Purpose : To evaluate the result of conservative management on recurrence, survival and prognostic factors of patient with operable breast cancer. Materials and Methods : Fifty three patients, treated for localized breast cancer by lumpectomy or quadrantectomy followed by radiotherapy between January 1985 and December 1996, were retrospectively studied. All patients followed up for at least five years. Their median age was 43 years $(range\;24\~72)$. The tumor stages were as follows : T1 in 30 patients, T2 in 21, Tis in 2. Thirty-eight patients had negative and 15 had positive axillary nodes. The histological types were 42 infiltrating ductal, 2 infiltrating lobular, and 2 intraductal carcinomas with 7 other histologies. The tumor locations were the outer quadrant in 38 breasts, the inner quadrant in 13 and central in 2. Radiation doses of $46\~50\;Gy$ were given to the entire breast areas with additional doses of 14-18 Gy delivered to the tumor bed areas. Results : The overall five and 10 yea actuarial and disease free survivals were $94.3\%\;and\;92.4\%,\;91.2\%\;and\;81\%$, respectively. The overall five year survivals were $100\%$ in stage I and IIa, and $66.7\%$ in stage IIb and IIIa tumors. Seven patients failed either locally or distantly. Incidence of local failure and distant metastasis for the first failure were $7.5\%\;and\;5.7\%$, respectively. Local recurrence appeared within 2 years of treatment at the primary site and after more than 8 years outside of primary lesion, whereas distant metastasis appeared between 2 and 6 years following treatment. The overall recurrences were high at a young age (< or = 35 years), with 5 out of 12 (2 local, 3 distant), and in T2 lesions with 5 out of 21 (1 local, 3 distant, and 1 in both). Distant metastasis was high in the positive axillary lymph node group with 4 out of 15 $(26.6\%)$. A high incidence in the axillary node was noted at a young age with 7 out of 12 $(58.3\%)$ and in T2 lesions with 8 out of 21 $(38.4\%)$. A young age, positive axillary node and large tumor size were all related with poor survival. Conclusion : Based on this study, lumpectomy or quadrantectomy, followed by radiation appears to be an adequate therapeutic method in operable breast cancer. A long term follow-up is necessary because a recurrence of breast can occur long time after treatment. The poor prognostic group, especially young patients with an aggressive biological behavior needs more effective treatment modalities to improve their survival.
Objectives We evaluated the improving effects of Taeksa-tang (TST) using 3T3-L1 cells and C57BL/6 mice were fed on a high-fat diet. Methods The anti-radical activities of TST were studied using 2,2-diphenyl-1-picrylhydrazyl and 2,2'-azino-bis (3-ethylbenzothiazoline-6-sulphonic acid). The content of total polyphenol was measured using Folin-Ciocalteu reagent, whereas aluminum chloride colorimetric method was used for the content of total flavonoid. Moreover, the factors related to lipid profile and the protein expressions such as 𝛽-oxidation and anti-oxidant enzyme were analyzed using serum and western blotting of 3T3-L1 cells. Additionally, we examined lipolysis through glycerol appearance in mouse adipose tissue. Results TST treatment showed strong free radical scavenging activities with half maximal inhibitory concentration and the presence of a amount of total polyphenol and total flavonoid. TST treatment significantly increased factors related to 𝛽-oxidation such as carnitine palmitoyl transferase-1 and uncoupling protein 2 via the phosphorlyation of liver kinase B1 (LKB1) and AMP-activated protein kinase (AMPK). Moreover, the protein expressions of anti-oxidant enzyme and lipolysis were significantly elevated by TST administration. In addition, TST supplementation lowered serum malondialdehyde, triglyceride, and total cholesterol levels compared with the control group. Taken together, these data suggest that TST treatment regulated lipid parameters via the increase of 𝛽-oxidation by LKB1-AMPK signaling pathway. Conclusions TST may have a potential remedy in the prevention and treatment of obesity. Therefore, this study may provide the scientific basis for TST use.

Background Three-dimensional (3D) model printing improves visualization of anatomical structures in space compared to two-dimensional (2D) data and creates an exact model of the surgical site that can be used for reference during surgery. There is limited evidence on the effects of using 3D models in microsurgical reconstruction on improving clinical outcomes. Methods A retrospective review of patients undergoing reconstructive breast microsurgery procedures from 2017 to 2019 who received computed tomography angiography (CTA) scans only or with 3D models for preoperative surgical planning were performed. Preoperative decision-making to undergo a deep inferior epigastric perforator (DIEP) versus muscle-sparing transverse rectus abdominis myocutaneous (MS-TRAM) flap, as well as whether the decision changed during flap harvest and postoperative complications were tracked based on the preoperative imaging used. In addition, we describe three example cases showing direct application of 3D mold as an accurate model to guide intraoperative dissection in complex microsurgical reconstruction. Results Fifty-eight abdominal-based breast free-flaps performed using conventional CTA were compared with a matched cohort of 58 breast free-flaps performed with 3D model print. There was no flap loss in either group. There was a significant reduction in flap harvest time with use of 3D model (CTA vs. 3D, 117.7±14.2 minutes vs. 109.8±11.6 minutes; P=0.001). In addition, there was no change in preoperative decision on type of flap harvested in all cases in 3D print group (0%), compared with 24.1% change in conventional CTA group. Conclusions Use of 3D print model improves accuracy of preoperative planning and reduces flap harvest time with similar postoperative complications in complex microsurgical reconstruction.
