Effect of Oral Water Intake to Globus Pharyngeus with Non-Organic Diseases (인후두 이물감을 호소하는 환자에서 경구 수분섭취가 증상 호전에 미치는 영향)

  • Background and Objectives : Globus pharyngeus is a sensation of foreign body in the throat. There are many studies evaluate relationships between globus pharyngeus and organic diseases such as laryngopharyngeal reflux, esophageal motility disorders as well as psychotic causes. But, Also many patients without etiologic causes complain of globus sensation. The authors performed a study that evaluate association between oral water intake and symptoms of globus sensation on the basis of a belief that pharyngeal dehydration due to lack of oral water intake causes globus sensation. Materials and Methods : A survey using visual analogue scale to evaluate symptoms was performed with 73 patients with globus pharyngeus. Results : Patients who drink water less than 500 mL per day (p=0.04) and less than five times per day (p=0.02) were improved after 6 months due to education and doctor's recommendation. Conclusion : Frequent and enough water intake should be recommended to resolve symptoms of globus sensation.

  • Eagle's syndrome is the term given to symtomatic elogation of the styloid process or mineralization of the stylohyoid or stylomandibular ligament. Since ossification of stylohyoid ligament at cadeva was fist described by De Manchetis in 1652 and Weinlecher described clinical symptom which produced by elongated styloid process and osteotomy of styloid process in 1872, Clinical symtom which include sensation of a foreign body on the pharynx, dysphagai, dysphonia, referred pain, and mouth opening disturbance was termed by Eagle as Eagle's syndrome. then, case reports of Eagle's syndrome are presented. in these cases, the patient's chief complaints included periauricular radiating pain, mouth opening disturbance, foreign body sensation, dysphagia, tenderness on the neck. Through vairous X-ray examination and palpation of tonsillar fossa, elongated styloid process were confirmed. Under the general anesthesia there were successfully removed out via transoral approach technique, described by Eagle. after resented styloid process, they were freed from the symptoms without further complication. Therefor we reported these cases treated by surgical resection of styloid process with good results.

Surgical Treatment of Vocal Cord Granuloma (성대돌기 육아종의 수술적 치료)

  • Background and Objectives: Vocal cord granuloma is a exophytic inflammatory mass and caused by gastroesophageal reflux, voice abuse, endotracheal intubation. There has been a controversy in the treatment of vocal cord granuloma. Our aim of study is to know the clinical characteristics and the results of surgical management for vocal cord granuloma. Materials and Methods: We have reviewed and analyzed medical records of 55 patients who were diagnosed and surgically treated as vocal cord granuloma in Asan medical center from 1997 to 2005 retrospectively. Results: 25 cases were intubation granuloma and 30 cases were contact granuloma. In intubation granuloma, the clinical manifestation was hoarseness(70%), foreign body sensation(44%), chronic cough(21%). In contact granuloma, the clinical manifestation was hoarseness(67%), foreign body sensation(60%), throat clearing(21%). The recurrence rate after surgery was 8% in intubation granuloma and 33% in contact granuloma. Mean recurrence time was 4.1months in intubation granuloma and 3.2months in contact granuloma. Conclusion: Although there is no significant difference, recurrence rate after surgery was high in contact granuloma compared to intubation granuloma(p=0.125). Although this study is retrospective, surgical management must be considered in resolving diagnostic doubt, treating airway obstruction, and failure in conservative treatments.

  • This study investigated the status of soft contact lens wear in college students. The results were as follows: 1. The most prevalent age of initial contact lens wear was 19 years or 20 years, with an age distribution in females from as early as 10 years to 20 years. 2. For almost all subjects, the test clinic for the contact lens wear assessment and purchase of the contact lenses was an optical shop. 3. The main advantage for most male subjects was use for sports, whereas most female subjects used contact lenses for cosmetic reasons. 4. The most common side-effects experienced due to contact lens wear were red eye, foreign body sensation, and eye pain. 5. The most frequently reported eye disease caused by contact lens wear was conjunctivitis, with only few subjects experiencing keratitis. 6. Following treatment for the side effects of contact lens wear, most subjects hoped to re-use the previous contact lenses, some subjects wanted to change to new contact lenses, and others subjects refused further contact lens wear.

