Management of an Inappropriately Treated Case of Auricular Hematoma

  • Othematoma is a condition in which blood collects beneath the perichondral layer of the pinna. Recurrent auricular hematoma can be a challenging medical problem, with complications resulting in ear deformity, therefore, appropriate management is important. Improper treatment can result in a deformity known as 'cauliflower ear' or 'wrestler's ear.' We report a case of othematoma that was inappropriately treated and present a review of current treatment options.

Comparison of Topical Agents for Bactericidal and Wound Healing Effect in Pseudomonas aeruginosa-infected Wound (Pseudomonas aeruginosa 감염창상에 사용되는 국소제제들의 항균효과 및 창상치유 효과의 비교)

  • Purpose: Pseudomonas aeruginosa is an etiologic agent in serious wound infection. Pseudomonas aeruginosa infection is problematic because this organism is resistant to many antimicrobial drugs. The purpose of this study was to compare the bactericidal effect of commonly used topical agents and their effect on wound healing. Methods: Pseudomonas aeruginosa-infected full-thickness skin defect was developed on the mouse to compare 3 commonly used topical agents-Betadine, 2% Gentamicin solution and 0.3% Acetic acid with the control group. Wound size change, bacterial colony counts and histologic findings of each groups were analyzed. Results: The wound size decreased in all treated groups as compared with the control group. However, there was no statistical difference. Gentamicin solution group was showed the lowest bacterial colony count and statistically significant difference compared with the control group(p=0.032). Other treated groups were also effective against Pseudomonas aeruginosa, but not different statistically. Histologic findings revealed that epithelialization, granulation tissue formation and microvessel proliferation were increased and necrosis and inflammation were decreased in all treated groups compared to the control group, but not different statistically. Betadine group significantly increased granulation tissue formation compared to the control group (p= 0.041). Conclusion: There is no universal topical agent that enhances most aspects of wound healing while simultaneously decreasing the bacterial concentration. However, Gentamicin solution may be an optimal topical agent for Pseudomonas aeruginosa infected wound. Further study should experiment on human with Gentamicin solution to confirm a effect on Pseudomonas aeruginosa infected wound for clinical applications.

  • An organic compound fertilizer was manufactured using wet oxidation human waste as principal source of phosphorus and organic matter. The waste was treated with sulfuric and glutamic acids to increase the available and water-soluble $P_2O_5$ contents. The treatment of 0.1 N sulfuric acid with 24 hours curing was best way in recovering the maximum percentage of $P_2O_5$ originally in the waste. The particle size distribution of trial product varied considerably in the amount of glutamic acid used for granulation. The number of relatively large fertilizer particles was increased as the amount of glutamic acid was increased. The granule crushing strength was generally high in large granules in which 12.5 weight percent of glutamic acid were used for granulation. The trial product showed high moisture absorption due to its porous structure and chemical makeup.

A Study on Genetically Optimized Fuzzy Set-based Polynomial Neural Networks (진화이론을 이용한 최적화 Fuzzy Set-based Polynomial Neural Networks에 관한 연구)

  • In this rarer, we introduce a new Fuzzy Polynomial Neural Networks (FPNNs)-like structure whose neuron is based on the Fuzzy Set-based Fuzzy Inference System (FS-FIS) and is different from that of FPNNs based on the Fuzzy relation-based Fuzzy Inference System (FR-FIS) and discuss the ability of the new FPNNs-like structurenamed Fuzzy Set-based Polynomial Neural Networks (FSPNN). The premise parts of their fuzzy rules are not identical, while the consequent parts of the both Networks (such as FPNN and FSPNN) are identical. This difference results from the angle of a viewpoint of partition of input space of system. In other word, from a point of view of FS-FIS, the input variables are mutually independent under input space of system, while from a viewpoint of FR-FIS they are related each other. In considering the structures of FPNN-like networks such as FPNN and FSPNN, they are almost similar. Therefore they have the same shortcomings as well as the same virtues on structural side. The proposed design procedure for networks' architecture involves the selection of appropriate nodes with specific local characteristics such as the number of input variables, the order of the polynomial that is constant, linear, quadratic, or modified quadratic functions being viewed as the consequent part of fuzzy rules, and a collection of the specific subset of input variables. On the parameter optimization phase, we adopt Information Granulation (IG) based on HCM clustering algorithm and a standard least square method-based learning. Through the consecutive process of such structural and parametric optimization, an optimized and flexible fuzzy neural network is generated in a dynamic fashion. To evaluate the performance of the genetically optimized FSPNN (gFSPNN), the model is experimented with using gas furnace process dataset.

