호기 및 흡기호흡운동이 상복부 수술을 한 노인 환자의 폐환기능과 폐 합병증에 미치는 효과 비교 (Comparison of Effects of Exhalation and Inhalation Breathing Exercises on Pulmonary Function and Complications in Elderly Patients with Upper-abdominal Surgery)

  • Purpose: The purpose of this study was to identify the effects of exhalation breathing exercises using expirometer and that of inhalation breathing exercises using incentive spirometry on pulmonary function and complications in elderly patients with upper-abdominal surgery. Methods: The research design was a nonequivalent control group non-synchronized design. Participants were 63 patients who underwent upper-abdominal surgery under general anesthesia (32 in experiment group, 31 in control group). They were recruited at P university hospital from August 1 to November 30, 2015. Effects were evaluated by measuring pulmonary functions (Forced Vital Capacity [FVC], Forced Expiratory Volume in 1 second [FEV1]) and pulmonary complications. Data were analyzed using SPSS/WIN 18.0 program. Results: There was no difference in FVC between the experimental group and the control group, but FEV1 in the experimental group increased significantly compared to the control group by time change (p=.001). Also, there were no pulmonary complications in the experimental group but there were 5 cases (16.1%)(p=.018) in the control group. Conclusion: Findings indicate that exhalation breathing exercises by elderly patients following upper-abdominal surgery is an effective nursing intervention in enhancing pulmonary function and preventing pulmonary complications.

The effect of Schroth's three-dimensional exercises in combination with respiratory muscle exercise on Cobb's angle and pulmonary function in patients with idiopathic scoliosis

  • Objective: The objective of this study was to investigate the effects of Schroth's three-dimensional exercises in combination with respiratory muscle exercise (SERME) on Cobb's angle and functional movement screen (FMS). Design: Randomized controlled trial. Methods: Fifteen subjects with scoliosis were randomly assigned to two groups. Eight subjects were assigned to the experimental group and seven subjects were assigned to the control group. The experimental group underwent SERME using SpiroTiger (Idiag, Switzerland), while the control group performed only the Schroth's three-dimensional exercises (SE). Both groups performed exercises for one hour per day, three times a week for eight weeks. Cobb's angle, pulmonary function (forced vital capacity, forced expiratory volume at one second, and peak expiratory flow) and FMS were measured before and after the experiment. Results: After intervention, the SERME group showed a significant difference in Cobb's angle, FMS scores, and pulmonary function as compared to before intervention (p<0.05). In the SE group, there was a statistically significant difference in Cobb's angle, pulmonary function, and FMS scores compared to before intervention (p<0.05). The SERME group showed a significant difference in Cobb's angle and peak expiratory flow in pulmonary function compared to the SE group (p<0.05). Conclusions: The results suggest that SERME could be a more effective intervention for improvement of the Cobb's angle and pulmonary function for scoliosis patients.

Asthma-COPD Overlap Syndrome: What We Know and What We Don't

  • Approximately one in four patients with chronic obstructive pulmonary disease (COPD) have asthmatic features consisting of wheezing, airway hyper-responsiveness or atopy. The Global initiative for Asthma/Globalinitiative for chronic Obstructive Lung Disease committee recently labelled these patients as having asthma-COPD overlap syndrome or ACOS. ACOS also encompasses patients with asthma, ${\geq}40$ years of age, who have been cigarette smokers (more than 5-10 pack years) or have had significant biomass exposure, and demonstrate persistent airflow limitation defined as a post-bronchodilator forced expiratory volume in 1 second ($FEV_1$)/forced vital capacity of <70%. Data over the past 30 years indicate that patients with ACOS have greater burden of symptoms including dyspnea and cough and show higher risk of COPD exacerbations and hospitalizations than those with pure COPD or pure asthma. Patients with ACOS also have increased risk of rapid $FEV_1$ decline and COPD mortality. Paradoxically, experimental evidence to support therapeutic decisions in ACOS patients is lacking because traditionally, patients with ACOS have been systematically excluded from therapeutic COPD and asthma trials to maintain homogeneity of the study population. In this study, we summarize the current understanding of ACOS, focusing on definitions, epidemiology and patient prognosis.

