Analysis of Lower Extremity Injury Mechanism Centered on Frontal Collision in Occupant Motor Vehicle Crashes (정면충돌 시 차량 탑승자의 하지 손상기전에 대한 분석)

  • Injury mechanisms of lower extremity injuries in motor vehicle accidents are focused on fractures, sprains, and contusions. The purpose of this study is to evaluate the analysis of lower extremity injury mechanism in occupant motor vehicle accident by using Hospital Information System (HIS) and reconstruction program, based on the materials related to motor vehicle accidents. Among patients who visited the emergency department of Wonju Severance Christian Hospital due to motor vehicle accidents from August 2012 to February 2014, we collected data on patients with agreement for taking the damaged vehicle's photos. After obtaining the verbal consent from the patient, we asked about the cause of the accident, information on vehicle involved in the accident, and the location of car repair shop. The photos of the damaged vehicle were taken on the basis of front, rear, left side and right side. Damage to the vehicle was presented using the CDC code by analytical study of photo-images of the damaged vehicle, and a trauma score was used for medical examination of the severity of the patient's injury. Among the 1,699 patients due to motor vehicle crashes, 88 (5.2%) received a diagnosis of lower extremity fracture and 141 (8.3%) were the severe who had ISS over 15. Nevertheless during 19 months for research, it was difficult to build up in-depth database about motor vehicle crashes. It has a limitation on collecting data because not only the system for constructing database about motor vehicle crash is not organized but also the process for demanding materials is not available due to prevention of personal information. For accurate analysis of the relationship between occupant injury and vehicle damage in motor vehicle crashes, build-up of an in-depth database through carrying out various policies for motor vehicle crashes is necessary for sure.

The Effectiveness and Safety of Acupuncture for Recovery of Patients with Vertebral Compression Fractures: A Systematic Review and Meta-analysis (척추 압박 골절 환자의 회복에 대한 침 치료의 유효성 및 안전성 평가: 체계적 문헌 고찰과 메타분석)

  • Objectives : The purpose of this study was to evaluate the effectiveness and safety of acupuncture treatment for recovery of patients with vertebral compression fracture(VCF). Methods : We searched ten English and Chinese and seven Korean database up to April 2018. Randomised controlled trials(RCTs), quasi-RCTs, non-radomised Controlled Trials(CCTs) were eligible. Quasi-RCTs and CCTs were assessed only for safety assessment. Pain and adverse events were primary outcome of this review. Quality of life, dysfunction, patient satisfaction, incidence of new vertebral compression fracture were regarded as secondary outcomes. The risk of bias was assessed by two independent authors using the Cochrane risk of bias tool. Level of evidence was tabulated using the GRADE methods. Results : Of 1656 screened, 15 RCTs, 1 quasi-RCT and 3 CCTs were included. Number of participants per study ranged from 45 to 135. Most of the studies had unclear or high risk of bias and considerable heterogeneity in terms of type of intervention, comparison and time-points for outcome measurement. Compared to usual care alone, acupuncture combined with usual care showed short-term favorable results for pain relief in patients with VCF(5 studies, n=252, MD -1.05 point on a 0 to 10 point scale, 95% CI -1.45 to -0.65, $I^2=74%$). Four studies reported mild and temporary adverse events, and no serious adverse events were reported. One study descriptively reported that acupuncture was effective for improving quality of life without providing numerical outcomes. There were no reports of patient satisfaction and incidence of new VCF. Conclusions : Level of evidence is very low for the effectiveness and safety of acupuncture for pain, harms and other clinical outcomes in patients with VCF. Included studies suffered from incomplete reporting, high or unclear risk of bias and substantial heterogeneity between studies. Future high-quality RCTs are needed to assess whether acupuncture is beneficial for recovery of patients with VCF.