A Feasibility Study on the Polymer Solidification of Evaporator Concentrated Wastes (폐액증발기 농축폐액 폴리머고화 타당성 연구)

  • The granulation equipment of concentrated wastes is manufactured for the polymer solidification of concentrated wastes. It uses liquid sodium silicate as a granulating agent for the granulating of dried powder containing boric acid. The granulating agent is sprayed in the form of droplet and mean size of dried granules is $2{\sim}4mm$. The new technology which has been used for the polymer solidification of spent resin in U.S. and certified by Nuclear Regulatory Commission (NRC) is successfully applied to concentrated wastes. This uses in-situ solidification process within drum without mechanical mixing. Maximum loading of waste can be achieved without increasing of waste volume. Polymer waste forms were evaluated with several test such as fire test, compressive strength test, leaching test, immersion test, irradiation test, and thermal cycling test according to standard test procedures.

Wall-Suction Assisted Vacuum Sealing for Treatment of Infected Diabetic Foot Ulcer (벽매립형 중앙 흡인장치를 이용한 감염성 당뇨병성 족부 궤양의 밀봉 치료)

  • Purpose: The infected diabetic foot patients were reviewed to analyze the result of new dressing methods using a wall-suction instruments. Materials and Methods: Eighteen patients treated with wall-suction assisted vacuum dressing were included. After debridement under local anesthesia, a sponge pad, a drain, and a surgical drape were used to seal the wound. Negative pressure applied by the wall-suction at around 200 mmHg and dressing were repeated in every two to three days. The results of repeated wound cultures, growth of granulation tissues, and CRP level were closely observed on the regular basis. Results: Rapid growth of granulation tissues was noticed around the wound in 16 cases. No organism was obtained in an average 5 days and wound coverage was possible in an average 18 days. The CRP level returned to normal in an average 4 weeks. Two patients with end stage renal disease, who were regularly hemodialised, underwent major amputation. Conclusion: New dressing method has the following advantages: a rapid wound improvement in the patients with infected diabetic foot, less expensive, less painful, impediment of bacterial contamination in the hospital room. However, further study will be needed for the end stage renal disease patients.

Application of a paste-type acellular dermal matrix for coverage of chronic ulcerative wounds

  • Background Chronic wounds occur due to failure of the normal healing process, associated with a lack of deposition of cellular components and a suitable microenvironment such as the extracellular matrix (ECM). Acellular dermal matrix (ADM) is viewed as an ECM substitute, and a paste-type ADM has recently been introduced. We hypothesized that CGPaste, an injectable paste-type ADM, could serve as a scaffold and promote wound healing. Methods We retrospectively studied seven patients in whom CGPaste was applied between 2017 and 2018, who had pressure ulcers, necrotizing fasciitis, diabetic foot ulcers, traumatic defects, and osteomyelitis. The goal of applying CGPaste was to achieve complete wound healing with re-epithelialization or growth of granulation tissue, depending upon the wound bed status. CGPaste was injected based on the wound size along with the application of a dressing. Results Four of the seven patients showed granulation tissue on their wound bed, while the other three patients had a bony wound bed. The mean wound area was $453.57mm^2$ and the depth was 10.71 mm. Wound healing occurred in five of the seven patients (71.43%). The mean duration of complete healing was 2.4 weeks. Two patients showed failure due to paste absorption (29.57%); these patients had wound beds comprising bone with relatively large and deep wounds ($40{\times}30$ and $30{\times}20mm^2$ in area and 15 and 10 mm in depth). Conclusions CGPaste is an effective option for coverage of small and deep chronic wounds for which a flap operation or skin grafting is unfeasible.

