Reconstruction of Postburn Scar Contracture of the Sole Using the Medialis Pedis Free Flap (내측 족부 유리 피판을 이용한 발바닥 화상 후 구축 반흔의 재건)

  • Postburn scar contracture of sole can cause musculoskeletal deformity, restricted range of motion, and decreased quality of life. It is very important to exhibit similar characteristics of the sole when reconstructing the sole because it has to resist shearing force and weight bearing. In this case, we performed medialis pedis free flap for the postburn scar contracture of the sole and the flap survived without complication. The patient satisfied with functional and aesthetic outcomes. Medialis pedis free flap, which is harvested adjacent to the sole, can show similar characteristic of the sole and maintain adequate contour. Moreover, this flap can be harvested without sacrifice of major vessel or nerve. Due to these advantages, medialis pedis free flap can be an ideal option for the reconstruction of the sole.

  • Mucinous Eccrine carcinoma (MEC) is a rare malignant tumor related to the eccrine sweat gland and is commonly located on the face, especially around the eyelids and scalp. Most of these tumors are diagnosed at age 40's to 60's and exhibit a wide variety of patterns in addition to the general appearance previously reported. MEC is difficult to diagnose clinically, but can be diagnosed by accompanying biopsy. We present the case of a 75-year-old man who complained of a gradually growing Left lower lid tumor of duration one year. Initially, the tumor was mistaken for an epidermal cyst and treated by surgical resection. However, biopsy findings resulted in a diagnosis of Mucinous Eccrine carcinoma. Therefore, we performed wide excision and flap reconstruction surgery. In a one year follow-up examination, the patient achieved successful functional and aesthetic results without regional or distant metastasis and recurrence.

  • Electrical burn injuries can cause more damage than clinical evaluations initially suggest. The energy waves penetrate from the surface to the deepest layers of tissue, causing extensive harm at every level. The neck is a critical area, both functionally and aesthetically. We present a case involving a young male patient with a severe fourth-degree electrical burn on the neck, who underwent a single-stage debridement and reconstructive surgery. The pectoralis major myocutaneous flap is a versatile option for various head and neck reconstructions. However, if the donor site cannot be closed primarily and requires split-thickness skin grafting, it can result in unsightly scars and deformities. For large flap paddles, it is ideal to reconstruct the secondary defect with locoregional flaps. In this case, we successfully reconstructed the donor site's secondary defect using a contralateral internal mammary artery perforator flap, without resorting to any skin grafts. The early postoperative results demonstrated satisfactory cosmesis, patient satisfaction, and functional outcomes.

Digital Replantation in Industrial Punch Injuries (천공 펀치 기계에 의한 수지 절단부의 재접합술)

  • Purpose: Industrial punch accidents involving fingers cause segmental injuries to tendons and neurovascular bundles. Although multiple-level segmental amputations are not replanted to regain function, most patients with an amputated finger want to undergo replantation for cosmetic as much as functional reason. The authors describe four cases of digital amputation by an industrial punch that involved the reinstatement of the amputated finger involving a joint and neurovascular bundle. Amputated segments were replanted to restore amputated surfaces and distal segments. Methods: A single institution retrospective review was performed. Inclusion criteria of punch injuries requiring replantation were applied to patients of all demographic background. Injury extent (size, tissue involvement), operative intervention, pre- and postoperative hand function were recorded. Result: Four cases of amputations were treated at our institute from 2004 to 2008 from industrial punch machine injury. Average patient age was 32.5 years (25~39 years) and there were three males and one female. Sizes of amputated segments ranged from $1.0{\times}1.0{\times}1.2\;cm^3$ to $3{\times}1.5{\times}1.6\;cm^3$. Tenorrhaphy was conducted after fixing fractured bone of the amputated segments with K-wire. Proximal and distal arteries and veins were repaired using the through & through method. The average follow-up period was thirteen months (2~26 months), and all replanted cases survived. Osteomyelitis occurred in one case, skin grafting after debridement was performed in two cases. Because joints were damaged in all four cases, active ranges of motion were much limited. However, a secondary tendon graft enhanced digit function in two cases. The two-point discrimination test showed normal values for both static and dynamic tests for three cases and 9 mm and 15 mm by dynamic and static testing, respectively, in one case. Conclusion: Though amputations from industrial punch machines are technically challenging to replant, our experience has shown it to be a valid therapy. In cases involving punch machine injury, if an amputated segment is available, the authors recommend that replantation be considered for preservation of finger length, joint mobility, and overall functional recovery of the hand.

