충남지역 초등학생의 채소와 과일 섭취 행동 변화 단계에 따른 비타민과 무기질 섭취상태 비교 (Comparison of Mineral and Vitamin Intakes According to the Stage of Change in Fruit and Vegetable Intake for Elementary School Students in Chungnam Province)

  • 본 연구는 충남지역 초등학생을 대상으로 채소와 과일섭취 증진을 위한 행동 변화 단계별 비타민과 무기질 섭취 상태를 비교해 보고자 2007년 4월 19일부터 5월 1일까지 충남 연기군 1개 초등학교 4, 5, 6학년 남자 122명, 여자 134명, 총 256을 대상으로 조사하였다. 조사대상 아동의 채소와 과일 섭취 행동 변화 단계의 구분은 설문에 의해 현재보다 더 먹으려고 생각하는가, 계획하는가, 실천하는가를 순차적으로 예나 아니오로 답하게 하여 고려전, 고려 및 준비, 행동의 3단계로 분류하였다. 1) 자신의 건강상태에 대해 좋다, 보통이다, 나쁘다로 답하게 하였을 때 고려 및 준비 단계에 있는 아동에서 건강상태가 '좋다'는 응답이 35.9%로 다른 단계에 비해 높았다. 영양제를 '항상 섭취 한다'고 답한 대상자가 고려전 단계에 있는 아동의 45.8%로 다른 두 군보다 높았다. 2) BMI 85백분위수 미만에서는 남아가 여아에 비해 고려전 단계에 속한 비율이 높고 고려 및 준비단계 이상에 속한 비율이 낮았다. 3) 영양소 섭취량의 차이는 행동 단계의 아동에서 비타민 C 섭취량 (p < 0.05)이 가장 높게 나타났다. 4) 여아에서는 행동 변화 단계가 진전됨에 따라 평균필요량 미만 섭취자의 비율이 비타민 B2 (p < 0.05)와 비타민 C 섭취 (p < 0.05)에서 낮게 나타났으나 남아에서는 행동 변화단계간에 섭취부족자의 비율에 차이를 내는 영양소가 없었다. 5) 행동 변화 단계에 따른 식품 섭취 상태는 채소류 (p < 0.01)와 김치 (p < 0.05)섭취량이 행동 단계의 대상자에서 다른 단계보다 높았고, 난류에서도 행동 변화 단계의 진전에 따라 섭취량이 점차 높아졌다. 본 연구 결과 행동 변화 단계에 따라서 김치와 채소류의 섭취량이 행동전 단계에서는 차이가 없었으나 행동단계에서 증가하였고, 비타민 C의 섭취량도 행동단계에서 확실하게 증가하였다. 또한 다른 비타민과 무기질도 증가하는 경향을 보였으므로 아동들의 비타민과 무기질 섭취를 증진시키기 위해서는 채소와 과일섭취 행동변화단계를 높일수 있는 방안에 관한 연구가 필요하다.

Associations between food consumption/dietary habits and the risks of obesity, type 2 diabetes, and hypertension: a cross-sectional study in Jakarta, Indonesia

  • BACKGROUND/OBJECTIVES: This study aimed to assess the current mean daily intake of 10 food groups, analyze the sociodemographic factors associated with food consumption, and determine the associations between food consumption/dietary intake and the prevalence rates of obesity, type 2 diabetes (T2D), and hypertension (HTN) in Jakarta, Indonesia. SUBJECTS/METHODS: A total of 600 participants aged 20-85 yrs were included in this cross-sectional study. Food consumption and dietary habits were assessed using a food frequency questionnaire. To determine the association between food consumption/dietary habits and the abovementioned diseases, logistic regression analysis was performed. RESULTS: The average vegetable and fruit intake was lower, while sugar and salt consumption were higher than that recommended by Indonesia's national dietary guidelines. A high intake of ultra-processed foods (UPFs) was associated with young age, men, "single" status, a high education level, and employment with a high monthly income. Obesity and T2D were positively correlated with high intakes of cereals and tubers, UPFs, sugars, fats, and oils. Conversely, an inverse association was found between legume, vegetable, and fruit consumption and obesity risk. An inverse correlation was also observed between vegetable consumption and T2D risk. Moreover, a high salt intake was inversely correlated with fruit consumption in terms of HTN risk. Non-indulgence in habitual late-night snacking and refrainment from consuming more than one dish at each meal were also negatively related to the prevalence of obesity, T2D, and HTN. Inverse correlations were also observed between the prevalence rates of T2D and HTN and abstaining from adding sugar to beverages. CONCLUSION: Foods high in fat, sugar, and sodium were strongly associated with the risks of obesity, T2D, and HTN. Additionally, poor eating habits were also associated with disease development.