폐절제술시 정량 폐관류스캔을 이용한 폐기능 변화 예견에 대한 평가 (Evaluation of Prospective Pulmonary Function Change for Pulmonary Resection Using Quantitative Perfusion Lung Scan)

  • Spirometry and regional function studies using 99m-Technetium were performed preoperatively to predict postoperative pulmonary function change in 34 patients who had various pulmonary resectional procedures at the Department of Thoracic and Cardiovascular Surgery, Seoul National University Hospital. Between two months and fourteen months postoperation all the patients were reinvestigated with spirometry and clinical examination to evaluate their functional respiratory status. The postoperative obtained values, especially forced vital capacity [FVC] and forced expiratory volume in one second [FEV1] among the other parameters were compared with the postoperative predicted values. Estimated values of FVC and FEV1 derived from preoperative spirometry and quantitative perfusion lung scan correlated well with the measured postoperative values. The linear regression line derived from correlation between postoperative estimated[X] and postoperative measured[Y] values of FVC and FEV1 in all patients are as follows; 1. Y=0.76x + 0.39 in correlation of FVC [r=0.91] 2. Y=0.88x + 0.17 in correlation of FEV1 [r=0.96],br> This method of estimation was one of the best methods of predicting postoperative pulmonary function change and valuable in determining the extent of safe resection and postoperative prognosis to a poor risk patient with chronic obstructive lung disease.

목빗근의 근막통증에 키네시오테이핑 적용 후 호흡기능의 변화 (Change of Pain and Breathing Function following Kinesio Taping of Myofascial Pain in Sternocleidomastoid Muscle)

  • Purpose: This study was conducted in order to determine the changes in pain and breathing function when kinesio taping is applied to patients with myofacial trigger points on sternocleidomastoid (SCM) muscle. Methods: The subjects were 25 males and females aged 20 to 30 years (male 10, female 15). They were randomly divided into the control group and the experimental group to be applied with kinesio taping. Kinesio taping was applied to SCM muscle three times per week for two weeks. Pain triggered when the taut band or nodule was palpated was measured. Pain intensity was measured using the visual analog scale (VAS). The breathing function was measured using forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), and FEV1/FVC ratio. In all subjects, VAS, breathing function was measured before and after intervention. Results: In the experimental group, pain in the SCM was relieved as VAS showed a significant decrease and FVC, FEV1, and FEV1/FVC ratio showed a significant increase. Comparison between the groups, showed significant differences in VAS and the FVC, FEV1, and FEV1/FVC ratio. Conclusion: These results suggest that myofascial pain on SCM muscle is thought a factor that affects the breathing function.

Do the Types of Seat Surface influence the pulmonary Functions during Prolonged Sitting?

  • Purpose: The purpose of this study was to identify the effects of the types of seat surface (static or dynamic seat surface) on the pulmonary functions during prolonged sitting. Methods: Thirty-four participants (20 males and 14 females) were recruited, and distributed randomly into dynamic prolonged sitting (DPS, n=17) and static prolonged sitting (SPS, n=17) groups. The DPS group was seated on a chair with a dynamic air cushion, and the SPS group was seated on a chair without a dynamic air cushion. The pulmonary function was assessed before sitting, and after participants had been seated for one hour. The pulmonary function [forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), and Peak expiratory flow (PEF)] was measured using a spirometer. Results: Statistical analyses revealed significant differences in the time x group interactions of FVC, FEV1, PEF, and FEV1/FVC. The DPS group were significantly different in FVC, FEV1, PEF, and FEV1/FVC after prolonged sitting for one hour, compared to the SPS group (p<0.05). Conclusion: These findings suggest that dynamic sitting can prevent a decrease in the physiological function, such as pulmonary functions, rather than static sitting during prolonged sitting.

과거 무기분진에 노출된 이직근로자들의 연간 폐기능 변화 (Annual Changes of Lung Function in Retired Workers Exposed to Inorganic Dusts)

  • Background: The aim of this study was to investigate annual changes in pulmonary function in workers who were exposed to inorganic dust. Methods: The subjects were 2,922 male patients who had been diagnosed with pneumoconiosis more than twice during 6 years from 2005 to 2010. Results: Of the 2,922 cases, forced vital capacity (FVC) decreased by 54 mL in 1 year. In contrast, the annual change of forced expiratory volume in one second ($FEV_1$) decreased by 56 mL. Conclusion: This is the first study that has investigated the annual change in pulmonary function in workers exposed to inorganic dust. The results will help estimate the pulmonary condition of patients who are unable to perform a pulmonary function test due to age or a disorder.

