A study on the use of dental hygiene devices in Korean adolescents (청소년의 구강관리보조용품 사용에 관한 연구)

  • Objectives: The objective of the study was to investigate the use of dental hygiene devices in Korean adolescents. Methods: The study subjects were 70,362 adolescents in 800 schools who completed the 2015 Korean Youth Risk Behavior Web-based survey. Dependent variables included usage of dental floss, interdental brushes and mouthwash solutions. Independent variables included demographic characteristics of the subjects, health state and behaviors, and oral health behaviors and experience of oral diseases. Results: The related factors of usage of dental hygiene devices included gender, where females showed higher usage (OR=1.10) compared to males, father and mother's level of education where usage was higher in above university graduates (OR=1.20, OR=1.14) compared to less than high school graduation, economic status where usage was higherin high and middle (OR=1.93, OR=1.26) compared to low, vigorous physical activity where usage was higher in those who responded yes (OR=1.35) compared to no, subjective weight recognition where usage was higher in normal (OR=1.07) compared to under weight, sleep time where usage was higher in enough (OR=1.12) compared to not enough, number of toothbrushing (day) where usage was higher in 2 times or over 3 times (OR=1.35, OR=1.75) compared to below 1, oral health education experience (OR=1.10), sealant experience (OR=1.17) and scaling experience (OR=1.45) where usage was higher in those who responded yes compared to no, school where usage was lower in high school (OR=0.64) compared to middle school, residential type where usage was lower in rural area (OR=0.74) compared to metropolitan area, living form where usage was lower in other (OR=0.77), compared to with family, smoking (OR=0.93), and alcohol drinking (OR=0.90) where usage was lower in those who engage in the activities compared to those who didn't, BMI where usage was lower in normal (OR=0.87) and over weight (OR=0.98) compared to under weight. Conclusions: To expand the use of dental hygiene devices in the adolescents, it is necessary to improve the continuing education program for need and motivation of dental hygiene device usage.

The effect of oral health behavior of the visually impaired on DMFT index (시각장애인의 구강보건행태가 DMFT지수에 미치는 영향)

  • Objectives: This study aimed at helping oral health prevention of the blind and related management plan, which is defined as the influence factors between missing and filled permanent teeth index and general feature and oral health behavior of the blind in Korea (estimates 229,678 persons) using data of the 6th Korea National Health and Nutrition Examination Survey from 2014 Korea Centers For Disease Control and Prevention. Methods: The blind over the age of 30 were selected as study subjects who have conducted health survey and dental inspections in KNHANES VI-2. Estimates of the subjects were 229,67 persons. For analyzing data, general linear models: GLM and covariance analysis were conducted to identify the relation between general feature and oral health behavior and missing and filled permanent teeth index. SPSS 21 statistical program was used, which is possible to conduct complex sampling design, and the significance level was 0.05. Results: The missing and filled permanent teeth index was 8.58 points. Regarding the results of the analysis, R-squared of the missing and filled permanent teeth index depending on general features of the blind was 0.839 points, which shows gender, age, residence, education level, individual income, disability rating, kinds of health insurance, marital status and recipient of basic living had an effect on the missing and filled permanent teeth index. R2 of the missing and filled permanent teeth index depending on oral health form of the blind was 0.728 points, which shows oral examination, dental treatment, smoking and toothbrushing after lunch had an effect on the missing and filled permanent teeth index. Conclusions: With the result of this study, we found the oral health actual condition of the blind in Korea. Therefore, it is considered that the government needs to introduce the personalized oral health education program to maintain oral health of the blind and to develop a program that uses braille and voice device which enables to access and utilize to improve oral health behavior that the government could use it as a reference to establish the policy plan.

The opinions of health care workers on the inclusion of dental hygienists in the category of medical personnel (치과위생사의 의료인화에 대한 의료종사자들의 견해)