  • Borehole elemental concentration logging, measuring neutron-induced gamma rays by inelastic scattering and neutron capture interactions between neutron and formation, delivers concentrations of the most common elements found in the minerals and fluids of subsurface formation. X-ray diffraction and X-ray fluorescence analysis from core samples are traditionally used to understand formation composition and mineralogy, but it represents only part of formations. Additionally, it is difficult to obtain elemental analysis over the whole intervals because of poor core recovery zones such as fractures or sand layers mainly responsible for groundwater flow. The development of borehole technique for in situ elemental analysis plays a key role in assessing subsurface environment. Although this technology has advanced consistently starting from conventional and unconventional resources evaluation, it has been considered as exclusive techniques of some major service company. As regards domestic research and development, it has still remained an unexplored field because of some barriers such as the deficiency of detailed information on tools and calibration facility for chemistry and mineralogy database. This article reviews the basic theory of spectroscopy measurements, system configuration, calibration facility, and current status. In addition, this article introduces the domestic researches and self-development status on borehole elemental concentration tools.

Cytological Study on the Cause of the Osteoporotic Side Effects of Adefovir Dipivoxil (아데포비어의 부작용인 골다공증 원인 규명을 위한 세포학적 연구)

  • Osteoporosis is a disease that increases the risk of fractures by inducing a decrease in bone strength by the changes in hormones and a decrease in minerals. Recent reports have indicated that the long-term administration of Adefovir dipivoxil (ADV), which is used as a treatment for the hepatitis virus and AIDS, may have osteoporotic side effects. On the other hand, there are few studies on the cytopathic correlation of these causes. In this study, the biological relevance of ADV was evaluated using osteoblast hFOB1.19 and vascular endothelial cell HUVEC. First, the cells were treated with ADV at different concentrations, and DAPI and crystal violet staining were performed for morphological analysis of each cell and nucleus. A CCK-8 assay, real-time PCR, alkaline phosphatase (ALP) staining, and activity was performed to evaluate the drug effects on cell proliferation, gene expression, and osteoblast differentiation. As a result, ADV induced cell hypertrophy in hFOB1.19 cells and HUVEC cells. Furthermore, ADV not only inhibited cell proliferation and TGF-${\beta}$ expression but was also involved in osteoblast differentiation. Overall, these results provide basic data to help better understand the mechanism of ADV-induced osteoporosis and its clinical implications.

A study on the occlusal wear patterns in maxillary posterior teeth with palatal side abfractions (구개측 굴곡파절이 있는 상악 구치의 교합면 마모 양상에 대한 연구)

  • Purpose: Aims to analyze the occlusal wear patterns in maxillary posterior teeth with palatal side abfractions and study the association between occlusal force and abfractions. Materials and Methods: This study was conducted in a total of 308 teeth from 148 patients with palatal side abfractions in maxillary posterior teeth. The occlusal wears in maxillary premolars and molars with palatal side abfractions were classified and recorded. The classification was done by type of teeth, age, and gender, and in order to evaluate the statistical significance between groups, chi-square test was conducted (${\alpha}=0.05$). Results: Palatal side abfractions in maxillary posterior teeth were observed at the highest frequency in the 1st molars, and in all teeth with palatal side abfractions, more than one occlusal wear was observed. In classification by type of teeth, by age, and by gender, the occlusal wears in teeth with palatal side abfractions were observed at high frequency in cuspal inclined plane, central fossa, and marginal ridge, and there was a statistical significance (P < 0.05). Conclusion: Palatal side abfractions were observed at the highest frequency in maxillary 1st molars, and in all maxillary posterior teeth where palatal side abfractions were found, the occlusal wears were observed. And the occlusal wears were observed at high frequency in cuspal inclined plane, central fossa and marginal ridge. Such results show that abfraction is associated with occlusal force.