최대부하운동 후 20대 남성 흡연자와 비흡연자의 폐기능과 심박수 회복 반응 (The Pulmonary Function and Heart Rate Recovery Response of Smoker and Nonsmoker in Males Aged 20s after Graded Maximal Exercise)

  • Purpose : The purpose of this study was to investigate the response of pulmonary function and heart rate recovery of smoker and nonsmoker in males aged 20s after graded maximal exercise. Method : The subjects were composed of smoker group (n=12) and nonsmoker group (n=12) in males aged 20s. Each groups completed an graded maximal exercise with Bruce protocol and were assessed on the pulmonary function(forced vital capacity : FVC, forced expiratory volume-one second : FEV1, FEV1/FVC) and heart rate. Result : The results were as follows: First, heart rate in the measurement point was a statistically significant difference for smoker and non-smoker group after maximal exercise, but FVC, FEV1, FEV1/FVC was no difference. Second, FEV1/FVC between smoker and nonsmoker group was a statistically significant difference after maximal exercise, but FVC, FEV1, heart rate was no difference. Conclusion : The results of this study is that smoking is negative effects on FEV1/FVC of pulmonary function in males aged 20s after maximal exercise.

The Effects of Water-based Exercise on Respiratory Function in Children with Spastic Diplegic Cerebral Palsy

  • Purpose: We investigated to evaluate the effectiveness of water-based exercise (WE) program on respiratory functions for children with spastic diplegic cerebral palsy (CP). Methods: Fourteen children with spastic diplegic CP were randomly assigned, to either the experimental group (EG, n=7), or the control group (CG, n=7). Respiratory function was measured by a spirometer, a CardioTouch 3000S ( Bionet, Seoul, Korea) at a chair-sitting posture. Forced vital capacity (FVC), forced expiratory volume at one second (FEV1), peak expiratory flow (PEF) were measured. The intervention program will last 8 weeks, with three 40 minutes sessions per week (24 training session). The usual care and the addition of a WE program, were compared in the CG and EG, respectively. Results: The EG showed a significant increase in the FVC, FEV1, PEF after training (p<0.05), whereas there was no significant difference in the CP after training. In the EG, FVC increased significantly, compared to the control group (p<0.05), but not FEV and PEF. Conclusion: These findings suggest that WE program have an effect on the respiratory function in children with spastic diplegic CP.

양측 액와개흉을 통한 양측 폐기낭 동시절제 (Simultaneous bilateral bleb resection through bilateral trans-axillary thoracotomy)

  • Simultaneous bilateral bleb resection was done through bilateral transaxillary thoracotomy in 10 patients with spontaneous pneumothorax during the period from May 1991 to Novemver 1992 in whom bilateral bulla or bleb was detected with using simple chest X-ray and chest CT scanning. To compare the effectiveness of bilateral transaxillary thoracotomy, we investigated 10 unilateral transaxillary thoracotomy patients with spontaneous pneumothorax and two clinical reports from other institutes which dealt the results of bilateral bleb or bulla resection through median sternotomy also. In bilateral transaxillary thoracotomy group,mean operation time was 115 minute,mean intraoperative bleeding was 329 cc, mean postoperative hospital stay was 7.5 days. Postoperative ABGA[Arterial Blood Gas Analysis] was in normal range and postoperative recovery rates of FVC[Forced Vital Capacity], FEV1[Forced Expiratory Volume at 1 second], TV[Tidal Volume] were 84.3%, 93.4%, 88.7%,respectively. In median sternotomy group,mean operation time was 129 minute,mean intraoperative bleeding was 490 cc, mean postoperative hospital stay was 12.4 days. Postoperative ABGA was in normal range and postoperative recovery rates of FVC, FEV1 were 97.3%, 97.4%, respectively. In unilateral transaxillary thoracotomy group, postoperative ABGA was in normal range also and postoperative recovery rates of FVC, FEV1, TV were 91.6%, 99.0%, 96.0%,respectively. In conclusion, simultaneous bilateral bleb resection through bilateral transaxillary thoracotomy should be considered in pneumothorax patients with bilateral bleb or bulla because of cost-effectiveness[reducing hospital days] and better cosmetic result without any impairment in recovery of respiratory function.
