족부에 발생한 선천성 축후성 다지증에 대한 자기공명영상 검사 결과 분석 (Analysis of Congenital Postaxial Polydactyly of the Foot Using Magnetic Resonance Imagings)

  • 목적: 족부에 발생한 선천성 축후성 다지증에 자기공명영상 검사를 실시하여 그 결과에 대해 평가하고자 한다. 대상 및 방법: 족부에 발생한 선천성 축후증 다지증 및 다지합지증으로 수술을 시행받은 347예(288명)에 대하여 단순 방사선 사진상에 나타나는 변형이 시작되는 부위에 따라 다섯 개의 군으로 분류하였다(넓은 중족골두, 이분중족, 유합된 복제, 불완전 복제, 완전복제 군). 골화가 이루어지지 않아 단순 방사선 사진상 나타나지 않는 부위에 대하여 자기공명영상 검사를 실시하여 유합 혹은 분리여부를 확인하였다. 또한 단순 방사선 사진상 지골 형성이 되지 않은 것처럼 보이는 부분에 대해서도 자기공명영상 검사를 실시하였다. 결과: 단순 방사선 사진상 골화가 이루어지지 않은 부분에 있어서 자기공명영상 검사를 실시하여 보니 잉여지와 고유지 간 유합 혹은 분리되는 양상이 다양하게 관찰되었다. 또한 지골 형성이 이루어지지 않은 듯 보이는 부분에 대해서도 지골의 상태를 효과적으로 알 수 있게 하였다. 결론: 족부에 발생한 선천성 축후성 다지증에 실시하는 자기공명영상 검사는 단순 방사선 사진에서 확인되지 않는 부분에 대한 정확한 해부학적 상태를 알려줄 수 있는 유용한 장치로 사용될 수 있다.

Prediction of Residual Axillary Nodal Metastasis Following Neoadjuvant Chemotherapy for Breast Cancer: Radiomics Analysis Based on Chest Computed Tomography

  • Objective: To evaluate the diagnostic performance of chest computed tomography (CT)-based qualitative and radiomics models for predicting residual axillary nodal metastasis after neoadjuvant chemotherapy (NAC) for patients with clinically node-positive breast cancer. Materials and Methods: This retrospective study included 226 women (mean age, 51.4 years) with clinically node-positive breast cancer treated with NAC followed by surgery between January 2015 and July 2021. Patients were randomly divided into the training and test sets (4:1 ratio). The following predictive models were built: a qualitative CT feature model using logistic regression based on qualitative imaging features of axillary nodes from the pooled data obtained using the visual interpretations of three radiologists; three radiomics models using radiomics features from three (intranodal, perinodal, and combined) different regions of interest (ROIs) delineated on pre-NAC CT and post-NAC CT using a gradient-boosting classifier; and fusion models integrating clinicopathologic factors with the qualitative CT feature model (referred to as clinical-qualitative CT feature models) or with the combined ROI radiomics model (referred to as clinical-radiomics models). The area under the curve (AUC) was used to assess and compare the model performance. Results: Clinical N stage, biological subtype, and primary tumor response indicated by imaging were associated with residual nodal metastasis during the multivariable analysis (all P < 0.05). The AUCs of the qualitative CT feature model and radiomics models (intranodal, perinodal, and combined ROI models) according to post-NAC CT were 0.642, 0.812, 0.762, and 0.832, respectively. The AUCs of the clinical-qualitative CT feature model and clinical-radiomics model according to post-NAC CT were 0.740 and 0.866, respectively. Conclusion: CT-based predictive models showed good diagnostic performance for predicting residual nodal metastasis after NAC. Quantitative radiomics analysis may provide a higher level of performance than qualitative CT features models. Larger multicenter studies should be conducted to confirm their performance.

소형 품종 견에서 경추의 척추척수증 (Cervical Spondylomyelopathy in Small-Breed Dogs)

  • 경추 척추척수증(CSM)은 소형 품종 견에서 드물게 발병하는 질병이다. 두 마리의 중성화 하지 않은 페키니즈(체중 5 kg 이하)가 경부 통증과 마비성 보행을 주증으로 내원하였다. 영상 진단을 통해 척추 불안정성에 의한 CSM으로 진단하고, 배쪽 창냄술과 해면질골 이식, 외고정을 조합한 방법을 적용하였다. 수술 후 임상 증상이 현저하게 개선되었고 합병증이나 재발은 없었다. 경추의 척추척수증이 있는 소형 품종 견에서 배쪽 창냄술을 통한 감압과 골 유합을 통한 안정화를 조합한 방법은 효과적인 치료법임을 확인하였다.

