스피로피란 기반 친수성 가교제를 활용한 다중 자극 감응형 하이드로젤 (Multi-responsive hydrogel cross-linked synthesized spiropyran-based hydrophilic cross-linker)

  • 온도, pH, 빛 및 힘 등의 외부 자극에 반응하여 그 구조나 물리 화학적 특성이 변화 가능한 자극 감응형 하이드로젤에 대한 연구가 활발하게 진행되고 있다. 본 연구에서는 응력 감응형 분자인 스피로피란을 사용하여 응력 및 pH 감응형 하이드로젤을 제조하였다. 먼저, 폴리에틸렌 다이아크릴레이트(PEGDA)를 스피로피란 분자 양 끝에 접목시켜, 수용액에 쉽게 용해될 뿐만 아니라 하이드로젤 가교제 역할이 가능한 아령모양(PEG-spiropyran-PEG)의 SP-PEGDA 분자를 합성하였다. 이렇게 합성한 SP-PEGDA로 가교된 하이드로젤은 팽윤에 의해 발생하는 내부 응력에 의해 노란색의 스피로피란(SP) 분자를 보라색의 메로사인(MC) 형태로 변환시켰다. 또한 pH에 따라 양성화된 메로사인(MCH) 형태로 변환하여 팽윤과 수축을 시각화 하였다.

Cone-beam computed tomographic comparison of chin symphysis bone particles and allograft versus iliac crest bone graft alone for reconstruction of alveolar bone defects in cleft patients

  • Objectives: The aim of this study was to compare morbidities and duration of surgery, as well as bone formation in alveolar defects reconstructed with symphysis bone combined with allograft and iliac crest bone graft in patients with cleft palate. Patients and Methods: This randomized clinical trial was performed with 22 patients with unilateral alveolar cleft with a follow-up period of 12 months. In 12 patients, alveolar defects were reconstructed with chin bone graft plus allograft (Group A), while for the other 10 patients, iliac bone crest was used as donor site (Group B). Duration of surgery as well as occurrence of morbidities and complications were recorded. In addition, cone-beam computed tomographic (CBCT) scans were performed before surgery and 12 months after surgical procedures in order to compare bone formation between the two groups. Results: Postoperative CBCT demonstrated a mean bone fill percentage of 76.9% of the alveolar defect in Group A, compared with 77.0% in Group B. Paresthesia in the lower lip or chin did not occur in any patients of Group A. The mean duration of the surgical process was significantly shorter for Group A (40 minutes vs 76 minutes, P<0.001). In addition, patients in Group A regained normal gait faster than patients in Group B (1 day vs 9.5 days). Conclusion: Mandibular symphysis bone graft in combination with allograft results in favorable outcomes in patients with unilateral alveolar clefts.

화상을 동반한 당뇨발 환자에게 Razor Blade를 이용한 부분층피부조각 이식술의 효용성 (Fragmented Split-Thickness Skin Graft Using a Razor Blade in Burn Induced Diabetic Foot)

  • Diabetic patients have an increased risk of burn injuries on foot. Because of their diabetic neuropathy, they could contact with hot water or warming device without being aware of it. Split-thickness skin graft (STSG) is successful in treatment of various wound types; however, donor site wounds are sometimes problematic, and complications such as pain and impaired healing often occur. Although, donor site wounds in healthy young individuals can rapidly heal without complications, the wound-healing capacity of elderly patients or those with a comorbidity has been reported to be low. The dermatome is the most commonly used tool because it can harvest a large skin graft in one attempt. However, it is difficult to harvest tissues if the area is not flat. Furthermore, because the harvested skin is usually rectangular, additional skin usually remains after skin grafting. Therefore, use of razor blade and fragmented STSG on a large defect area is advantageous for harvesting a graft with a desired size, shape, and thickness. From January 2018 to July 2018, fragmented STSG was used in 9 patients who suffered from burn induced open wound on foot with diabetic neuropathy. With this approach, healing process was relatively rapid. The mean age of patients was 70 (57~86 years) and all of 9 patients had diabetes mellitus type 2. In all patients, the skin graft on the defect site healed well and did not result in complications such as hematoma or seroma.

