기능성 위장관 질환의 한의학적 치료 연구전략 수립을 위한 연구 동향 분석 (Latest Research Trends Analysis of Korean Medicine for Research Strategy Establishment of Functional Gastrointestinal Disorders)

  • Obejectives : The purpose of this study was to analyze the papers from 2015 to 2019 to identify trends in gastrointestinal studies in Korean medicine. Methods : The search for related papers was conducted in 10 oriental medicine journals that were actively reported and published, easy to search using the Internet, and freely accessible to researchers. Results : There were many studies related to the stomach, and in particular, there were many papers confirming the effectiveness of Korean medicine treatment for functional digestion, hypersensitive intestinal syndrome, and gastrointestinal reflux diseases. It is regrettable that only about 60 papers of research on gastrointestinal diseases have been published in 10 journals over the past five years of research on gastrointestinal diseases. Conclusions : These results suggest that the need to promote practical research on functional gastrointestinal disorders should be evaluated higher in the Korean medical community, and the development of gastrointestinal-related treatments for Korean medicine can improve the quality of life for all of us.

  • In patients with functional gastrointestinal disorders, dietary factors have an important effect on the development or worsening of gastrointestinal symptoms. Therefore, nutritional advice is often needed for the treatment of these patients. Although no firm conclusions can be drawn from current studies, the use of low-fat diets can be recommended in patients with functional dyspepsia. Constipation can be treated with a high-fiber diet. There are few studies in irritable bowel syndrome patients with diarrhea. The diet recommendations must be applied to each patient depending on symptoms.

소아의 기능성 위장관 질환과 기질 및 성격과의 관계 (The Relationship Between Children's Temperament and Character to Functional Gastrointestinal Disorders)

  • Objectives The purpose of this study is to establish relationship between functional gastrointestinal disorders in children and Temperament and Character using objective criteria and questionnaires. Methods This study surveyed 3-6 years old 130 children who were attending the kindergarten or daycare center in the vicinity of Gangdong from May 15th, 2014 to November 30th, 2014. The Korean-translated Questionnaire on Pediatric Gastrointestinal Symptoms-Rome III (QPGS-Rome III) and The Junior Temperament and Character Inventory 3-6 (JTCI 3-6) questionnaires were used. Results 1. Between the groups, functional gastrointestinal disorder group scored significantly higher on NS, NS1, NS4, HA4, ST and ST1. 2. As the number of functional gastrointestinal disorders increase, the group scored higher on NS, NS1, NS4, HA4, ST and ST1. Conclusions This study showed functional gastrointestinal disorder group exhibited higher NS, NS1, NS4, HA4, ST and ST1 scores.

상부 소화관 질환증상에 대한 제산, 소화효소 및 생약 세립의 유용성 (Clinical Evaluation of Granules Containing Antacids, Digestive Eneymes and Herb Drugs in Patients with Various Symptoms of Upper Gastrointestinal Disorders)

  • The purpose of this study is to assess the clinical efficacy and safety of NRM granules which were newly developed as a combination product containing antacids, digestive enzymes and herb drugs for the treatmet of various symptoms of upper gastrointestinal disorders taking SGN granules of similiar but different composition as a control. One hundred and twenty one patients were randomly allocated to receive one pack of either NRM granule(80 cases) or SGN granules(41 cases) three times a day after meal and undergone symptomatic, endoscopic and clinical laboratory assessment before and after two weeks of treatment. The results were as follows; 1. All symptoms except anorexia showed improvements in more than 90% of total cases with each symptom in the NRM group. 2. The general improvement rate was 97.5% in NRM group, and was superior to the(92.5%) of SGN group. 3. Two cases of NRM group and one case of SGN group developed mild nausea, and onecase of SGN group developed mild epigastric soreness. No cases of the two groups developed laboratory(blood, biochemical, urine) abnormalities. 4. The overall clinical efficacy rate was 95.0% in NRM group, 87.8% in SGN group. kccording to the result of this trial, it was concluded that NRM granule was an effective and safe drug and(but not statistically significant) was slightly more efficient than SGN for treatment of various symptoms in upper gastrointestinal disorders.

