Study on the Efficacy of Acupuncture with Real and Sham Needle on Healthy Subjects: Single-Blind Test using Electrogastrogram (건강인을 대상으로 거짓침과 진짜 침을 이용한 침의 효능에 대한 연구: 위전도를 이용한 단일맹검)

  • This study aimed to prove the effects of acupuncture treatment on acupoint Joksamni(ST36) related to gastrointestinal movement. Electrogastrogram (EGG) data were obtained in 11 healthy subjects treated by real or sham needle in single-blinded condition for 40 min, including before, during acupuncture treatment, and after removing needle. According to frequency analysis of EGG, median frequency(MF) was increased by real needle in 4 subjects, MF for another 2 subjects were increased by real and sham needle and no trend shift in MF showed in other 5 subjects. Our result showed that acupuncture treatment on Joksamni has some effects on gastrointestinal movement. In this study we have applied both real acupuncture and sham acupuncture on the same subjects. So its blinding was regarded as good but was effective in about 36%~55%. Two subjects were assumed that they showed placebo effects on both real and sham acupuncture. Therefore it is necessary to perform further study on case-control and a larger number of subjects with functional dyspepsia, in order to confirm our findings.

Diagnosis of Functional Gastrointestinal Disorders with Rome III Criteria in Korean Pediatric and Adolescent Patients: Clinical Usefulness of QPGS (한국 소아청소년에서 로마 III 기준을 이용한 기능성 위장관 질환의 진단: QPGS (Questionnaire on Pediatric Gastrointestinal Symptoms)의 유용성)

  • Purpose: We diagnosed pediatric functional gastrointestinal disorders in Korean children and adolescents using Rome III criteria and investigated the clinical validity of QPGS-Rome III. Methods: Diagnosis based on QPGS was compared with the physician's diagnosis based on Rome III criteria. One hundred and thirty eight children and their parents completed the QPGS. Agreement rates were measured using Kappa method. Results: In physician's diagnoses, the most prevalent disorders were functional dyspepsia (39.1%), irritable bowel syndrome (38.4%), and functional abdominal pain (18.8%). Among QPGS based diagnoses, the most prevalent disorders were irritable bowel syndrome (39.1%), functional dyspepsia (29.7%), and functional abdominal pain (21.7%). The agreement rate was substantial (${\kappa}$=0.72, p=0.00). Diagnostic disagreements probably resulted from different patient responses to bowel movement form and bowel frequency. Conclusion: Functional dyspepsia, irritable bowel syndrome, and functional abdominal pain were the most common disorders by Rome III criteria in the Korean pediatric and adolescent patients. The agreement rate between physician's diagnoses and QPGS based diagnoses supported the validity of the QPGS-Rome III in Korean pediatric and adolescent patients. QPGS seems to be useful in diagnosis of patients with functional gastrointestinal disorders by Rome III criteria.

Radiation Exposure Dose Reduction on Upper Gastrointestinal Series (위장조영검사에서 피폭선량 저감)

  • Upper gastrointestinal series is non-invasive examination, and it is useful for patients or elderly patients who have difficulty in endoscopic examination because of absence of any side effects other than temporary constipation or abdominal pain. The entire image of the gastrointestinal tract can be seen and have been widely used in the diagnosis of upper gastrointestinal diseases. However, there is a possibility that radiation dose increases due to improper movement and breath control, when examination is carried out by lack of understanding the overall inspection process for the upper gastrointestinal series. In upper gastrointestinal series to increase understanding of examination, to induce appropriate cooperation during examination, to reduce the number of retakes and shorten examination time, and to reduce dose of the subject, the procedure and precautions of the gastrointestinal test were made as a movie. We investigated the effectiveness of pre-education using the movie to reduce the inspection time, the number of re-shoots, and the reduction of exposure dose by watching the movie during the waiting time before examination. 120 patients that were selected each 20 patients aged from 30s to 80s were evaluated were evaluated for exposure dose, examination time, and the number of retakes before and after the movie training. The radiation dose, the examination time, and the number of retakes were respectively $3171.83{\mu}Gy{\cdot}m^2$ and $2931.73{\mu}Gy{\cdot}m^2$, 8.05 min, and 6.75 min, and 1.68 times and 1.22 times before and after movie training. It can be concluded that the movie training on the gastrointestinal examination influences the reduction of the examination time, the number of retakes and the reduction of the radiation dose.