50세 이상 남성의 식품 및 영양소 섭취실태와 골밀도와의 관계 (Associations Between Daily Food and Nutrient Intake and Bone Mineral Density in Men Aged 50 Years and Older)

  • Osteoporosis is a rising problem, as the older age population is increasing due to prolongation of life. Genetic and environmental factors play key roles in bone metabolism, and diet is also an important factor. We investigated the relationship among factors affecting bone mineral density (BMD), including daily food intake and nutrient intake in men aged >50 years. Seventy-eight men, who visited the health promotion center at one of the university medical centers, were divided into normal and osteopenia groups according to their BMD. The body weight of the normal group was significantly higher than that of the osteopenia group. The osteopenia group showed significantly higher carbohydrate intake and lower calcium and vegetable calcium intake compared to those in the normal group. Lumbar spine BMD was negatively correlated with energy, fat, vitamin B1, and sodium intake in the normal group. Additionally, femoral neck BMD was negatively correlated with total animal protein, energy, protein, fat, phosphorous, iron, animal iron, potassium, vitamin B1, B2, B6, and niacin intake. Lumbar spine BMD was positively correlated with fruit, calcium, vegetable calcium, animal calcium, and vitamin C intake in the osteopenia group. Femoral neck BMD was negatively correlated with meat, dairy product, total animal protein, plant protein, animal protein, vitamin A, and cholesterol intake. A stepwise multiple regression analysis revealed that several dietary factors affected BMD, including energy, fat, vitamin B1, B2, B6, niacin, sodium, protein, iron, animal iron, phosphorous, potassium, and animal protein in the normal group and zinc, calcium, vegetable calcium, animal calcium, vitamin C, fruit, protein, animal protein, meat, dairy product, carbohydrates, cholesterol, vegetables, mushrooms, and seasonings in the osteopenia group. These results indicate that adequate nutrient intake plays an important role maintaining optimum bone health in middle aged men.

Evaluation of the effect of a school garden as an educational didactic tool in vegetable and fruit consumption in teenagers

  • BACKGROUND/OBJECTIVES: Increasing the consumption of vegetables and fruits in Mexico remains a challenge. Promoting sustainable food production systems through schools may be an effective way to educate young people about food and nutrition issues. A study of nutritional education in adolescents, based on the school garden, is necessary in order to evaluate its effects on the consumption of fruits and vegetables among middle- and upper-income segments of the population. The objective of this study was to evaluate the effect of an educational intervention, accompanied by a school garden as an educational teaching tool, to improve vegetable and fruit consumption by Mexican teenagers attending a private middle/high school. SUBJECTS/METHODS: Teenagers between 12 and 18 years of age (n = 126) attending a private middle/high school in Queretaro, Mexico participated in a 3-arm, controlled, comparative impact study using a vegetable and fruit consumption frequency questionnaire, food consumption diaries, a psychosocial factor assessment questionnaire of vegetable and fruit consumption, and structured interviews. The participants were randomized into 3 experimental groups: 1) food education + school garden (FE + SG), 2) FE only, and 3) control group (CG). RESULTS: The FE + SG and FE groups significantly increased the frequency and daily intake of vegetables and fruits compared to the CG. The FE + SG group showed greater understanding of, reflection upon, and analysis of the information they received about vegetable and fruit consumption, as well as a greater willingness to include these in their daily diet. CONCLUSIONS: FE accompanied by a SG as a teaching tool is more effective at promoting vegetable and fruit consumption than either education alone or control in teenagers in middle-upper income segments of the population.

충남지역 중학생의 채소 섭취실태와 식습관에 관한 연구 (A Study on Vegetable Intakes and Dietary Habits of Middle School Students in Chungnam)

