The Treatment Outcome of Elderly Patients with Idiopathic Trigeminal Neuralgia : Micro-Vascular Decompression versus Gamma Knife Radiosurgery

  • Objective : This study was designed to compare the efficacy of micro-vascular decompression (MVD) and Gamma knife radiosurgery (GKRS) for elderly idiopathic trigeminal neuralgia patients by analyzing the clinical outcome. Methods : In the past 10 years, 27 elderly patients were treated with MVD while 18 patients were treated with GKRS (>65-years-old). We reviewed their clinical characteristics and clinical courses after treatment as well as the treatment outcomes. For patients who were treated with MVD, additional treatment methods such as rhizotomy were combined in some areas. In GKRS, we radiated the root entry zone (REZ) with the mean maximum dose of 77.8 (70-84.3) Gy and one 4 mm collimator. Results : The mean age was 68.1 years for MVD, and 71.1 years for GKS group. The average time interval between first presenting symptom and surgery was 84.1 (1-361) months, and 51.4 (1-120) months, respectively. The mean follow-up period after the surgery was 35.9 months for MVD, and 33.1 months for GKRS. According to Pain Intensity Scale, MVD group showed better prognosis with 17 (63%) cases in grade I-II versus 10 (55.6%) cases in GKRS group after the treatment. The pain recurrence rate during follow up did not show much difference with 3 (11.1%) in MVD, and 2 (11.1%) in GKRS. After the treatment, 2 cases of facial numbness, and 1 case each of herpes zoster, cerebrospinal fluid (CSF) leakage, hearing disturbance, and subdural hematoma occurred in MVD Group. In GKRS, there was 1 (5.6%) case of dysesthesia but was not permanent. Three cases were retreated by GKRS but the prognosis was not as good as when the surgery was used as primary treatment, with 1 case of grade I-II, and 1 case of recurrence. The maximal relieve of pain was seen just after surgery in MVD group, and 1 year after treatment in GKRS group. Conclusion : For trigeminal neuralgia patients with advanced age, MVD showed advantages in immediately relieving the pain. However, in overall, GKRS was preferable, despite the delayed pain relief, due to the lower rate of surgical complications that arise owing to the old age.

Hearing Outcome after Gamma Knife Stereotactic Radiosurgery in Vestibular Schwannoma Patients with Serviceable Hearing

  • Objective : The authors conducted a retrospective study to evaluate the preservation rates of serviceable hearing and to determine its prognostic factors after gamma knife stereotactic radiosurgery[GK SRS] in the patient with vestibular schwannomas. Methods : Between December 1997 and March 2005, 54 patients with a sporadic vestibular schwannoma and serviceable hearing [Gardner Robertson grade I-II] were enrolled in this study. Electronic database of medical records and radiological examinations before and after GK SRS were investigated to the last follow up. The mean marginal dose was $12.3{\pm}0.7Gy$. The mean maximum dose delivered to the tumor center was 24.7Gy [$22{\sim}30Gy$]. The median tumor volume was 2cc [$0.1{\sim}9.1cc$]. The median follow-up period of magnetic resonance[MR] imaging was 31 months [$6{\sim}99\;months$], and the mean follow-up period of audiometry was 24 months [$4{\sim}70\;months$]. Results : The tumor control rate was 100% in the patients with the follow up period more than 2 years. The trigeminal and facial nerve preservation rates were 98% and 100%, respectively. Twenty-eight [52%] of the 54 patients preserved serviceable hearing and 16 [30%] patients retained their pre-GK G-R grade level after GK SRS. In the univariate and multivariate analysis, there was no significant prognostic factor in preservation of the serviceable hearing. Conclusion : The hearing preservation rate is still unsatisfactory compared with the results of other cranial nerve preservation and tumor control in the treatment of vestibular schwannoma by GK SRS. More sophisticated strategy during and after GK SRS is necessary to improve long-term hearing preservation.

