  • The authors studied the natural evaluation of lumbar soft disc herniation in 20 patients with lumbar radiculopathy that was successfully treated with conservative theraphy only. The first computed tomography (CT) examination was performed during the acute phase of the radiculopathy, and the second performed from 3 to 24 months after healing. The initial CT allowed classification of the herniations according to size: ten were considered small. ten medium. Comparison with follow-up scans showed that 18 of Herniations decreased between 0 and 25%, two decreased between 75 and 100%.

Clinical experience with $^{18}F$-fluorodeoxyglucose positron emission tomography and $^{123}I$-metaiodobenzylguanine scintigraphy in pediatric neuroblastoma: complementary roles in follow-up of patients

  • Purpose: To evaluate the potential utility of $^{123}I$-metaiodobenzylguanine ($^{123}I$-MIBG) scintigraphy and $^{18}F$-fluorodeoxyglucose ($^{18}F$-FDG) positron emission tomography (PET) for the detection of primary and metastatic lesions in pediatric neuroblastoma (NBL) patients, and to determine whether $^{18}F$-FDG PET is as beneficial as $^{123}I$-MIBG imaging. Methods: We selected 8 NBL patients with significant residual mass after operation and who had paired $^{123}I$-MIBG and $^{18}F$-FDG PET images that were obtained during the follow-up. We retrospectively reviewed the clinical charts and the findings of 45 paired scans. Results: Both scans correlated relatively well with the disease status as determined by standard imaging modalities during follow-up; the overall concordance rates were 32/45 (71.1%) for primary tumor sites and 33/45 (73.3%) for bone-bone marrow (BM) metastatic sites. In detecting primary tumor sites, $^{123}I$-MIBG might be superior to $^{18}F$-FDG PET. The sensitivity of $^{123}I$-MIBG and $^{18}F$-FDG PET were 96.7% and 70.9%, respectively, and their specificity were 85.7% and 92.8%, respectively. $^{18}F$-FDG PET failed to detect 9 true NBL lesions in 45 follow-up scans (false negative rate, 29%) with positive $^{123}I$-MIBG. For bone-BM metastatic sites, the sensitivity of $^{123}I$-MIBG and $^{18}F$-FDG PET were 72.7% and 81.8%, respectively, and the specificity were 79.1% and 100%, respectively. $^{123}I$-MIBG scan showed higher false positivity (20.8%) than $^{18}F$-FDG PET (0%). Conclusion: $^{123}I$-MIBG is superior for delineating primary tumor sites, and $^{18}F$-FDG PET could aid in discriminating inconclusive findings on bony metastatic NBL. Both scans can be complementarily used to clearly determine discrepancies or inconclusive findings on primary or bone-BM metastatic NBL during follow-up.

  • Unresolved major pulmonary embolism (UMPE) is an uncommon condition which causes pulmonary hypertension, cor pulmonale and death. An accurate and prompt diagnosis of UMPE is very important in the management of such patients with pulmonary embolectomy. Follow-up lung scans can lead to earlier diagnosis of UMPE especially on patients who have a history of acute pulmonary embolism in the past and. present with pulmonary hypertension, respiratory insufficiency and cor pulmonale. We report a case of UMPE strongly suggested by follow-up lung scans and subsequently confirmed by pulmonary angiography and postmortem examination.

원발성 유방암에서 질병병기 분류 및 추적검사 $^{99m}Tc-MDP$ 골 신티그램의 의의 ($^{99m}Tc$ MDP-Bone Scintigraphy for Preoperative and Follow up Assessment in Primary Breast Cancer)

  • Because carcinoma of breast is known to readily metastasize to the bone, early detection of metastasis spread is very important. Two hundred thirty four consecutive patients who had $^{99m}Tc-MDP$ bone scans prior to treatment and during the follow up period were studied retrospectively to determine the contribution of the scans to staging, treatment and follow up examination between Jan. 1980 and Apr. 1987 in Seoul National University Hospital. 1) Positive bone scan rates in initial clinical stage I, II, III and IV were 0%, 4%, 14% and 53%, respectively. 2) The higher clinical stage is, the more cumulative risk of conversion to positive bone scan is during the same follow up period. 3) Conversion rate to positive bone scan in patients with lymph node metastasis is higher (50%) than those without metastasis (18%) (P < 0.001). We concluded that the bone scan in primary breast cancer was very useful to evaluate initial clinical stage and to perform the follow up examination.

