BACKGROUND/OBJECTIVES: Recent studies have reported an association of the angiotensin II type 2 receptor (AT2R) 3123Cytosine/Adenine (3123C/A) polymorphism with essential hypertension and cardiovascular diseases. The purpose of the study was to investigate whether the AT2R 3123C/A polymorphism affects blood pressure for free-living hypertensive men during a 5-month intervention period. SUBJECTS/METHODS: The subjects were free-living hypertensive Japanese men aged 40 to 75 years who agreed to intervention in the period from 2004 to 2011. Detection of the AT2R 3123C/A polymorphism was determined by polymerase chain reaction. The dietary intervention was designed to decrease salt level and to increase potassium level through cooking instructions and self-monitoring of the diet. The exercise session consisted of activities such as stretching, resistance training, and walking. Blood pressure, urinary sodium and potassium excretion, dietary and lifestyle data, and non-fasting venous blood sample were collected at baseline and after the intervention period. RESULTS: Thirty nine subjects were eligible for participation and the follow-up rate was 97.4%. The C allele proportion was 57.9%. AT2R 3123C/A polymorphism was X-chromosome-linked, therefore we analyzed the C and A genotypes. At baseline, no significant differences were observed between the genotype groups. After the intervention, there were no significant differences in lifestyle habit between the groups. Nevertheless, the estimated salt excretion (g/day) was significantly decreased only in the C genotype (13.0-10.3, P = 0.031). No significant change was observed in systolic blood pressure (SBP) (mmHg) in the A genotype, but a significant decrease was observed in the C genotype (150.0-141.5, P = 0.024). CONCLUSTIONS: In the C genotype, it might be easy to improve SBP through lifestyle intervention in free-living hypertensive Japanese men, however generalization could not be achieved by the small sample size.

In terms of quality of life, leisure and health have become important issues with increasing incomes and decreasing working hours in Korea. This study empirically investigates the effects of urban open space on outdoor leisure activities, emphasizing that parks, river banks, and physical activity sites can provide opportunities such as walking, jogging, stretching, and cycling, free of charge to all residents. Based on 2010 sample survey data on leisure activities, multiple regression model as well as hierarchical linear model are estimated, taking account of both individual characteristics on demand and environmental/areal factors on supply side, including open space. Major findings include: first, urban open space significantly increases residents' outdoor leisure activities, second, the effect is more significant for the elderly and third, the effect is more valid for those with relatively low incomes and less education. These results imply that urban open space could be available as a local public good to cope with population aging and to realize health city and social welfare, since this space is not only a leisure place but also public health and welfare facilities.
This study was carried out to establish a management program and soil restoration plan through analysis of soil properties and visitor questionnaires in Songrim in Hadong, Korea. Soil bulk density in Songrim was slightly higher in the closed-woodlands ($1.31g/cm^3$) than in the open-woodlands ($1.39g/cm^3$). Soil bulk density in the closed-woodlands was higher in walking trails ($1.74g/cm^3$) than in forest areas ($1.39g/cm^3$), while the rates of pore space were lower in walking trails (42.6%) than in forest areas (50.5%). The soil porosity were lower in the closed-woodlands (34.6%) than in the open-woodlands (42.6%). Soil strength in surface soil was slightly lower in the open-woodlands ($8.5kgf/cm^2$) than in the closed-woodlands ($10.5kgf/cm^2$). The content of organic matter, total nitrogen and exchangeable cations of the woodlands was low compared with the optimum nutrient content for tree growth in Korea forest soil. According to the survey, the objective of visiting in Songrim was to enjoy recreation and landscape views. To conserve pine forest ecosystems in Songrim, the respondents said that it needs to the implement of closed-woodland periods, the establishment of smoking free zone, and the prohibition of garbage throwing and alcohol including disciplinary rules and education. Also the respondents said that pine forest ecosystem in Songrim is relatively sound, but the woodlands require the intensive management to the ecosystem and the introduction of native understory vegetation, such as grasses under pine forest ecosystem. It is recommended to designate the rest-year forest for a proper period in all woodlands to restore the Songrim soil rather than the alternation application between the rest-year for three years or non-rest-year trails, and to open partially the walking trails across the woodlands after the period. In addition, the forest within the woodland is need to designate a long-term rest-year.

