Pretreatment Neutrophil-to-Lymphocyte Ratio and Smoking History as Prognostic Factors in Advanced Non-Small Cell Lung Cancer Patients Treated with Osimertinib

  • Background: The remarkable efficacy of osimertinib in non-small cell lung cancer (NSCLC) with acquired T790M mutation has been widely documented in clinical trials and real-world practice. However, some patients show primary resistance to this drug. Even patients who initially show a favorable response have inconsistent clinical outcomes later. Therefore, the aim of this study was to identify additional clinical predictive factors for osimertinib efficacy. Methods: A prospective cohort of patients with acquired T790M positive stage IV lung adenocarcinoma treated with osimertinib salvage therapy in Hallym University Medical Center were analyzed. Results: Sixty-one eligible patients were analyzed, including 38 (62%) women and 39 (64%) who never smoked. Their mean age was 63.3 years. The median follow-up after treatment with epidermal growth factor receptor (EGFR) tyrosine kinase inhibitors (TKIs) was 36.0 months (interquartile range, 24.7-50.2 months). The majority (n=45, 74%) of patients were deceased. Based on univariate analysis, low baseline neutrophil-to-lymphocyte ratios (NLR), age ≥50 years, never-smoking history, stage IVA at osimertinib initiation, and prolonged response to previous TKIs (≥10 months) were associated with a significantly longer progression-free survival (PFS). Multivariate analysis showed that never-smoking status (hazard ratio [HR], 0.54; 95% confidence interval [CI], 0.30-0.98; p=0.041) and a baseline NLR less than or equal to 3.5 (HR, 0.23; 95% CI, 0.12-0.45; p<0.001) were independently associated with a prolonged PFS with osimertinib. Conclusion: Smoking history and high NLR were independent negative predictors of osimertinib PFS in patients with advanced NSCLC developing EGFR T790M resistance after the initial EGFR-TKI treatment.

자력 선별 전로슬래그 미분말을 결합재로 활용한 모르타르의 기초특성 (Fundamental Properties of Mortar with Magnet-Separated Converter-Slag Powder as SCM)

  • 전로슬래그는 용광로에서 생산된 선철을 용융강으로 정제하는 과정에서 배출되는 부산물로, 컨버터에서 생산되는 양의 약 15 %가 배출된다. 이 전로슬래그는 유리 CaO 함량이 높아 콘크리트 골재로 사용 시 팽창균열 가능성이 있어 재활용이 어렵다. 이를 해결하기 위해 전로슬래그를 야드에서 숙성시키지만 완벽하게 제어하기는 어렵다. 본 연구에서는 전로슬래그에 모르타르 제제를 혼합하여 연구를 수행함으로써 골재가 아닌 시멘트 대체재로 전로슬래그를 사용할 수 있는 가능성을 검토하였다. 실험에 사용된 전로슬래그의 EDS 결과, 분말상 대비 골재상일 때 자력선별을 진행하는 것이 T.Fe를 제거하는데 더 효과적인 것을 확인하였다. 분쇄효율 실험은 T.Fe가 많이 분류된 비자착분이 가장 높은 분말도를 나타내었다. 이런 고분말도로 인해 전로슬래그를 15 % 치환했을 때 Plain 대비 비자착분의 플로가 가장 많이 감소하였다. 전로슬래그를 혼입한 모르타르의 길이변화는 재령이 증가할수록 변화율이 커지지만 Plain 대비 유사한 변화율이 나타났다. 압축강도에서는 Plain 대비 비자착분은 치환율 15 %까지 91.9~97.1 %의 강도 발현을 나타내었다. 추후 전로슬래그 미분말을 시멘트 대체재로 활용할 경우, 분쇄 전 자력(Gauss) 조절을 통한 최적의 자력선별 조건을 찾아 더 효율적인 T.Fe 선별 방법이 필요할 것으로 판단되며, EDS 및 Mapping은 국부적인 분석방법으로 큰 편차율을 보이기 때문에 다수의 시료 평가가 필요할 것으로 판단된다.

