Clinical outcomes and prognostic factors in patients with mycosis fungoides who underwent radiation therapy in a single institution

  • Purpose: We aimed to evaluate clinical outcomes including progression-free survival (PFS), overall survival (OS), partial response, and complete response in patients who underwent radiation therapy (RT) for mycosis fungoides (MF). Also, we sought to find prognostic factors for clinical outcomes. Materials and Methods: Total 19 patients confirmed with MF between 1999-2015 were retrospectively reviewed. Clinical and treatment characteristics, clinical outcomes, and and toxicities were analyzed. Results: Eleven patients were treated with total skin electron beam radiotherapy (TSEBT) and 8 patients with involved field radiation therapy (IFRT) with median dose of 30 Gy, respectively. The median time interval from diagnosis to RT was 2.6 months (range, 0.4 to 87.3 months). The overall response rate was 100%; 11 patients (57.9%) had a complete response and 8 patients (42.1%) a partial response. The presence of positive lymph node at the time of consultation of RT was associated with lower OS (p = 0.043). In multivariate analysis, PFS was significantly lower for patients with increased previous therapies experienced following RT (p = 0.019) and for patients showing PR during RT (p = 0.044). There were no reported grade 3 or more skin toxicities related with RT. Conclusion: Both IFRT and TSEBT are effective treatment for MF patients. Patients with short disease course before RT or complete response during RT are expected to have longer PFS. Positive lymph node status at the initiation of RT was associated woth poor OS, suggesting other treatment modalities such as low-dose RT for patients with low life-expectancy.

  • A new blade section of propeller is developed to obtain higher propeller efficiency and better cavitation performance. Eleven foil sections are carefully designed and manufactured to compare the lift-drag characteristics and cavitation performances. It is expected that the developed section behaves better in the vicinity of the ship's wake, where the angle of attack variation is large, because of its wider width in lift-drag and cavitation-free bucket diagrams. A propeller design method using the selected foil section is presented. Three chordwise loading shapes are selected to investigate the influence of the lift-drag ratios on the propeller efficiencies and cavitation performances. Three propellers are designed, which correspond to the selected chordwise loading shapes. Two more propellers which use existing foil sections are designed to compare the section performance.

Effects of Hold-Relax and Active Range of Motion on Thoracic Spine Mobility

  • Few studies address the use of manual muscle stretching to improve spinal active range of motion(AROM). There is evidence that' Hold-Relax'(HR) is effective for increasing ROM in the extremities, which leads the researchers to anticipate similar benefits in the spine. The purpose of this study is to investigate the effects of HR(trunk flexors) and active thoracic flexion and extension on thoracic mobility, specifically flexion and extension in healthy individuals. A convenience sample of 30 physical therapy students(22-38 years) were randomly assigned to intervention sequence 'A-B' or 'B-A', with at least 7 days between interventions. Intervention' A' consisted of HR of the ventral trunk musculature while 'B' consisted of thoracic flexion-extension AROM. Thoracic flexion and extension AROM were measured before and after each intervention using the double inclinometer method. Paired t-tests were used to compare AROM pre and post-intervention for both groups, and to test for carry-over and learning effects. There was a statistically significant increase(mean=$3^{\circ}$ ; p=0.006) in thoracic extension following HR of the trunk flexors. There were no significant changes in thoracic flexion following HR, or in flexion or extension following the AROM intervention. No carryover or learning effects were identified. HR may be an effective tool for improving AROM in the thoracic spine in pain free individuals. Further investigation is warranted with symptomatic populations and to define the minimal clinical difference(MCD) for thoracic spine mobility.