  • The flexible solid state device has been widely studied as portable and wearable device applications such as display, sensor and curved circuits. A zero-bias operation without any external power consumption is a highly-demanding feature of semiconductor devices, including optical communication, environment monitoring and digital imaging applications. Moreover, the flexibility of device would give the degree of freedom of transparent electronics. Functional and transparent abrupt p/n junction device has been realized by combining of p-type NiO and n-type ZnO metal oxide semiconductors. The use of a plastic polyethylene terephthalate (PET) film substrate spontaneously allows the flexible feature of the devices. The functional design of p-NiO/n-ZnO metal oxide device provides a high rectifying ratio of 189 to ensure the quality junction quality. This all transparent metal oxide device can be operated without external power supply. The flexible p-NiO/n-ZnO device exhibit substantial photodetection performances of quick response time of $68{\mu}s$. We may suggest an efficient design scheme of flexible and functional metal oxide-based transparent electronics.

Nutritional Components Content of Oriental Melon Fruits Cultivated under Different Greenhouse Covering Films (시설하우스 외피복재 종류에 따른 참외과실의 성분 함량)

  • This study was conducted to investigate the effect of functional cover film on oriental melon fruit quality under unheated plastic greenhouse cultivation in cold period. The 6 kind of films having different characteristics were covered at plastic greenhouses and oriental melon seedlings were cultivated at 2 regions of Seongju, respectively. The air-temperatures in plastic greenhouses of J-1 and J-2, having high infrared absorption rate, were about $2{\sim}5^{\circ}C$ higher than K-3. The contents of ${\beta}-carotene$ and sugar of fruit showed significant difference between functional and normal films. The sucrose, determining a sweetness during maturation, and soluble solids content of fruits cultivated in J-1, J-2, J-3, and K-1 were higher than those of K-2 and K-3. Ascorbic acid of fruits was highest in K-2 having low light transmission rate and thermo-keeping capacity. There was no significant difference in mineral content among all of cover films. These results indicated that the use of functional greenhouse covering films could improve fruit quality such as ${\beta}-carotene$ and sugar content of oriental melon.

Increase of Synapsin I, Phosphosynapsin (ser-9), and GAP-43 in the Rat Hippocampus after Middle Cerebral Artery Occlusion

  • The loss of neurons and synaptic contacts following cerebral ischemia may lead to a synaptic plastic modification, which may contribute to the functional recovery after a brain lesion. Using synapsin I and GAP-43 as markers, we investigated the neuronal cell death and the synaptic plastic modification in the rat hippocampus of a middle cerebral artery occlusion (MCAO) model. Cresyl violet staining revealed that neuronal cell damage occurred after 2 h of MCAO, which progressed during reperfusion for 2 weeks. The immunoreactivity of synapsin I and GAP-43 was increased in the stratum lucidum in the CA3 subfield as well as in the inner and outer molecular layers of dentate gyrus in the hippocampus at reperfusion for 2 weeks. The immunoreactivity of phosphosynapsin was increased in the stratum lucidum in the CA3 subfield during reperfusion for 1 week. Our data suggest that the increase in the synapsin I and GAP-43 immunoreactivity probably mediates either the functional adaptation of the neurons through reactive synaptogenesis from the pre-existing presynaptic nerve terminals or the structural remodeling of their axonal connections in the areas with ischemic loss of target cells. Furthermore, phosphosynapsin may play some role in the synaptic plastic adaptations before or during reactive synaptogenesis after the MCAO.