삼차신경통에 대한 감마나이프방사선 수술 (Outcome of Gamma Knife Radiosurgery for Trigeminal Neuralgia)

  • 목 적 : 본 연구는 삼차신경통에 대한 감마나이프 방사선 수술을 한 경우에 있어서 그 장기추적 결과를 분석하고자 하였다. 방 법 : 총 증례수는 11례였고 신경근 입구부(nerve root entry zone)에 수술의 목표점으로 최대량 67~85Gy를 조사하였다. 4mm collimator를 사용하였고 목표점은 뇌교의 표면으로부터 1~6mm 원위부 신경근에 위치하였다. 초기 3례에 있어서는 신경근과 뇌교가 만나는 접합부를 방사선수술의 목표점으로 삼았다. 이들 증례에서는 뇌교에 56 혹은 60Gy가 조사되었다. 후반 8례에서는 목표점을 다소 신경근의 원위부로 이동하여 뇌교의 가장자리가 최대량의 20% 이하로 조사되도록 계획하였다. 결 과 : 평균추적 기간은 25개월(13~50개월)이었다. 통증감소의 시작시점은 수술후 일주일내에서부터 길게는 5개월째였다. 통증의 치료결과는 삼례에서 완전소실, 3례에서 현격히 감소(80~90%)하였고 4례에서는 의미있게 감소하였다. 단 한례에서 최종추적결과 수술전과 같은 정도의 통증재발이 있었다. 전례에서 감마나이프 방사선수술과 관련된 의미있는 정도의 부작용은 관찰되지않았다. 결 론 : 장기추적 결과 감마나이프 방사선 수술이 삼차신경통의 치료에 효과적임이 관찰되었고 향후 이 질환에 대한 일차적 치료법으로 정하기 위하여 좀더 많은 치료경험이 필요할 것으로 사료된다.

Radiosurgical Techniques and Clinical Outcomes of Gamma Knife Radiosurgery for Brainstem Arteriovenous Malformations

  • Objective : Brainstem arteriovenous malformation (AVM) is rare and radiosurgical management is complicated by the sensitivity of the adjacent neurological structures. Complete obliteration of the nidus is not always possible. We describe over 20 years of radiosurgical procedures for brainstem AVMs, focusing on clinical outcomes and radiosurgical techniques. Methods : Between 1992 and 2011, the authors performed gamma knife radiosurgery (GKRS) in 464 cerebral AVMs. Twenty-nine of the 464 patients (6.3%) reviewed had brainstem AVMs. This series included sixteen males and thirteen females with a mean age of 30.7 years (range : 5-71 years). The symptoms that led to diagnoses were as follows : an altered mentality (5 patients, 17.3%), motor weakness (10 patients, 34.5%), cranial nerve symptoms (3 patients, 10.3%), headache (6 patients, 20.7%), dizziness (3 patients, 10.3%), and seizures (2 patients, 6.9%). Two patients had undergone a previous nidus resection, and three patients had undergone a previous embolization. Twenty-four patients underwent only GKRS. With respect to the nidus type and blood flow, the ratio of compact type to diffuse type and high flow to low flow were 17 : 12 and 16 : 13, respectively. In this series, 24 patients (82.8%) had a prior hemorrhage. The mean target volume was 1.7 $cm^3$ (range 0.1-11.3 $cm^3$). The mean maximal and marginal radiation doses were 38.5 Gy (range 28.6-43.6 Gy) and 23.4 Gy (range 18-27 Gy), and the mean isodose profile was 61.3% (range 50-70%). Results : Twenty-four patients had brainstem AVMs and were followed for more than 3 years. Obliteration of the AVMs was eventually documented in 17 patients (70.8%) over a mean follow-up period of 77.5 months (range 36-216 months). With respect to nidus type and blood flow, the obliteration rate of compact types (75%) was higher than that of diffuse types (66.7%), and the obliteration rate of low flow AVMs (76.9%) was higher than that of high flow AVMs (63.6%) (p<0.05). Two patients (6.9%) with three hemorrhagic events suffered a hemorrhage during the follow-up period. The annual bleeding rate of AVM after GKRS was 1.95% per year. No adverse radiation effects or delayed cystic formations were found. Conclusion : GKRS has an important clinical role in treatment of brainstem AVMs, which carry excessive surgical risks. Angiographic features and radiosurgical techniques using a lower maximal dose with higher isodose profiles are important for lesion obliteration and the avoidance of complications.