Recent Advance in the Management of Dysplasia in the Ulcerative Colitis

  • In patients having long-standing ulcerative colitis (UC), the risk of colorectal cancer (CRC) increased compared with general population. Dysplasia is a precancerous lesion of colitic patients, and traditionally total proctocolectomy was considered as a standard therapy to prevent colorectal cancer in UC patients. However, even with ileal pouch-anal anastomosis (IPAA), patients who underwent total proctocolectomy may experience early and late postoperative complications, such as ileus, bleeding, pouchitis, and so on. In addition, the bowel movement after proctocolectomy with IPAA reaches a median of seven times per day, and a considerable proportion of patients require daytime and nighttime pads. Change in the strategy for managing dysplasia started from two early studies, which suggested polypectomy for polypoid dysplasia to prevent CRC in colitic patients. After that, many studies supported that polypectomy should be the first option for the management of polypoid dysplasia. Moreover, recent studies suggested the feasibility of endoscopic submucosal dissection as a therapeutic option for non-polypoid dysplasia, although long term, large studies should be followed.

The Comparison Study on the Effect of Bowel Movement between Bo-Ryu Enema(보류관장, Bao-Liu Enema) and General Enema in Patients at the Acute Stage of Cerebrovascular Accident (급성기 중풍 환자에 있어서 보류관장과 일반관장의 사하효과에 대한 비교 연구)

  • Objectives : In Oriental Medicine, the improvement of constipation is very important for patients at the acute stage of cerebrovascular accident. For constipation, oral medicine has usually been used but is not available to stroke patients with dysphagia and mental disorder, and may bring about side effects on the gastrointestinal tract. In such cases, general enema has been used for assistant therapy, We needed to develop a more effective enema technique on constipation of stroke patients. Methods : Sixty-three patients were researched. We treated 29 patients with Bo-Ryu enema and 34 patients with general enema. To compare the effect of the Bo-Ryu enema group with that of the general enema group, we analyzed general characteristics, bowel movement, abdominal examination and related symptoms. Results : Sixty-three patients comprised the Bo-Ryu (n=29) and general (n=34) enema groups. Between the Bo-Ryu and general enema groups, the Bo-Ryu enema was more effective than the general enema in the number of bowel movements, total and corrected amount of stool, tonus of rectus abdominis muscle and abdominal discomfort. However, there were not significant differences in the power of abdominal muscle, digestion and physical power. Among the subgroups, Bo-Ryu enema group was most effective in the number of bowel movements, total amount of stool and tonus of rectus abdominis muscle, and Bo-Ryu enema group was best in the corrected amount of stool and abdominal discomfort. Conclusions : Bo-Ryu enema should be considered as an effective and safe treatment for patients with constipation during the acute stage of cerebrovascular accident.

  • We investigated the effect of seatangle drink and seatangle extract on lipid oxidation, blood coagulation and intestinal movement in rats fed a hyperlipidemic diet. In the dietary hyperlipidemic induced group, the serum superoxide dismutase activity decreased and formation of hydroxy radical increased when compared to normal group, but these were controlled by seatangle drink treatment. The decreased of bleeding time and increased of tissue factor in the dietary hyperlipidemic rats were regulated by seatangle drink and seatangle extract, and especially the activity of tissue factor was remarkably decreased. Seatangle drink and seatangle extract were increased contraction on intestinal movement. Therefore, it can be concluded that seatangle drink or seatangle extract treatment depresses changing in absorption of gastrointestinal track in rats fed a hyperlipidemic diet.

The Effects of Interferential Current and Kaltenborn-Evjenth Orthopedic Manual Therapy on Functional Constipation

  • 2 week study was conducted to investigate the effects of Interferential Current(IC) and Kaltenborn-Evjenth Orthopedic Manual Therapy(KEOMT) on functional constipation. Interventions were applied to spinal segments between $T_9-L_2$ which provides innervations to the gastrointestinal tract. Subjects(n=24) were randomly allocated to two treatment groups: the IC group or the KEOMT group. Results for the IC therapy demonstrated significant decrease with the colonic transit time(CTT) as well as scores on the constipation assessment scale(CAS). The frequency of defecations per week had increased significantly(p<0.05). The KEOMT displayed decreased CTT in the left colon region. The scores on the CAS were reduced and frequency of defecations per week had increased significantly (p<0.05). This study not only showed that both modes of therapy improved symptoms of constipation, but also optimized gastrointesti. nal content movement, eventuating in a more normalized CTT. In conclusion, both the IC therapy and the Kaltenborn-Evjenth Orthopedic Manual Therapy have shown to be effective interventions for improving functional constipation.