Prognostic Value of 18F-FDG PET/CT Radiomics in Extranodal Nasal-Type NK/T Cell Lymphoma

  • Objective: To investigate the prognostic utility of radiomics features extracted from 18F-fluorodeoxyglucose (FDG) PET/CT combined with clinical factors and metabolic parameters in predicting progression-free survival (PFS) and overall survival (OS) in individuals diagnosed with extranodal nasal-type NK/T cell lymphoma (ENKTCL). Materials and Methods: A total of 126 adults with ENKTCL who underwent 18F-FDG PET/CT examination before treatment were retrospectively included and randomly divided into training (n = 88) and validation cohorts (n = 38) at a ratio of 7:3. Least absolute shrinkage and selection operation Cox regression analysis was used to select the best radiomics features and calculate each patient's radiomics scores (RadPFS and RadOS). Kaplan-Meier curve and Log-rank test were used to compare survival between patient groups risk-stratified by the radiomics scores. Various models to predict PFS and OS were constructed, including clinical, metabolic, clinical + metabolic, and clinical + metabolic + radiomics models. The discriminative ability of each model was evaluated using Harrell's C index. The performance of each model in predicting PFS and OS for 1-, 3-, and 5-years was evaluated using the time-dependent receiver operating characteristic (ROC) curve. Results: Kaplan-Meier curve analysis demonstrated that the radiomics scores effectively identified high- and low-risk patients (all P < 0.05). Multivariable Cox analysis showed that the Ann Arbor stage, maximum standardized uptake value (SUVmax), and RadPFS were independent risk factors associated with PFS. Further, β2-microglobulin, Eastern Cooperative Oncology Group performance status score, SUVmax, and RadOS were independent risk factors for OS. The clinical + metabolic + radiomics model exhibited the greatest discriminative ability for both PFS (Harrell's C-index: 0.805 in the validation cohort) and OS (Harrell's C-index: 0.833 in the validation cohort). The time-dependent ROC analysis indicated that the clinical + metabolic + radiomics model had the best predictive performance. Conclusion: The PET/CT-based clinical + metabolic + radiomics model can enhance prognostication among patients with ENKTCL and may be a non-invasive and efficient risk stratification tool for clinical practice.

Japanese Chin 종에서 발생한 Ivermectin에 반응을 보이지 않은 전신성 농성 모낭충 감염증에 대한 0.1% Amitraz와 4% Chlorhexidine의 국소치료 일례 (Efficacy of Weekly 0.1% Amitraz Dip with 4% Chlorhexidine Shampoo on Juvenile Onset Generalized Pyodemodicosis Unresponsive to Ivermectin Therapy in Japanese Chin Dog)

  • 중성화 수술을 받지 않은 3.1 kg의 1년령 암컷 일본 찡견이 자견발생모낭충증의 전형적인 증상으로 경북대학교 수의과대학 부속동물병원에 내원하였다. 일반적인 신체검사, 피부박리검사, 혈액화학 검사, 내분비검사와 세균배양검사 등을 실시하였다. 내원 당시 피부 병변을 제외한 환축의 상태는 양호하였으며, 혈액화학치, $T_3$, $T_4$, $T_4$는 모두 정상이었다. 그러나, 피부박리검사에서 다수의 모낭충이 관찰되었고, 농포를 이용한 세균배양 결과 구균이 검출되어 속발성 표재성농피증온 동반한 자견발생모낭충증으로 확진하였다. 치료를 위하여 13주간 매일 1회 600 $\mu\textrm{g}$/kg의 ivermectin을 매일 2회 25mg/kg의 cephalexin과 함께 투여하였으나 임상증강이 호전되지 않았으므로 일주일 간격으로 30 mg/kg의 selamectin을 cephalexin과 한께 국소 누여하는 방법을 실험적으로 2주간 실시하였다. 그러나, 임상증상이 오히려 더 악화되어 주 1회 0.1% amitraz dip을 주 2회 4% chlorhexidine 샴푸와 함께 실시하였다. 그 결과, 환축은 급속도로 호전되어 속발성 농피증에 의한 피부병변과 이에 동반된 소양증은 치료 실시 후 2주 경에 거의 사라졌으며 9주 경에는 피부박리검사에서 모낭충이 검출되지 않았을 뿐만 아니라 환축의 피모도 거의 정상으로 회복되었다. 재발을 막기 위하여 피부박리검사에서 음성 격과를 얻은 후에도 4주간 더 치료하였다. 현재 환축은 임상적으로 정상이며, 모낭충증의 재발여부를 관찰 중에 있다.