  • Eagle's syndrome refers to a symptomatic elongation of the styloid process or calcification of the stylohyoid and stylomandibular ligaments that usually accompanies facial and pharyngeal pain, dysphagia, and foreign-body sensation in the throat. Making an accurate diagnosis is difficult as Eagle's syndrome can be accompanied with referred pain to various areas of head and neck. Treatment options for Eagle's syndrome include non-surgical and surgical methods. The surest and the most effective method for relieving the symptom is surgical removal or reduction of the elongated styloid process. The present study reports two cases of female whom were diagnosed as "Eagle's syndrome" and whose chief complain was foreign-body sensation and pain in the throat on movement. Surgical removal via transoral approach of the elongated styloid process is a safe surgical option with favorable results compared to extraoral approach.

Two Cases of Human Thelaziasis and Brief Review of Korean cases

  • The present study was performed to describe 2 cases of human thelaziasis (HT) which occurred in Gyeongsangnam-do and to briefly review the previously reported Korean cases. A 58-year old woman, residing in Hadong-gun, Gyeongsangnam-do, came to Gyeongsang National University Hospital (GNUH) complaining of foreign body sensation and itching of the right eye in March 2000. Total 6 adult nematodes of Thelazia callipaeda (2 males and 4 females) were detected in her right eye. A 80-year old man, residing in Jinju-si, Gyeongsangnam-do, came to GNUH complaining of foreign body sensation, itching, and pain of the right eye in December 2007. A total of 5 worms (4 females and 1 degenerated) were removed from his right eye. We analyzed characteristics of the total 39 Korean HT cases reported to date, including the present 2 cases. Most of the cases (71.8%) occurred in Seoul and Gyeonggi-do before 2000, and 21 cases (53.8%) were males and 18 (46.2%) were females. The prevalence was higher in younger ages below 30 years (48.7%) than 31-60 years (41.0%) and over 61 years (10.3%). The seasonal prevalence showed a higher incidence in autumn (43.6%) than in other seasons. Most of the cases (94.9%) were conjunctival sac infections and only 2 (5.1%) were intraocular cases. The present 2 HT cases are the first reported cases in Gyeongsangnam-do. Some characteristics of Korean HT cases were analyzed.

  • Increasing of Korean old women's rate is demographic characteristics attributed an aging society, silver generation would lead to silver fashion market with steady competitiveness and interest in fashion. So the study attempts to use foundation material for silver's fashion design by analyzing design characteristics and processing development of design representative women's top. Selecting brand for collecting photo using study represents a mid- and premium-priced with reference Brand Yearbook. After selecting brand on domestic and foreign I have collected picture from Internet shopping mall and web-site of fashion brand, from October 2013 to July 2014, but I selected 840 pictures based on front poses for easy decision. In order to search design characteristics according to sort of silver blouse design on domestic and foreign. I have examined shape, color, pattern, fabric sensation, inside composition line and decoration with reference standard of analysis in preceding research. Data analysis method was conducted on analysis of frequency and cross analysis using SPSS statistical package 21.0. The conclusion of design characteristic according to type of blouse design for silver, H-silhouette in shirt blouse seems to fit in body transformation and expose silhouette with correcting the body flaw. Using soft material in T-shirt blouse they are able to conceal a body defect and express beautiful silhouette. By using detail in pullover shirt blouse and tunic blouse for point of design, it express glamour and luxurious. Most of blouse are printed various pattern, it would looks more gorgeous using pattern.