방광요관역류와 신반흔 (Vesicoureteral Reflux and Renal Scar)

  • Purpose : Vesicoureteral reflux(VUR) is the major risk factor of urinary tract infection(UTI) in children and may result in serious complications such as renal scarring and chronic renal failure. The purpose of this study was to evaluate the relationship between VUR and renal scar formation, the usefulness and correlation of various imaging studies in reflux nephropathy, and the spontaneous resolution of VUR. Methods : We retrospectively reviewed 106 patients with VUR with no accompanying urogenital anomalies in the Department of Pediatrics, Bundang CHA Hospital during the period from Jan. 1996 to Mar. 2005. Ultrasonography and $^{99m}Tc$-dimercaptosuccinic acid(DMSA) scan were performed in the acute period of UTI. Voiding cystourethrography(VCUG) was performed 1 to 3 weeks after treatment with UTI. Follow-up DMSA scan was performed 4 to 6 months after treatment and a follow-up VCUG was performed every 12 months. Results : The mean age at detection of VUR was $13.8{\pm}22.2$ months and the male to female ratio was 2:1. The incidence of renal scarring showed a tendency of direct correlation between severity of VUR(P<0.001) and abnormal findings of renal ultrasonography(P<0.01). 63.2%(24 of 38 renal units) of renal parenchymal defects present in the first DMSA scan disappeared on follow-up DMSA scans. Follow-up DMSA scans detected renal scars in 7(14%) of 50 renal units with ultrasonographically normal kidneys. Meanwhile, ultrasonography did not show parenchymal defects in 7(36.8%) of 19 renal units where renal scarring was demonstrated on a follow-up DMSA scan. The spontaneous resolution rate of VUR was higher(75%) in cases with low grade(I to III) VUR(P<0.01). Conclusions : The presence and severity of VUR and abnormal findings of renal ultrasonography significantly correlated with renal scar formation. DMSA scan was useful in the diagnosis of renal defects. Meanwhile renal ultrasonography was an inadequate method for evaluating renal parenchymal damage. Therefore, follow-up DMSA scans should be performed to detect renal scars even in children with low-grade VUR and normal renal ultrasonography.

초기 폐암의 정위방사선치료후 반응평가 분석 (Response Evaluation after Stereotactic Ablative Radiotherapy for Lung Cancer)

  • 정위 방사선치료를 받은 폐암환자에서 결과를 후향적으로 분석하고자 하였다. 연속된 흉부 컴퓨터단층촬영(CT)의 종양의 크기변화 분석을 통해 치료 반응 평가를 조사하였다. 11명의 초기 비소세포폐암환자를 대상으로 정위 방사선치료 선량의 중앙값은 6,000 cGy이고 분할 조사 회수의 중앙값은 5회였다. 경과 관찰기간의 중앙값은 28개월로 치료후 종양의 크기 변화는 총 51회의 CT를 통해 분석하였고, 각 환자당 중앙값 3회의 CT 촬영이 시행되었다. 본 연구에서 치료의 총 반응률은 90.9%로 5명의 환자에서 완전관해와 5명에서 부분관해가 관찰되었다. 부분관해와 완전관해까지의 기간의 중앙 값은 각각 3개월과 5개월이었다. 경과관찰에서 3명의 환자가 병의 진행양상을 나타내었다. 본 연구에서 초기 폐암의 정위방사선치료후 CT 분석을 통해 반응 평가와 함께 치료 반응 시점을 확인할 수 있었다.

급성신우신염과 방광요관역류 환아에서 DMSA신주사의 추적관찰 (A Follow-up Study of DMSA Renal Scan in Children with . Acute Pyelonephritis and Vesicoureteral Reflux)

