목적 : 기존의 시각 자극과 관련한 뇌기능 연구가 일반적인 이차원 시각 자극을 이용한 데 반하여 본 논문에서는 삼차원 스트레오스코픽 비젼에 의한 시각 자극을 이용한 뇌기능 영상 연구를 수행하였다. 본 연구를 통하여 삼차원 시각처리와 관련한 뇌의 기능 영역 및 특징을 이차원 시각 자극 결과와 비교 검토하였다. 대상 및 방법 Anaglyph(이차원 그림을 칼라 코딩한 후 왼쪽과 오른쪽에 서로 다른 색깔의 렌즈를 사용하여 스트레오스코픽 비젼을 구현)를 이용하여 삼차원 시각 자극을 주었으며, 이와 비교를 위하여 동일한 영상으로 이차원 시각 자극을 구현하였다. 건강한 volunteer들에 대하여 삼차원 및 이차원 시각 자극에 대한 뇌기능 영상을 3.0 Tesla MRI 시스템에서 얻었다. 결과 : 삼차원 시각 자극에 의하여 활성화된 뇌의 영역은 이차원 시각 자극과 동일하게 후두엽으로 나타났으며, 삼차원 시각 자극에 의한 활성영역이 이차원 시각 자극에 의한 영역 보다 약 $18\%$ 더 넓게 나타났다. 결론 : 사람은 양 눈을 사용하여 삼차원 물체를 인식한다. 삼차원 물체 인식은 스테레오스코픽 비젼에 기인하는데, anaglyph 를 이용하여 삼차원 시각 자극에 대한 뇌기능 영상을 3.0 Tesla MRI 시스템에서 수행하였다. 삼차원 시각 자극에 의하여 활성화된 뇌의 영역은 이차원 시각 자극과 동일하게 후두엽으로 나타났으나, 활성영역이 이차원 자극에 비하여 약 $18\%$ 더 넓게 나타났다. 이것은 삼차원 영상 처리가 이차원 영상 처리에 비하여 왼쪽 영상과 오른쪽 영상의 차이에서 거리 정보를 추출하는 삼차원 시각 처리 기능이 추가됨에 따라 뇌의 처리 영역이 넓어진 것으로 판단된다. 이러한 삼차원 시각 자극 뇌기능 영상은 가상현실이나 3-D 디스플레이, 3-D 멀티미디어 콘텐츠 연구 등 다양한 분야에 활용될 수 있을 것으로 기대한다.
Objectives : Posttraumatic stress disorder(PTSD) has been primarily associated with emotional problems. Recently, however, the impact of PTSD on cognitive processes has interested a growing number of researchers. The current study is aimed at investigating the cognitive aspects of PTSD at both behavioral and neurological levels. Methods : We recruited individuals with PTSD who survived the Daegu subway explosion in 2003 as well as non-PTSD individuals as a control group. To evaluate the inhibitory processes and the neural mechanisms, we had these individuals perform the negative priming task simultaneously with functional MRI scanning. Results : Behaviorally, the negative priming effect was intact in the control group but was not evident in the PTSD group. In the imaging results, only the PTSD group showed the negative priming effect (i.e., increased activation of the negative priming condition as opposed to the neutral condition) in the dorsolateral prefrontal cortex, anterior cingulate cortex, and inferior temporal gyrus. The PTSD group also showed increased activity for the positive priming condition as opposed to the neutral condition in the claustrum. These results confirm and extend the previous findings that the integrity of the ACC is compromised in the trauma survivors due to disrupted white matter tract. Conclusions : The current results suggest that deteriorated performance of the PTSD group may be due to the functional problem as well as the structural abnormalities.