Electroanalytical Applications Based on Carbon Nanotube/Prussian Blue Screen-printable Composite

  • A single step fabrication process of carbon nanotube/Prussian Blue (CNT/PB) paste electrodes based on screen printing technology has been studied as an amperometric sensor for the determination of hydrogen peroxide and free chlorine. Compared to the classical carbon paste (CP) electrode, the CNT paste electrode greatly enhanced the response in the presence of hydrogen peroxide due to the electrocatalytic activity of the CNT. Based on the CNT/binder paste, PB was also incorporated into a network of CNT paste and characterized. The best electroanalytical properties of PB-mixed sensors to hydrogen peroxide were obtained with PB ratio of 10 wt % composition, which showed fast response time ($t_{90}{\leq}5$ s; 0.2 - 0.3 mM), low detection limit of 1.0 ${\mu}M$, good linear response in the range from $5.0{\times}10^{-5}$ - $1.0{\times}10^{-3}$ mol $L^{-1}$ ($r^2$ = 0.9998), and high sensitivity of -8.21 ${\mu}AmM^{-1}$. In order to confirm the enhanced electrochemical properties of CNT/PB electrode, the sensor was further applied for the determination of chlorine in water, which exhibited a linear response behavior in the range of 50 - 2000 ppb for chlorine with a slope of 1.10 ${\mu}Appm^{-1}$ ($r^2$ = 9971).

A Study on VoiceXML Application of User-Controlled Form Dialog System (사용자 주도 폼 다이얼로그 시스템의 VoiceXML 어플리케이션에 관한 연구)

  • VoiceXML is new markup language which is designed for web resource navigation via voice based on XML. An application using VoiceXML is classified into mutual-controlled and machine-controlled form dialog structure. Such dialog structures can't construct service which provide free navigation of web resource by user because a scenario is decided by application developer. In this paper, we propose VoiceXML application structure using user-controlled form dialog system which decide service scenario according to user's intention. The proposed application automatically detects recognition candidates from requested information by user, and then system uses recognition candidate as voice-anchor. Also, system connects each voice-anchor with new voice-node. An example of proposed system, we implement news service with IT term dictionary, and we confirm detection and registration of voice-anchor and make an estimate of hit rate about measurement of an successive offer from information according to user's intention and response speed. As the experiment result, we confirmed possibility which is more freely navigation of web resource than existing VoiceXML form dialog systems.

An Authobiographical Narrative Interview Study on Life-Driveing Forces of A, a Female Farmer from Chonbuk Rural Area (전북농촌 여성노인 A의 생애구술에서 드러난 삶의 원동력)

  • This autobiographical narrative interview study aims at exploring how A, one Korean 82-year-old female farmer strived vigorously to learn by herself and to teach her children (4 boys and 3 girls) despite the fact that she was not afford to pay tuitions on time. From 40 times of interview-data three major findings emerged: (1) A learned how to read Korean Japanese and Chinese characters and how to calculate at a free-of-charge teaching center although her father didn't approve of her learning; (2) A tried very hard to earn money inside and outside home to support her children's education, organizing many mutual fraternity meetings to seek mutual financial support, selling mostly farm products as well as farming almost all day and everyday; (3) Although it was so hard to educate three daughters, A was proud of the fact that she was able to put her second daughter to a high school with a promise to pay her tuition later. Some implications of the findings are added.

Update on Current Role of Perioperative Chemotherapy in Upper Tract Urothelial Carcinoma (상부 요로상피암에서 신보조 항암요법 및 보조 항암요법의 최신 지견)

  • Upper tract urothelial carcinoma (UTUC) has a relatively low prevalence rate of about 1.8 per 100,000 people. According to the recent literature, the development of diagnostic techniques has gradually increased the prevalence and diagnosis rate. In the past, when UTUC was diagnosed, more than 60% of the patients were diagnosed as locally advanced or metastatic cancer. However, since 2010, approximately 70% of the patients have been diagnosed as operable stage. Although radical nephroureterectomy is known as the basis of treatment for UTUC, overall survival is poor in patients with lymph node invasion. Especially, the finding that a localized UTUC is associated with a high risk of cancer metastasis in approximately 50% of patients suggests that these patients may not have sufficient treatment through surgery alone. The European Association of Urology and the National Comprehensive Cancer Network guideline 2017 suggested that postoperative adjuvant chemotherapy may be considered in patients with advanced UTUC beyond pT2. Also, recent meta-analyses have reported that cisplatin-based adjuvant chemotherapy can be expected to have a synergistic effect of overall survival and disease-free survival. However, many patients with UTUC undergo postoperative renal failure, which may result in failure to perform cisplatin-based adjuvant chemotherapy with adequate dose. For this reason, several researchers have suggested that it is beneficial to apply neoadjuvant chemotherapy when the preoperative renal function is maintained to a certain extent. But, neoadjuvant chemotherapy has not been used by many clinicians because of the lack of studies and the rarity of the disease. We are currently discussing the outcomes and prospects of perioperative chemotherapy.