  • Objectives: The study investigated health professionals working in the metropolitan area and Jeolla-bukdo, South Korea, from April 30 to May 17, 2017 to understand their views on the issue of including dental hygienists in the scope of medical personnel of South Korea. Methods:A total of 270 surveys were analyzed in this study. The survey consists of 5 questions on general issues; 10 on the awareness of present work of dental hygienist; and 1 on opposition or approval about including dental hygienist in medical personnel. The collected data were analyzed using SPSS for Windows 18.0, as well as frequency analysis, cross analysis and logistic regression analysis. Statistical significance level (${\alpha}$) is 0.05. Results: 1. Broken down by gender, male subjects showed more oppositions against including dental hygienists in the category of medical personnel while female subjects showed more favorable opinions (p<0.05). In terms of academic background, those who had graduated from graduate school or higher showed a higher propensity for opposition while those who had graduated from a 3-year college showed a higher tendency for approval on the idea (p<0.05), In terms of occupational type, health professionals showed more opposing views whereas medical technologists showed more approvals than others (p<0.001). 2. With respect to the awareness of work specialty and proficiency of dental hygienist according to general characteristics, the higher the age, the higher the awareness level was. In terms of the occupational type, medical technologists were found to have higher awareness level than health professionals (p<0.001). 3. With respect to the relationship between general characteristics and view on including dental hygienists in medical personnel, the occupational type of health professional was found to have 6.33 times more oppositions than medical technologists. When the awareness level on proficiency of dental hygienist was low, opposition was 6.52 times more frequent (p<0.05). Conclusions: Based on the findings above, the inclusion of dental hygienists in medical personnel seems necessary in properly establishing the specialty and role of dental hygienist in the dentist medical environment of the country in order to enhance national oral health related preventive dental service and expand the demand. To this end, it is necessary to provide nationwide promotion, work to change the awareness of health professionals in other occupational types, and facilitate public promotion for legal ground establishment.

Degummed crude canola oil, sire breed and gender effects on intramuscular long-chain omega-3 fatty acid properties of raw and cooked lamb meat

  • Background: Omega-3 long-chain (${\geq}C_{20}$) polyunsaturated fatty acids (${\omega}3$ LC-PUFA) confer important attributes to health-conscious meat consumers due to the significant role they play in brain development, prevention of coronary heart disease, obesity and hypertension. In this study, the ${\omega}3$ LC-PUFA content of raw and cooked Longissimus thoracis et lumborum (LTL) muscle from genetically divergent Australian prime lambs supplemented with dietary degummed crude canola oil (DCCO) was evaluated. Methods: Samples of LTL muscle were sourced from 24 first cross ewe and wether lambs sired by Dorset, White Suffolk and Merino rams joined to Merino dams that were assigned to supplemental regimes of degummed crude canola oil (DCCO): a control diet at 0 mL/kg DM of DCCO (DCCOC); 25 mL/kg DM of DCCO (DCCOM) and 50 mL/kg DCCO (DCCOH). Lambs were individually housed and offered 1 kg/day/head for 42 days before being slaughtered. Samples for cooked analysis were prepared to a core temperature of $70^{\circ}C$ using conductive dry-heat. Results: Within raw meats: DCCOH supplemented lambs had significantly (P < 0.05) higher concentrations of eicosapentaenoic (EPA, $20:5{\omega}3$) and EPA + docosahexaenoic (DHA, $22:6{\omega}3$) acids than those supplemented with DCCOM or DCCOC; Dorset sired lambs contained significantly (P < 0.05) more EPA and EPA + DHA than other sire breeds; diet and sire breed interactions were significant (P < 0.05) in affecting EPA and EPA + DHA concentrations. In cooked meat, ${\omega}3$ LC-PUFA concentrations in DCCOM (32 mg/100 g), DCCOH (38 mg/100 g), Dorset (36 mg/100 g), White Suffolk (32 mg/100 g), ewes (32 mg/100 g) and wethers (33 mg/100 g), all exceeded the minimum content of 30 mg/100 g of edible cooked portion of EPA + DHA for Australian defined 'source' level ${\omega}3$ LC-PUFA classification. Conclusion: These results present that combinations of dietary degummed crude canola oil, sheep genetics and culinary preparation method can be used as effective management tools to deliver nutritionally improved ${\omega}3$ LC-PUFA lamb to meat consumers.

Factors influencing birth weight premature infants (미숙아들의 출생 체중에 영향을 주는 요인)

  • Purpose : The purpose of this study was to identify factors influencing premature infants who are small for their gestational ago. Methods : The medical records of 1,010 premature infants of 26 to 35 weeks of gestational age born at Il-Sin Christian Hospital, Busan from January 2000 to August 2006 were reviewed. We collected data on gestational age, birth weight, infant gender, birth order, maternal age and previous abortion history and analyzed the factors influencing premature infants who were small for their gestational ago at birth. Results : In our study more female than male (P=0.042) in premature infants who were small for their gestational ago were born from mothers aged younger than 20 or older than 35 (P=0.041). But association between smallness for gestational age and birth order or maternal previous abortion history was statistically insignificant (P=0.228, P=0.129). Conclusion : Considering the association of birth weight and the survival rate of premature infants, it is thought that maternal age had an influence on the survival rate of premature infants. Social and political support to lower the teenager pregnancy and older pregnancy is expected to increase the survival rate of premature infants and the birth of healthy normal neonates.