Purpose : To compare 12 and 32-element surface coil arrays for highly accelerated coronary magnetic resonance angiography (MRA) using parallel imaging. Materials and Methods : Steady state free precession coronary MRA was performed in 5 healthy volunteers at 1.5 T whole body MR scanner using both 12 and 32-element surface coil arrays. Left anterior descending and right coronary artery data sets were acquired for each volunteer. Data sets were sub-sampled for parallel imaging using reduction factors from 1 to 6. Mean geometry factor (g-factor), maximum g-factor, and artifact level were calculated for each of the two coil arrays. Results : Over all reduction factors, the mean and maximum g-factors and artifact level were significantly reduced using the 32-element array compared to the 12element array (P << 0.1). The mean g-factor was sensitive to the imaging orientations of coronary arteries while the maximum g-factor and artifact level were independent of orientation. Conclusion : The 32-element surface coil array significantly improves artifact and noise suppression for highly accelerated coronary MRA using parallel imaging. The increased acceleration factors made feasible with the 32-element array offer the potential to enhance spatial resolution or increase volumetric coverage for 3D coronary MRA.
These commensal intestinal bacteria can enhance the immune system and aid in nutrient absorption but can also act as opportunistic pathogens. Among these intestinal bacteria, the anaerobic Bacteroides fragilis are divided into enterotoxigenic B. fragilis (ETBF) which secrete the B. fragilis toxin (BFT) and non-enterotoxigenic B. fragilis (NTBF) which do not secrete BFT. ETBF can cause diarrhea and colitis in both humans and livestock but can also be found in asymptomatic individuals. ETBF is predominantly found in patients with inflammatory diarrheal diseases and traveller's diarrhea. Several clinical studies have also reported an increased prevalence of ETBF in human patients with inflammatory bowel disease (IBD), colitis and colorectal cancer. In small animal models (C57BL/6 wild-type mice, germ-free mice, multiple intestinal neoplasia (Min) mice, rabbits and Mongolian gerbils), ETBF have been found to initiate and/or aggravate IBD, colitis and colorectal cancer. BFT induces E-cadherin cleavage in intestinal epithelial cells resulting in loss of epithelial cell integrity. Subsequent activation of the ${\beta}$-catenin pathway leads to increased cellular proliferation. In addition, ETBF causes acute and chronic colitis in wild-type mice as well as enhances tumorigenesis in Min mice via activation of the Stat3/Th17 pathway. Currently, ETBF can be detected using a BFT toxin bioassay and by PCR. Advances in molecular biological techniques such as real-time PCR have allowed both researchers as well as clinicians to rapidly detect ETBF in clinical samples. The emergence of more sensitive techniques will likely advance molecular insight into the role of ETBF in colitis and cancer.
Background: Matrix metalloproteinase 9 (MMP-9) is related to tumor invasion and metastasis. However, the role of MMP-9 expression in breast cancer survival remains controversial. The purpose of this study was to accomplish a more accurate estimation of the association between MMP-9 expression and survival results in breast cancer patients through meta-analysis. Methods: A meta-analysis of published studies investigating the effects of positive MMP-9 expression on both relapse free survival (RFS) and overall survival (OS) was performed. Relevant literature was confirmed by searching electronic databases including PubMed, Ovid, EMBASE and China National Knowledge Infrastructure (CNKI) before November 1, 2012. Individual hazard ratios (HRs) and 95% confidence intervals (CIs) were extracted and pooled HRs with 95% CIs were used to evaluate the strength of the association between positive MMP-9 expression and survival results of breast cancer patients. Funnel plot and Egger's regression tests were used to evaluate publication bias. Heterogeneity and sensitivity analysis was also conducted. All the work was completed using STATA. Results: A total of 2,344 patients from 15 evaluative studies were finally included. Pooled HRs and 95% CIs suggested that MMP-9 overexpression had an unfavorable impact on both OS (HR: 1.70, 95% CI: 1.41-2.04) and RFS (HR: 1.54, 95% CI: 1.17-2.01) in breast cancer patients. There was no significant heterogeneity observed in the studies reported for OS (P=0.360, $I^2$=8.8%), but not RFS (P=0.002, $I^2$=67%). Publication bias was absent among the studies both in OS and RFS cases (t=-0.54, P=0.605 and t=1.71, P=0.131, respectively). Omission of any single study had little effect on the combined risk estimates on sensitivity analysis. Conclusion: The results of this meta-analysis suggest that positive MMP-9 expression confers a higher risk of relapse and a worse survival in patients with breast cancer. Larger prospective studies are now needed to evaluate the clinical utility of MMP-9 expression.