Implications of Children's Pure-Yang Characters (소아 순양의 의미)

  • After the author of 'Luxinjing' explained the children's characters using the concept of Pure-Yang, the term has been widely exploited in the literature of Oriental pediatrics, Its meaning could be summed up in three general categories, namely: Shengyang(盛陽), Zhiyinzhiyang(稚陰稚陽) and Shaoyang(少陽). The implications of Pure-Yang were different in each dynasties of China. By the early Qing(淸) period, it meant mostly Shengyang(盛陽), but its meaning moved in the direction of Zhiyinzhiyang(稚陰稚陽) after Wu Jutong(吳鞠通). And this movement in the interpretation of this term was largely triggered by the pharmaceutical concerns of doctors as too much medicine of cooling and gastrointestinal effect was used to 'cool off' the over-charged Yang characters(盛陽) of children, causing considerable detrimental side effects to the children's body. The concept of Zhiyinzhiyang(稚陰稚陽) which emphasizes the fragility of children's body thus came in action. The meaning of physiological terms in Oriental Medicine is essentially linked to its clinical applications as we have seen in the case of Pure Yang. In that respect, the recent interpretation of Pure-Yang as Shaoyang(少陽) among physicians in China seems lacking in its crucial counterpart, which is its clinical applications. No theory can prove fruitful in the absence of its practice.

The Expression of nNOS in Hirschsprung's Disease (히르쉬스프룽병의 병변부위에서 nNOS 발현)

  • Abnormal distribution of enteric nerves such as adrenergic, cholinergic and non-adrenergic non-cholinergic nerves (NANC) may cause the failure of relaxation at the involved bowel segment in Hirschsprung's disease (HD). Nitric oxide (NO) is a major inhibitory NANC neurotransmitter in the gastrointestinal tract. NO is synthesized by activation of nNOS (neuronal nitric oxide synthase) in the intramural ganglion cells and regulates bowel movement. To assess the distribution of nNOS in HD, immunohistochemical staining to nNOS was utilized on paraffin embedded specimens. Ten control colon specimens were tested for feasibility of staining. Immunohistochemisrty was done on ganglionic colon as well as aganglionic segment of 15 patients with HD. nNOS immunoreactivity was observed in the neuronal cells, small cells and nerve fibers in the muscle layer and submucosal neuronal cells of control specimens. This finding was also observed in the ganglionic segments of HD. But, there was no nNOS immunoreactivity in aganglionic segments of HD. In conclusion nNOS immunohistochemical staining of paraffin embedded specimen is feasible and reliable. And the results suggest that the relaxation failure of the aganglionic bowel in HD is related to the absence of nNOS containing cells and nerve fibers.

Pediatrician Perspectives on the Evaluation and Treatment of Acute Gastrointestinal Infections, Jeonbuk, South Korea, 2002 (전라북도 소아과 개원의의 급성 위장염 환자에 대한 인식 조사)

  • Purpose : This study was conducted to assess clinical practices related to the evaluation and treatment of patients with acute gastrointestinal infection(AGI) in infants and children. This survey also evaluated the current opinion regarding the use of rotavirus vaccine. Methods : This survey was conducted using a self-administered questionnaire provided to 82 pediatricians in private clinics of Jeonbuk province. From April to June, 2002, 63 of 82 pediatricians(76.8 %) responded to the survey. Results : The annual proportion of patients with AGI was 13%. The highest proportion of patients with diarrhea were reported for the months of October through December. For the treatment of diarrhea, pediatricians preferred to prescribe lactobacillus products, oral rehydration solution, enzyme products, anti-emetics, anti-diarrheal formula, nothing by mouth(NPO) and bowel movement inhibitors in descending order of frequency. Most pediatricians(79.4%) prescribed antibiotics in less than 20% of patients with AGI. Amoxicillin was the most commonly prescribed antibiotics followed by trimethoprim/sulfamethoxazole(TMP/SMX) and amoxicillin/clavulanate. Clinical manifestations that pediatricians considered as important factors in prescribing antibiotics were bloody diarrhea, mucoid diarrhea, high fever, persisting disease and abdominal pain. Diagnostic procedures that pediatricians preferred for AGI patients were stool examination(rotavirus antigen, RBC, WBC), abdominal X-ray, and serum electrolytes. Pediatricians had a strong interest in rotavirus vaccine(71.4%). Conclusion : Acute gastrointestinal illness remains a common problem, especially during the winter months in Korea. AGI manifesting as watery diarrhea is appropriately treated with commonly available agents but pediatricians appear ready to consider additional approaches to decrease the sizable disease burden of AGI among children in Korea.