  • The purpose of this study was to provide basic data for nutritional education for juveniles' desirable vegetable intake by analyzing their vegetable intake, dietary habit and nutrient intakes according to gender of middle school students in Chungnam. The average age of the subjects was 15.0, height was 162.5 cm, weight was 53.5 kg and their average BMI was $20.1kg/m^2$. The frequency of eating supper in girls was lower than that in boys. Many respondents answered that a meal-time was 10~20 minutes. Without gender difference, more than 90% respondents thought that vegetables were good for health. As for the preference of vegetable, subjects responded, "I am in the middle", "I like them", "I dislike them", "I like them very much", and "I dislike them very much" in order, without gender difference. The frequent eaten leaf vegetable was Chinese cabbage, the frequent eaten fruit vegetable was cucumber, and the frequent eaten root vegetable was radish. The favorite leaf vegetable was lettuce, the favorite fruit vegetable was corn, and the favorite root vegetable was sweet potato. The preference degrees of taro and ginger were very low. The reasons why they liked a vegetable were that it was delicious and they ate it at home often. In addition, the reason why they disliked vegetables was that they are untasty in flavor or texture and it showed that many students had a prejudice that vegetables were untasty. The intakes of plant protein, dietary fiber, ash and INQs of dietary fiber, calcium, vitamin C, folate, vitamin E in the group with high preference of vegetables were significantly higher than those of low preference group. The study results indicate that intake frequency and preference of root vegetables in juveniles are low and the major reasons of these results are taste and eating experience of vegetables. In addition, the intake amounts of dietary fiber and folate are poor in the subjects with low preference of vegetables. Therefore, families and schools should make efforts that juveniles can recognize the importance of vegetable intake and select various vegetables properly through the development of cooking methods and systematic nutrition education.

Nutritional value and in situ degradability of fruit-vegetable byproducts and their feeding effects on performance of growing Hanwoo steers

  • Objective: This study was conducted to evaluate nutritional value and in situ degradability of fruit-vegetable byproducts and their feeding effects on performance of growing Hanwoo steers. Methods: Nutritional value and in situ degradability of cabbage, Chinese cabbage and fruit-vegetable byproducts were assessed. In vivo feeding trial was also performed for 12 weeks. Thirty-six growing steers were randomly allocated into three groups according to body weight (BW) and age in 12 pens (4 replications/treatment) and assigned to one of the three dietary treatments: control (byproduct 0%), FV-B (fruit-vegetable byproduct 20%), and CA-B (cabbage peel 15% plus Chinese cabbage peel 15%, total byproduct 30%). Results: The crude protein contents of cabbage, Chinese cabbage and fruit-vegetable byproducts were 18.69%, 20.20%, and 10.07%, respectively. Concentrations of neutral detergent fiber (NDF) were higher in cabbage (22.31%) and Chinese cabbage (28.83%) than fruit-vegetable (13.94%). Higher concentrations of non-fiber carbohydrate were observed for fruit-vegetable (66.72%) than cabbage (44.93%) and Chinese cabbage byproducts (24.69%). The effective degradability (ED) of both dry matter (DM) and NDF for fruit-vegetable byproduct (DM, 84.69%; NDF, 85.62%) was higher (p<0.05) than cabbage (DM, 68.47%; NDF, 55.97%) and Chinese cabbage byproducts (DM, 68.09%; NDF, 54.22%). The DM intake was not different among treatments because the amount of feed was kept constant according to the BW of growing steers to prevent overweight during the growing period. The average daily gain during the whole experimental period was not different among treatments (1.26, 1.25, and 1.34 kg/d for control, FV-B, and CA-B). The ED of both DM and NDF degradability of the total mixed ration (TMR) diets were very similar among treatments. Feed conversion ratio during the whole period showed no significant difference among treatments. Conclusion: This study demonstrates that fruit-vegetable and cabbage byproducts up to 20% and 30% (as fed basis), respectively can be included in TMR diets for growing beef cattle.

인제지역 노인의 식품군 섭취 빈도에 따른 영양섭취량 조사 (Nutrient Intake Assessment of Korean Elderly Living in Inje Area, According to Food Group Intake Frequency)

  • The consumption of a wide variety of food groups is considered one of the key components of nutritional adequacy. The 2005 Dietary Guidelines for Koreans includes the consumption of a variety of foods from diverse food groups as a component of a normal diet. A survey was conducted to evaluate the relationship between the dietary diversity of food groups and nutrient intake in elderly patients (age 65 and above) at a rural area in Korea (Inje). 296 subjects (111 male and 185 female) were probed in a 3 day 24-recall dietary survey. Subjects were grouped according to food group intake frequency, based on six food groups (grain, meat/fish/legume/egg, vegetable, fat/oil, dairy, fruit). Nutritional quality was evaluated according to the numbers of nutrients under EAR (Estimated Average Requirements), and MAR (mean adequacy ratios). The frequency of elderly subjects consuming a meat/fish/legumes/egg food group less than once per day was 29.4%. The frequency of elderly subjects consuming fat/oil food group less than once per day was 65.8%. The percentage of subjects who did not eat dairy food was 88.8%, and that of subjects who did not eat fruit was 57.5%. A stepwise multiple regression analysis was used to develop models relating nutritional quality to possible food group intake frequency factors. Using the number of nutrients under EAR as a dependent variable, the meat/fish/legume/egg food group intake frequency explained 9.9% of variance, followed by the grain group, fat/oil group, dairy group, and vegetable and fruit group (Model $R^2$=0.260). For mean nutrient adequacy ratio as a dependent variable, the model $R^2$ was 0.326. The results of this study suggest that a highly varied diet in elderly might be associated with better nutritional quality, as assessed by nutrient intake. Accordingly, dietary guidelines should take into consideration nutritional characteristics in order to improve intake from all major food groups and to provide a variety of foods in the diet.