Interpretation of Paleostress using Geological Structures observed in the Eastern Part of the Ilgwang Fault (일광단층 동편에서 관찰되는 지질구조를 이용한 고응력사 해석)

  • In the southeastern part of the Korean Peninsula, huge fault valleys, including the Yangsan and Ulsan faults, are recognized. These NNE-SSW trending lineaments are called as a whole Yangsan Fault System. However, this fault system is relatively poorly studied except the Yangsan and Ulsan faults. This study deduced the paleostress history based on the mutual cross-cutting relationships between geologic structures developed in the granite body near the Ilgwang fault, which is compared with previous studies. In the study area, four lineaments parallel to the Ilgwang fault are recognized, and three of them show evidences of faulting. In each lineament, both slip-senses of left-lateral and right-lateral are recognized. It indicates that these faults consistently underwent multiple deformations of inversion along the faults. The inferred paleostress directions based on the mutual cross-cutting relationships of the geological structures are as follows: 1) Tensile fractures developed in the late Cretaceous under the ENE-WSW direction of compressive stress, 2) NW-SE trending maximum horizontal principal stress generated conjugate strike-slip faults, and 3) selective reactivations of some structures were derived under the compression by the NE-SW trending principal stress.

Analysis of the Risk Factors for Unfavorable Radiologic Outcomes after Fusion Surgery in Thoracolumbar Burst Fracture : What Amount of Postoperative Thoracolumbar Kyphosis Correction is Reasonable?

  • Objective : The aims in the management of thoracolumbar spinal fractures are not only to restore vertebral column stability, but also to obtain acceptable alignment of the thoracolumbar junction (T-L junction) to prevent complications. However, insufficient surgical correction of the thoracolumbar spine would be likely to cause late progression of abnormal kyphosis. Therefore, we identified the surgical factors that affected unfavorable radiologic outcomes of the thoracolumbar spine after surgery. Methods : This study was conducted in a single institution from January 2007 to December 2013. A total of 98 patients with unstable thoracolumbar spine fracture were included. In these patients, fixation was done through transpedicular screws with rods by three surgical patterns. We reviewed digital radiographs and analyzed the images preoperatively and postoperatively during follow-up visits to compare the change of the thoracolumbar Cobb angle with radiologic parameters and clinical outcomes. The unfavorable radiologic group was defined as the patients who were measured as having greater than 20 degrees of thoracolumbar Cobb angle on the last follow-up, or who underwent kyphotic progression of thoracolumbar Cobb angle greater than 10 degrees from the immediate postoperative state to final follow-up, or who had overt instrument failure with/without additional surgery. We assessed the risk factors that affected the unfavorable radiologic outcomes. Results : We had 43 patients with unfavorable radiologic outcomes, including 35 abnormal thoracolumbar alignments and 14 instrumental failures with/without additional surgery. The multivariate logistic regression test showed that immediate postoperative T-L junction Cobb angle less than 10.5 degrees was a statistically significant risk factor, as well as the presence of osteoporosis (p=0.017 and 0.049, respectively). Conclusion : Insufficient correction of thoracolumbar kyphosis was considered to be a major factor of an unfavorable radiological outcome. The spinal surgeon should consider that having a T-L junction Cobb angle larger than 10.5 degrees immediately after surgery could result in an unfavorable radiological outcome, which is related to a poor clinical outcome.

Understanding and Prevention of Fall-related Injuries in Older Adults in South Korea: A Systematic Review (한국 노인의 넘어짐과 연계된 인체손상에 대한 이해와 예방: 체계적 문헌 고찰)

  • Background: Fall-related injuries in older adults are a major health problem, and the risks and mechanisms of these injuries should be affected by race, culture, living environment, and/or economic status. Objects: Research articles have been systematically reviewed to understand fall-related injuries in older adults in South Korea. Methods: 128 published research papers have been found through the Korea Citation Index and the Korean Studies Information Service System, and reviewed in various perspectives, including incidents, fall death rates, medical costs, causes, injury sites and types, locations where falls occurred, prevention strategies, scholarly fields interested in fall injuries, and the role of physical therapy. Results: Fall-related injuries were found to be more common in women than in men, and the number of incidents increased with age, with the highest rate found in individuals over 85 years old. Risk of fall injury was associated with education level, comorbidities, and fear of falling. Common places where falls occurred included the bathroom, living room, stairs, and hallway. Common types of injury included bruises, fractures, and sprains in the lower extremities. Intervention strategies included exercise programs, education, and protective clothing. Scholarly fields interested in fall-related injuries in older adults included medicine, nursing, physical therapy, occupational therapy, physical education, pharmacology, oriental medicine, biomedical engineering, design, clothing, and textiles. Physical therapy intervention using proprioceptive neuromuscular facilitation has been used to improve one's balance. Conclusion: Any movement during the activities of daily living can lead to a fall. Physical therapists are highly educated to analyze human movements and should be involved in more research and practices to solve fall-related injuries in older adults.