  • 목 적 : 99m-TCDMSA(이하 DMSA)신주사는 발열성 요로감염환아 혹은 방광요관역류가 있는 환아에서 급성신우신염과 신반흔을 평가하는데 유용한 검사로 알려져 있다. 저자들은 이를 해석하는데 있어서 정확성을 기하고 효율적인 이용을 위해서 본 연구를 시행하였다. 방 법 : 1992년 1월부터 1995년 6월까지 연세대학교 세브란스병원에 내원하여 발열, 측복부 동통, 농뇨 등의 요로감염 증세 혹은 요 배양검사상 세균이 $10^5/mL$이상 동정된 환아 155명을 대상으로 DMSA신주사, 신초음파 및 배뇨성 요도방광조영술을 시행하였으며, 이중 방광요관역류가 없으면서 DMSA신주사에서 이상 소견을 보인 환아는 29명, 방광요관역류가 있었던 환아는 32명이 있었다. 초기 DMSA신주사상 병변이 발견되었던 환아는 추적검사를 통해 병변의 변화를 조사하였으며, 방광요관역류가 있었던 환아는 역류 정도에 따라 DMSA 신주사 및 신초음파 소견을 분석하였다. 결 과 : 방광요관역류 없이 DMSA신주사상 비정상소견을 보였던 29명의 환아는 요로감염 증상이 시작된 지 평균 $7.5{\pm}4.8$일내에 DMSA신주사를 시행하였으며, 평균 $0.99{\pm}0.46$개월에 추적 검사를 실시한 결과 21명 ($72.4\%$)에서는 정상으로 되었고, 3명($10.3\%$)은 병변이 감소하였으며, 5명($17.3\%$)은 병변이 지속되었다. 병변이 남아있는 8명을 3개월내 DMSA신주사를 시행한 결과 2명에서만 병변이 남아있었다. DMSA신주사로 급성신우신염이 확인된 29명중 12명($41.4\%$)이 신초음파에서 이상소견을 보였다. 방광요관역류가 있었던 32명(53역류신장)은 역류정도에따라 Grade $I:25.0\%,\;II:44.5\%,\;III:64.3\%,;IV:92.9\%,\;V:100\%$에서 초기 DMSA 신주사상 이상소견을 보였다. 53역류신장중 전체적으로 DMSA신주사에서 36신장($68.0\%$), 신초음파에서 26신장($49.1\%$)이 이상소견을 보여 유의한 차이를 보였으며(P<0.05). 특히 Grade IV 역류신장에서 유의한 차이가 있었다(P<0.05). 결 론 : DMSA신주사를 이용한 급성신우신염의 진단은 신초음파검사 보다 유용하며, 초기 DMSA신주사 소견상 이상소견을 보인 경우 약 8-12주 후 추적검사를 시행하여 변화를 관찰하고 섭취결손 부분이 남아있는 경우에는 향후 새로운 병소의 출현 혹은 정상화 여부를 보기 위한 추적검사가 필요하리라 사료된다. 방광요관역류 환아에서 DMSA신주사소견은 방광요관역류의 정도가 심할수록 이상소견을 보일 확률이 높으며 신초음파 검사보다 민감도가 높은 것을 알 수 있었다.

소아의 상부 요관확장증에서 폐쇄 유무 감별에 있어 Tc-99m DTPA 이뇨 신장 신티그램의 유용성 (The Value of Tc-99m DTPA Diuretic Renography for Assessment of Dilated Upper Urinary Tract in Children)

  • 목적: 본 연구의 목적은 Tc-99m DTPA를 이용한 표준화된 이뇨 신장스캔의 소아의 수신증의 진단에 있어서의 유용성을 알고자 하였다. 대상 및 방법: 초음파로 진단된 일측성 상부 요로 확장을 가진 14명의 환아(연령: 생후 3일-4세)를 대상으로 초기와 추적 이뇨 신장스캔을 분석하였다. Tc-99m DTPA를 이용한 이뇨 신장스캔은 표준화된 방법에 따라 시행했다. 이들 중 신생아 3 명에서는 초기 이뇨 신장스캔을 생후 1주 내에, 3-7개월 후에 추적검사를 시행하였다. 결과: 6명의 신우성형술을 시행한 환자는 수술상 요관 신우이행부 협착증이 있었고, 수술 후 3-12개월 지난 뒤 1명은 이뇨 신장스캔에서 기능을 하지 않았고 5명의 환아에서 비폐쇄성으로 전환되어 요관 신우이행부 협착이 진단되었다. 이들은 모두 이뇨 신장스캔에서 폐쇄성 레노그램을 보였다. 그러나 이들 환아 중 신생아기에 시행된 이뇨 신장스캔은 모두 불확정성 또는 비폐쇄성이었으나, 추적검사상 폐쇄성으로 전환되었다. 8명의 환아는 비폐쇄성으로 진단되었는데, 추적 초음파검사 상 수신증의 정도가 좋아졌고 수술적 치료도 필요하지 않았다. 결론: 이뇨 신장스캔은 편측성 수신증이 있는 소아에서 그 폐쇄 유무를 진단하는 데 있어 초기와 수술 후 추적검사로 유용한 검사 방법임을 확인하였다. 그러나, 신생아에서는 스캔상 불확정성 또는 비폐쇄성으로 해석되어도, 초음파 소견이나, 추적검사를 통하여 폐쇄로 진행되는지를 확인하는 것이 필요하리라 생각된다.