Bacterial cellulose matrix and acellular dermal matrix seeded with fibroblasts grown in platelet-rich plasma supplemented medium, compared to free gingival grafts: a randomized animal study

  • Purpose: Mucogingival defects (MGDs), such as dental root recessions, decreased vestibular depth, and absence of keratinized tissues, are commonly seen in dental clinics. MGDs may result in functional, aesthetic, and hygienic concerns. In these situations, autogenous soft tissue grafts are considered the gold-standard treatment. This study compares the healing process of free gingival grafts (FGGs) to bacterial cellulose matrix (BCM) and human acellular dermal matrix (ADM) seeded with fibroblasts from culture supplemented with platelet-rich plasma in a rat model. Methods: Surgical defects were made in rats, which received the following treatments in a randomized manner: group I, negative control (defect creation only); group II, positive control (FGG); group III, BCM; group IV, BCM + fibroblasts; group V, ADM; and group VI, ADM + fibroblasts. Clinical, histological, and immunological analyses were performed 15 days after grafting. Clinical examinations recorded epithelium regularity and the presence of ulcers, erythema, and/or edema. Results: The histological analysis revealed the degree of reepithelization, width, regularity, and presence of keratin. The Fisher exact statistical test was applied to the results (P<0.05). No groups showed ulcers except for group I. All groups had regular epithelium without erythema and without edema. Histologically, all groups exhibited regular epithelium with keratinization, and myofibroblasts were present in the connective tissue. The groups that received engineered grafts showed similar clinical and histological results to the FGG group. Conclusions: Within the limitations of this study, it was concluded that BCM and ADM can be used as cell scaffolds, with ADM yielding the best results. This study supports the use of this technical protocol in humans.

치아회분과 석고혼합제재 매식과 자가골 동시 이식후 치유과정에 관한 실험적 연구 (AN EXPERIMENTAL STUDY ON THE HEALING PROCESS OF TOOTHASH, PLASTER OF PARIS AND AUTOGENOUS BONE COMPOSITE GRAFTING IN DOGS)

  • The purpose of this study was to evaluate the efficacy of adding autogenous bone to the toothash-plaster mixture in the healing process of bone. Full-thickness round osseous defects with the diameter of 20mm were made at the calvarial bone of adult dogs (n=19) bilaterally, which were thought to be critical size defect. The right defects were repaired with the toothash-plaster mixture plus autogenous bone (compressed volume 0.3cc) and the left defects with only toothash-plaster mixture. At 2-, 4-, 8-, 12- and 20- week after implantation, dogs were sacrificed and evaluated the osseous healing of bony defects clinically, radiographically, and microscopically. The results were as follows; 1. At the clinical observation, the wound healed very well without any problem except severe swelling in the early period after operation. Slight depression was recognized at the both sides when the portions of cranial defect were palpated. 2. There were statistically significant differences between toothash-plaster mixture groups and autogenous bone added groups at the same period, and among the groups in the bone density of the digital radiograms (P<0.001). There was a tendency that bone density was increasing with time. 3. In light microscopic examination, new bone formation was more active in the autogenous bone added groups than toothash-plaster mixture groups at the early period after implantation but there is little difference at 20-week after implantation. 4. In fluorescent microscopic examination, the fluorescent band could be observed at the area of active bone formation and the band was more distinct in the autogenous bone added groups then toothash-plaster mixture groups. 5. In transmitted electron microscopic examination, organelles such as rER, Golgi complex and secretory granule and osteoblast were observed. In summary higher volume ratio of autogenous bone is needed to improve the bone healing in that there is little difference between toothash-plaster mixture group and autogenous bone added group at the 20-week after implantation in spite of new bone formation was more active in the autogenous bone added groups than toothash-plaster mixture groups at the early period after operation.

성견 하악골 절단 후 기계적 골 견인에 의해 형성된 골 신장부에 대한 시기별 조직학적 변화 (HISTOLOGICAL CHANGES IN THE ELONGATED BONE AFFECTED BY OSTEODISTRACTION OF THE MANDIBLE IN THE DOG)

