Topical Tacrolimus Resulting in a Marked Resolution of Perianal Crohn's Disease (Tacrolimus 연고제로 호전을 보인 항문 주위 크론병 3례)

  • Crohn's disease of the perineum is notably resistant to treatment. We describe our experiences of tacrolimus ointment application in three patients with intractable perianal Crohn's disease. At the time of diagnosis patients showed anal fissure with ulcer, anal fistula, or perianal abscess, respectively. These lesions did not respond to steroid, metronidazole, mesalazine, or azathioprine, though the pediatric Crohn's disease activity index was markedly improved during the treatment period (4 to 12 months). Topical tacrolimus was applied and resulted in marked healing and a rapid granulation of cavity edges within 6 weeks to 3 months. Topical tacrolimus at a low dose shows a promise for the management of childhood perianal Crohn's disease. Further controlled studies are required to assess the efficacy and safety of this treatment.

  • Ultrastructural changes in endomysial connective tissue, sarcolemma, transverse ridges and myofibrillar structures with particular attention given to Z-discs. A and I-filaments induced by heating to $80^{\circ}C$ were observed by transmission and scanning electron microscopy. Muscle heated to $80^{\circ}C$ produced granulation of sarcolemma aid ultrastructural changes and coagulation were observed in endomysial and sarcolemmal connective tissue. The edvient changes in myofibrillar structure were an increase in coagulation compactness of the A-band portion of sarcomere and disintegration of the I-bands. Z-discs appeared to be relative resistant to heat but I-filaments were observed to be most heat labile.

Application of a Composite Skin Equivalent using Collagen and Acellular Dermal Matrix as the Scaffold in a Mouse Model of Full-thickness Wound (콜라겐과 무세포진피를 이용한 혼합형 인공피부 개발 및 쥐 모델에서 창상치료 적용)

  • The aim of this study was to develop a composite human skin equivalent for wound healing. Collagen type1 and acellular dermal matrix powder were utilized as the scaffold with dermal fibroblasts and keratinocytes for the development of a composite human skin equivalent. Fibroblast maintained the volume of composite skin equivalent and also induced keratinocytes to attach and proliferate on the surface of composite skin equivalent. The composite human skin equivalent had a structure and curvature similar to those of real skin. Balb-C nu/nu mice were used for the evaluation of full-thickness wound healing effect of the composite human skin equivalent. Graft of composite skin equivalent on full-thickness wound promoted re-epithelialization and granulation tissue formation at 9 days. Given the average wound-healing time (14 days), the wound in the developed composite skin equivalent healed quickly. The overall results indicated that this three-dimensional composite human skin equivalent can be used to effectively enhance wound healing.

Influence of Solid Loading on the Granulation of 3Y-TZP Powder by Two-Fluid Spray Drying

  • The influence of solid loading in the slurry composition on the morphology of 3Y-TZP granules fabricated by two-fluid spray drying was investigated for solid contents varying between 30 wt% and 50 wt%. The resulting 3Y-TZP granules showed a sphere-like shape with diameters of $40-70{\mu}m$. However, a donut-like shape and a few cracks were observed on the granule surfaces fabricated using the slurry with 50 wt% solid content. The green density after cold isostatic pressing at 200 MPa was $2.1-2.2g/cm^3$, and a homogeneous fracture surface was obtained by complete destruction of granules. After sintering at $1500^{\circ}C$ for 2 h, all specimens had relative densities of 96.2 - 98.3%. With increasing solid content, the relative density decreased from 98.3% to 96.2%, but the grain size increased from $0.3{\mu}m$ to $0.6{\mu}m$. Highly sinterable zirconia granule powder could be obtained by controlling the slurry composition.

Mitomycin-C in Endoscopic Dacryocystorhinostomy (비내누낭비강문합술에서 Mitomycin-C의 효과)

  • Background and Objectives: The most common cause of the failure of endoscopic dacryocystorhinostomy is closure of the osteotomy site due to granulation or adhesion. We used mitomycin-C, an antineoplastic antibiotic agent, soaking over the osteotomy site to suppress fibrous proliferation and scar formation during the endoscopic dacryocystorhinostomy. Materials and Methods : A total of 20 Patients diagnosed with nasolacrimal duct obstruction were assigned randomly to either a mitomycin-C group or a control group. Endoscopic dacryocystochinostnmy has been used in both groups. In the mitomycin-C group, a piece of merocel soaked with 0.2 mg/ml mitomycin-C was applied to the osteotomy site and then after 30 minutes was removed. Results : All patients in the mitomycin-C group remained symptom free after removal of their silicone tube (100% success), and there were two patients in the control group who had recurrent epiphora (67% success). In the mitomycin-C group, the average surface area of the osteotomy at the end of the sixth postoperative month was 4.1 $\textrm{mm}^2$, whereas that of the control group was 2.5 $\textrm{mm}^2$. Neither serious systemic nor local toxicity were noted in the mitomycin-C group. Conclusion : Intraoperative mitomycin-C may possibly improve success rates over the endoscopic dacryocystorhinostomy procedure.