요오드화 조영제가 MR영상에 미치는 신호 변화 (Signal Change of Iodinated Contrast Agents in MR Imaging)

  • 본 연구에서는 CT에서 사용되는 요오드화 조영제가 가돌리늄조영제와 비교하여 MR영상신호에 어떠한 영향을 미치는지 알아보고자 하였으며, 이에 따른 CT조영증강 검사 이후 MRI검사를 시행하는 프로토콜이 적정한지에 대하여 논하고자 하였다. 실험은 iodine과 gadolinium의 두 개의 팬텀을 제작하여 MRI에서의 통상적인 T1, T2, T2 FLAIR, 3D Angio 검사를 시행 후, 이에 대한 정량적 분석이 이루어졌다. 실험결과 체내 자유수(Free water)와 유사한 셀라인의 신호강도 SSI(Saline's Signal Intensity)는 iodine팬텀에서 각 175, 1231, 333, 37 [a.u]을 보였고, gadolinium팬텀에서 101, 1021, 321, 31 [a.u]을 기록하였다. 셀라인의 SI(Signal Intensity)를 기준으로 가장 큰 차이의 신호강도 BDEPS(the Biggest Difference of EPS)는 iodine팬텀에서 각 1297, 123, 757, 232 [a.u]를 보였고, gadolinium팬텀에서 793, 6, 1495, 365 [a.u]를 기록하였다. 이때 셀라인과 비교한 신호증강정도 EPS(Enhancement Percentage to Saline)는 iodine팬텀에서 641.1 -90.0, 127.3, 527%를 보였고, gadolinium팬텀에서 685.1, 99.4, 365.7, 1077.4% 기록하였다. BDEPS를 보이는 지점인 BP(BDEPS's point)는 iodine팬텀에서 900, 900, 477, 900 mmol을 보였고, gadolinium팬텀에서 4, 0.2, 0.2, 40 mmol을 기록하였다. 셀라인과 비교 하여 육안으로 SI변화를 확인할 수 있는 지점 CPSS(Change Point of SI to SSI)는 iodine팬텀에서 63, 423, 63, 29 mmol을 보였고, gadolinium팬텀에서 각 [50, 30], [4, 0.2], [4, 1], 0.2 mmol을 기록하였다. 본 연구를 통하여 iodine 역시 MR신호에 영향을 끼치며, 이는 gadolinium과는 다른 패턴을 보이는 것을 확인하였다. 이에 따라 임상현장에서 본 연구의 정량화 데이터를 감안하여 CT와 MRI 검사 순서 프로토콜을 결정한다면 유용한 진단학적 MR영상을 구현 할 수 있을 것으로 사료된다.

측두하악장애에서의 적외선 체열 촬영 검사의 유용성 (Infrared Thermography in the Assessment of Temporomandibular Joint Dysorder)