The Development of Functional Photocatalytic $TiO_2$-Biodegrdable Plastic Composite Material by HVOF Spraying (고속가스플래임 용사법을 이용한 광촉매 $TiO_2$-생분해성 플라스틱 복합재료의 개발)

  • For the production of functional $TiO_2$-biodegradable plastic (polybutylene succinate:PBS) composite material with photocayalytic activity, we attempted to prepare $TiO_2$ coatings on PBS substrate by HVOF and plasma spraying techniques under various conditions. The microstructures of coatings were characterized with SEM and XRD analysis, and the photocatalytic efficiency of coatings was evaluated by the photo degradation of gaseous acetaldehyde. The effects of primary particle size and spraying parameters on the formation behavior, photocatalytic performance of the coatings have been investigated. The results indicated that for both the HVOF sprayed $P_{200}$ and $P_{30}$ coatings, the high anatase ratio of 100% can be achieved regardless of fuel gas pressure. On the other hand, the HVOF sprayed $P_7$ coating exhibited a largely decreased anatase ratio (from 100% to 49.1%) with increasing the fuel gas pressure, which may be attributed to much higher susceptibility of heat for 7 nm agglomerated powder. HVOF sprayed $P_{200}$ and $P_{30}$ coatings show better performance as compared to that of plasma sprayed $P_{200}$ coatings owing to the higher anatase ratio. However, the HVOF sprayed $P_7$ coatings did not show the photocatalytic activity, which may result from the extremely small reaction surface area to the photocatalytic activity and low anatase ratio.

Extensor Tendon Gliding Surface Reconstruction Using Adipoascia Extended Free Anterolateral Thigh Flap - Case Report - (지방 근막 피판이 연장된 전외측 대퇴 유리 피판술을 이용한 신전건 활주막의 재건 - 증례 보고 -)

  • Purpose: In reconstructing a defect on the dorsum of the hand, there are many cases of extensor tendons exposed or even missing. The repaired or reconstructed tendons need relevant gliding environment for good functional recovery. The anterolateral thigh flap offers a vascular fascial component with large amounts that can be used for covering exposed tendons and we report a unique case of single-stage hand dorsum and gliding surface reconstruction. Methods: A 35-year-old man had severe post-traumatic scarring in his left dorsal hand and coverage of the flap with split-thickness skin graft has been done before. After scarred skin excision and extensor tendon graft for missed part, a free anterolateral thigh adipofascial flap was used to resurface the hand dorsum and to reconstruct a two-layer gliding surface of the extensor tendons. The extensor tendons were wrapped in the fascial component with the fat layer inside. Results: He had an uneventful postoperative course without infection, dehiscence and flap necrosis. Good overall functional recovery and tendon excursion were observed. He was also satisfied with postoperative appearance. Conclusion: A free anterolateral thigh adipofascial flap was used successfully for reconstruction of a two-layer tendon gliding surface to treat a patient with severe scarring in the dorsal hand.

Great Toe Pulp Graft for the Reconstruction of the Postburn Flexion Contracture in the Fingers (수지 화상 후 굴곡성 구축 치료 시 족질부 이식)

  • Purpose: In case of postburn flexion contracture of the fingers, skin graft, geometrical relaxation techniques, local flap, and free flap have been used. Among these procedures, full-thickness skin grafts from the inguinal area are widely used to reconstruct a postburn flexion contracture in the fingers. But there are many esthetic and functional problems in this procedure. Especially, hyperpigmentation of the skin-grafted fingers poses a troublesome problem, particularly in the patients who have dark colored skin. To solve the problem, we have used pulp graft which was harvested from the lateral aspect of great toe. In the present study, we report pulp graft, with which we have obtained a good result in the treatment of postburn flexion contracture of the fingers. Methods: Between September of 2004 and August of 2006, great toe pulp graft was performed to 20 sites of 15 patients. After release of the postburn flexion contracture using Z-plasty, the composite tissue (pulp) harvested from the lateral aspect of great toe was grafted on the raw surface. Moisture dressing with ointment and foam dressing material was performed. Stratum corneum of the graft got stripped off in two to four weeks after pulp graft. The color of the pulp graft was slightly reddish, then it became similar to the adjacent tissue. Results: There was complete take in all the patients who were treated with pulp graft. Great toe pulp graft provided similar color and texture to the adjacent skin, high rate of graft take, and left only a minimal scar at donor site. Conclusion: Thick keratin layer and inelastic nature of the pulp make this type of the graft much easier and simpler, and ensure a better take. Pulp graft is useful method for the reconstruction of the postburn flexion contracture in fingers.