Coke strength was increased by adding ash-free coal (AFC) binder. In this study, the effect of the AFC binder on the physical and chemical properties of coke was experimentally investigated to understand the molecular mechanism for the improved coke strength. For reduced $CO_2$ emission in steelmaking industry, torrefied biomass fuel mixed with coal binder is also considered. The interface between the base coal and AFC was thus examined using Scanning Electron Microscope (SEM). The coke strength was commonly measured by performing the indirect tensile test and Nuclear Magnetic Resonance (NMR) spectroscopy in $^1H$ and $^{13}C$ modes. For comprehensive mechanism study of the enhanced coke strength thus obtained, ordinary coal for thermal power plant use was carbonized with AFC for subsequent SEM examination. The NMR spectroscopy results of coke samples positively revealed that the tensile strength was proportional to the average number of aromatic rings.

In this study, the cellular protective effect of resveratrol on oxidative damage and its antioxidative activity were investigated. The free radical-scavenging activity ($FSC_{50}$) of resveratrol was measured to be $103{\mu}M$. The reactive oxygen species-scavenging activity ($OSC_{50}$) of resveratrol on the ROS generated in a $Fe^{3+}-EDTA/H_2O_2$ system was investigated using the luminol-dependent chemiluminescence assay. Resveratrol displayed $0.042{\mu}M$ ROS scavenging activity, which is 9.6-fold higher than that of L-ascorbic acid ($0.405{\mu}M$) and had a more prominent cellular protective effect than (+)-${\alpha}$-tocopherol. When HaCaT cells were exposed to $800mJ/cm^2$ of UVB or treated with $30{\mu}M$ rose bengal, resveratrol protected the cells against oxidative stress in a concentration-dependent manner; however, it was unable to protect the cells when the damage was induced by 10 mM $H_2O_2$. These results indicate that resveratrol could be employed to improve and prevent the skin aging through its antioxidative and cellular protective activities.
The study was compared for ability to supply macro-nutrients between time and height of mowing to establish alternate technique of a chemical fertilizer with 9-year old 'Niitaka' pear (Pyrus pyrifolia Nakai) trees in a pesticide-free orchard in 2009. Rye and hairy vetch were sown together during fall 2008. Four types of mowing time treatments were included; mowing on April 18+May 28, on April 28+June 8, on May 8+June 18, and on April 18+May 18+June 18. Mowing height treatment included mowing at $3{\pm}1cm$, $7{\pm}1cm$, and $13{\pm}1cm$ on April 18 or June 8. Mowing on May 8+June 18 increased the most dry weight of ground cover and vegetation to 14,480 kg per ha among the mowing time treatments, resulting in the greatest production of T-N (total nitrogen; 292 kg) and K (396 kg) per ha on the soil. Mowing at $7{\pm}1cm$ decreased the most dry weight of ground cover and vegetation to 11,970 kg per ha among the mowing height treatments, resulting in the lowest production of T-N (265 kg), P (111 kg) and K (333 kg) per ha in a soil. Ground cover treatments increased soil organic matter contents, regardless of mowing treatment methods.
Objectives Total intracranial volume (TIV) is a major nuisance of neuroimaging research for interindividual differences of brain structure and function. Authors intended to prove the reliability of the atlas scaling factor (ASF) method for TIV estimation in FreeSurfer by comparing it with the results of manual tracing as reference method. Methods The TIVs of 26 normal children and 26 children with attention-deficit hyperactivity disorder (ADHD) were obtained by using FreeSurfer reconstruction and manual tracing with T1-weighted images. Manual tracing performed in every 10th slice of MRI dataset from midline of sagittal plane by one researcher who was blinded from clinical data. Another reseacher performed manual tracing independently for randomly selected 20 dataset to verify interrater reliability. Results The interrater reliability was excellent (intraclass coefficient = 0.91, p < 7.1e-07). There were no significant differences of age and gender distribution between normal and ADHD groups. No significant differences were found between TIVs from ASF method and manual tracing. Strong correlation between TIVs from 2 different methods were shown (r = 0.90, p < 2.2e-16). Conclusions The ASF method for TIV estimation by using FreeSurfer showed good agreement with the reference method. We can use the TIV from ASF method for correction in analysis of structural and functional neuroimaging studies with not only elderly subjects but also children, even with ADHD.