한국 청소년의 과일, 채소 섭취빈도와 정신건강: 제10-13차 (2014-2017) 청소년건강행태조사를 이용하여 (Fruit and vegetable consumption frequency and mental health in Korean adolescents: based on the 2014-2017 Korea Youth Risk Behavior Survey)

  • 본 연구는 2014-2017년도 청소년건강행태조사 원시자료를 이용하여 만 12-18세에 해당하는 남학생 137,101명, 여학생 130,806명을 대상으로 과일과 채소 섭취빈도와 주관적 행복상태, 스트레스 인지, 우울 증상 경험 및 자살 생각과의 관련성을 알아보고자 수행되었다. 연구 결과, 우리나라 청소년의 단 66%가 행복한 편이라고 답하였으며, 2.7명 중 1명은 과도한 스트레스, 4명 중 1명은 우울 증상, 8명 중 1명은 자살 생각을 경험하는 등의 정신건강 문제에 노출되어 있었다. 과일과 채소 섭취빈도는 성별, 나이, 가정경제 수준, 주거 형태, 주관적 학업 성취도, 비만도, 흡연 여부 및 음주 여부에 따라 유의적인 차이가 있었으며, 가당음료, 우유, 패스트푸드 섭취 및 아침 결식 여부와 같은 다른 식습관 요인에 의해서도 유의적인 차이가 있었다. 인구사회학적 특성과 생활습관 변수를 보정한 후 과일과 채소 섭취빈도에 따른 정신건강과의 관계를 살펴본 결과, 남학생과 여학생에서 모두 과일과 채소 섭취가 증가함에 따라 비섭취군 대비 주관적 행복상태에 대한 오즈비는 유의적으로 증가하고, 스트레스 인지, 우울 증상, 자살 생각에 대한 오즈비는 유의적으로 감소하였다. 이러한 관계는 다른 식생활 요인 변수들을 추가로 보정한 후에도 여전히 유의적으로 나타나, 과일과 채소 섭취빈도 증가가 청소년의 긍정적인 정신건강과 관련성이 있는 주요 요인임을 확인하였다. 이상의 연구 결과를 종합해 볼 때, 청소년기 건강한 정신건강을 위해 과일과 채소의 섭취가 중요한 것으로 생각되며, 청소년의 과일과 채소 섭취를 증가시킬 수 있는 효과적인 정책과 교육방안이 개발되어야 할 것으로 생각된다.

채소와 과일섭취 증진을 위한 영양교육이 행동전단계 초등학생의 사회심리적 요인의 변화에 미치는 효과 (The Effect of Nutrition Education on the Improvement of Psychosocial Factors Related to Vegetable and Fruit Intake of Elementary School Children in Pre-Action Stages)

