Anaphylactic Shock Care during General Anesthesia -A Case Report- (전신마취 도중 유발된 아나필락시스 -증례보고-)

  • Generalized anaphylaxis is a most dramatic and acutely life-threatening allergic reaction. Most fatalities from anaphylaxis occur within the first 30 minutes postantigenic exposure. The mechanism of generalized anaphylaxis is the reaction of IgE antibodies to an allergen that causes the release of histamine, bradykinin, and others. These chemical mediators cause the contraction of smooth muscles of the respiratory and intestinal tracts, as well as increased vascular permeability. Four major clinical symptoms are recognized: skin reactions, smooth muscle spasm (gastrointestinal and genitourinary tracts and respiratory smooth muscle), respiratory distress, and cardiovascular collapse. Epinephrine is the drug of choice for the management. Its syrnpathomimetic effects directly counteract most aspects of the attack. Respiration must be immediately supported by the establishment of a patent airway along with artificial ventilation. The circulation should be supported and the existing hypotension overcome by placing the victim in a position to allow gravity to aid venous return and by administering intravenous fluids, vasopressors, and corticosteroids. When an imperceptible pulse is evident, external cardiac compression must also be instituted. This is a case report of anaphylactic shock care during general anesthesia, possibly due to penicillin, pancuronium and others.

  • Let(X, $\Im$) be a probabilistic metric space with a t-norm. Common fixed point theorems and convergence theorems generalizing the results of Ćirić, Fisher, Sehgal, Istrătescu-Săcuiu and others are proved for three mappings P,S,T on X satisfying $F_{Pu, Pv}(qx){\geq}min\left{F_{Su,Tv}(x),F_{Pu,Su}(x),F_{Pv,Tv}(x),F_{Pu,Tv}(2x),F_{Pv,Su}(2x)\right}$ for every $u, v {\in}X$, all x>0 and some $q{\in}(0, 1)$.   • A semi-Lagrangian finite element scheme with objective time stepping algorithm for solving viscoelastic flow problem is presented. The convection terms in the momentum and constitutive equations are treated using a quasi-monotone semi-Lagrangian scheme, in which characteristic feet on a regular grid are traced backwards over a single time-step. Concerned with the generalized midpoint rule type of algorithms formulated to exactly preserve objectivity, we use the geometric transformation such as pull-back, push-forward operation. The method is applied to the 4:1 planar contraction problem for an Oldroyd B fluid for both creeping and inertial flow conditions.

  • Valve dysfunction can be caused by thickening or contraction of a fibrous sheath covering a cusp of a porcine bioprosthesis, but this is uncommon. This complication appears to more frequent in other bioprostheses, such as fascia late valves and homografts, in which fibrous sheaths seems to grow more rapidly. rapidly. Thus the slow and limited growth of fibrous sheath in porcine bioprostheses is advantageous in this respect. Recently, we experienced a case of valve dysfunction caused by fibrous tissue overgrowth on Hancock mitral xenograft in 45 year old female. 3.5 years ago, the patient was received valve replacement due to mitral stenoinsufficiency. But since 2.5 years elapsed after operation, she has complained of generalized edema and dyspnea, and their symptoms were aggravated progressively. So reoperation was performed under the diagnosis as valve dysfunction of mitral xenograft and newly developed tricuspid insufficiency. Her postoperative courses were good.

  • In this paper, first, we prove some common fixed point theorems for the generalized contraction condition under newly defined modified simulation function which generalize and include many results in the literature. Second, we give two numerical examples with graphical representations for verifying the proposed results. Third, we discuss and study a set of common fixed point theorems for two pairs (finite families) of self-mappings. Finally, we give some applications of our results in discrete and functional fractional economic systems.

  • We define new classes of expansive-type mappings in the setting of modular 𝜔G-metric spaces and prove the existence of common unique fixed point for these classes of expansive-type mappings on 𝜔G-complete modular 𝜔G-metric spaces. The results established in this paper extend, improve, generalize and compliment many existing results in literature. We produce some examples to validate our results.

An Omnidirectional Vision-Based Moving Obstacle Detection in Mobile Robot

  • This paper presents a new moving obstacle detection method using an optical flow in mobile robot with an omnidirectional camera. Because an omnidirectional camera consists of a nonlinear mirror and CCD camera, the optical flow pattern in omnidirectional image is different from the pattern in perspective camera. The geometry characteristic of an omnidirectional camera has influence on the optical flow in omnidirectional image. When a mobile robot with an omnidirectional camera moves, the optical flow is not only theoretically calculated in omnidirectional image, but also investigated in omnidirectional and panoramic images. In this paper, the panoramic image is generalized from an omnidirectional image using the geometry of an omnidirectional camera. In particular, Focus of expansion (FOE) and focus of contraction (FOC) vectors are defined from the estimated optical flow in omnidirectional and panoramic images. FOE and FOC vectors are used as reference vectors for the relative evaluation of optical flow. The moving obstacle is turned out through the relative evaluation of optical flows. The proposed algorithm is tested in four motions of a mobile robot including straight forward, left turn, right turn and rotation. The effectiveness of the proposed method is shown by the experimental results.

Numerical study on the characteristics of the flow through injector orifice by multi-block computations (다중블럭계산에 의한 분사기 오리피스 유동특성 해석)

  • Numerical computations were conducted to characterize the three-dimensional laminar flow through an injector orifice having an inclined angle of 30 .deg.. For this study, the incompressible Navier-Stokes equations in generalized curvilinear coordinates, using a pseudocompressibility approach for continuity equation, were solved. The computations were performed using the finite difference implicit, approximately factored scheme of Beam and Warming and multi-block grids of complete continuity at block interfaces. The multi-block computations were validated for the steady state using direct comparison of multi-block solutions with equivalent single-block ones, including 2-D 180.deg. TAD and 3-D 90.deg. pipe bend. The comparisons between the numerical solutions and the flow field measurements for a tube with sudden contraction were presented in this work for solution validation. Computational results showed the nature of complex flow fields within the inclined injector orifice, including strong pressure-driven secondary flows in the cross stream induced by the effect of streamline curvature. In addition, asymmetric secondary flows were induced in the Reynolds number range above assumed laminar flow regime considered. However, turbulence calculations and grid dependency studies are needed for more accurate computations.

Idiopathic Atrial Standstill in an Old English Sheepdog Cross Dog (Old English Sheepdog Cross Dog에서의 특발성 심방정지)

  • An 1 year old intact male mixed dog (weighing 20 kg) was presented with primary complaints of abdominal distension and severe exercise intolerance. Electrocardiogram found profound bradycardia (46-79 beats/min) with atrial standstill. Laboratory studies found no particular abnormalities. Serum cortisol and T4 concentration were within normal range. Diagnostic imaging studies revealed generalized cardiomegaly, ascites, dilation in all cardiac chambers, absence of atrial contraction, absence of transmitral A-peak, mitral and tricuspid valve insufficiency and normal left ventricular systolic dimension. Based on diagnostic findings, the case was diagnosed as idiopathic atrial standstill. The dog was treated with conventional therapy for heart failure.