  • Background: Temporomandibular joint disorder (TMD) is a group of musculoskeletal conditions characterized by pain in the pre-auricular area, limitation of jaw movement and palpable muscle tenderness. Thermography is a nonionizing, noninvasive diagnostic alternative for the evaluation of TMD. This study was conducted to evaluate the usefulness of thermography in the assessment of TMD. Methods: Thermography was conducted on the 61 patients who had been diagnosed with TMD, and on the 34 normal symptom-free volunteers. The temperature differences between opposite sides of the temporomandibular joint (${\Delta}T_{TMJ}$) and the masseter muscle (${\Delta}T_{MST}$) were calculated. The sensitivity and specificity of thermography was calculated at the cut off values of 0.2, 0.3, and $0.4^{\circ}C$. Results: In the patient group, the ${\Delta}T_{TMJ}$ was $0.42{\pm}0.38^{\circ}C$ and the ${\Delta}T_{MST}$ was $0.38{\pm}0.33^{\circ}C$, whereas in the control group the ${\Delta}T_{TMJ}$ was $0.10{\pm}0.07^{\circ}C$ and the ${\Delta}T_{MST}\;0.15{\pm}0.10^{\circ}C$. In addition, the patient group demonstrated a significantly lower level of thermal symmetry than the control group (P < 0.001) in both the temporomandibular joints and the masseter muscles. The sensitivity of thermography at the cut off values of 0.2, 0.3 and $0.4^{\circ}C$ was 67.2, 49.2, and 42.6% in the temporomandibular joint (TMJ) and 60.7, 49.2 and 37.7% in the masseter muscle, respectively. The specificity of thermography at the cut off values of 0.2, 0.3 and $0.4^{\circ}C$ was 88.2, 100, and 100% in the TMJ and 61.8, 91.2 and 100% in the masseter muscles, respectively. The accuracy of thermography at the cut off values of 0.2, 0.3 and $0.4^{\circ}C$ was 74.7, 67.4, and 63.2% in TMJ and 61.1, 64.2 and 60.0% in the masseter muscles, respectively. Conclusions: Temperature differences exist between the opposite sides of the TMD and masseter muscles in patients with TMD. Although the sensitivity of thermography in the diagnosis of TMD is low, it has high specificity in the evaluation of TMD, and is therefore applicable to patients with TMD.

대학생들의 패스트푸드 이용 실태와 기호도에 관한 연구 -의정부 지역 대학생 중심으로- (A Study on the actual conditions of college students′ fast-food use and their likings -Centering around the students in the Uijungbu region-)

  • The demographic characteristics of research examinees showed that male students were 122(44%) and female ones 155(56%), and teenagers 142 per age. The average monthly pocket money appeared that 96 students received more than 200,000(34.7%) and it was analyzed that 243(87.7%) resided in their homes. The research of fast-food choice factors resulted in the fact that female students who were easily influenced by their friends were 83, while males 57. And, mascom took influence on 40 female students and 20 male ones, besides other included 39 male and 30 female students. They most preferred hamburger and pizza(94) among the fast-food menus, and taste like seasame was most preferred among the characteristic factors of foods taste. And as a result of factor analysis of menu, Cronbach alpha coefficient Fc 2 of reliability appeared highest as 0.7694, next, Fc 3 as 0.6965, and Fc 1 lowest as 0.6403. The reliability among the specific items of this study may be regarded high. And the difference between groups according to gender appeared that Fc 1, Fc 2, and Fc 3 were significantly different at 5% level as a result of T-examination analysis and the distributed analysis of the average monthly pocket money resulted in the fact that the difference between groups appeared significantly at F3 below significant level of 0.05. The testing of the difference of reason why the fast-food is preferred according to frequency of use per week lead to Chi-square value of 41.399 with free degree of 12, so significant degree became 0.000. The zero hypothesis that the two variables are not independent each other is not reserved because it has a small value of signigicant degree below 0.05, and the permanent hypothesis is adopted.

경남일부 청소년의 인터넷 식생활 정보이용에 관한 연구 (Adolescent' Internet Utilization Status of Dietary Information in Kyungnam)

  • This study was carried out to investigate the utilization status of internet dietary information by gender(boys: 363, girls: 366) in adolescent(middle & high school students). The results were summarized as follows. The internet using frequency of 6-7times per week had 45.0% of subjects and the using time of internet per a time was shown mainly'<2hours(68.5%)'. The main place for internet use was home(79.0%) and favorite search engine was 'Yahoo'(45.7%) and 'Daum'(19.3%). As main purpose using internet were mentioned 'social intercourse'(45.0%) and data search'(24.8%). The organization that offer to reliable internet information was educational institution'(49.4%). The problems in using information site were 'poor information'(26.4%), 'slow connection speed'(22.6%), and 'don't arouse interest'(18.8%). The search experience about dietary information had only 27.9% of subjects and search purpose was 'for homework'(33.3%) and 'for health'(32.0). The satisfaction degree of dietary information was not high. The connection motive to dietary information was mainly 'by site navigation casualty'(55.7%). Only 7.7% of subjects had experience of nutrition counseling using internet, and the motive of nutrition counseling was also 'by site navigation causally'(55.8%). The purpose of counseling was 'for diet'(41.5%) and 'for health problem'(30.2%), and the satisfaction degree of counseling result was very low. As the ask of improvement for counseling site were pointed out 'poor in answer content'(44.8%) and 'lazy answer'(31.0%). The subjects wanted to get the dietary information about 'growth in status'(41.4%), 'diet related skin beauty'(14.6%), the update period less 1 month, and the way of 'free board'(32.3%), 'game'(21.1%) and 'animation'(19.3%) as offer tool. The results of this study showed that although the internet using percent and frequency of subjects was high, they used dietary information very seldom and they are dissatisfied with internet nutritional information. Therefore, the information donor should consider which dietary information was needed and what is the optimal tool for adolescent.

