Retromandibular approach for the management of subcondylar fractures, followed by treatment of parotid gland fistulae: Case report (하악 과두하 골절시 후하악 접근법에 의한 수술 후 발생한 이하선 누공 형성과 이에 대한 치료 증례)

  • Condylar fractures account for one-third of all mandibular fractures. There are many surgical methods for the open reduction of condylar fractures, such as the transoral, submandibular, preauricular, and retromandibular approaches. Two patients suffering from condylar fractures, a 45-year-old man and a 25-year-old man, were admitted to our hospital. Both patients' condylar fractures were positioned too high for us to use the transoral approach. Therefore, we employed the retromandibular method to expedite the approach to the fracture site and minimize the size of the incision. After the surgical procedures in both cases, we experienced complications in the form of parotid gland fistulae, which rarely result from the retromandibular approach. A combination of botulinum toxin injection and amitriptyline medication was effective for the management of these parotid gland fistulae. Here, we report these two cases and offer a review of the literature on this article.

Effectiveness of Dual-Maneuver Using K-Wire and Dingman Elevator for the Reduction of Unstable Zygomatic Arch Fracture

  • Background: The zygoma is the most prominent portion of the face. Almost all simple zygomatic arch fractures are treated in a closed fashion with a Dingman elevator. However, the open approach should be considered for unstable zygomatic arch fractures. The coronal approach for a zygomatic arch fracture has complications. In this study, we introduce our method to reduce a special type of unstable zygomatic fracture. Methods: We retrospectively reviewed zygomatic arch view and facial bone computed tomography scans of 424 patients who visited the Wonju Severance Christian Hospital from 2007 to 2010 with zygomaticomaxillary fractures, among whom 15 patients met the inclusion criteria. Results: We used a Dingman elevator and K-wire simultaneously to manage this type of zygomatic arch fracture. Simple medial rotation force usually collapses the posterior fractured segment, and the fracture becomes unstable. Thus, the posterior fracture segment must be concurrently elevated with a Dingman elevator through Keen's approach with rotation force applied through the K-wire. All fractures were reduced without any instability using this method. Conclusion: We were able to reduce unstable and difficult zygomatic arch fractures without an open incision or any external fixation device.

  • Most capitate fractures occur in association with additional carpal injuries, particularly scaphoid fractures. Isolated fractures of the capitate account for only 0.3% of carpal injuries, and stress fractures are one form of this fracture. We report the case of a 20-year-old male who had a stress fracture of the capitate after serving as an honor guard in the military. Conventional radiographs and computed tomography of the right wrist revealed a minimally displaced fracture line located at the midcarpal aspect of the right capitate. A magnetic resonance imaging scan demonstrates a subarticular capitate fracture with diffuse bone marrow edema, small osteophytes, and irregularity of the midcarpal articular cartilage. We also review the carpal kinematics which possibly caused the stress fracture. Although stress fractures of the capitate are rare, they should also be accounted for with patients who perform repetitive motions of the wrist to a considerable extent.

SEVEN-YEAR RETROSPECTIVE STUDY OF FACIAL FRACTURE (안면골 골절의 발생 양상에 관한 7년간의 추적조사연구)

  • This report constitutes a 7-year study of facial fractures based on a series of 616 patients who were treated for facial fractures as in-patients at Chosun University Dental Hospital between 1998 and April 2005. The following results were obtained : 1. The ratio of men to women was 5 : 1. 2. The major etiological factors were falls (36.2%), traffic accidents (23.4%), punches (18.1%), sports (8.3%), and accidents related to work (2.8%). 3. They were most frequent in the second (28.8%), first (26.6%), and third (12.3) decades, in that order. 4. The sites of frequent mandible fracture are the symphysis (32%), left mandibular angle (25.3%), and right mandibular angle (10.9%). 5. The most frequent maxillary fractures were zygomatic fractures (46.4%), Le Fort I fractures (28.8%), and Le Fort II (12.0%) fractures.