Closure of Petersen's Space Lowers the Incidence of Gastric Food Retention after Distal Gastrectomy with Gastrojejunostomy in Gastric Cancer Patients

  • Purpose: Delayed gastric emptying usually manifests as gastric food retention. This study aimed to evaluate the incidence of gastric food retention after distal gastrectomy with gastrojejunostomy in gastric cancer patients and identify the risk factors for its development. Materials and Methods: We retrospectively enrolled 245 patients who underwent distal gastrectomy with gastrojejunostomy for gastric cancer at Boramae Medical Center between March 2017 and December 2019. We analyzed the presence of gastric food residue via computed tomography (CT) scans at 3 and 12 months postoperatively and analyzed the risk factors that may influence the development of gastric food retention. Results: CT scans were performed on 235 patients at 3 months and on 217 patients at 12 months postoperatively. In the group that received closure of Petersen's space, the incidence of gastric food retention was significantly low as per the 3- and 12-month postoperative follow-up CT scans (P=0.028 and 0.003, respectively). In addition, hypertension was related to gastric food retention as per the 12-month postoperative follow-up CT scans (P=0.011). No other factors were related to the development of gastric food retention. In the multivariate analysis, non-closure of Petersen's space (hazard ratio [HR], 2.54; 95% confidence interval [CI], 1.20-5.38; P=0.010) was the only significant risk factor for gastric food retention at 3 months postoperatively, while non-closure of Petersen's space (HR, 2.81; 95% CI, 1.40-5.64; P=0.004) and hypertension (HR, 2.30; 95% CI, 1.14-4.63; P=0.020) were both significant risk factors for gastric food retention at 12 months postoperatively. Conclusions: Closure of Petersen's space has an effect on decrease the incidence of gastric food retention after distal gastrectomy with gastrojejunostomy in gastric cancer patients.

Imaging Patterns of Bacillus Calmette-Guérin-Related Granulomatous Prostatitis Based on Multiparametric MRI

  • Objective: To categorize multiparametric MRI features of Bacillus Calmette-Guérin (BCG)-related granulomatous prostatitis (GP) and discover potential manifestations for its differential diagnosis from prostate cancer. Materials and Methods: The cases of BCG-related GP in 24 male (mean age ± standard deviation, 66.0 ± 9.4 years; range, 50-88 years) pathologically confirmed between January 2011 and April 2019 were retrospectively reviewed. All patients underwent intravesical BCG therapy followed by a MRI scan. Additional follow-up MRI scans, including diffusion-weighted imaging (DWI), were performed in 19 patients. The BCG-related GP cases were categorized into three: A, B, or C. The lesions with diffusion restriction and homogeneous enhancement were classified as type A. The lesions with diffusion restriction and a poorly enhancing component were classified as type B. A low signal intensity on high b-value DWI (b = 1000 s/mm2) was considered characteristic of type C. Two radiologists independently interpreted the MRI scans before making a consensus about the types. Results: The median lesion size was 22 mm with the interquartile range (IQR) of 18-26 mm as measured using the initial MRI scans. The lesion types were A, B, and C in 7, 15, and 2 patients, respectively. Cohen's kappa value for the inter-reader agreement for the interpretation of the lesion types was 0.837. On the last follow-up MRI scans of 19 patients, the size decreased (median, 5.8 mm; IQR, 3.4-8.5 mm), and the type changed from A or B to C in 11 patients. The lesions resolved in four patients. In five patients who underwent prostatectomy, caseous necrosis on histopathology matched with the non-enhancing components of type B lesions and the entire type C lesions. Conclusion: BCG-related GP demonstrated three imaging patterns on multiparametric MRI. Contrast-enhanced T1-weighted imaging and DWI may play a role in its differential diagnosis from prostate cancer.