Effects of Recognition of Elderly Education on Provision for Old Age among Middle-Aged and Aged People in Korea (한국 중고령자의 노인교육이 노후준비에 미치는 영향)

  • The purpose of this study is to analyze the statistical data to get the government to have positive recognition of education for middle-aged and aged people about provision for old age after retirement and present a model of policy direction of realistic elderly education for middle-aged and aged people and an ideal program for middle-aged and aged people in Korea. Such an empirical research obtained the following results. First, as for effects of recognition of the need of elderly education and the desire to participate in elderly education on provision for retirement, the need of elderly education and the desire to participate in elderly education were found to have effects. Second, as for effects of the level of retirement expectation on provision for retirement, new departure, career completion, and continuous expectation were found to have effects. Third, as for mediating effects of retirement expectation in terms of effects of recognition of the need of elderly education and the level of desire for participation, the need of elderly education has retirement expectation partly affect physical, psychological, and economic preparation. As for the desire to participate in elderly education, expectation of retirement had completely mediating effects on physical preparation and partly mediating effects on psychological and economic preparation. Fourth, the need of elderly education, the desire for participation, retirement expectation, and recognition of provision for retirement by general properties were found to have statistically significant results in terms of gender, age, occupation, and educational background.

Factors affecting Amputation Level in Diabetic Foot (당뇨발 환자에서 절단 부위 결정에 영향을 주는 요인에 관한 연구)

  • Purpose: To evaluate the factors which affect the level of the amputation for treatment of DM foot excluding arterial occlusion. Materials and Methods: We selected 30 patients (10 of major amputations, 20 of minor amputations) who were amputated from May, 1999 to April, 2001 because of DM foot. Major amputation is BK amputation, and minor amputation is amputation below ankle joint. Gender, age, size of the wound, extent of the necrosis, infectious organism, medical com orbidity, duration of DM and blood glucose level, duration of DM foot and treatment history were investigated. Results: In major amputations, male to female ratio was 9:1, average of the age was 63.8, the average of sizes of the wound was 16cm2, duration of DM was 15.0 years, duration of DM foot was 10.6 weeks, and 80% of patients had necrosis and the organisms were S. aureus, E. faecium, Streptococcus, P. vulgaris, average of the blood glucose levels was 301 and 40% of them had been treated for DM foot. In minor amputations, male to female ration was 9:1, average of the age was 56.6, the average of sizes of the wound was 4.8cm2, duration of DM was 11.2 years, duration of DM foot was 5.7 weeks, and 40% of patients had necrosis and the organisms were S. aureus, Streptococcus, M. morganini, E. faecium, average of the blood glucose levels was 257 and 20% of them had been treated for DM foot. Conclusion: In DM foot patients, extents of the necrosis, duration of DM, duration of DM foot, the infectious organism were significant factors to decide extent of the amputation level.

Intraneural Ganglion Cyst in Foot and Ankle (족부와 족관절에서의 신경내 결절종)

  • Purpose: Pathogenesis of intraneural ganglion is controversial, however, the synovial theory that the intraarticular region is the origination of disease has come into the spotlight nowadays. But there are a few researches about intraneural ganglion in foot and ankle. We studied 7cases of intraneural ganglion. We are going to prove the synovial theory by indentifying articular branch of intraneural ganglion. Materials and Methods: From August 2003 to May 2011, we evaluated 7 ouf of 8 patients diagnosed as a intraneural ganglion in foot and ankle. The gender ratio were 4 male and 3 female, and the mean age at the time of surgery was 52.9 years. Clinically, we checked pre and post operative symptom, muscle tone and whether loss of muscle tone and sensation exists. We analyzed surgical records and preoperative MRI and compared those with intra-operative finding. Results: In MRI analysis of 7cases, the connection around the joints were confirmed, and 1 case was confirmed in the retrospective analysis of MRI. Intraneural ganglions occurred in medial plantar nerve 3 cases, lateral plantar nerve 1 case, superficial peroneal nerve 1 case and sural nerve 1 case. We could not found recurrence during the follow up periods. Most patients relieved pain after operation, but recovery of sensation was unsatisfactory. We could find some cases pathological finding of the nerve intraoperatively, and clinical result of that cases was poor. Conclusion: Intraneural ganglion can occur in various parts in foot and ankle. We concluded that the intranneural ganglion originated from joint by identifying the artichlar branch of ganglion. Due to its small size, it is difficult to find articular branch in operation field. But we do our best to find and remove articular branch. Currently, considering the small amount of research in foot and ankle, more research about articular brach is needed.