  • In order to establish methods of potato processing for mixed cooking with rice as main dish, the size and shape of potato granules and favorable ratio of potato granules to rice for cooking were studied preliminary. As the results cylindrical shaped potato granule of around 6mm in diameter and $10{\sim}13mm$ in length are more favorable than rectangular shape for mixed cooking according to panel test. Mixed cooking of 30% of treated potato granules with rice is the most favorable in the point of palatability.

Optimization on Pretreatment and Granule Tea Recipe of Polygonatum sibiricum Delar (둥굴레의 전처리 및 과립차 배합비 최적화)

  • The organoleptic properties of granule tea was optimized for granulation of Polygonatum sibiricum Delar(Dunggulle) tea for exclusion of scorched smell and increase of consumption. The length of 2 mm was suitable to cutting size of Dunggulle for preparation of the roasted Dunggulle. The optimum sensory conditions for aroma of Dunggulle granule tea showing 7.85 organoleptic score were 80.61% in ratio of Dunggulle extracts to total extracts, 12.77% in content of total extracts and 37.33% in rate of glucose to total sugar. The highest score of overall palatability was 5.96 at 61.11% in rate of Dunggulle extracts to total extracts, 13.79% in content of total extracts, and 60.92% in rate of glucose to total sugar.

  • These experiments were conducted to investigate the effect of Taklee Hwangki Tang(THT) on inflammation. THT extract did not affected on the leakage of evans blue into peritoneal cavity and mouse paw edema induced by histamine, but decreased the cottom pellet granuloma formation. Using proliferation of Balb/c 3T3 fibroblast cell line as an in vitro model of granulation tissue formation, the ability of THT to stumulate cellular proliferation of fibroblast cells was investigated. When the cells were seeded at $1{\times}10^4$ cells/well, balb/c 3T3 cells are reached to the late expponential phase at 3rd day. Under the conditions established above, THT increased the proliferation of Balb/c 3T3 cells at concentration of $10^-,\;10^{-6}\;and\;10^{-5}g/ml$. The treatment of $10^{-6}g/ml$ of THT did not influence onthe NDA syntesis and proteinsynthesis of the cells. The $10\%$ serum from THT treated mice(500mg/kg/day for 4 days) increased the proliferation of Balb/c 3T3 fibroblast markedly, but decreased the DNA synthesis and protein sythesis of the cells. The results suggest that THT may be of practical therapeutic use at the period of the last in. flammation.

  • The dissolution characteristics of DDB were markedly enhanced by preparing solid dispersions of drug with polyethylene glycol 6000. Solid dispersions of various weight fraction were formed by a melting method. And various tablets$(A{\sim}E)$ were prepared from these solid dispersions with excipients (lactose, com starch, Avicel and PVP) by wet granulation method. There were no significant differences in dissolution rates between physical mixture and DDB alone. But dissolution rates of solid dispersions were $1.4{\sim}2.0$ times greater than that of DDB alone and $1.2{\sim}1.8$ times greater than those of a commercial tablet.

Algin-Impregnated Vascular Graft II. Preliminary Animal Study

  • Microvel $^{\textregistered}$ double velour graft impregnated with a biodegradable algin was studied as a new vatscular graft. It is impervious to blood but still retains high porosity. This graft does not require preclotting during implantation and has good tissue ingrowth and biological healing properties. Two vascular grafts impregnated with algin (6mm in diameter) were implanted in the aorta of mongrel dogs without preclotting. Two identical grafts were preclctted and served as controls. The grafts were harvested 2 and 4 months postoperatively, and the healing pattern was examined by a light microscope after hemRtoxylineosin staining. It was observed that endothelial cells were incompletely covered on both algin-impregnated and control grafts after 2 month Implantation, while they were fully covered on both grafts after 4 month. There were no significant differences in subendothelial granulation tissue organization and fibrinoid material absorption between the algin-impregnated and control grafts. The algin-impregnated graft did not show any harmful effect on the healing and thus can be a new promising graft which is not necessary preclotting during the implantation.
