Research Design for the Verification of Constitutional Prescription Determinants (체질처방 결정요인 확인을 위한 연구설계)

  • Objectives The purpose of this study is to design a research to find clinical factors involved in the decision-making process for determining the constitutional prescription based on prospective clinical data. Methods We have created a draft of the case report form. Therefore, seven constitutional experts was interviewed for additional items to be important used to determine the constitutional prescription in clinic. Interviews were done from December 2012 to March 2014, experts per person 2-3 times, took time of about 120 minutes per interview. Since then, we developed the final case report form through the expert meeting. At the same time, the developing the electronic case report form (eCRF) and the protocol to collect constitutional treatment cases was also discussed. Results & Conclusions The items of the case report form were subject general, lifestyle, health measurement, record of expert, prescription and evaluation after medication. The part of the clinical symptoms of the record of expert allowed to be recorded in the 5-point scale for the collection of quantitative data as much as possible. Assuming a re-visit of the patient, if necessary, twice the recording were to be possible. At the same time, the eCRF and the protocol to collect constitutional treatment cases were also developed. In this study, it will be able to more objectively standardize the medical decision making process that the experts of constitutional prescription decision. As a result, it will be possible to provide the standardized constitutional medical services.

Proximal-type Epithelioid Sarcoma Involving Perineum - Case Report - (회음부를 침범한 근위형 유상피육종 - 증례보고 -)

  • Epithelioid sarcoma has been well characterized as a distincitve entity. In the classic form, it occurs in the distal extremities of young adults. However, in the proximal form, it was found mostly in the pelvis, perineum, and genital tract of young to middle-aged adults. Morphologically, the tumor cells had promient epithelioid or rhabdoid features with marked cytologic atypia, and grew in a multinodular pattern. Immunohistochemically, cytokeratin, epithelial membrane antigen, and vimentin were positive in the tumor cells. This reports dealt with 32-year-old male lesion on his perineum. The tumor shows typical features of proximal type epithelioid sarcoma. We also discussed pathological differential diagnosis and prognosis on this particular disease.

  • Shone complex is a rare congenital disorder that involves 4 obstructive lesions of the left heart, as follows: parachute mitral valve, supravalvular mitral ring, subaortic stenosis, and coarctation of the aorta. Incomplete forms with 2 or 3 of these lesions in adult patients have been rarely reported in the literature, meaning that insufficient general data exist concerning the surgical strategy and clinical follow-up. Herein, we report the case of a 31-year-old woman with a diagnosis of incomplete form of Shone complex with parachute mitral valve and coarctation of the aorta who underwent successful single-stage surgical repair.

The Current Evaluation State of Post-Anesthetic Recovery after General Anesthesia (전신마취 후 회복 평가도구 활용 실태)

  • Purpose: This research was conducted to evaluate, analyze, and determine the limitation of the anesthesia that is used in there covery room in order to provide the foundation for developing the effectiveness of it. Method: There covery records of this study were collected from 41 hospitals in Seoul, Kyung Kii-Do, and Inchon province. The post anesthetic recovery records consisted of the evaluation of type, evaluative items, frequency, time, score and $SaO_2$. These records were collected from September to December of 2005. Results: The most commonly used post anesthetic evaluation tool was Alderete Score, which was used in 73% of hospitals. The second was Aldrete Score with $SaO_2$(17%). Also, 5% of the hospitals used the modified AS form. There were 2.5% of the hospitals applying the adult and child in the evaluation separately. Also, the last 2.5% of the hospitals did not use AS but measured BP, pulse, respiration, temperature, and $SaO_2$ with observing nausea, vomiting, urinary retention, backache, laryngitis, shock, and neurologic assessment. Conclusion: It is necessary to develop a post-anesthetic recovery evaluation tool including the application of $SaO_2$, so that the early-diagnosis of hypoxia can be possible. In addition, it is necessary to develop a post- anesthetic recovery evaluating method that can distinguish a difference between adults and children.

Food contributing to fermentable oligosaccharide, disaccharide, monosaccharide, and polyols intake in Korean adults

  • BACKGROUND/OBJECTIVES: The dietary intake of foods with fermentable oligosaccharides, disaccharides, monosaccharides, and polyols (FODMAP) is known to adversely affect patients with irritable bowel syndrome (IBS). However, the effects of FODMAP have been studied predominantly among Western populations. This study aimed to identify foods high in FODMAP content which form a part of the Korean adult diet and obtain basic data for the preparation of IBS guidelines. SUBJECTS/METHODS: An online survey of 1,000 adults from the general population in the age group of 20 to 40 years was performed. Data from 787 participants (men, 386; women, 401) were analyzed. The general characteristics of the participants, health status, IBS diagnosis using the Rome III diagnostic criteria, semi-quantitative food frequency questionnaire findings, and food items causing symptoms were analyzed. RESULTS: Overall, 169 participants (21.5%) had IBS. The contribution of the FODMAP nutrients in both IBS and healthy groups was as follows: fructan > lactose > excess fructose > sorbitol > mannitol > galacto-oligosaccharides (GOS). The fructan intake was 4.6 ± 2.2 g/day and 4.3 ± 2.5 g/day in the IBS and healthy groups (P = 0.014), respectively. In the IBS group, the ratio of the intake of fructan to the total FODMAP intake was 39.5%, 29.8%, and 5.8% through onions, garlic, and bananas, respectively. CONCLUSIONS: Fructan was the FODMAP nutrient most consumed by Korean adults. Therefore, given the difference in the dietary habits of each country's population, the dietary guidelines for IBS should be country specific.

