both groups, the first MTP joint angles and the distance of the MSB were significantly smaller, while the second IMA was significantly greater in the weight bearing condition. The difference in the distance of the MSB between the two postures was significantly greater in the group with HV. Conclusion: Weight bearing can affect the first MTP joint angle, second IMA, and position of the MSB; the change in the position of the MSB in weight bearing was greater in the group with hallux valgus. The difference in these variables between weight bearing and non-weight bearing conditions may be considered when measuring HV.