  • Many types of drugs affect functions of tile gastrointestinal tract. Investigators may be interested in discovery or pharmacological characterization of drugs as therapeutic agents intended for treatment of gastrointestinal disorders or in identification of gastrointestinal side effects of drugs intended for non-gastrointestinal indications. Examples of drug categories often associated with significant gastrointestinal side effects include cardiovascular drugs, antibiotics (erythromycin in particular), anti-inflammatory drugs, antiemetics, analgesics (especially opiates), antihistamines, antidepressants, and antipsychotics. Whether tile objective is development of gastrointestinal therapeutic agents or evaluation of gastrointestinal side effects, appropriate laboratory models for experimentation are essential.

Role of colonoscopy in the diagnosis and treatment of pediatric lower gastrointestinal disorders

  • The safety and effectiveness of colonoscopy in the investigation of lower gastrointestinal tract pathology in children has been established for more than 2 decades in Korea. The skill and experience have since advanced to the point that both diagnostic and therapeutic colonoscopy are now routinely performed by most pediatric gastroenterologists. Pediatric colonoscopy differs significantly from its adult parallels in nearly every aspect including patient and parent management and preparation, selection criteria for sedation and general anesthetic, bowel preparation, expected diagnoses, instrument selection, imperative for terminal ileal intubation, and requirement for biopsies from macroscopically normal mucosa. Investigation of inflammatory bowel disease, whether for diagnosis or follow-up evaluation, and suspected colonic polyps are the most common indication for pediatric colonoscopy. The child who presents with signs and symptoms of lower gastrointestinal disorder should undergo colonoscopy with biopsy to make the diagnosis, as well as to help determine the appropriate therapy. This review introduces practical information on pediatric colonoscopy, the author's experiences, and the role of colonoscopic examination in the diagnosis and treatment of pediatric lower gastrointestinal disorders.

Functional Gastrointestinal Disorders in Infancy: Impact on the Health of the Infant and Family

  • Functional gastrointestinal disorders (FGIDs) such as infantile colic, constipation and colic occur in almost half of the infants. The aim of this paper is to provide a critical and updated review on the management of FGIDs and their impact on the health of the infant and family to health care physicians. Guidelines and expert recommendations were reviewed. FGIDs are a frequent cause of parental concern, impairment in quality of life of infants and relatives, and impose a financial burden to families, health care, and insurance. Therefore, primary management of the FGIDs should be focused on improving the infants' symptoms and quality of life of the family. If more than parental reassurance is needed, available evidence recommends nutritional advice as it is an effective strategy and most of the time devoid of adverse effects. The role of healthcare providers in reassuring parents and proposing the correct behavior and nutritional intervention by avoiding inappropriate use of medication, is essential in the management of FGIDs.

한국 소아청소년에서 로마 III 기준을 이용한 기능성 위장관 질환의 진단: QPGS (Questionnaire on Pediatric Gastrointestinal Symptoms)의 유용성 (Diagnosis of Functional Gastrointestinal Disorders with Rome III Criteria in Korean Pediatric and Adolescent Patients: Clinical Usefulness of QPGS)