  • A measurement of conduction veloctiy of the action potentials along the muscle fibres has been applied to various diagnosis. When we measure muscle fiber conduction velocity, it occurs that not only change of conduction velocity but alos inclusion of mipulse component by physiological and experimental reason. So, robuster time delay estimation algorithm than general methods[1] is needed to find correct time delay form these signals. In this paper we, propose new time delay estimation algorithms, robust in impulsive noise, by using characteristic of .alpha.-stable distribution whcih defines impulsive noise well. Then we apply proposed algorthms to measure muscle fiber conduction velocity and compare them with other studies.

  • In general, long-term changes of ecological factors take a pulse form in which they interact with other factors and go through a repeated increasing and decreasing cycle. The coupling of the two approaches the grid model and the box model in ecological modeling can lead to an in-depth understanding of the environment. The study analyzes temporal variations of major storages with an energy system model that formulizes effectively the relationships among nutrients, phytoplankton, and zooplankton in the Yellow Sea and the East China Sea. An increase of light intensity and standing stock of nutrient increase the magnitude and frequency of pulsing. Also, an immense reduction of nutrient concentration can cause extinction of the pulsing and bring about a steady state. It is concluded that the nutrient loads in freshwater discharge from the Yangtze affect the cycles of major ecological components as well as water quality variables and play an important role in the marine ecosystem.

Comparative Study on the Selection Algorithm of CLINAID using Fuzzy Relational Products

  • The Diagnostic Unit of CLINAID can infer working diagnoses for general diseases from the information provided by a user. This user-provided information in the form of signs and symptoms, however, is usually not sufficient to make a final decision on a working diagnosis. In order for the Diagnostic Unit to reach a diagnostic conclusion, it needs to select suitable clinical investigations for the patients. Because different investigations can be selected for the same patient, we need a process that can optimize the selection procedure employed by the Diagnostic Unit. This process, called a selection algorithm, must work with the fuzzy relational method because CLINAID uses fuzzy relational structures extensively for its knowledge bases and inference mechanism. In this paper we present steps of the selection algorithm along with simulation results on this algorithm using fuzzy relational products, both harsh product and mean product. The computation results of applying several different fuzzy implication operators are compared and analyzed.

  • A 65-year-old woman presented with a solid mass on the right temporal area. The mass had grown for over 2 years without any initiating event of trauma or inflammation. Before excision, the patient went through a computed tomography scan, revealing a calcified mass without bony connection. Under general anesthesia, an excisional biopsy was performed. Microscopic examination confirmed a diagnosis of soft tissue osteoma. Soft tissue osteoma is rare, especially in the head and neck region. Osteomas in the temporal region have not been reported yet. Due to its rarity, osteoma might be misdiagnosed as another soft tissue or bone origin tumor. Its treatment of choice is simple excision. In this review, we present an unusual clinical form of soft tissue osteoma.

Patients' Profile, Clinical Presentations and Histopathological Features of Colo-rectal Cancer in Al Hassa Region, Saudi Arabia

  • Background: Patients' demographics, presenting clinical and histopathological features for colo-rectal cancer (CRC) are important factors for patients' outcome and disease prognosis. This study aimed to describe the pattern of CRC in terms of patients' demographics, main presenting symptoms and histopathological features in Al Hassa region of Saudi Arabia. Subjects and Methods: A retrospective hospital records-based study which included reviewing of patients' records diagnosed with CRC at three general hospitals in Al Hassa region, Saudi Arabia. A compilation form was designed to collect information regarding socio-demographics, age at diagnosis; referral sites and the main presentations at CRC diagnosis. sites and the main presentations at CRC diagnosis. Histopathological reports were reviewed to delineate the main cytopathologic features, prominent cytological characteristics, the presence of predisposing pathology, and disease stage. Results: Of the 142 cases with CRC, 15.5% and 33.1% were affected before 40 and 50 years of age respectively. Emergency rooms were main referral sites for CRC cases (31.0%). Right colon was affected in 16.9% while left colonic lesions accounted for 62.7%. Intestinal obstruction was the main presentations (41.5%), and 26.1% presented with symptoms indicating distant metastastic lesions. Adenocarcinoma was the predominant pathological lesions (86.6%). Metastastic CRC was diagnosed in 62.7%. Duke's staging showed that 22.5% and 40.1% of lesions were classified into C and D categories respectively. Conclusion: Saudi patients with CRC present late with distant metastasis, and advanced disease stage. A sizeable proportion of patients developed the lesions at relatively young age. Screening recommendations should be enacted and revised in response to current change with younger age of affection.