Correlation between a Rupture of the Hypovascular Zone and Early Single Heel Raising after Achilles Tendon Repair (아킬레스건 봉합술 후 조기 단일 하지 거상과 아킬레스건 허혈성 구간 침범과의 상관관계)

  • Purpose: To analyze the correlation between a rupture of the hypovascular zone and early single heel raising after Achilles tendon repair. Materials and Methods: From January 2012 to August 2015, 68 patients, who underwent surgical treatment for a Achilles tendon rupture using Krackow method, were analyzed retrospectively. The patients were divided into two groups according to possibility of single heel raises within 3 months postoperatively. During the periodic outpatient observations, the visual analogue scale, Achilles tendon total rupture score (ATRS), and timing capable single heel raises were evaluated. In addition, the preoperative defect size and distance between the calcaneal osteotendinous junction and the rupture site were measured by ultrasound in all cases. Results: Twenty-three patients could perform a single heel raise within 3 months after surgery (early single heel raise group), and fortyfive patients could perform a single heel raise after 3 month postoperatively. The age, gender, body mass index, smoking, and operation delay were similar in the two groups. In addition, the defect size and distance between the calcaneal osteotendinous junction and rupture site as measured by preoperative ultrasound were similar (p=0.379 and p=0.631, respectively). On the other hand, when the rupture site was divided into the hypovascular zone (4~7 cm from calcaneal osteotendinous junction) and non-hypovascular zone, the hypovascular zone rupture rate was significantly lower in the early single heel raise group (60.9%, 14/23; 91.1%, 41/45; p=0.003). In logistic regression analysis, the odds of the hypovascular zone rupture group being capable of early single heel raise were 0.189 (p=0.017). The ATRS score at 3 months and 1 year after surgery were significantly higher in the early single heel raise group (p<0.001). Conclusion: Achilles tendon rupture at the hypovascular zone is a poor prognostic factor for early single heel raise and might affect the prognosis significantly after an Achilles tendon rupture operation.

Eating habits, physical activity, nutrition knowledge, and self-efficacy by obesity status in upper-grade elementary school students

  • BACKGROUND/OBJECTIVES: Childhood obesity has increased in recent decades in Korea. This study was designed to examine differences in the eating habits, physical activity (PA), nutrition knowledge, and self-efficacy of children by obesity status. SUBJECTS/METHODS: Subjects were 5th-grade children from 70 elementary schools in 17 cities nationwide. Two-stage stratified cluster sampling was employed. Survey questionnaire included items related to general characteristics, eating habits, PA, nutrition knowledge and self-efficacy. Excluding incomplete responses, 3,531 data were analyzed using SPSS. Subjects were categorized into overweight obesity (OW) and normal weight (NW) groups based on body mass index percentiles for age by sex. RESULTS: A total of 21.5% of subjects was overweight or obese. There were significant differences in gender, perceived stress, perception of body shape, body satisfaction, and interest in weight control between the OW and NW groups (P < 0.001). With respect to eating habits, the OW group ate breakfast (P < 0.05) and snacks (P < 0.01) less frequently, ate bigger meals (P < 0.001), and demonstrated less desirable behaviors during meals (P < 0.05 in boys) compared to the NW group. The OW group participated in less PA than the NW group, especially boys. OW boys spent less time walking during weekdays (P < 0.05) or the weekend (P < 0.001), spent more time being sedentary during weekdays or the weekend (P < 0.001), and exercised a fewer number of days (P < 0.01). For girls, the OW group spent more time being sedentary during the weekend (P < 0.01) and exercised a fewer number of days by walking or bicycle riding (P < 0.05) than the NW group. Nutrition knowledge was not significantly different between the OW and NW groups. Self-efficacy (P < 0.01 in boys), especially PA self-efficacy (P < 0.01), was significantly lower in the OW than NW group. CONCLUSIONS: This study revealed differences in eating habits, PA, and self-efficacy between OW and NW children. Obesity management programs for children need to focus on increasing self-efficacy, modifying eating habits, and increasing PA.