Studies on the Problems about Preparation and Packagin of Granular Enzymic Digestives (과립소화효소제(顆粒消化酵素劑)의 제조(製造) 및 포장(包裝)에 따른 문제점(問題點) 검토(檢討))

  • It is well known that the commonly used granular enzymic digestives are easy to diminish it's potency through preparation and custody. We made an experinent to know the proper way of manufacturing and keeping the granules without a loss of potency by $L_8$ orthogonal Tables. The factors, we considered, decomposition of enzyme activity through manufacturing are (1) sufficiency of granules drying (2) with or without sodium bicarbonate $(NaHCO_3)$ in gramules (3) separate wet granulation of enzyme and the rest of the component or mix together (4) kind of packing materials (Polycello or quartet silner paper) Besides we counted (5) humidity (6) temperature as block factors. Then we tested the potency of each sample by intervals of. a week, 2 weeks and we foretelled the potency of after one year by the life test. From the experimental results, main factor of activity loss is preserving humidity. Therefor the sufficiency of packing is became a main problem and the granules have to be made by separate wet granulation to inhibit it's decomposition. Temperature is the factor that accelerates loss of activity by humidity but does not so much by itself mixing enzyme toge therwith $NaHCO_3$ is not became a matter so far as it is dried.

Increased Microbial Resistance to Toxic Wastewater by Sludge Granulation In Upflow Anaerobic Sludge Blanket Reactor

  • The relationship between the layered structure of granules in UASB reactors and microbial resistance to toxicity was investigated using disintegrated granules. When no toxic materials were added to the media, the intact and disintegrated granules exhibited almost the same ability to decrease COD and to produce methane. However, when metal ions and organic toxic chemicals were added to a synthetic wastewater, he intact granules were found to be more resistant to toxicity than the disintegrated granules, as determined by the methane production. The difference in resistance between the intact and disintegrated granules was maximal, with toxicant concentrations ranging from 0.5 mM to 2 mM for trichloroethylene with toluene and 5 mM to 20 mM for metal ions (copper, nickel, zinc. chromium, and cadmium ions). The augmented COD removal rate by granulation compared to disintegrated granules was also measured in the treatment of synthetic and real wastewaters; synthetic wastewater, $-2.6\%$; municipal wastewater, $2.8\%$; swine wastewater, $6.4\%$; food wastewater, $25.0\%$; dye works wastewater, $42.9\%$; and landfill leachate, $61.8\%$. Continuous reactor operation also demonstrated that the granules in the UASB reactor were helpful in treating toxic wastewater, such as landfill leachate.

Optimization of self-dispersible floating granule (UG) of flucetosulfuron and its herbicidal performance (Flucetosulfuron 수면부상형 입제의 최적화 및 제초효과)

  • This study was conducted to optimize formulation factors of a self-dispersible floating granule of flucetoaulfuron that can be used at a low application volume (5 kg $ha^{-1}$). SPA and xanthan gum were selected as a binder because blending of them showed good granulation and floatation. Potassium chloride selected as a filler also showed good granulation, floatation and disintegration properties. Among wetters and dispersants, Surfynol 440 gave the best dispersibility The optimum granule size was determined to be 1.0 mm considering floating time and dispersibility. Flucetosulfuron was mixed with the blend of SPA and xanthan gum, potassium chloride, and Surfynol 440 and formulated as a floating self-dispersible granule at 1.0 mm in diameter. Indoor and outdoor field experiments showed that the floating granule of flucetosulfuron performed well, covering long range from the application spot up to 15 m in weed control. Therefore, this floating technology may be used for laborsaving herbicide formulation development and help to reduce workloads for herbicide application.