악성 성상세포종 및 교모세포종의 적정 방사선 조사 영역에 대한 고찰 (Optimal Radiation Therapy Field for Malignant Astrocytoma and Glioblastoma Multiforme)

  • 목적 : 본 연구의 목적은 악성 성상세포종 및 교모세포종 환자들의 방사선 치료 시 가장 적절한 조사 영역을 알아 보고자 시행하였다. 대상 및 방법 : 1994년 1월부터 2000년 3월까지 악성 성상세포종 및 교모세포종으로 진단되어 수술 및 방사선 치료를 받은 후 MRI로 추적관찰이 시행된 환자 중 재발이 확인된 21 명을 대상으로 분석하였다. 원발 병소 바깥 경계에서부터 처음 재발이 확인된 병소까지의 거리를 측정하였다. 그 외에 종양의 크기, 부종의 정도, 수술 절제의 범위, 감마나이프를 이용한 정위방사선수술, 다발성 병변 등이 재발 양상에 미치는 영향에 대하여 분석을 하였다. 결과 : 총 21명 중 18명$(86\%)$이 2 cm 이내에서 재발을 하였다. 이들 중 1 cm 이내가 12명, $1\~2\;cm$ 사이의 재발이 6명이었다. 나머지 3명의 재발은 3 cm, 4 cm, 5 cm, 떨어져서 각각 재발을 하였다. 2 cm 이상 떨어져 재발한 3명은 모두 다발성 병변이 있는 환자였다. 종양의 크기, 부종의 범위, 수술 절제의 범위, 감마나이프 시행 유무에 따른 재발의 양상에 차이가 없었다. 다만 다발성 병변일 경우 더 멀리서 재발하는 경향을 보였다. 결론 : 악성 성상세포종 및 교모세포종에서 재발 양상은 원발 병소 준위의 2 cm 이내 재발이 주 재발 양상이었다. 방사선 조사영역의 넓이는 부종의 범위나, 병소의 크기, 감마 나이프 수술 등에 따라 더 넓힐 필요는 없는 것으로 판단된다. 그러나 다발성 병변의 경우에는 단일 병소보다 더 넓은 조사 범위가 필요할 것으로 생각된다.

Comparison of Target Approximation Techniques for Stereotactic Radiosurgical Plan

  • The aim of radiosurgery cures a patient to deliver the lower dose at the normal organ and the higher dose at the tumor. Therefore accuracy of the dose is required to gain effect of radiosurgery in surgical planning. In this paper, we developed the methods of target approximation for a fast treatment planning. Nominally, the stereotactic radiosurgery(SRS) using Linac and Gamma knife produces spherical dose distribution through circular collimators using multiple arcs and 201 holes on semi-spherical helmet by $^{60}Co$. We developed an automatic radiosurgical plan about spherical packing arrangement. To automatically plan the SRS, new planning methods based on cylinder and cube structure for target shaping was developed. This approach using heuristic and stochastic algorithm is a useful radiosurgical plan without restrictions in the various tumor shapes and the different modalities.

Intradural Retroclival Chordoma

  • A 43-year-old woman presented with dizziness, ataxia and right hearing difficulty. Her magnetic resonance images demonstrated an inhomogeneously contrast-enhanced large tumor growing into right cavernous sinus and Meckel's cave located totally within intradural retroclival region. She underwent retromastoid suboccipital craniotomy to resect the tumor mass and adjuvant gamma knife radiosurgery for remnant tumor at 1 month after operation. Adjuvant radiosurgery after surgical excision seems to be effective for the treatment of intradural extraosseous chordomas.