Research trend in Fabrication of Metastable-phase Iron Nitrides for Hard Magnetic Applications (준안정상 기반의 질화철계 영구자석소재 제조연구동향)

  • Rare earth magnets are the strongest type of permanent magnets and are integral to the high tech industry, particularly in clean energies, such as electric vehicle motors and wind turbine generators. However, the cost of rare earth materials and the imbalance in supply and demand still remain big problems to solve for permanent magnet related industries. Thus, a magnet with abundant elements and moderate magnetic performance is required to replace rare-earth magnets. Recently, $a^{{\prime}{\prime}}-Fe_{16}N_2$ has attracted considerable attention as a promising candidate for next-generation non-rare-earth permanent magnets due to its gigantic magnetization (3.23 T). Also, metastable $a^{{\prime}{\prime}}-Fe_{16}N_2$ exhibits high tetragonality (c/a = 1.1) by interstitial introduction of N atoms, leading to a high magnetocrystalline anisotropy constant ($K_1=1.0MJ/m^3$). In addition, Fe has a large amount of reserves on the Earth compared to other magnetic materials, leading to low cost of raw materials and manufacturing for industrial production. In this paper, we review the synthetic methods of metastable $a^{{\prime}{\prime}}-Fe_{16}N_2$ with film, powder and bulk form and discuss the approaches to enhance magnetocrystalline anisotropy of $a^{{\prime}{\prime}}-Fe_{16}N_2$. Future research prospects are also offered with patent trends observed thus far.

Comparison of physicochemical traits of dry-cured ham from purebred Berkshire and crossbred Landrace × Yorkshire × Duroc (LYD) pigs

  • This study was conducted to compare the physicochemical traits of dry-cured hams made from two different pig breeds: Berkshire and $Landrace{\times}Yorkshire{\times}Duroc$ (LYD). Pigs were slaughtered at a live weight of approximately 110 kg and cooled at $0^{\circ}C$ for 24 h in a chilling room. Then, the ham portion of the carcasses were cut and processed by dry-curing for physicochemical analyses. The dry-cured hams from Berkshire contain higher crude protein, fat, and ash level than those from LYD, whereas the hams from LYD had higher moisture contents than those from Berkshire(p < 0.05). The pH values of the hams from Berkshire were lower than those from LYD (p < 0.05). The hams from Berkshire had lower $L^*$ and $b^*$ values than those from LYD (p < 0.05). Palmitoleic acid (C16:1), oleic acid (C18:1), elaidic acid (C18:1t), monounsaturated fatty acids, and ratio of n-6 and n-3 fatty acids (n-6/n-3) in the ham from Berkshire were higher than LYD (p < 0.05). Free amino acids such as aspartic acid, threonine, serine, asparagine, glutamic acid, and lysine in hams from Berkshire were higher than those from LYD (p < 0.05). The microbial population had no significant difference between Berkshire and LYD dry-cured ham. The cross sections of dry cured ham showed difference from different breeds using scanning electron microscope and indicates some differences in texture. Considering the meat quality parameters of ham, hams from Berkshire could provide variety of ham for consumer who are seeking various different qualities and stories.

  • In the 4th industrial revolution, Artificial Intelligence (AI), big data, Internet of Things (IoT) are already around us, making our society hyper-connected and blurring the lines between the digital and biological spheres. We witness drastic changes not only in the food industry, but also in economy, society and our life as a whole. Technologies bring industrial reorganization and greater changes at the system level and the food industry is not exceptional. Human demand for foods continues to grow and the very nature of the food industry remains unchanged, but its production, distribution and marketing face unprecedent innovations. Passing through the global pandemic, the food industry has been evolved into 'contact-free', as the safety become our top priority. Amid the gradual shift to technology-oriented society, the smarter food safety management skills and tools are being adopted in many countries exerting greater efforts to enhance traceability and to upgrade AI-powered safety management system.