  • Purpose : Traditionally, the treatement of choice has been a bone grafting procedure to increase the length of bone in case of actual length discrepancy. But, bone grafting procedure has many disadvantages, for example, graft resorption, donor site morbidity, and so on. So, many trials have been performed to avert the use of autogenous bone graft via introducing new materials or methods. And, one of those trials has been realized by the development of a technique inducing bone lengthening by osteotomy (or corticotomy) and slow gradual distraction of the osteotomized segments. This new technique of bone lengthening dates back to the early 20th century. But, the majority of information concerning the biology of new bone formation during bone lengthening and technical details of the procedure were produced by extensive clinical and experimental studies performed by Ilizarov, a Russian surgeon. According to Ilizarov, with adequate blood supply, preservation of periosteum, rigid fixation of the osteotomized segments, and proper rate and rhythm of distraction, intramembranous bone rapidly develops within the distraction gap in the limb lengthening procedure. In the limb lengthening, many orthopedic surgeons try to observe the biologic and clinical principles recommended by Ilizarov. In the oral and maxillofacial region, however, not a few studies must be performed to apply this surgical technique in the clinical cases. Besides, the mechanism of bone formation in the distraction gap is not clear, yet. The purpose of this experiment was to scrutinize serially the histological changes in the elongated bone affected by osteodistraction of the mandibular body in an adult canine model. In addition, it was performed to confirm the presence of specific region(s) which was important in the bone formation in the gap through the observation of the expression pattern of osteocalcin and osteonectin with the immunohistochemical examination. Materials and Methods : The experimental and control specimens were obtained from seven adult male mongrel dogs weighing over 20kg. The distractors were custom-made linear extraoral devices and bicortical fixation screws were 2.3mm in diameter, 50mm in total length, 15mm in screw length. The distractors were devised to produce a linear gap of 0.75mm between two bony segments every $360^{\circ}$ turn of the rotation rod of the device. The mandibular body of the right side of each animal was corticotomized perpendicular to the occlusal plane and then two bony segments were separated completely by careful manipulation of the segments with bone forceps. The left side of each animal was left intact. This side was served as control. At sixth day after osteotomy and fixation of the segments were performed, distraction of the segments was commenced with a rate of 1.1mm/day and a rhythm of two/day for ensuing 7 days. The animals were euthanized at the 16th. 29th, and 44th day after the osteotomy. The bony specimens were decalcified, embedded in paraffin, sectioned $5{\mu}m$ thick and stained with H&E. The prepared specimens were examined under the light microscope. And, immunohistochemical examinations using anti-osteocalcin antibody (OC1, Biodesign, USA) and anti-osteonectin antibody (Haematologic Technologies Inc., Essex, VT) to locate the expressions of osteocalcin and osteonectin, respectively, were performed. Results : 1. New bone was observed already at the 16th. day after osteotomy. This suggests that new bone formation in osteodistraction was commenced at an early stage of the regenerative process. But, radiologically and microscopically, bony union was not completed in the distraction gap at the 44th. day after osteotomy. Therefore, rigid fixation must be maintained between the bony fragments till the complete bony union is confirmed clinically rather than one month or so after the completion of distraction.

채소 접목묘 일본 선박 수출 현황 및 수송 환경 분석 (Current Status of Vegetable Grafted Seedling Shipping Export to Japan and Analysis of Transportation Environment)

  • 농산물 검역통계를 통해서 채소 모종 수출 현황과 채소 접목묘의 일본 선박 수송 시의 온도 및 상대습도 변화를 분석하여 국내 채소 모종 수출 연구와 정책 수립을 위한 기초자료를 제공하기 위하여 본 연구를 수행하였다. 2007~2016년 사이 수출된 채소 모종은 배추, 양배추 등 엽채류 4종, 토마토, 오이 등 과채류 7종, 근채류 1종으로, 전세계 20여 국가에 46,146,536주(총 1,839건) 수출 되었다. 상업적 목적을 위한 채소 모종의 주요 수출국은 일본이었고, 2016년 기준 일본에 수출된 모종은 주로 과채류 접목묘인 가지, 토마토, 오이, 수박 및 고추 등으로 총 2,575,446주가 수출되었으며, 총 수출 금액으로는 0.7~1.6백만불로 추산된다. 일본으로 수출되는 접목묘는 도시농업용과 농가용으로 구분되고 있으며, 온도 $15{\sim}18^{\circ}C$의 냉장 컨테이너를 이용한 선박 수송은 포장 ${\rightarrow}$ 선적 ${\rightarrow}$ 검역(한국 부산항) ${\rightarrow}$ 검역(일본 시모노세키항) ${\rightarrow}$ 바이어 수령의 과정으로 대략 24시간 내외가 소요되고 있었다. 일본의 도시농업 발달로 인한 채소 모종 수요의 급격한 증가와 러시아 등 인근 국가들의 채소 접목묘에 대해 높아지고 있는 관심과 수요는 우리나라 채소 접목묘 수출 규모를 확대할 수 있는 기회가 될 것이다. 채소 모종 수출을 확대하기 위해서는 본 연구 결과를 기반으로 정확한 채소 모종수출 정보의 수집 및 분석이 이루어질 수 있도록 정부의 적극적인 정책 지원과 수출 전용 모종 생산, 모종 저장 및 수송 관련 기술 개발을 위한 연구가 필요하다.

명반처리에 의한 견직물개선연구 -Sericin 정착을 중심으로 하여- (Studies on Sericin Fixation by Use of Alum Meal)

