간호조무사의 직무영역별특성이 직무만족도와 스트레스에 미치는 영향 -서울시 의료기관 근무자 중심으로- (A Study on Job Satisfaction and Stress in Relation to Job Features of Nursing Assistants - Case Study with Nursing Assistants in Seoul Metropolitan City -)

  • 본 연구는 간호조무사의 직무영역별 직무분석을 통한 직무특성이 직무만족도와 스트레스에 미치는 영향을 연구하고자 한다. 연구대상자는 서울시에 소재하고 있는 의료기관에 근무 중인 간호조무사 469명을 대상으로 2013년 5월 1일부터 9월 4일까지 연구를 실시하였다. 본 연구의 실증분석은 모두 유의수준 5%에서 검증하였으며 통계처리는 SPSS/WIN18.0프로그램을 사용하였고 빈도분석과, t-test와 일원변량분석 및 다중회귀분석을 실시하였으며 다중범위검증을 통계적으로 분석하였다. 분석결과 간호조무사의 자격증취득 연도와 연령, 최종학력에 따른 직무만족도에서 유의미한 차이가 나타났다. 그리고 직무특성에 따른 직무만족도 차이에서는 근무지와 담당직무영역 및 근무형태에서 가장 높게 나타났다. 본 연구의 시사점은 간호조무사의 직무능력 향상과 전문 간호 인력이 되도록 담당 직무영역별로 표준화된 업무지침과 교육프로그램을 통해 지속적인 관리가 이루어 져야 할 것으로 사료된다.

융합적 사고를 적용한 응급구조학의 문제중심학습 (Application of convergence thinking in Problem-based learning on paramedic education)

  • 본 연구의 목적은 융합적 사고를 적용하여 응급구조학과 학생들의 현장문제해결에 도움되고자 하는데 있다. 응급구조과 3학년에 재학 중인 학생 78명을 대상으로 15주간 동안 문제중심학습 시나리오 수업을 20시간 진행하였으며, 수업 진행 후 미응답자 5명을 제외한 73명 학생의 자료를 분석하였다. 학습관심도에 대한 질문 중 조사대상자의 "문제중심학습 시나리오 수업방식이 강의식 수업보다 학습과 지식습득에 효과적이었다'라는 질문에 각각 보통이라고 하였고, 38%로 가장 높았다. 수업에서 학습한 내용들이 문제해결에 있어 적절한 해결방안을 생각해 내는 능력 향상에 도움이 되었다"라는 질문에 57.5% '그렇다'라고 하였고, 수업에서 학습한 내용들이 응급 현장에서 자신감을 갖는 데에 도움이 되었다"에 50.7%가 그렇다고 답하였다. 문제중심학습은 어느 한 학년에 국한되지 않고 단계별, 지속적으로 현장상황을 이해할 수 있는 학습교과과정으로 적용된다면 효과적이고, 효율적인 교육방법이 될 것이다.

의과대학 학생들의 환자안전에 대한 인식 (Medical Students' Perceptions and Intentions Regarding Patient Safety)

  • Background: The purpose of this study was to examine undergraduate medical students' perceptions and intentions regarding patient safety during clinical clerkships. Methods: Cross-sectional and self-administered questionnaire survey was conducted on 34 students from one medical school using a modified version of the Medical Student Safety Attitudes and Professionalism Survey (MSSAPS). We assessed $4^{rd}-year$ medical students' perceptions of the cultures ('safety', 'teamwork', and 'error disclosure'), 'behavioural intentions' concerning patient safety issues and 'overall patient safety'. The overall response rate was 66.4%. Results: Among safety domains, "teamwork culture" was rated highest. "Error disclosure culture" received the lowest ratings. Regarding the error disclosure domain, only 10% of respondents reported that they have received education or training on how to disclose medical error to patients. Independent of survey domains, when students were asked "Overall, do you think your hospital is safe based on your clinical rotation?", 61.8% reported that the hospital was safe. Conclusions: Assessing students' perceptions of safety culture can provide clerkship directors and educators with information that enhances the educational environment and promotes patient safety. Discussions of medical errors, patient safety, and how best to incorporate an analysis of these issues into the existing curriculum are needed.