Gamma Knife Radiosurgery for Ten or More Brain Metastases

  • Objective : This study was performed to assess the efficacy of GKS in patients with ten or more brain metastases. Methods : From Aug 2002 to Dec 2007, twenty-six patients (13 men and 13 women) with ten or more cerebral metastatic lesions underwent GKS. The mean age was 55 years (32-80). All patients had Karnofsky performance status (KPS) score of 70 or better. According to recursive partitioning analysis (RPA) classification, 3 patients belonged to class I and 23 to class II. The location of primary tumor was lung (21), breast (3) and unknown (2). The mean number of the lesions per patient was 16.6 (10-37). The mean cumulated volume was 10.9 cc (1.0-42.2). The median marginal dose was 15 Gy (9-23). Overall survival and the prognostic factors for the survival were retrospectively analyzed by using Kaplan Meier method and univariate analysis. Results : Overall median survival from GKS was 34 weeks (8-199). Local control was possible for 79.5% of the lesions and control of all the lesions was possible in at least 14 patients (53.8%) until 6 months after GKS. New lesions appeared in 7 (26.9%) patients during the same period. At the last follow-up, 18 patients died; 6 (33.3%) from systemic causes, 10 (55.6%) from neurological causes, and 2 (11.1 %) from unknown causes. Synchronous onset in non-small cell lung cancer (p=0.007), high KPS score (${\geq}80$, p=0.029), and controlled primary disease (p=0.020) were favorable prognostic factors in univariate analysis. Conclusion : In carefully selected patients, GKS may be a treatment option for ten or more brain metastases.

Cerebral Arteriovenous Malformation Associated with Moyamoya Disease

  • The coexistence of moyamoya disease (MMD) with an arteriovenous malformation (AVM) is exceedingly rare. We report two cases of AVM associated with MMD. The first case was an incidental AVM diagnosed simultaneously with MMD. This AVM was managed expectantly after encephalo-duro-arterio-synangiosis (EDAS) as the main feeders stemmed from the internal carotid artery, which we believed would be obliterated with the progression of MMD. However, the AVM persisted with replacement of the internal carotid artery feeders by new external carotid artery feeders from the EDAS site. The AVM was eventually treated with gamma knife radiosurgery considering an increasing steal effect. The second case was a de novo AVM case. The patient was initially diagnosed with MMD, and acquired an AVM eight years later that was slowly fed by the reconstituted anterior cerebral artery. Because the patient remained asymptomatic, the AVM is currently being closely followed for more than 2 years without further surgical intervention. Possible differences in the pathogenesis and the radiologic presentation of these AVMs are discussed with a literature review. No solid consensus exists on the optimal treatment of MMD-associated AVMs. Gamma knife radiosurgery appears to be an effective treatment option for an incidental AVM. However, a de novo AVM may be managed expectantly considering the possible risks of damaging established collaterals, low flow characteristics, and probably low risks of rupture.

Reliability of Stereotactic Coordinates of 1.5-Tesla and 3-Tesla MRI in Radiosurgery and Functional Neurosurgery

  • Objective : The aims of this study are to identify interpersonal differences in defining coordinates and to figure out the degree of distortion of the MRI and compare the accuracy between CT, 1.5-tesla (T) and 3.0T MRI. Methods : We compared coordinates in the CT images defined by 2 neurosurgeons. We also calculated the errors of 1.5T MRI and those of 3.0T. We compared the errors of the 1.5T with those of the 3.0T. In addition, we compared the errors in each sequence and in each axis. Results : The mean difference in the CT images between the two neurosurgeons was $0.48{\pm}0.22mm$. The mean errors of the 1.5T were $1.55{\pm}0.48mm$ (T1), $0.75{\pm}0.38$ (T2), and $1.07{\pm}0.57$ (FLAIR) and those of the 3.0T were $2.35{\pm}0.53$ (T1), $2.18{\pm}0.76$ (T2), and $2.16{\pm}0.77$ (FLAIR). The smallest mean errors out of all the axes were in the x axis : 0.28-0.34 (1.5T) and 0.31-0.52 (3.0T). The smallest errors out of all the MRI sequences were in the T2 : 0.29-0.58 (1.5T) and 0.31-1.85 (3.0T). Conclusion : There was no interpersonal difference in running the Gamma $Plan^{(R)}$ to define coordinates. The errors of the 3.0T were greater than those of the 1.5T, and these errors were not of an acceptable level. The x coordinate error was the smallest and the z coordinate error was the greatest regardless of the MRI sequence. The T2 sequence was the most accurate sequence.