  • 본연구는 백반이 sericin 정착하는 작용을 구명하기 위해서 시행된 것이며 이미 유혁기술에서 사용하여 오던 실예를 생사에 적용해보려는 것이었다. 타인의 보고는 Cr-명반이나 tannin, formalin 또는 vinyl acetate로 sericin 정착한 사실이 있기는 하나 이들은 섬유의 흡습성을 도외시한 것이어서 본보는 섬유의 흡습성을 보존하면서 sericin 정착을 하는 방식을 취하기로 했다. 한편 sericin 정착기구를 상세히 알기위해 섬유의 침간 mechanism과 정착 mechanism도 고찰하여 보았다. 본실험에서 얻어진 결과는 다음과 같다. 1. 백반을 sericin 정착에 이용할 때는 타인의 보고에서와 같이 고온처리는 금물이며 고온처리는 섬유를 경화시켰다. 2. 백반을 저온처리하는 대신 3 시간 이상 처리함이 바람직하였고 이때 백반만으로도 sericin 정착과 더불어 방수효과를 보였다. 3. 1% 백반농도이면 sericin 정착이 가능하였다. 4. sericin 정착만 고려할때는 0.5% NaOH 10분간 처리후 1% 백반용액 3시간처리방식이 가장 좋았다. 5. 그러나 sericin 정착과 흡습성을 고려할때는 상기 방식의 역방법이 가장 좋았다. 6. sericin 정착검정은 습윤시료의 건조곡선도시로 더욱 선명하게 다루었다. 7. 연감률은 백반 -NaOH 처리의 경우 4.3%였는데 이러한 정도이면 실용 선습에 능히 sericin 유실을 막을 것으로 본다. 8. 흡습성도 백반-NaOH 처리방식에서 미처리손과 동일한 흡습성을 유지했다. 9. 처리직후와 처리 3개월후의 강력경도에 변화가 없었다. 10. 결론적으로 본처리는 무연무색처리로서 유색처리(탄닌, Cr-명반)방법과 취기처리(formalin, vinyl acetate 처리)보다 개선된 방식이었다.

혈소판 농축 섬유소가 골모세포 증식과 분화에 미치는 영향 (The Effects of Platelet-Rich Fibrin on Osteoblast Proliferation and Differentiation: Effects of Platelet-Rich Fibrin on Osteoblasts)

  • 임플란트 식립 시 가장 빈번하게 맞게 되는 문제점으로 임플란트 식립 부위에서의 불충분한 골량과 해부학적 구조에 의한 접근성의 문제를 들 수 있다. 일반적으로 성장 인자들은 치유 과정이나 조직 형성에 있어서 가장 기본적인 필수 요소로 인정되고 있다. 이러한 이유로 골 이식 재료의 효과를 증진시키기 위한 성장 인자들이 최근에 주목을 받고 있다. 혈소판 내 granules에는 높은 농도의 다양한 성장 인자들이 포함되어 있다. 특히, platelet-rich fibrin (PRF)는 2세대 혈소판 농축 인자로 항응고제가 들어있지 않은 상태로 얻을 수가 있고, 혈소판과 많은 성장 인자들이 풍부한 섬유소 막을 포함하고 있다. 이번 연구의 목적은 in vitro 상에서 골아 세포에 대한 PRF의 영향을 알아보고자 하였다. 특히 치유와 재생에 연관된 주요 기능으로써 증식과 분화에 대한 영향을 조사하고자 하였다. 이를 위해서, PRF 내에서 방출되는 성장 인자(platelet-derived growth factor subunit B와 transforming growth factor-${\beta}1$)의 농도, 세포의 생존능력, alkaline phosphatase (ALP) activity, type 1 collagen 합성, 골아 세포의 분화 지표로써 ALP와 Runx2의 발현 정도와 골 기질 단백질로써 type 1 collagen의 발현 정도에 대해서 조사하였다. 이 실험을 통하여 PRF는 치유 시 필요한 타당한 기간 동안에 충분히 자가 성장 인자의 방출을 유지하고 있음을 알 수 있었고, 골아 세포의 증식과 분화에 대해서 긍정적인 효과가 있음을 보여 주였다. 제한적인 실험이지만, 골재생을 위한 PRF의 사용은 골 치유와 골 개조에 있어서 증진 효과를 가져다줄 수 있는 촉망되는 방법 중 하나가 될 수 있을 것이다.

하악 자연치열에 대합되는 상악 임플란트 전악 수복 증례 (Implant-supported maxillary full-arch fixed prosthesis opposing mandibular natural dentition: A clinical report)

  • 치주질환에 의해 수평적, 수직적 골흡수를 보이는 환자에게 전악 임플란트 고정성 보철수복 치료를 계획한 경우에는 저작 기능, 심미, 발음 등을 고려한 임플란트 식립이 필요하다. 이를 위해 철저한 임상적, 방사선학적 진단이 필요하며 광범위한 골이식과 연조직 이식술이 요구되기도 한다. 아직까지도 골이식한 상악에 대한 적절한 임플란트 식립 개수, 하부구조물 분할 여부 및 상부 구조물의 유지 방법 등에 대한 명확한 지침이 마련되어 있지 않기 때문에 진단과정에서 이를 고려한 검사가 요구된다. 본 증례에서는 치주질환 병력이 있는 환자에서 상, 하악 다수 치아 발거 및 치주 치료 후, 골이식 및 연조직 이식을 동반한 다수 임플란트 식립과 하악 자연 치열에 대합되는 상악 임플란트 전악 보철수복을 통하여 기능적, 심미적으로 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.