Purpose: This study is to compare the distinct brain activation between implicit and explicit memory retrieval tasks using a non-invasive blood-oxygenation-level-dependent (BOLD) functional magnetic resonance imaging(fMRI). Materials & Methods: We studied seven right-handed, healthy volunteers aged 21-25 years(mean;22 years) were scanned under a 1.5T Signa Horizon Echospeed MR imager(GE Medical Systems, Milwaukee, U.S.A.). During the implicit and explicit memory retrieval tasks of previously teamed words under the conceptual processing, we acquired fMRI data using gradient-echo EPI with 50ms TE, 3000ms TR, 26cm${\times}$26cm field-of-view, 128${\times}$128 matrix, and ten slices(6mm slice thickness, 1 mm gap) parallel to the AC-PC(anterior commissure and posterior commissure) line. By using the program of statistical parametric mapping(SPM99), functional activation maps were reconstructed and quantified.

Objective: Prior functional magnetic resonance imaging (fMRI) work has revealed that children/adolescents with disruptive behavior disorders (DBDs) show dysfunctional reward/non-reward processing of non-social reinforcements in the context of instrumental learning tasks. Neural responsiveness to social reinforcements during instrumental learning, despite the importance of this for socialization, has not yet been previously investigated. Methods: Twenty-nine healthy children/adolescents and 19 children/adolescents with DBDs performed the fMRI social/non-social reinforcement learning task. Participants responded to random fractal image stimuli and received social and non-social rewards/non-rewards according to their accuracy. Results: Children/adolescents with DBDs showed significantly reduced responses within the caudate and posterior cingulate cortex (PCC) to non-social (financial) rewards and social non-rewards (the distress of others). Connectivity analyses revealed that children/adolescents with DBDs have decreased positive functional connectivity between the ventral striatum (VST) and the ventromedial prefrontal cortex (vmPFC) seeds and the lateral frontal cortex in response to reward relative to non-reward, irrespective of its sociality. In addition, they showed decreased positive connectivity between the vmPFC seed and the amygdala in response to non-reward relative to reward. Conclusion: These data indicate compromised reinforcement processing of both non-social rewards and social non-rewards in children/adolescents with DBDs within core regions for instrumental learning and reinforcement-based decision-making (caudate and PCC). In addition, children/adolescents with DBDs show dysfunctional interactions between the VST, vmPFC, and lateral frontal cortex in response to rewarded instrumental actions potentially reflecting disruptions in attention to rewarded stimuli.
목적: 정상인을 대상으로 과제의 규칙을 모른체 의사결정를 하는 경우 시간에 따른 뇌활성 영역의 차이를 lowa Gambling 과제를 수행하는 자기공명 뇌기능 영상을 사용하여 알아보고자하였다. 대상 및 방법: 14명의 오른손잡이 정상인(여자 5명, 남자 9명, 평균나이 26.8세)을 대상으로 100개로 구성된 lowa Gambling과제를 수행하는 동안 자기공명 뇌기능영상을 획득하였다. GE 3.0T 자기공명영상 장치에서 혈액산소포화 의존(BOLD)기법을 사용하여 EPI 데이터 (에코시간 40 ms, 반복시간 2000 ms, 매트릭스 $64{\times}64$)를 획득한 후 MATLAB버전 7.2 (The Mathworks Inc., Natick, MA, USA)와 Statistical Parametric Mapping (SPM 2; Wellcome Department of Imaging Neuroscience, London, UK) 분석툴을 이용하여 총 100개 중 20개 단위로 구성된 5개의 블록으로 나누어 분석하였다. 결과: 과제 수행초기 블록에서는 위쪽 중앙 전전두뇌엽에서의 뇌활성화를 나타내고 과제를 수행해 감에 따라 뇌활성화 영역이 점차 중앙 전전두뇌엽의 아래족으로 이동하는 결과를 나타내었다 (p < 0.005). 결론: 의사결정 과제의 규칙을 모르는 경우 과제를 수행하며 규칙을 알아가는데는 위쪽 중앙 전전두뇌엽의 영역을 사용하고 과제의 규칙을 깨달은 다음 의사결정을 수행하는 경우에는 아래쪽 중앙 전전두뇌엽의 영역을 사용함을 알수 있었다.