응급처치 교육과정을 배우는 학생들의 개인의료정보 보호에 대한 인식도 조사 - 응급구조과와 간호과 학생을 중심으로 - (The Perception Survey for Personal Health Information Protection of First Aid Training Courses Students - Focused of EMT students and Nursing students -)

  • Purpose. The checked of perception for the protection of personal medical information of EMT student and Nursing student. Methods. Nursing students and EMT students 200 questionnaires were collected and Frequency analysis, Chi-square test, one-way ANOVA was performed for using the Windows SPSS(ver. 12.0). Results. Most of the subjects were aware of the protection law of personal information and Infringement of the privacy of personal information will be exposed. also, Education is needed privacy(EMT students $3.84{\pm}0.96$, Nursing students $3.73{\pm}0.99$). EMT($3.99{\pm}1.00$) and Nursing($4.07{\pm}0.94$)students due to exposure to both the patient's personal information privacy was violated would get recognized. Exposure to the computerization of information privacy will be exploited in other agencies(EMT students $3.78{\pm}0.88$, Nursing students $3.95{\pm}0.94$) was called. Conclusions. For the protection of personal health information, education needs to be expanded.

어린이 성장판 영상화를 위한 초음파와 X-선 방식의 비교 평가 (Comparison Between Ultrasonic and X-ray Methods for Imaging the Children′s Growth Plate)

  • 본 연구에서는 진단 방법의 정확성이 아직까지 구체적으로 규명되지 않고 있는 어린이의 성장 발육을 체계적이고 과학적으로 측정할 수 있는 방법을 제시하고자 시도하였다. 현재 많이 이용되고 있는 성장 진단 방법은 X-ray 영상을 이용한 각 관절 부위의 성장판의 개폐 분석이다. 그러나 X-ray 방식은 성장 진단에서 요구되는 주기적이고 반복적인 측정을 하지 못함과 동시에 방사선 노출이라는 단점을 가지고 있다. 따라서 인체에 무해한 초음파 영상을 이용한 성장판 개폐 분석 대체 가능성을 평가하기 위해 종골의 초음파 광역 감쇄 영상을 이용한 프로파일 분석 및 성장판 영상 분석 알고리즘을 제시하였다. 본 연구에서는 7세~16세의 어린이 269명에 대해서 X-ray를 이용하여 무릎, 손가락, 종골에서의 성장판 영상을 얻었다. 또한 초음파 방식을 이용하여 종골의 성장판 영상을 얻었다. 각 관절에서의 성장판 개폐 유무 시기는 각 개인별로 측정 방법에 따라 조금의 차이는 있으나 전반적으로 일치하였다. 또한. X-ray방식과 초음파 방식에 의한 성장판 영상을 비교한 결과 상호 높은 연관성을 나타내었다. 이와 같이 성장판을 정확하게 측정하기 위해 도입된 초음파 프로파일 분석 알고리즘은 기존의 X-ray 영상 방식에 의한 성장판의 측정을 대체할 수 있을 것으로 사료된다.

의료재활교육이 요통에 미치는 실증적 연구 (The Experimental Study Effected on Low Back Pain by MRE(medical rehabilitation education))

  • The purpose of study was to analyze and clarify the new facts which might help the patients with low back pain be rehabilitated by giving MRE(medical rehabilitation educations) for DLH(Daily Living Habit) based on body types. The outcome of examining hypothesis is as following. 1. The 1st hypothesis : It has been proved that the patients of experimental group which received the DLH will recovery and rehabilitate to social lives much faster than those of non-experimental group(p<.001). 2. The 2nd hypothesis : It has been proved that those of experimental group have much more changes in bodies and body types than those of non-experimental group(p<.001). 3. The 3rd hypothesis : It has been proved that those of experimental group will be better at self-care of back pain and satisfied with it much more than those of non-experimental group(p<.001). In above results, DLHE(daily living habit education) based on body types can be recommended to be a desirable education for low back patients who need a long-care. As it is more subdivided and specialized than the existing MHE, the effects of therapeutic self -care involving spinal manipulation are expected to be higher.