This study was conducted in order to analyze user behavior and preference factors in the outdoor spaces of mental hospitals. Among hospitals with 250 or more beds, 5 hospitals were selected in consideration of size of garden and diversity of garden elements. The subject of the study was restricted to mild cases of schizophrenia while 30~50 patients were selected on the recommendation of their doctor from 5 hospitals, respectively. The physical environment was analyzed, focusing on space components, after visiting the sites of study. A face to face interview method was selected in consideration of patients' cognitive abilities, a total of 230 questionnaires were used for the analysis. The results of the study can be summarized as follows. Rest facilities occupy the largest numbers in the components of garden, and those are followed by landscape facilities, walking/exercise facilities, and experience facilities. Outdoor walking/exercise programs are classified into group walks and free walks with most patients taking group walks. Most of the patients visit these outdoor spaces every day but some of them rarely use the outdoor areas. In order to increase the efficiency of using these outdoor spaces, the percentage of space for ensuring a sense of control should properly harmonize with the percentage of space to facilitate patients in having social contact. With regard to the reasons for preferring the most widely-used outdoor spaces, landscape/environment property was the most important, followed by functionality and then accessibility. Major activities in the preferred space are mainly composed of walking/exercise and rest. The preferred facilities are waterscape facilities such as ponds, waterfalls and fountains, rest facilities such as pergolas and shade trees, and lawn. It was understood that naturalness should be considered to be the most important factor in constructing a new healing garden, followed by aesthetics and amenities. Single facilities rated by preference for introduction were flower beds, trails, and lawn. According to type, waterscape facilities such as fountains, ponds, waterfalls and waterwheels were most preferred. Space for natural distraction and programs for the cultivation of flower beds were also preferred. The ideal image of a healing garden should be bright, familiar, and orderly as a whole, having plenty of introduced facilities. Open spaces were preferred to enclosed spaces. Finally, the image of a garden that helps patients feel calm was thought to be that of the most ideal garden.

Today, Deoksugung Doldam-gil, which is a well-known area in Seoul, has become a mixed place as many places reaching a critical age have been converted into parks. However, the previous research on the Deoksugung Doldam-gil was deficient in that the user, an essential variable, was not considered when assessing the place. Based on that, this study aims to analyze and interpret the perception of the places in Deoksugung Doldam-gil and to analyze factors to further enrich the place to visitors. According to the research, the representative idea of Deoksugung Doldam-gil is "the distance you want to go" and that has influencing factors, such as vehicle restrictions and the improvement of the walking environment. The analysis of classifying the variables that make up the perception of the place, physical environments, activities and meanings showed high awareness in, "streets of green (3.95)" and "stone walls of curves (3.88)." In the category of activities, "walking activities in the inner city (4.01)" and "love and romance (3.57)" were high. These results seem to reflect the spatial characteristics of the streets and the familiar image of the place were important. Five factors were extracted from the factor analysis to provide a more detailed understanding of the place perception, the correlation between each factor, and the place atmosphere of Deoksugung Doldam-gil. These factors confirmed a high correlation between 'green landscape' and 'historicity.' This can be attributed to the fact that the analysis reflects vital space, visual experience, and free walking conditions to be important, and these variables are present in urban parks. It also indicates the long-accumulated image and behavior near the site of Deoksugung Palace, including the historical and cultural heritage. It was confirmed that the factors related to the cognitive perception of Deoksugung Doldam-gil and the formation of the atmosphere of the place were strongly recognized. It found that there was a need to reflect the value and importance of 'green' in the future as culture or in the use of preservation and management related to heritage. This study presented a direction to be noted from the perspective of a user's place awareness, but considered only a fraction of the variables that affect the multidimensional sense of place and location recognition, and thus must be supplemented in the future.
Purpose: Retronychia is the embedding of the nail into proximal nail fold. Retronychia starts with disruption of the longitudinal growth of the nail. With the growth of a new nail, the old one is pushed upwards and backwards. This leads to embedding of the top nail into the ventral aspect of the proximal nail fold and results with chronic paronychia. We present a case of retronychia that was rarely reported in the literature. Methods: A 46-year old female presented with a 3-month history of painful right first, 2nd, 3rd toenail changes. Although she was initially treated with broad spectrum antibiotics, she did not response to therapy. Later, she presented to our department because of progressively worsening pain that impaired her walking. Physical examination revealed with proximal nail fold erythema, painful swelling, yellowish nail discoloration, and distal onycholysis. Bacterial and fungus culture showed no organism. Treatment was surgical nail avulsion under local anesthesia. Results: The postoperative course was uneventful. 10 months later, the patient had a normal growing nail and was free of symptoms. Conclusion: We report a case of retronychia on toenail. Retronychia is a proximal nail plate ingrowth into the proximal nail fold which is associated with multiple generations of nail plate misaligned beneath the proximal nail. Management consisted of simple avulsion of superimposed nail. Retronychia is suspected with a persistent paronychia, particularly in the setting of trauma. Avulsion of the top nail confirms the diagnosis and may be curative if the underlying nail appears healthy.