목 적 : 정상인 여성을 대상으로 기능적 자기공명영상법(functional magnetic resonance imaging : fMRI)을 이용하여 성적 흥분반응과 관련되는 시각자극과 삼음교(SP6, 三陰交) 경혈자극 의한 대뇌 활성화 영역의 차이점을 밝히고, 나아가서는 성적흥분의 기전을 신경해부학적인 측면에서 규명하고자 하였다. 대상 및 방법 : 성기능이 정상적인 오른손잡이 여자 21명(19$\sim$32세 : 평균 22세)을 대상으로 3.0 Tesla MR scanner를 이용하여 fMRI 영상을 얻었다. 성적 흥분을 유도하기 위한 패러다임은 3분간의 에로틱 비디오 시청과 삼음교(SP6)자침에 의한 자극을 주었으며, 자극 전 후에는 각각 1분간의 휴지기를 두었다. 또한 광명혈(GB37) 후방으로 3 cm 위치에 있는 비경혈점에 자극을 주어 경혈(acupoint)과 비경혈(shampoint) 자침에 의한 대뇌의 활성화 양상을 비교하였다. 기능적 자기공명영상의 획득은 AC-PC line을 기준으로 20개의 횡단면으로부터 총 2,000개의 기초영상을 얻었으며, SPM99 프로그램을 이용하여 평균 활성화 영상을 얻었다. 결 과 : 삼음교 경혈과 비경혈의 침자극에 의한 비교실험에서 경혈은 비경혈에 비하여 대뇌 신피질에서 평균 5배, 변연계에서 2배 높은 활성화도를 보였다. 비경혈 자극시에 간뇌의 HTHL, 기저핵의 GLO, AMYG, 그리고 두정엽의 SMG 등에서는 활성화 신호가 관찰되지 않았다. 성적흥분과 관련된 대뇌의 해부학적인 영역을 시각자극과 삼음교 경혈자극 별로 비교 분석한 결과, 두 자극간의 평균 활성화율(activation ratio)은 신피질과 변연계 모두에서 유의한 차이가 없었다(p < 0.05). 반면 평균 활성화도(activity)는 신피질의 경우에는 두 자극간에 유의한 차이를 보이지 않았으나, 변연계의 경우에는 경혈 자극시에 유의하게 더 높은 활성화도를 보였다(p < 0.05). 결 론 : 기능적 자기공명영상법을 이용하여 성적흥분과 관련된 시각자극과 경혈 침술자극에 의하여 유발된 대뇌 활성화 양상의 차이점과 신경해부학적 기전의 차이점을 규명할 수 있었으며, 이러한 결과를 바탕으로 향후 한방의 경혈, 경락 이론을 과학적인 측면에서 입증할 수 있을 것으로 기대한다.
목적 : 언어기능의 지도화(language mapping) 및 우세반구 결정에 있어서 기능적 자기공명영상의 유용서을 알아보고자 하였다. 대상 및 방법 : 자원자 10명(남자 6명, 여자 4명, 오른손잡이 7명, 왼손잡이 3명)을 대상으로 단어생성 과제(명사만들기, 동사만들기)와 판단과제를 이용하여 언어영역의 활성화를 유도하였다. 1.5T 초진도 자기공명영상장치에서 EPI로 BOLD 기법을 이용하여 기능적 영상을 얻었으며, 여앙후처리는 SPM 분석 프로그램을 이용하였다. 기능적 영상은 2번의 활성기와 3번의 휴식기를 번갈아 시행하였고 스캔 시간은 각각30초였으며 총 영상획득시간은 162초였다. 유의수준 p<0.001을 기준ㅇ로 자극 과제에 따라 활성화된 뇌영역을 분석하였고 반구별로 활성화된 화소의 수를 측정하여 편재화지수(lateralization index)를 구하였다. 결과 : 왼손잡이 1명을 제외한 9명에서 성공적으로 기능적 영상을 얻었으며 양측 전운동영역(premotor area), 하전두회, 두정엽, 중측두회 등의 여러 영역에서 활성화 신호가 나타났다. 오른손잡이의 경우 편재화지수는 평균 0,64(0.16~1)로 모두 좌측으로 편재화 되었다. 과제별로는 동사만들기 과제가 명사만들기 및 판단 과제에 비해 언어 영역의 활성화가 많았고 편재화지수의 값도 높았다. 결론 : 기능적 자기공명영상은 언어영역의 뇌기능지도화 및 우세반구 결저에 있어서 유용한 검사 방법이 될 수 있으리라 생각된다.