  • 본 연구에서는 채소와 과일 섭취 증진을 위한 영양교육이 행동변화단계의 진전에 따라 사회심리적요인에 변화를 초래하는지 파악하고자 초등학생을 대상으로 영양교육을 실시한 후 행동전단계를 두 단계로 나누어 이 두 단계 아동들의 채소와 과일 섭취의 장점과 단점에 대한 인지, 인지적 행위적 변화과정, 자아효능감의 수준 변화를 조사하였다. 충남 연기군의 한 초등학교 5학년 아동 71명에 대한 설문 조사에 의해 행동전단계 (고려전, 고려, 준비)로 분류된 47명을 대상으로 2007년 11월 19일부터 12월 24일까지 6주 동안 채소와 과일섭취 영양교육을 실시하고 교육전과 후에 사회심리적요인의 수준을 설문지법에 의해 조사하였다. 1) 영양교육 후 채소섭취의 장점에 대한 인지도 점수는 고려전단계나 고려 및 준비단계에 속한 대상자 모두에서 증가하였고, 단점에 대한 인지도 점수는 두 단계 모두에서 감소하였다. 그러나 과일섭취에 있어서는 장점에 대한 인지도 점수에서는 고려전단계와 고려 및 준비단계 모두에서 교육 후 변화가 없었으나, 단점에 대한 인지도 점수는 고려전 단계에 속한 대상자에서만 유의적인 감소가 나타났다 (p < 0.01). 채소섭취에 대한 장점과 단점에 대한 인지 개선 영양교육은 두 단계 모두에서 효과적이었다. 2) 영양교육 후 채소섭취의 인지적 변화과정에서는 고려전단계의 대상자에서 의식향상 (p < 0.001)점수가 증가되었고, 고려 및 준비단계에 속한 대상자에서는 걱정해소 (p < 0.01), 자기재진단 (p < 0.05), 환경재평가 (p < 0.01), 사회규범변화(p < 0.05) 등 여러 변수의 점수가 증가하였다. 과일섭취의 인지적 변화과정에서는 고려전단계에 있는 대상자에서 환경재평가 (p < 0.01)점수의 증가를 나타내었고, 고려 및 준비단계에서는 변화를 보인 변수가 없었다. 채소와 과일섭취의 행위적 변화과정에서는 모두 변화가 없었다. 3) 영양교육 후 채소섭취에 관한 자아효능감은 고려전단계에 있는 대상자에서 '채소를 보이는 곳에 둔다' (p < 0.01)와 '혼자 채소를 먹을 때 권장섭취 횟수만큼 먹을 수 있다' (p < 0.01). '채소 먹을 시간을 낼 수 있다' (p < 0.05)에서 증가를 나타내었다. 고려 및 준비단계에서는 고려전단계에서 나타난 '혼자 채소를 먹을 때 권장섭취 횟수만큼 먹을 수 있다' (p < 0.01)와 '채소 먹을 시간을 낼 수 있다' (p < 0.01)의 항목을 포함하여 '다양한 종류의 채소를 살 수 있다' (p < 0.01)에서 교육 후에 자아효능감이 높아졌다. 즉 채소섭취의 행동변화단계가 높아질수록 자아효능감의 수준이 성숙됨을 보였다. 과일섭취에 대해서는 영양교육 후 고려전단계에 속한 대상자에서 '과일을 보이는 곳에 둔다'의 자아효능감의 점수가 증가되었다 (p < 0.05). 이상의 연구결과에서 영양교육 후 행동전단계에 있는 아동들의 행동변화단계의 진전이 일어났고, 고려전단계와 고려 및 준비단계에서 모두 사회심리적요인의 수준이 높아졌으나 두 단계 간에는 교육효과의 유의적인 차이가 없었다. 과일에 비해 채소섭취와 관련된 사회심리적요인에서 교육 효과가 더 크게 나타났으며, 사회심리적요인들 중에서 판단 균형의 단점에 대한 인지도, 인지적 변화과정, 자아효능감이 영양교육의 평가지표로 사용될 수 있는 가능성을 보였다.

Evaluation of a theory-based community intervention to increase fruit and vegetable intakes of women with limited incomes

  • The study objectives were to increase both the stage of readiness to eat fruit and vegetables as well as the intakes of women who participated in the Expanded Food and Nutrition Education Program (EFNEP) for families with limited incomes. The intervention was to enhance the currently used curriculum, Eating Right Is Basic III (ERIB3), with stage-specific processes based on the Trans-Theoretical Model of readiness to change. Trained EFNEP community workers taught the enhanced curriculum to 90 mothers in the experimental county and to 53 mothers in the control county. Pre- and post-intervention measures included stages of readiness to eat fruit and vegetables and to intake as assessed by 24-hour dietary recalls and staging questions. Most women recruited into EFNEP were in Action and Preparation Stages (53.5%). Fruit and vegetable intakes showed a linear trend along with the Stage of Change for fruit and vegetable. After intervention, some combination of the ERIB3 and the fruit and vegetables-enhanced ERIB3 resulted in a reported 1/2 servings/day increase in fruits and vegetables in both the control and the experimental counties, although changes were not significant. EFNEP women also moved along the Stage of Change Continuum for fruits and vegetables in both counties. The percentage of people who ate 5 or more servings of fruit and vegetables was significant, however, only in the experimental group. We encourage health professionals to apply lessons learned from this intervention and to continue to pursue theoretically based interventions to change dietary behaviors.
