Medial Malleolar Fracture Combined with Deltoid Ligament Rupture - one case report - (삼각 인대 파열을 동반한 족근 관절 내과 골절 - 1예 보고 -)

  • Authors report one patient of concurrent fracture of medial malleolus and rupture of deltoid ligament. To the author's knowledge, combined failure of both structures has not been previously reported in Korea. Injury mechanism was classified into pronation -abduction type according to Lauge-Hansen classification. Plain radiograph revealed trimalleolar fracture and posterior ankle dislocation. MRI showed complete rupture of both deep and superficial layer of the deltoid ligament. In conclusion, therefore, concurrent rupture of deltoid ligament should be considered for the surgical treatment in trimalleolar ankle fracture with posterior ankle dislocation resulted from pronation-adduction injury.

Prevalence Of Radiographic Ankle Osteoarthritis (방사선적 족관절염의 유병율)

  • Objectives: This study was done to determine the prevalence of radiographic ankle osteoarthritis (OA) in Korean women with clinical primary ankle OA. Methods: This analysis involved 160 Korean women who visited our out patient clinic due to ankle joint pain. Ankle AP and lateral weight-bearing radiographs were evaluated retrospectively to check whether they have radiographic OA or not. Radiographs were read for features of ankle OA using School classification. Demographic factors and radiographic findings in both radiographic ankle OA group and non-radiographic ankle OA group were analyzed by the student t-test, the chi square test and the Pearson's analysis. A p value <0.05 was considered significant. Results: The prevalence of radiographic ankle OA in Korean women is 18.2 % in patient between 18 and 39 years and is 55.6 % in patient over 40. There was significant difference between two groups with age (P<0.001) and relatively strong correlation between radiographic OA and age (r=0.474, P<0.001). Conclusion: These results show that the prevalence of Korean women with radiographic changes of ankle OA was 45 % and there was relatively strong correlation between age and radiographic ankle OA.

Surgical Management of Neuropathic Arthropathy due to Rheumatoid Arthritis: A Case Report (류마티스 관절염 환자에서 발생한 신경병성 관절병증의 수술적 접근: 증례 보고)

  • Neuropathic arthropathy (Charcot arthropathy) is a progressive joint disease often associated with conditions such as diabetes, leading to severe joint deformity and pain. However, its occurrence in patients with rheumatoid arthritis (RA) is rare and not well documented. This case report describes a 48-year-old woman with a long history of RA who developed a severe deformity of her right ankle, identified as neuropathic arthropathy extending to the subtalar joint (Brodsky classification type 3A). After excluding other potential causes, the condition was determined to be secondary to RA-associated peripheral neuropathy. Surgery, including allograft and autograft fixation, was performed to correct the deformity. Post-operative complications, such as wound infection, were treated with negative pressure wound therapy and skin grafting. At the 18-month follow-up, the patient was able to walk without pain, demonstrating successful joint fusion.

Technology and Policy Measures for Landfill Sites of Foot-and-Mouth Disease in Korea (구제역 매몰지에 대한 기술 및 정책적 대응 방안)

  • In 2010-11 outbreak of foot-and-mouth disease, around 3.5 million livestock were slaughtered and buried at more than 4,500 landfill sites. These landfill sites have been managed according to the Domestic Animal Infectious Disease Control Law. However, the control will be removed from December 2013 since the control period is 3 years. That means the sites will be reused without any interference. In this situation, if the buried livestock has not fully resolved and opened to surface, many dangerous happening is expected. This study explores technology and policy measures to meet the expected problems after December 2013. We will suggest the structure of technology for management of landfill sites based on the needs of the sites, and we will analysis Korean patents under this classification. This is the first try in the world because Korean style of landfill is unique. Based on analysis, we will suggest policy recommendations.