개에서 발생한 폐 원발성 혈관육종의 척수로의 전이 (Invasion of Hemangiosarcoma from Lung to the Spinal Cord in a Dog)

  • 후구마비를 나타낸 11세 숫컷 웰시코기가 내원하였다. 신경검사상 양측 뒷다리는 자세반응이 감소되었으며, 그 밖의 이상소견은 확인되지 않았다. MRI 및 CT 영상에서는 T11-12부위에서 척수 부종 소견을 나타내었으며, 폐의 우측 후엽에 종괴가 관찰되었으며, 종괴는 흉막을 통해 척주관으로 침입되어 있었다. 확정진단 및 척수의 감압을 위해 hemilaminectomy를 실시하였다. 종괴는 혈관육종으로 진단되었다. 종괴 제거후 임상증상의 개선은 확인되지 않았으며, 수술후 58일 후에 폐수종으로 인하여 폐사하였다.

Surface Structures and Thermal Desorption Behaviors of Cyclopentanethiol Self-Assembled Monolayers on Au(111)

  • The surface structures, adsorption conditions, and thermal desorption behaviors of cyclopentanethiol (CPT) self-assembled monolayers (SAMs) on Au(111) were investigated by scanning tunneling microscopy (STM), X-ray photoelectron spectroscopy (XPS), and thermal desorption spectroscopy (TDS). STM imaging revealed that although the adsorption of CPT on Au(111) at room temperature generates disordered SAMs, CPT molecules at $50^{\circ}C$ formed well-ordered SAMs with a $(2{\surd}3{\times}{\surd}5)R41^{\circ}$ packing structure. XPS measurements showed that CPT SAMs at room temperature were formed via chemical reactions between the sulfur atoms and gold surfaces. TDS measurements showed two dominant TD peaks for the decomposed fragments ($C_5H_9^+$, m/e = 69) generated via C-S bond cleavage and the parent molecular species ($C_5H_9SH^+$, m/e = 102) derived from a recombination of the chemisorbed thiolates and hydrogen atoms near 440 K. Interestingly, dimerization of sulfur atoms in n-alkanethiol SAMs usually occurs during thermal desorption and the same reaction did not happen for CPT SAMs, which may be due to the steric hindrance of cyclic rings of the CPT molecules. In this study, we demonstrated that the alicyclic ring of organic thiols strongly affected the surface structure and thermal desorption behavior of SAMs, thus providing a good method for controlling chemical and physical properties of organic thiol SAMs.

Targeting a Safe Entry Point for C2 Pedicle Screw Fixation in Patients with Atlantoaxial Instability

  • Objective : This investigation was conducted to evaluate a new, safe entry point for the C2 pedicle screw, determined using the anatomical landmarks of the C2 lateral mass, the lamina, and the isthmus of the pars interarticularis. Methods : Fifteen patients underwent bilateral C1 lateral mass-C2 pedicle screw fixation, combined with posterior wiring. The C2 pedicle screw was inserted at the entry point determined using the following method : 4 mm lateral to and 4 mm inferior to the transitional point (from the superior end line of the lamina to the isthmus of the pars interarticularis). After a small hole was made with a high-speed drill, the taper was inserted with a 30 degree convergence in the cephalad direction. Other surgical procedures were performed according to Harm's description. Preoperatively, careful evaluation was performed with a cervical X-ray for C1-C2 alignment, magnetic resonance imaging for spinal cord and ligamentous structures, and a contrast-enhanced 3-dimensional computed tomogram (3-D CT) for bony anatomy and the course of the vertebral artery. A 3-D CT was checked postoperatively to evaluate screw placement Results : Bone fusion was achieved in all 15 patients (100%) without screw violation into the spinal canal, vertebral artery injury, or hardware failure. Occipital neuralgia developed in one patient, but this subsided after a C2 ganglion block. Conclusion : C2 transpedicular screw fixation can be easily and safely performed using the entry point of the present study. However, careful preoperative radiographic evaluation, regardless of methods, is mandatory.

요추부 척추관 협착증과 동반된 경막내 신경초종: 증례 보고 (Intradural Schwannoma Associated with Lumbar Spinal Stenosis: A Cese Report)

  • 요추부 척추관 협착증과 경막내 종양이 동반되어 있는 환자에서, 척추관 협착증에 대한 수술적 치료만 시행하였다가 수술 후 증상이 잔존할 수 있다. 본 증례는 척추 신경이 척추관 협착증으로 인한 외적인 압박과 함께, 종양의 공간 점유로 경막내 압박을 동반하였던 경우로, 신경 감압술과 경막내 종양의 제거를 동시에 시행하여 성공적으로 치료되었기에 문헌 고찰과 함께 보고하는 바이다. 71세 여자환자가 하부 요통 및 양측 하지로의 방사통을 주소로 내원하였다. 자기 공명 영상에서 제 4-5 요추부는 척추관 협착증 소견과 함께 제 5 요추-제 1 천추부에 경도의 척추 전방 전위증 소견이 관찰되었고, 제 4 요추체 부위에서 타원형의 경막내 공간의 대부분을 점유하는 종괴가 관찰되었다. 후방 도달법으로 감압술 및 유합술을 시행한 다음, 정중 경막 절개술을 통해 종괴를 제거하였다. 조직 검사상 신경초종으로 진단되었고, 수술 후 증상은 현저히 호전되었다.