Effect of Emergency Treatment Education Program for Self-leadership, Career Decision Making Self Efficacy and Nursing Performance Ability of Nursing Students (응급처치 교육프로그램이 간호대학생의 셀프리더십, 진로결정자기효능감 및 수행능력에 미치는 영향)

  • This study was conducted to examine the effects of an emergency treatment education program on nursing students' self-leadership, career decision-making self-efficacy, and nursing performance ability. The study was designed using a nonequivalent control group pretest-posttest design, and data were collected from June 1 to August 31, 2017. The participants consisted of 52 nursing students in D College of D City who were assigned to an experimental group (n=27) or control group (n=25). The pretest poll was performed on June 1, 2017 and posttest poll was performed 2 weeks later without any treatment in the control group. In the experimental group, the pretest poll was performed on July 25 before the emergency treatment education program and the posttest poll was performed on August 31 after the education program. The emergency treatment education program for the experimental group consisted of 2 hours of emergency treatment instruction, autonomously watching videos, and 4 hours of teaching elementary students about CPR for adults and first aid for airway obstruction, burns, bleeding, and fractures as a lecturer. The data were analyzed using descriptive analysis, independent t-test, $x^2$ test, and paired t-test with SPSS/WIN 20.0. After receiving the emergency treatment education program, a difference was observed in self-leadership (t=2.08, p<.05), career decision-making self-efficacy (t=4.22, p<.05) and nursing performance ability (t=5.02, p<.05) in the experimental group. The results of this study indicate that the emergency treatment education program was effective at increasing self-leadership, career decision-making self-efficacy, and nursing performance ability in nursing students.

Effect of Lonicerae Japonicae Flos on Bone Density in Ovariectomized Rat Model of Osteoporosis (난소 적출 흰쥐 골다공증 모델에서 금은화(金銀花)가 골밀도 증가에 미치는 효과)

  • Objectives : Osteoporosis is a systemic skeletal disease that decreases bone density and increases the risk of fractures. Bisphosphonates and SERMs are mainly used to treat osteoporosis, but, long-term use increases the risk of side effects such as jaw bone necrosis and breast cancer. Therefore, it is necessary to develop a therapeutic agent for a natural product with few side effects. Water extract of Lonicerae Japonicae Flos (wLF) was mainly found to have anti-cancer and anti-inflammatory effects. However, the effect of wLF on osteoporosis has not been elucidated. Therefore, this experiment investigated the effect of wLF on osteoclasts, osteoblasts and osteoporosis models. Methods : In order to study the effect of wLF on osteoporosis, the OVX-induced rat model was used for in vivo study. After 8 weeks, we measured body weight, uterine weight, liver weight, femur weight, bone density, trabecular area and tibia ash weight. To determine the effect of wLF on osteoclast differentiation, we measured the number of TRAP-positive cells and TRAP activity. To examine the effect of wLF on the expression of osteoblast-related genes, we measured the mRNA expression of alkaline phosphatase (ALP, Alpl) and osteocalcin (OCN, Bglap2). Results : In vivo experiment, wLF inhibited the reduction of femur weight, trabecular area, bone density and tibia ash weight. In vitro experiment, wLF had no significant effect on osteoclast differentiation. However, wLF increased the mRNA expression of Alpl and Bglap2 in MC3T3-E1 cell. Conclusions : This result suggested that wLF may be used for the treatment and prevention of postmenopausal osteoporosis.