The aim of this study was to evaluate effects of short-tenn repetitive-bilateral excercise on the activation of motor network using functional magnetic resonance imaging (fMRI). The training program was performed at 1 hr/day, 5 days/week during 6 weeks. Fugl-Meyer Assessments (FMA) were performed every two weeks during the training. We compared cerebral and cerebellar cortical activations in two different tasks before and after the training program: (1) the only unaffected hand movement (Task 1); and (2) passive movements of affected hand by the active movement of unaffected hand (Task 2). fMRI was performed at 3T with wrist flexion-extension movement at 1 Hz during the motor tasks. All patients showed significant improvements of FMA scores in their paretic limbs after training. fMRI studies in Task 1 showed that cortical activations decreased in ipsilateral sensorimotor cortex but increased in contralateral sensorimotor cortex and ipsilateral cerebellum. Task 2 showed cortical reorganizations in bilateral sensorimotor cortex, premotor area, supplemetary motor area and cerebellum. Therefore, this study demonstrated that plastic changes of motor network occurred as a neural basis of the improvement subsequent to repetitive-bilateral excercise using the symmetrical upper-limb ann motion trainer.
Purpose: To investigate whether or not acupuncture of GB34 produces a significant response of the modulation of somatomotor areas by functional magnetic resonance imaging (fMRI) study. Methods: The acupoint, GB34, located in the back of the knee, is known to be effective in recovering motor function after stroke. Using 3T MRI scanner, functional MR imaging of the whole brain was performed in 12 normal healthy subjects during two stimulation paradigms; acupuncture manipulation on GB 34 and sham points. This study investigates the activation of the motor cortex elicited by a soft and an intensified stimulation of GB 34. Three different paradigms were carried out to detect any possible modulation of the Blood Oxygenation Level Dependent (BOLD) response in the somatomortor area to motor stimulation through acupuncture. Results: Group analysis from seven individuals showed that bilateral sensorimotor areas (BA 3,4,6 and 7) showed stimulation related BOLD signal contrast of approximately 6% whereas very few areas were activated when sham stimulation is given. Conclusions: The present study shows that acupuncture fMRI study can be safely conducted in 3T MRI environment, and acupuncture stimulation in GB34 modulates the cortical activities of the soma- to motor area in human. The present findings may shed light on the CNS mechanism of motor function by acupuncture and form a basis for future investigations of motor modulation circuits in the stroke patients. We developed a symmetrical upper limb motion trainer for chronic hemiparetic subjects. This trainer enabled the practice of a forearm pronatio $n^ination and wrist flexion/extension. In this study, we have used functional magnetic resonance imaging(fMRI) with the developed symmetrical upper limb motion device, to compare brain activation patterns elicited by flexion/extension wrist movements of control and hemiparetic subject group. In control group, contralateral somatosensory cortex(SMC) and bilateral cerebellum were activated by dominant hand movement(Task 1), while bilateral movements by dominant hand(Task 2) activated the SMC in both cerebral hemispheres and ipsilateral cerebellum. However, in hemiparetic subject group, contralateral supplymentary motor area(SMA) was activated by unaffected hand movement(Task 1), while the activation of bilateral movements by unaffected hand(Task 2) showed only SMA in the undamaged hemisphere. This study, demonstrating the ability to accurately measure activation in both sensory and motor cortex, is currently being extended to patients in clinical applications such as the recovery of motor function after stroke.ke.