Operative Treatment of Hallux Valgus Deformity in Male Patients (남성 무지 외반증 환자에 대한 수술적 치료)

  • Purpose: We evaluated the result of operative treatment of the hallux valgus in male patients. Materails and Methods: Total 11 cases (10 patients) of the hallux valgus deformity that treated with operation were evaluated. Following Mann's radiological classification system, there was 1 cases of mild, 8 cases of moderate, and 2 cases of severe. Preoperative, postoperative, postoperative 3 months and postoperative 6 months follow up standing radiographs were used as radiologic evaluation. And we evaluated radiological outcomes by hallux valgus angle (HVA), first-second intermetatarsal angle (IMA), distal metatarsal articular angle (DMAA), proximal phalangeal articular angle (PPAA) and clinical outcomes by hallux-metatarsophalangeal scale of American Orthopaedic Foot and Ankle Society (AOFAS) score. Result: Radiologically, the mean preoperative HVA $37^{\circ}$ and IMA $13.7^{\circ}$ were improved postoperatively as HVA $11.9^{\circ}$ and IMA $4.7^{\circ}$, and the mean preoperative DMMA $29.4^{\circ}$ and PPAA $8.6$ were improved postoperatively DMMA $13.9^{\circ}$ and PPAA $7^{\circ}$. But, postoperative 6 months follow up HVA, IMA, DMMA and PPAA was increased at $14.2^{\circ}$, $6.3^{\circ}$, $16.1^{\circ}$ and $8.3^{\circ}$. Average AOFAS score were improved from 61.2 points to 75.2 points. Conclusion: In our study, operative treatment of hallux valgus in male patients with proximal metatarsal osteotomy and distal soft tissue procedure showed good results but it was necessary to pay attention to increase aspect of follow up radiologic measurements.

Geographical distribution and molecular epidemiology of the foot-and-mouth disease viruses of major groups (주요 혈청형 구제역바이러스의 발생분포와 분자역학적 분석)

  • Foot-and-mouth disease (FMD) virus exists in seven serotypes and is known to be a highly contagious disease that is hard to eradicate from the world. The O, A, Asia1 and SAT2 serotypes commonly infected cattle, sheep and goats during 2007~2009 throughout the world. In particular, the outbreak of the Asia1 serotype in China appeared in all areas from 2005 and is still present. Surprisingly, in 2009, Taiwan reported the first outbreak of the type O serotype since 2001. Then type A appeared in China for the first time since the early 1960s. The virus shows a close relationship to the viruses from Southeast Asia suggesting one or more recent introductions into China in the OIE reports. Recently the subtype of A/Iran05 spread to nearby countries exhibiting genomic evolution. The use of molecular epidemiology is an important tool in understanding and consequently controlling the FMD virus. The phylogenetic analysis with VP1 gene was especially useful for molecular epidemiological studies and showed the same pattern which matches with serotype classification. This paper describes basic information about the disease, and the serotype-specific characteristics and evolution to perform molecular epidemiological analysis. Furthermore, we show the importance of the genetic evolution on the FMD serotypes in global surveillance and molecular epidemiology of FMD for outbreak investigation.

Outcomes of Immediate Operative Treatment of Ankle Trimalleolar Open Fractures (족관절 개방성 삼과골절의 즉각적인 수술의 결과)

  • Purpose: Generally, the treatment of ankle trimalleolar open fractures is divided into two stages: external fixation and debridement; and secondary internal fixation. On the other hand, this two-stage operation takes considerable treatment time and is challenging in procedures requiring reduction. The purpose of this study was to evaluate the radiologic and clinical results of an immediate one-stage internal fixation operation considering the wound conditions to overcome two stage operation disadvantages. Materials and Methods: From September 2009 to January 2018, 24 cases of ankle trimalleolar open fractures, who underwent immediate internal fixation and were followed up for at least one year, were studied retrospectively. The open wound was divided into the Gustilo-Anderson classification. Open reduction and internal fixation were performed on every medial and lateral malleolar fracture. On the other hand, with posterior malleolar fractures, surgical or conservative treatment was performed depending on the fragment size. The radiologic outcome was evaluated using the Burwell and Charnley criteria and American Orthopaedic Foot and Ankle Society (AOFAS) scores, and complications, such as infection and posttraumatic arthritis, were used for the clinical evaluation. Results: The wound was classified into eight cases (33.3%) of type I, 11 cases (45.8%) of type II, and five cases (20.8%) of type IIIa. The degree of reduction was anatomical, fair, and poor in 16 cases (66.7%), six cases (25.0%), and two cases (8.3%), respectively. The mean AOFAS score was 79 points, and there were complications, such as infection in three cases (12.5%) and post-traumatic arthritis in two cases (8.3%). Conclusion: Satisfactory results were obtained through immediate surgical treatment in ankle trimalleolar open fractures of types I, II, and IIIa.