Purpose: The manoeuvring space at doors must be secured for users of walking aids, wheelchair users, stroller users, luggage carriers, etc., so clear criteria must be established. The korean standards for the manoeuvring space at doors need to be improved to present clear criteria depending on the type of entrance and the direction of opening and closing. Method: Korean standard, International Standard(ISO 21542, 2011), German Standard(DIN 18040-1, 2010), Austrian Standard(OENORM B 1600, 2017) and Swiss Standard(Norm SIA 500, 2009) were investigated and analyzed. A comprehensive improvement of the criteria for the manoeuvring space at doors is proposed. Results: The manoeuvring space at doors, which is essential for the disabled to use the door, shall be installed with a comprehensive consideration of the width of doors, the Circulation space, the direction of access, and the method of opening and closing of the door. Korean standards, such as the width of doors, the Circulation space, and the manoeuvring space at doors, are different from international and international standards, which require improvement of standards. The criteria for the manoeuvring space at doors shall be clearly improved so as not to be misinterpreted during the installation of doors and the BF-certification. Implication: The criteria for the installation of doors shall be presented as a comprehensive alternative through continued study of details. In addition, improvements in standards that reflect changes in international standards should be made.
Objective : To evaluate the neuroprotective effects of lacosamide after experimental peripheral nerve injury in rats. Methods : A total of 28 male wistar albino rats weighing 300-350 g were divided into four groups. In group I, the sciatic nerve exposed and the surgical wound was closed without injury; in group II, peripheral nerve injuries (PNI) was performed after dissection of the nerve; in group III, PNI was performed after dissection and lacosamide was administered, and in group IV, PNI was performed after dissection and physiological saline solution was administered. At 7 days after the injury all animals were sacrificed after walking track analysis. A 5 mL blood sample was drawn for biochemical analysis, and sciatic nerve tissues were removed for histopathological examination. Results : There is low tissue damage in lacosamide treated group and antioxidant anzymes and malondialdehyde levels were higher than non-treated and placebo treated group. However there was no improvement on clinical assessment. Conclusıon : The biochemical and histological analyses revealed that lacosamide has neuroprotective effect in PNI in rats. This neuroprotective capacity depends on its scavenger role for free oxygen radicals by increasing antioxidant enzyme activity.

Also, it tries to analyze on features in a form, material, kind, and color by period in a bag. Thus, the aim is to be conducive to a future qualitative research on bag Through the research process, it could be known about which a bag has been changed into diverse forms while experiencing several processes in order to conform with a flow of clothing, which is required by society according to a change in period, and about its historical significance, value, and aesthetic characteristics. It was certainly necessary for walking about with putting commodity necessary for people in it. As one of accessories that cannot be separated from clothing, it makes it available for knowing user's social standing and position and living scale, and makes it realized for the importance influence upon establishing a woman's identity.

Spontaneous pneumomediastinum(SPM) is an uncommon disease that is defined as the nontraumatic presence of free air in the mediastinum without any apparent underlying disease. We report a case of recurrent spontaneous pneumomediastinum without any direct precipitating cause. A 21-year-old woman was admitted to hospital suffering from chest pain that occurred during walking (?). She had previously been well. She did not complain of cough or vomiting prior to this presentation. Upon admission, the physical examination and Electrocardiogram findings were normal. The chest x-ray and chest computed-tomography showed a pneumomediastinum. She was successfully managed conservatively. Twenty seven months later, she was readmitted with chest pain, which again occurred during the same activity. The recurrent SPM was confirmed by the chest x-ray and chest computed-tomography. She was managed in the same manner as before and made an uneventful recovery. This is the first case of recurrent SPM in Korea.