Optimization of Multi-Atlas Segmentation with Joint Label Fusion Algorithm for Automatic Segmentation in Prostate MR Imaging

  • Purpose: Joint label fusion (JLF) is a popular multi-atlas-based segmentation algorithm, which compensates for dependent errors that may exist between atlases. However, in order to get good segmentation results, it is very important to set the several free parameters of the algorithm to optimal values. In this study, we first investigate the feasibility of a JLF algorithm for prostate segmentation in MR images, and then suggest the optimal set of parameters for the automatic prostate segmentation by validating the results of each parameter combination. Materials and Methods: We acquired T2-weighted prostate MR images from 20 normal heathy volunteers and did a series of cross validations for every set of parameters of JLF. In each case, the atlases were rigidly registered for the target image. Then, we calculated their voting weights for label fusion from each combination of JLF's parameters (rpxy, rpz, rsxy, rsz, β). We evaluated the segmentation performances by five validation metrics of the Prostate MR Image Segmentation challenge. Results: As the number of voxels participating in the voting weight calculation and the number of referenced atlases is increased, the overall segmentation performance is gradually improved. The JLF algorithm showed the best results for dice similarity coefficient, 0.8495 ± 0.0392; relative volume difference, 15.2353 ± 17.2350; absolute relative volume difference, 18.8710 ± 13.1546; 95% Hausdorff distance, 7.2366 ± 1.8502; and average boundary distance, 2.2107 ± 0.4972; in parameters of rpxy = 10, rpz = 1, rsxy = 3, rsz = 1, and β = 3. Conclusion: The evaluated results showed the feasibility of the JLF algorithm for automatic segmentation of prostate MRI. This empirical analysis of segmentation results by label fusion allows for the appropriate setting of parameters.

하늘타리(Trichosantes kirilowii Maximowicz)추출물의 항산화 효과 (Antioxidantive Effectiveness of Trichosanthes kirilowii Maximowicz Extracts)

  • 최근 피부친화성, 흡수력, 나노 입자의 제조 등에 우수한 특성을 지닌 불포화 레시친에 대한 관심이 높아지고 있다. 이에 따라 불포화레시친의 산화안정성을 높이고, 변질되기 쉽고 독성을 지닌 합성화합물을 대체하는 천연물을 개발하고자 하였다. 결과, 하늘타리(T. kirilowii Maximowicz)의 열매와 뿌리의 성분분석을 한 결과 항산화 성분으로는 갈릭산, 카페인산, 3,5-디하이드록시벤조익산, 플라보논이 검출되었다. 총 폴리페놀 함량은 열매추출물(53.5 mg/g)보다 뿌리추출물(133.85mg/g)에서 높게 나타났고 프리라디칼 제거 및 지질산패 억제효과 또한 뿌리 추출물이 우수하였으며 뿌리 추출물은 폴리페놀 함량이 100 ppm 이상일 때 대조군으로 사용한 BHT보다 약 20.1%, 19.2% 높게 나타났다. 또한 하늘타리 추출물은 1250ppm 이하에서 95% 이상의 높은 세포 생존율을 나타내어 안전성이 우수하였다. 리포좀의 안정성에 대한 효과는 열매보다 뿌리추출물이 우수하였으며 뿌리추출물은 대조군인 BHT에 비해 항산화 효과 15.1%, 제타전위 13.9% 높았으며, 입자크기는 9.3% 낮아 기존의 합성 원료보다 우수하였다