  • 목 적: 저자들은 한국 소아청소년의 기능성 위장관 질환을 진단하는 데 로마 III 기준을 기초로 한 소아 위장관 증상에 관한 설문(QPGS)이 유용한 지 알아보고자 하였으며, 로마 III 기준을 이용하여 소아 청소년의 기능성 위장관 질환의 유병률을 살펴보았다. 방 법: 소아 소화기 전문의의 문진과 신체검진을 통한 진단과 소아 위장관 증상에 관한 설문(QPGS)을 기초로 한 진단의 일치 정도를 비교하였다. 138명의 환자와 그들의 보호자들이 설문지를 완성해주었다. 진단의 일치도는 카파(Kappa) 값을 구하여 측정하였다. 결 과: 소아 소화기 전문의의 진단에서 유병률은 기능성 소화불량(39.1%), 과민성 장 증후군(38.4%), 기능성 복통(18.8%) 순이었다. 설문을 기초로 한 진단에서는 과민성 장 증후군 (39.1%), 기능성 소화불량(29.7%), 기능성 복통(21.7%) 순이었다. 진단 일치도는 높았으며 (${\kappa}$=0.72, p=0.00), 진단이 일치되지 않은 경우는 대부분 대변 형태 및 대변 횟수에 관한 항목에서의 차이 때문이었다. 결 론: 소아 청소년 기능성 위장관 질환 진단의 선별 검사로서 로마 III 기준을 기초로 한 소아 기능성 위장관 증상에 관한 설문(QPGS)은 비교적 유용한 것으로 생각되며 특히 우리나라처럼 짧은 시간에 많은 환자를 진료해야 하는 환경에서 도움이 될 것으로 보인다.

섬유제조업 여성 근로자들의 교대작업에 따른 수면실태와 위장관장애 (Sleeping Patterns and Gastrointestinal Disorders According to the Shift Works in Female Textile Workers)

  • To investigate the sleeping patterns and gastrointestinal disorders of shift workers, the author studied 434 female workers who worked at textile industry ill Taejon city from September 1,1992 to September 31,1992. Shift pattern were divided into 3 categories ; 3 shift(shifts per 8 hours), 2 shift(day and night shift) and day work. The results obtained were as follows : 1 The average sleeping hours when adjusted for the education level of the total study subjects was 6.1 hours. That of 3 shift workers was 6.1 hours, 2 shift workers was 6.0 hours, but that of day workers was 6.5 hours. There were no significant difference among the shift workers. 2. For the 3 shift workers, the average physiologic adjusted duration in day shift (2.2 days) was shorter than that of night shift(2.7 days) and there were significant difference among the rotating shift works (p<0.001). The sleeping problems in day shift was less than those of night shift (p<0.001). 3. 44.9% of 3 shift workers, 39.3% of 2 shift workers and 33.1% of day workers complained gastrointestinal symptoms when adjusted for the age, education level, job tenure, work post. And the rates of gastrointestinal symptoms complained increase with job tenures (p<0.001). 4. The most frequent gastrointestinal diseases were gastitis and gastric ulcer with 14.2% , Irritable bowel syndrome with 3.1 %, duodenitis and duodenal ulcer with 2.1% and combined gastrointestinal disease with 2.1%. Age, eating habit, amount of coffee per day, job tenure, work post and shift pattern showed no significant difference with the gastrointestinal diseases when adjusted for the age. According to the above results, the author suggested that the shift pattern and job tenure can affect to the sleeping problem and gastrointestinal symptoms.

소화기 증상을 보이는 소아 정신신체 질환에 대한 정신과적 고찰 (Psychiatric Considerations on Pediatric Psychosomatic Disorders with Gastrointestinal Symptoms)

  • Gastrointestinal (GI) symptoms in children and adolescents are influenced by diverse psychiatric components such as psychosocial stresses, familial environment, school-related situations, and comorbid psychiatric conditions. Absolutely psychiatric symptoms of pediatric patients are also affected by problems of GI system. Lots of symptoms including anorexia, dyspepsia, nausea and so on are commonly originated from either GI or psychiatric causes or both. Sometimes the negative interactions between GI and psychiatric problems aggravate the severity and eventually decline the functions of children and adolescents with GI symptoms. We summarized the common GI and psychiatric conditions which have GI and psychiatric associations. To a clinician who manages pediatric GI disorders, psychiatric considerations can be beneficial to understand the clinical manifestations of patients and to find the way to relieve them. This short and somewhat superficial review may help to have a bird's-eye view on this topic.