120 Cases of Facial Foreign Body Granuloma (안면부 이물 육아종 120례의 치험례)

  • Purpose: As the use of soft tissue fillers becomes more popular, complications such as foreign body granuloma (FBG) are increasing. We report 120 cases of facial FBG and review the available literatures. Methods: 120 patients of facial FBG in our clinic from Mar. 2003 to Feb. 2008 were complied and analyzed. A retrospective chart review was done and patient satisfaction was evaluated with a questionnaire using 5 score scale. Patients with severe inflammation sign or bizarre deformity underwent surgical excision and those with minimal symptoms or a history of hyaluronic acid injection received injection therapy using hyaluronidase. Results: 100 females and 20 males were observed. The average age was 43.7 years (from 16 to 74). 84 patients received surgical therapy and 36, injection therapy. Deformity of facial contour, foreign body sensation and inflammation sign were the three main chief complaints. 84 patients did not know what the injection materials were. The known materials are as follows: collagen, hyaluronic acid, silicone oil, paraffin. 92 cases were performed by unlicensed practitioners, 29 by physicians. Anatomical site most frequently affected by the foreign body granuloma was the cheek (25.8%), followed by forehead (19.2%), lips (15.8%), nose (9.2%), mentum (8.3%), eyelid and eyebrow (4.3%) and temple (0.8%). In 21 patients (17.5%), FBGs were found on multiple sites. Patients with inflammation sign got the highest satisfaction ($3.19{\pm}0.73$) (p=0.001) among 3 chief complaints. And patient satisfaction was statistically higher in surgical therapy group ($3.43{\pm}0.72$) than in injection therapy group ($2.97{\pm}0.88$) (p=0.003). Conclusion: We suggest that it may be beneficial to tailor the type of treatment for FBG relying on wound state and patient's chief complaints. In surgical therapy, resolute approach is necessary to correct facial deformity definitely and to minimize inflammation. Injection therapy could be another option for those with minimal symptoms or a history of hyaluronic acid injection. To prevent foreign body granuloma, not only plastic surgeons but also other physicians should inject soft tissue fillers with great caution and we should warn the public of disastrous consequences associated with illegal medical practice.

Dental needle foreign body in the neck: a case report

  • Foreign body (FB) ingestion is commonly seen in the ear nose and throat (ENT) field, with different presentations and sequelae. FBs can arrest in the upper aerodigestive tract or continue further down into either the airway tract to the bronchus or the digestive tract to the intestines. The pathway of an FB depends on the size and shape of the FB and how sharp its edges are. Since the 20th century, the use of disposable stainless-steel needles in the oral cavity has proven to be an effective and safe method for performing various intraoral procedures like dental infiltration or a root canal wash. Complications from their use are rare. Generally, dental needle breakages are caused by patients biting the needle, incorrect injection techniques, or inadequate preventative measures. The sudden movement of a patient during a procedure is one of the most common causes of breakage. Occasionally, needles are swallowed during dental procedures such as a root canal. Here, we report a case of a patient that swallowed a broken needle during a dental procedure. A few days later, the patient presented with neck pain, swelling, and a FB sensation. When the patient presented, she claimed that her symptoms had onset after consuming a meal containing duck meat. Initially, the patient was diagnosed as having ingested a duck bone. However, intraoperatively, the FB was discovered to be an injection needle that had migrated from the throat to the neck.

The Relationship between Habitual Patient-Reported Symptoms and Signs in the Soft Contact Lens Wearers (소프트 콘택트렌즈 착용자에서 평상시 나타난 증상에 관한 연구)

  • Purpose: To understand symptoms and signs in soft contact lens wearers the authors evaluate the attribution of the tear test to the contact lens related symptoms and signs. Methods: Sixty-two healthy soft contact lens wearers (23.95${\pm}$4.38 years old, 18.60${\pm}$16.92 months of prior lens wear) were participated in this study. 3 kinds of tear test and McMonnies' questionnaire test were performed for the soft contact lens wearers and subjective symptoms and objective signs were graded using CCLRU scales during the study period. Results: In this surveys, we found ocular dryness, redness and tiredness are the most common frequent and severe symptoms in soft contact lens wearers. Frequency of the ocular dryness, redness and tiredness are associated with grittiness, burning sensation and foreign body sensation respectively, and severity of the photophobia, foreign body sensation are associated with unstable vision and grittiness and which are associated with score of McMonnies' Questionnaire. Conjunctival redness and limbal redness are major signs but those are not need to be clinical care and rarely occurred corneal and conjunctival staining which are associated with tear break-up-time statistically. Conclusions: Dryness, redness and tiredness are primary common symptoms in Korean soft contact lens wearers. The frequency of those symptoms are increased with the period of soft contact lens wear and those are associated with other symptoms but have no relationship with signs.