  • Groundwater flow is primarily influenced by the presence of fractures, functioning as conduits. To block the flow, grouting operation is commonly used. Thereby the fractures are then expected to be sealed, which will add to enhance the shear strength in rock. This far, regarding the assessment of grouting efficiency, however, there's been a considerable uncertainty That is, several geophysical methods of high resolution such as tomography, S-wave logging have produced a significant amount of measurable response caused by grouting, but they can inevitably be used only for the qualitative assessment. Thus, this paper deals with an accurate quantitative assessment about the grouting result. In this, a new strategy is introduced, based mainly on evaluating the opening of fractures. For fracture-opening investigation purposes, borehole Televiewer has already proven to be an excellent logging technique that produces both amplitude image and traveltime image. As well known, the traveltime image can be converted to a high precision 3D caliper log with max. 288 arms, which allows to observe the opening of fractures. To evaluate the fracture opening from the traveltime image, an algorithm of practical use was developed, in which image correction due to the borehole deviation, feature discrimination of wall roughness from fractures, automatic evaluation procedure etc. were considered. Field examples are shown to confirm the efficiency of the suggested method.

Clinical Study of 123 Facial Bone Fractures in Elderly (노인 안면골 골절 123례에 대한 임상적 고찰)

  • Purpose: Aging society was realized after persons over 65 was rated above 7% in 2000. It is inevitable fact that society gets older. Few study about facial bone fracture in elderly was reported until now. This study provides a retrospective statistical analysis of facial bone fracture and reports of some demographical information from medical records. Methods: From January 2000 to December 2005, 123 cases of facial bone fracture in above 55 year-old persons were reviewed and analysed. Statistic data was related to distribution, age, sex, causes, occupations, occurrence, time, incidence of facial bone fracture, treatment and it's complications. Results: Facial bone fractures in elderly tend to increase and rated to 4.7%. Facial bone fractures in elderly were most frequently occurred in farmers, cultivator accidents and zygoma fractures. A few minor complications were checked, but easily improved. Conclusion: Facial bone fractures in elderly have small proportion of the whole facial bone fractures, but gradually have been increased. This study was observed trends in changes of facial bone fracture in elderly for 5 years and expected to provide statistical index to prevent facial bone fracture in elderly.

Guided Bone Regeneration in Comminuted Long-Bone Fractures Using Recombinant Human Bone Morphogenetic Protein-2 and a Collagen Membrane

  • A dog aged two years and seven months and a cat aged seven years were referred owing to fractures of long bones. Preoperative radiographs revealed comminuted bone fractures close to joints. Conventionally, long-bone fractures are treated using intramedullary pins, plate and screw systems, or an external fixator system. In cases of non-reducible fractures, various graft materials have been used in fracture treatments to stimulate bone repair. Here, recombinant human bone morphogenetic protein-2 (rhBMP-2) and a collagen membrane were applied. Four weeks after surgery, fractured bone fragments began to unite and the bone union was observed using radiography four months after surgery. No complications occurred related to grafted materials. We successfully applied rhBMP-2 and collagen membranes in two different species to support the healing process of comminuted fractures, according to the concept of guided bone regeneration.

Outcomes after rib fractures: more complex than a single number

  • Purpose: Rib fractures are common injuries that can lead to morbidity and mortality. Methods: Data on all patients with rib fractures admitted to a single trauma center between January 1, 2008 and December 31, 2018 were reviewed. Results: A total of 1,671 admissions for rib fracture were examined. Patients' median age was 57 years, the median Injury Severity Score (ISS) was 14, and the median number of fractured ribs was three. The in-hospital mortality rate was 4%. Age, the number of rib fractures, and Charlson Comorbidity Index scores were poor predictors of mortality, while the ISS was a slightly better predictor, with area under the receiver operating characteristic curve values of 0.60, 0.55, 0.58, and 0.74, respectively. Multivariate regression showed that age, ISS, and Charlson Comorbidity Index score, but not the number of rib fractures, were associated with significantly elevated adjusted odds ratios for mortality (1.03, 1.14, and 1.28, respectively). Conclusions: Age, ISS, and comorbidities were independently associated with the risk of mortality; however, they were not accurate predictors of death. The factors associated with rib fracture mortality are complex and cannot be explained by a single variable. Interventions to improve outcomes must be multifaceted.