전통장류에 대한 일부 초등학생 대상 영양교육의 효과 (Effects of Nutrition Education about Korean Traditional Fermented Soybean Products for Elementary School Students)

  • This study was conducted to investigate the effects of educating 5th graders in elementary school about Korean traditional fermented soy products. The percentage of correct answers from the educated group increased after nutrition education compared to the non-educated group (p<0.001). The perceptions and attitudes regarding Korean traditional fermented soy products increased significantly after nutrition education (p<0.05). Analysis of the preference for Korean traditional fermented soy products based on participation in nutrition education showed that both groups were not significantly different in all categories including soybean paste, soy sauce, and red pepper paste. The investigation on differences in intake frequency of Korean traditional fermented soy products showed that both groups did not display statistically significant differences in all categories including soybean paste, soy sauce, and red pepper paste. These results confirmed that the nutrition education activity program had positive effects in correcting 5th graders' knowledge, perceptions and attitudes regarding Korean traditional fermented soy products. However, it did not significantly affect their preferences and intake frequency. This indicates that short-term education cannot effect changes in preference or actual behaviors. Therefore, continuous nutrition education activities are required.

충북지역 일부 학교 영양(교)사의 건강기능식품에 대한 인식 및 교육 요구 (Perception and Educational Demand on Health Functional Foods among School Nutrition Teachers and Dietitians in Chungbuk)

  • We investigated perception and educational demand for health functional foods among nutrition teachers and dietitians working in schools. The subjects were 229 nutrition teachers (57.2%) and dietitians (42.7%) working in Chungbuk elementary, middle and high schools. Approximately 67% of the subjects had experience consuming health functional foods. Most of the subjects did not have experience with the side effects after consuming health functional foods. Subjects who were unaware of the legal specifications, classification, and labeling standards for health functional foods (78-91.2%) were much more common than those who were aware of those issues. Most of the subjects perceived health functional foods as a food (45.0%) or as a product between a food and a medicine (44.5%). The subjects recommended themselves (43.1%) or medical doctors/pharmacists (40.8%) as consultation/education specialists for health functional foods. Approximately 66% of the subjects had experience conducting nutritional consultation/education in their schools, but only 3.2% had experience with health functional foods. Only 17.8% of the subjects had experience receiving education on health functional foods. Most subjects (88.2%) demanded education on health functional foods for themselves. The results suggest that an effective education is needed for nutrition teachers and dietitians working in schools for them to play an active role in educating students and their parents about health functional foods in the near future.

  • Objectives: This paper aims to explain principles of gist-based health message design and discuss their implications for health promotion and public health education. Methods: After reviewing Reyna and Brainerd's Fuzzy Trace Theory(FTT), the authors explicate how to transform FTT into a practical guidance of health message design. Our explication is based upon FTT's reasoning that human intuition, rather than analysis, takes a primary role in message recall and comprehension, followed by judgment and decision making. We expect gist-based message design to be appropriate to serve such intuition. Results: Four principles of gist-based message design are offered: (1) provision of qualitative, as well as quantitative, information of gist, (2) inclusion of visual images corresponding to gist, (3) use of effective message formats to emphasize the gist (4) inclusion of relevant reasons and contextutal information. Conclusions: Gist-based message design has theoretical and practical implications for health promotion, specifically in the field of public health education, the press and governmental communication toward the public, and provider-patient communication in medical settings.

Low Frequency of Precore Mutants in Anti-Hepatitis B e Antigen Positive Subjects with Chronic Hepatitis B Virus Infection in Chennai, Southern India

  • The natural course of chronic hepatitis B (CH-B) virus infection is reportedly variable, and the long-term outcomes in hepatitis B e antigen (HBeAg)-negative chronic hepatitis B infection are distinct from HBeAg-positive chronic hepatitis. However, the molecular virological factors that contribute to the progression of liver disease in the south Indian setting remain largely unclear. We prospectively studied 679 consecutive patients for HBsAg, HBeAg, anti-HBe, and HBV DNA by qualitative PCR. Randomly selected samples were subjected to bidirectional sequencing to reveal core/precore variants. Of the total 679 chronic HBV cases investigated, 23% (154/679) were replicative HBV carriers. Furthermore, amongst the 560 HBV DNA samples analyzed, 26% (146/560) were viremic. Among the 154 HBeAg positive cases, HBV DNA was positive in 118 cases (77%), significantly (p<0.001) higher than the anti-HBe positive (7%) (28/406) cases. Significant increase in liver disease (p<0.01) with ALT enzyme elevation (p<0.001) was observed in both HBe and anti-HBe viremic cases. Interestingly, low frequencies of mutations were seen in the precore region of the HBV strains studied. HBV precore and core promoter variants were less often detected in subjects with "e" negative chronic HBV infection and, therefore, may not have a prognostic role in determining liver disease sequelae in this part of tropical India.