Treatment of Displaced Intra-articular Calcaneal Fracture using Ollier Approach (전위된 관절 내 종골 골절의 Ollier 접근법을 이용한 치료)

  • Purpose: We evaluated the clinical results of the intraarticular calcaneal fractures treated using Ollier approach by inexperienced orthopaedic surgeon. Materials and Methods: Between August 2003 and May 2007, Of the total 46 cases, 12 cases (9 patients) of displaced intraarticular calcaneal fracture who underwent open reduction and internal fixation using Ollier approach were evaluated. The means of age was 50.5 years. According to the Sanders classification, there was no type I case and 8 cases of type II, 1 case of type III, and 3 cases of type IV. We evaluated the treatment result by assessing radiologic parameters ($B{\ddot{o}}hler$ angle, Gissane angle, and calcaneal height/width) and clinical outcomes (VAS and AOFAS score). Results: The means of follow-up period was 25.3 months. The means of B.hler angle was improved from $2.4^{\circ}$ to $26.1^{\circ}$. Radiologic and clinical union was achieved in all cases without additional procedures. Excellent result were noted in 2 cases, good in 5 cases, fair in 4 cases, and poor in 1 case. We experienced 2 cases of minor complications; 1 case of mild wound infection and 1 case of hypoesthesia on foot dorsum. Radiologic findings of subtalar arthritis were present in 2 cases. Conclusion: Ollier approach seems to be helpful to inexperienced orthopaedic surgeons for the treatment of intraarticular calcaneal fractures in that it enables them to achieve considerable clinical outcomes without serious complications.

Arthroscopic Osteochondroplasty of the Osteochonral Lesion of the Talus (거골의 골연골 병변에 대한 관절경적 골연골 성형술)

  • Purpose: To evaluate the clinical characteristics and the functional results of the osteochondral lesions of the talus (OLT) treated with arthroscopic osteochondroplasty. Materials and Methods: The study was based on 25 cases (25 feet) of osteochondral lesion of talus that were treated with arthroscopic osteochondroplasty from May 1997 to June 2005 with at least 1 year follow-up. Follow-up period was average 21.8 months. The average age was 37.2 years old. Male patients were 22 while female were 3. Postoperative evaluation was performed with American Orthopedic Foot and Ankle Society (AOFAS) Functional score, patient satisfaction and complications. Results: Ninety-two percent of patients had trauma history. Seventy-two percent of the lesions were located at the medial dome. As for the cartilage classification, 56% were fibrillation/fissure lesions while 44% were flap tears. Visual analogue scale (VAS) pain score improved from 6.8 points to 2.2 points, and AOFAS score improved from 71.3 points to 87.3 points. Eighty-eight percent of the patients were satisfied with the surgery and average period of returning to previous job was 6.5 weeks. Conclusion: We concluded that the arthroscopic osteochondroplasty is a viable surgical option for the osteochondral lesion of the talus.

Surgical Treatment of Calcaneal Fractures of Sanders Type II and III by A Minimally Invasive Technique with 6.5 mm Cancellous Screw (6.5 mm 해면골 전산나사를 이용한 Sanders Type II 또는 III형 종골 골절의 최소 침습적 치료)

  • Purpose: This study evaluated the clinical and radiological results of 6.5 mm full threaded cancellous bone screw fixation of calcaneal fractures. Materials and Methods: Thirty seven patients diagnosed with Sanders type II or III calcaneal fractures, who underwent open reduction and internal fixation with a 6.5 mm full threaded cancellous bone screw between August 2014 and August 2017, were analyzed. Both the preoperative and postoperative Böhler angle and Gissane angle were measured radiographically. American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot scale on the final follow-up were also assessed. Results: The mean age of the patients was 52.7 years and the mean follow-up period was 29.5 months. In the Sanders classification, type II and III were 16 and 24 cases, respectively. The Böhler and Gissane angles improved from 21.2° and 122.6° preoperatively to 21.6° and 120.3°, respectively, in the postoperative radiographs. All cases achieved bony union, and the AOFAS ankle-hindfoot scale was 90.7 and 91.3 in Sanders type II and III, respectively, at the final follow-up. Conclusion: The treatment of calcaneal fractures using a 6.5 mm full threaded cancellous bone screw can reduce the complications with minimally invasive surgery and achieve firm fixation.