Bone Mineral Bensity and Bone Turnover Markers in Patients with Femur Fracture Who Visited the Emergency Department (응급센터를 내원한 대퇴골 골절 환자에서 골밀도와 생화학적 표지자들의 의의)

  • Background: The most frequent fracture seen at the emergency department (ED) in the elderly is a femur fracture whereas they do not know the degree of osteoporosis. We analyzed the degree of osteoporosis in patients with femur fractures and compared patients with only femur fractures (FX) to patients with femur and vertebral fractures (VX) by examining the clinical features, the bone mineral density (BMD), and biochemical markers. Methods: From January 2004 to December 2004, we enrolled prospectively 30 femur fracture patients who visited the ED. The bone mineral densities of the lumbar spine and the femur were examined. Total calcium, phosphate, alkaline phosphatase, osteocalcin, and serum C-terminal telopeptide (s-CTx) were measured. The patients with femur fractures were divided into two subgroups according to the presence of vertebral fracture. Results: All BMDs of the FX group showed osteoporosis. The s-CTx levels were higher than normal. The patients in the FX with VX were older than those in the FX only group, and had lower BMDs. There were no significant differences in markers between the subgroups, but the incidence of trochanteric fractures was higher in FX with VX group than in the FX only group. Conclusion: Femur fractures in the elderly were associated with osteoporosis. In our study, despite a considerable difference in BMD between patients with femur fractures and those with femur fractures combined with vertebral fractures, there was no difference in biochemical markers on bone formation nor in the those of bone resorption. We will further investigate the biochemical markers and BMD in the population of osteoporotic fractures. So those indicators should be helpful for planning treatment and for prevention of FX in the elderly.

Malunion of the Jaw Fractures Complicated Following the Primary Managements (악골절 치료후 부정유합에 관한 임상적 연구)

  • PURPOSE : This is to review the complicated jaw fractures that had been referred for revision of the unsatisfactory results, and to provide proper managements for the easily complicated jaw fractures. MATERIALS & METHODS : Twenty-nine patients who had been revised due to malunion or complicated fractures of facial bones for last 3 years were reviewed. The main problems required for revision, type of fractures complicated, the primary managements to be reclaimed, the specialties to be involved, the management to be reclaimed, time elapsed to seek reoperation, type of revision surgeries, residual complication were analysed with medical records, radiographs and final examinations. RESULTS: The major complaints were malocclusion(79.3%), facial disfigurement(41.3%), TMJ problems (13.7%), neurologic problems(10.3%), non-union(10.3%), and infection(6.8%). Unsatisfactory results were occurred most frequently after improper management of the multiple fractures of the mandible (62.2%), combined fractures of maxilla and mandible (20.6%), fracture of zygomatico-maxillary complex and midpalate (17.2%). The complications to be corrected were widened or collapsed dental arches (79.3%), improperly reduced condyles (41.3%), painful TMJ (34.4%), limited jaw excursion (31.0%), over-reduction of zygoma (13.7%), and nonunion with infection(13.7%). and dysesthesia (10.3%). The primary managements were nendereet by plastic surgeons in 82.7%(24/29) and by oral surgeons in 7.6%(2/29). Main causes of malunion are inadequate ORIF in 76%, unawareness & delay in 17%, and delayed due to systemic cares in 17%. 76% of 29 patients had been in state of intermaxillary fixation for over 4 weeks. Revision were done by means of "refracture and ORIF"in 48.2%(14/29), orthognathic osteotomies with bone grafts in 55.1%(16/29), and camouflage countering & alloplastic implantations in 37.9%(11/29), TMJ surgeries in 17.2%, micro-neurosurgeries in 11.6%. Residual complications were limited mouth opening in 24.1% (7/29), paresthesia in 13.7%, resorption of reduced condyle in 10.3%. CONCLUSIONS : Failure of initial treatment of jaw fractures is due to improper diagnosis and inadequate treatment with lack of sufficient knowledge of stomatognathic system. It is crucial to judge jaw fracture and patients accurately, moreover, the best way of treatments has to be selected. Consideration of these factors in treatment could minimize the complication of jaw fractures.