Convolutional Neural Networks기반 항공영상 영역분할 및 분류 (Aerial Scene Labeling Based on Convolutional Neural Networks)

  • 항공영상은 디지털 광학 영상 기술의 성장과 무인기(UAV)의 발달로 인하여 영상의 도입 및 공급이 크게 증가하였고, 이러한 항공영상 데이터를 기반으로 지상의 속성 추출, 분류, 변화탐지, 영상 융합, 지도 제작 형태로 활용되고 있다. 특히, 영상분석 및 활용에 있어 딥 러닝 알고리즘은 패턴인식 분야의 한계를 극복하는 새로운 패러다임을 보여주고 있다. 본 논문은 딥 러닝 알고리즘인 ConvNet기반으로 항공영상의 영역분할 및 분류 결과를 통한 더욱더 넓은 범위와 다양한 분야에 적용할 수 있는 가능성을 제시한다. 학습데이터는 도로, 건물, 평지, 숲 총 3000개 4-클래스로 구축하였고 클래스 별로 일정한 패턴을 가지고 있어 특징 벡터맵을 통한 결과가 서로 다르게 나옴을 확인할 수 있다. 본 연구의 알고리즘은 크게 두 가지로 구성 되어 있는데 특징추출은 ConvNet기반으로 2개의 층을 쌓았고, 분류 및 학습과정으로 다층 퍼셉트론과 로지스틱회귀 알고리즘을 활용하여 특징들을 분류 및 학습시켰다.

Implant Removal after Percutaneous Short Segment Fixation for Thoracolumbar Burst Fracture : Does It Preserve Motion?

  • Objective : The purpose of this study was to evaluate the efficacy of implant removal of percutaneous short segment fixation after vertebral fracture consolidation in terms of motion preservation. Methods : Between May 2007 and January 2011, 44 patients underwent percutaneous short segment screw fixation due to a thoracolumbar burst fracture. Sixteen of these patients, who underwent implant removal 12 months after screw fixation, were enrolled in this study. Motor power was intact in all patients, despite significant vertebral height loss and canal compromise. The patients were divided into two groups by degree of osteoporosis : Group A (n=8), the non-osteoporotic group, and Group B (n=8), the osteoporotic group. Imaging and clinical findings including vertebral height loss, kyphotic angle, range of motion (ROM), and complications were analyzed. Results : Significant pain relief was achieved in both groups at final follow-up versus preoperative values. In terms of vertebral height loss, both groups showed significant improvement at 12 months after screw fixation and restored vertebral height was maintained to final follow-up in spite of some correction loss. ROM (measured using Cobb's method) in flexion and extension in Group A was $10.5^{\circ}$ ($19.5/9.0^{\circ}$) at last follow-up, and in Group B was $10.2^{\circ}$ ($18.8/8.6^{\circ}$) at last follow-up. Both groups showed marked improvement in ROM as compared with the screw fixation state, which was considered motionless. Conclusion : Removal of percutaneous implants after vertebral fracture consolidation can be an effective treatment to preserve motion regardless of osteoporosis for thoracolumbar burst fractures.

MODIS영상의 고해상도화 수법을 이용한 오창평야 NDVI의 평가 (Assessment of the Ochang Plain NDVI using Improved Resolution Method from MODIS Images)

  • Remote sensing cannot provide a direct measurement of vegetation index (VI) but it can provide a reasonably good estimate of vegetation index, defined as the ratio of satellite bands. The monitoring of vegetation in nearby urban regions is made difficult by the low spatial resolution and temporal resolution image captures. In this study, enhancing spatial resolution method is adapted as to improve a low spatial resolution. Recent studies have successfully estimated normalized difference vegetation index (NDVI) using improved resolution method such as from the Moderate Resolution Imaging Spectroradiometer (MODIS) onboard EOS Terra satellite. Image enhancing spatial resolution is an important tool in remote sensing, as many Earth observation satellites provide both high-resolution and low-resolution multi-spectral images. Examples of enhancement of a MODIS multi-spectral image and a MODIS NDVI image of Cheongju using a Landsat TM high-resolution multi-spectral image are presented. The results are compared with that of the IHS technique is presented for enhancing spatial resolution of multi-spectral bands using a higher resolution data set. To provide a continuous monitoring capability for NDVI, in situ measurements of NDVI from paddy field was carried out in 2004 for comparison with remotely sensed MODIS data. We compare and discuss NDVI estimates from MODIS sensors and in-situ spectroradiometer data over Ochang plain region. These results indicate that the MODIS NDVI is underestimated by approximately 50%.
